Welcome everybody, you're listening AFSO 21's Weekend Wrap Up Podcast. Here is your host, Kevin Ferrara. If you've spent any time around a firehouse, or if you're a city manager, a council member, township supervisor, or a taxpayer, looking at where your money goes, this episode is arguably the most critical conversation we will ever have. We need to talk about an illusion, one that's killed thousands of firefighters. Not in a spectacular, tragic flashover, but quietly, slowly, over ten, fifteen, maybe twenty years in a hospital oncology ward. The illusion is simple. The smoke is gone, so the air is clean. Now imagine yourself at a structure fire. The main body of the fire is knocked down. The heavy black columns of smoke that you could see from miles away, well, they've dissipated. What's left is a scene that looks mostly clear. Maybe there's a light haze. Maybe to the naked eye, the air looks perfectly transparent. The windows are broken out. There's a positive pressure fan sitting at the front door, blowing fresh air inside, and you notice the interior crews, they're moving in with axes and hooks for what we typically call overhaul, tearing open walls and ceilings to check for hidden embers. Welcome everyone to another episode of AFSO 21's weekend wrap-up podcast. I'm your host, Kevin Ferrara. And folks, at this exact moment, across thousands of fire departments worldwide, a dangerous habit is occurring. Firefighters are ripping off their self-contained breathing apparatus, or SCBA, and they're letting their regulators and masks dangle, bounce off their gear, or worse, through charred remains inside a structure. They're taking a deep breath of that clear looking air, wiping the sweat from their brows, and they're getting ready to work. They think they're safe because they can see clearly across the room, but they're breathing in an invisible soup of highly concentrated lethal toxins. Folks, this episode stems from firefighters carelessly ignoring safety standards developed to protect them. Just yesterday, I was sent photos of a volunteer fire department who responded to a vehicle fire. The photo showed a lone fire chief pulling a line and attacking the fire. Now, that might sound like a leader who showed initiative, but when you look closer at the photos and see what was becoming more and more common, especially within the volunteer fire service, this chief wasn't even wearing a full set of turnout gear, just a coat, a ball cap, and blue jeans. While a bystander, I get you this, a bystander was holding the chief's white helmet. As for wearing an SCBA, nope, not in this scenario. It was still in the truck. Yet there he was, the fire chief. The leader of this particular department, enveloped by toxic smoke, billowing out from the engine compartment. A hero to some because, well, he showed initiative and was getting it done. Folks, that's not being smart. That's being stupid. How long to we hear this chief has cancer? Will we at all? Or will he slowly decay and pass away knowing that what he did was preventable, but he let his ego get the best of him? Will he look at his family standing beside his hospital bed and with difficulty breathing say he's sorry for not holding himself accountable? By then, folks, it's too late. So today I'm diving into the microscopic nightmare of the modern fire ground. I'll explain exactly what is in that clear air and why the human body cannot defend against it, and why local municipalities have a legal, ethical, and fiduciary duty to mandate SCBA use through strict, non-negotiable policies. So let's begin by stripping away the myth of the old school smoke eating firefighter. It's time for some hard truth. To understand why clear air is deadly, we have to look at how modern homes and buildings are manufactured. Fifty years ago, a house was built out of solid wood, cotton, wool, and metal. When solid wood burns, it produces carbon monoxide, carbon dioxide, and particulate matter. Sure it's dangerous, absolutely, but it's a slow, predictable burn. On the flip side, today's modern homes, well, they're packed with synthetic materials. Your living room furniture, it isn't stuffed with cotton down, it's polyurethane foam. Your walls aren't horsehair and plaster, they're coated with synthetic polymers. Your floors, they're vinyl or engineered laminates instead of solid wood. And the electronics you're likely listening to this podcast with, well, they're encased in flame retardant plastics. But when plastics, resins, and synthetic materials burn or heat up, they don't just create smoke. They undergo thermal degradation, releasing a toxic cocktail of gases. Crucially, these invisible gases, vapors, and fumes, they remain suspended in the air long after the visible soot particles have been ventilated out. So let's break down the big two of the toxic twins, along with the invisible carcinogens. The first one is carbon monoxide. CO is an odorless, colorless gas. We all know it's dangerous, but do you know why it's deadly during overhaul? It has an affinity for human hemoglobin, the protein in your blood that carries oxygen. That's over 200 times greater than oxygen. When you breathe in CO, it hitches a ride on your red blood cells, locking oxygen out. Your tissues suffocate on a cellular level. In an area with no visible smoke, CO levels can easily remain at 150 to 200 parts per million. The safe OSHA exposure limit for over an eight-hour period, well, that's just 50 parts per million. And then we have hydrogen cyanide. This is the real killer in modern fires. HCN is produced when nitrogen-containing plastics, nylon, and synthetic fibers burn. It's 35 times more toxic than carbon monoxide. Like CO, it's invisible. While CO stops oxygen from traveling in your blood, HCN goes straight to the cell's mitochondria and shuts off its ability to use oxygen at all. You have a lung full of pure oxygen, but if HCN is in your system, your cells literally starve to death. And then we have the volatile organic compounds, or VOCs, and carcinogens. Beyond the immediate killers, we have the slow poisons, benzene, formaldehyde, and polycyclic aromatic hydrocarbons, or PAHs. During overhaul, when these materials are off-gassing, slowly releasing trapped chemical vapors as they cool down, benzene and formaldehyde, well, they're radiating off every charred piece of drywall and melted synthetic carpet. They're invisible to the naked eye. You can't smell them over the general odor of a fire scene, but yet you're breathing them directly into your lungs. So what happens when a firefighter decides that their SCBA is too heavy, too hot, or too restrictive during a post-fire investigation or overhaul? Let's trace the physiological damage. Acute short-term effects. Well, in the immediate hours following exposure to a seemingly clear room without protection, firefighters often experience what's called the smoke hangover. It features splitting headaches, intense