systems,"},{"startTime":253.38,"endTime":256.71,"body":"which range from populations"},{"startTime":253.38,"endTime":256.71,"body":"as low as 500,000,"},{"startTime":256.71,"endTime":258.99,"body":"all the way to two and a"},{"startTime":256.71,"endTime":258.99,"body":"half million, 3 million."},{"startTime":258.99,"endTime":260.73,"body":"- Yeah."},{"startTime":258.99,"endTime":260.73,"body":"- So there is"},{"startTime":260.73,"endTime":262.11,"body":"a lot of inconsistency."},{"startTime":262.11,"endTime":265.62,"body":"This restructure brings"},{"startTime":262.11,"endTime":265.62,"body":"it down to around 25-ish,"},{"startTime":265.62,"endTime":267.51,"body":"where the average number"},{"startTime":265.62,"endTime":267.51,"body":"of populations are"},{"startTime":267.51,"endTime":269.22,"body":"two and a half million to 3 million."},{"startTime":269.22,"endTime":271.02,"body":"So more consistency."},{"startTime":271.02,"endTime":272.46,"body":"But also a bit more clarity"},{"startTime":272.46,"endTime":274.65,"body":"in some of the roles,"},{"startTime":272.46,"endTime":274.65,"body":"and responsibilities,"},{"startTime":274.65,"endTime":276.09,"body":"of neighbourhoods,"},{"startTime":294.12,"endTime":296.61,"body":"and what we are doing to really intervene"},{"startTime":296.61,"endTime":298.62,"body":"on seismic changes to their health?"},{"startTime":298.62,"endTime":302.756,"body":"So it's going to focus on some"},{"startTime":298.62,"endTime":302.756,"body":"of the real areas that ICSs"},{"startTime":302.756,"endTime":305.64,"body":"ICBs, and NHS England, and the department,"},{"startTime":305.64,"endTime":307.5,"body":"will need to focus on going forward."},{"startTime":307.5,"endTime":310.77,"body":"And thirdly, I think"},{"startTime":307.5,"endTime":310.77,"body":"this, for me, presents,"},{"startTime":310.77,"endTime":313.62,"body":"as I said, an opportunity but a challenge,"},{"startTime":313.62,"endTime":316.41,"body":"to really make some"},{"startTime":313.62,"endTime":316.41,"body":"transformational impact"},{"startTime":316.41,"endTime":318.72,"body":"to healthcare delivery more long term."},{"startTime":318.72,"endTime":322.11,"body":"Within our system, we talk"},{"startTime":318.72,"endTime":322.11,"body":"about generational change,"},{"startTime":322.11,"endTime":326.22,"body":"and making sure that we invoke"},{"startTime":322.11,"endTime":326.22,"body":"some sort of intervention"},{"startTime":326.22,"endTime":329.31,"body":"that impacts people today,"},{"startTime":326.22,"endTime":329.31,"body":"as well as tomorrow."},{"startTime":329.31,"endTime":331.41,"body":"I think with this restructure,"},{"startTime":331.41,"endTime":333.96,"body":"the challenge and opportunity sits in"},{"startTime":333.96,"endTime":336.48,"body":"being very clear in decision making,"},{"startTime":336.48,"endTime":340.14,"body":"and being very clear on more"},{"startTime":336.48,"endTime":340.14,"body":"impactful interventions."},{"startTime":340.14,"endTime":342.33,"body":"And we can talk about"},{"startTime":340.14,"endTime":342.33,"body":"that shortly, I'm sure."},{"startTime":342.33,"endTime":343.163,"body":"- Yeah."},{"startTime":343.163,"endTime":346.53,"body":"So just in terms of the biggest challenges"},{"startTime":346.53,"endTime":348.15,"body":"that ICBs have faced"},{"startTime":348.15,"endTime":350.1,"body":"and sort of taking on specialist services"},{"startTime":350.1,"endTime":351.69,"body":"previously managed by NHS England,"},{"startTime":351.69,"endTime":353.09,"body":"what are your views on that?"