dizziness, which are the early signs of mild CO and HCN poisoning. We have reactive airway dysfunction syndrome, or RADS. This is an asthma-like condition triggered by a single exposure to high concentrations of irritating vapor. And then we have elevated heart rates. Because your heart is working double-time to pump poorly oxygenated blood to your organs, exposure significantly spikes the risk of an acute myocardial infarction or a heart attack on the scene or in 24 hours following the call. How many times have we read about firefighters dying within 24 hours of a call? Too many. The real tragedy, it's not the short game, it's the long game. The International Agency for Research on Cancer, or the IARC, officially classifies the occupational exposure of a firefighter as a group one carcinogen, putting it in the same category as asbestos, tobacco smoking, and radiation. When you breathe in invisible particulate matter, specifically PM 2.5, these are particles smaller than 2.5 micrometers, those particles bypass the natural filters in your nose and throat. They travel deep into the alveoli of your lungs, cross directly into your bloodstream, and carry carcinogens to every organ in your body. The statistics, folks, well, they're they're herring. Firefighters face a 9% higher risk of being diagnosed with cancer than the general public. They face a 14% higher risk of dying from cancer. In certain cancers like mesothelioma, which is caused by asbestos exposure during demolition or overhaul, testicular cancer, non-Hodgkins lymphoma, and multiple melanoma, these cancers show significantly elevated rates among firefighters who spent careers skipping their air packs during the tail end of calls. Every time a firefighter inhales clear air on a fireground without an SCBA face piece sealed to their face, they're spinning a biological roulette wheel. So now let's shift the focus from the fire ground to the city hall. If you're a mayor, a city manager, a county commissioner, a township supervisor, maybe a borough council member, or a taxpayer, I want you to listen closely. This isn't just a health issue for the firefighters protecting your communities. It's a profound policy and financial issue for local governments. Every local municipality funds fire departments using public tax dollars in some manner. Some departments are fully funded, while others receive certain amounts, maybe from an annual fire tax or donations. Because of this, municipal leaders hold a dual responsibility, protecting the citizens who pay the taxes, and protecting the public servants contracted to do the job. For decades, the fire service operated on a culture of bravado. A dirty helmet and a smoke-stained face were badges of honor. If a chief leaves SCBA use up to personal preference once the main fire is out, well, folks, the toxic culture wins. Younger firefighters, well, they're going to take their masks off at the first chance they get because they want to look tough in front of the salty veterans or the local news photographer capturing the action. Therefore, municipalities must enact strict mandatory top-down policies regarding PPE. It can't be an option, folks. It has to be a mandate. If you're within the hot zone or warm zone of an incident, regardless of visible smoke, you're on breathing air until air monitoring equipment officially clears the atmosphere. So let's talk a little bit about dollars and cents because effective and enforced safety policies save municipalities millions of tax dollars. There's the thing called presumptive laws. Many states and provinces have enacted legislation that now dictates if a firefighter develops cancer, it's an automatically presumed to be a work-related injury. This means the municipality's workers' compensation insurance must foot the entire bill for the treatment, disability, and eventual line of duty death benefits. A single cancer claim can easily cost a municipality between half a million and several million dollars. Then there's the civil liability. If a municipality fails to enforce modern safety standards, like NFPA 1500, the standard on fire department occupational safety, health, and wellness program, and a firefighter falls ill or dies, the municipality opens itself up to massive civil lawsuits for negligence. And then there's asset protection. Taxpayers fund expensive SCBA equipment, often costing upwards of $8,000, maybe $10,000 per unit. Leaving these units sitting on the apparatus while firefighters breathe and poison. Well, that's a severe misuse of public equipment designed specifically to prevent these exact financial and physical losses. By enacting strict PPE and SCBA enforcement policies, municipal leaders aren't micromanaging. They're practicing responsible fiduciary oversight. They're protecting tax dollars from catastrophic workers' compensation payouts while ensuring their emergency workforce stays healthy and operational. So as we wrap up this breakdown, let's bring it back to the human element for a second. This might come as a shock to some of you listening, but folks, the old way of firefighting, it's dead. And if it's not in your department, it needs to be. True bravery isn't breathing in toxic chemicals to prove you can take it. True bravery is having the discipline to protect yourself and others so everybody can go home to your families, and so you can continue serving your communities for 30 or more years instead of cutting your life short at 50. To the firefighters out there listening, look at the air monitoring devices you have, not just through your visor. If the gas detectors are reading clear, remember that those four gas those four gas detectors, they don't screen for every synthetic carcinogen floating in the air. Keep your mask on until you're completely packed up and out of the hot zone. Before you unmask, think of why your department is practicing the clean cab environment, hopefully it is, and then look at yourself operating inside a burnout building or stretching a handline to attack a vehicle fire. You know that microscopic particulates are being sent into the air and are being disrupted with every move you make during overhaul. So why then are you deliberately putting yourself in a situation knowing that just because you don't see smoke, there's things floating around that are just as, if not more, harmful. To our municipal leaders listening, back up your fire chiefs when they create and enforce PPE policies. Hold accountable anyone who willingly violates these management documents. Seek out funding for advanced air monitoring equipment, extra air cylinders, decontamination supplies to include PPE washing equipment, and help fire departments write effective standard operating policies and guidelines that have teeth. Doing so has proven to save lives, tax dollars, and basically it's the right thing to do. So, friends, that's going to do it for this episode. I want to thank each of you for listening.