},{"startTime":354.27,"endTime":358.45,"body":"- Yeah, so delegated"},{"startTime":354.27,"endTime":358.45,"body":"commissioning is really a shift"},{"startTime":358.45,"endTime":361.2,"body":"of moving some of that autonomy and power"},{"startTime":361.2,"endTime":363.15,"body":"from the centre to ICBs."},{"startTime":363.15,"endTime":364.95,"body":"We've seen that in some"},{"startTime":363.15,"endTime":364.95,"body":"disciplines already,"},{"startTime":364.95,"endTime":368.76,"body":"such as a pharmacy, optometry,"},{"startTime":364.95,"endTime":368.76,"body":"and dentistry, the pod hub,"},{"startTime":368.76,"endTime":370.74,"body":"where, you know, the market access,"},{"startTime":370.74,"endTime":373.38,"body":"the commissioning of those"},{"startTime":370.74,"endTime":373.38,"body":"particular contractors,"},{"startTime":373.38,"endTime":376.47,"body":"moving to systems."},{"startTime":376.47,"endTime":378.387,"body":"That includes also the money involved,"},{"startTime":378.387,"endTime":381.0,"body":"and the personnel that"},{"startTime":378.387,"endTime":381.0,"body":"were sitting at centre,"},{"startTime":381.0,"endTime":382.86,"body":"and that's been very interesting."},{"startTime":382.86,"endTime":385.59,"body":"Specialist services, spec com, again,"},{"startTime":385.59,"endTime":388.71,"body":"another big body of people, and money,"},{"startTime":388.71,"endTime":391.65,"body":"being delegated to systems,"},{"startTime":391.65,"endTime":394.756,"body":"because of the structural"},{"startTime":391.65,"endTime":394.756,"body":"changes is being a halt"},{"startTime":394.756,"endTime":397.608,"body":"in the mechanics of that."},{"startTime":397.608,"endTime":399.54,"body":"- Okay, yeah."},{"startTime":397.608,"endTime":399.54,"body":"- So, I think,"},{"startTime":399.54,"endTime":400.913,"body":"we are going to get influencing"},{"startTime":444.54,"endTime":446.49,"body":"the prescribing of high cost drugs"},{"startTime":446.49,"endTime":447.75,"body":"within community settings?"},{"startTime":447.75,"endTime":449.76,"body":"And do you think there's any barriers"},{"startTime":449.76,"endTime":452.4,"body":"that still it comes to medicines, mean,"},{"startTime":467.19,"endTime":469.02,"body":"you need to really be"},{"startTime":467.19,"endTime":469.02,"body":"very clear what that means"},{"startTime":469.02,"endTime":470.58,"body":"when it comes to medicines."},{"startTime":470.58,"endTime":473.34,"body":"The ICB blueprint, the 10 year plan,"},{"startTime":473.34,"endTime":474.78,"body":"and other policy documents"},{"startTime":474.78,"endTime":476.49,"body":"give us an idea on that."},{"startTime":476.49,"endTime":479.22,"body":"But, I think, over time,"},{"startTime":476.49,"endTime":479.22,"body":"over the last six months,"},{"startTime":479.22,"endTime":481.724,"body":"I think pharmacy leaders"},{"startTime":479.22,"endTime":481.724,"body":"have been quite clear"},{"startTime":481.724,"endTime":484.59,"body":"in understanding the"},{"startTime":481.724,"endTime":484.59,"body":"strategic commissioning"},{"startTime":484.59,"endTime":485.73,"body":"when it comes to medicines"},{"startTime":485.73,"endTime":489.93,"body":"really focuses on long-term,"},{"startTime":485.73,"endTime":489.93,"body":"population-based approach"},{"startTime":489.93,"endTime":494.19,"body":"to planning, commissioning,"},{"startTime":489.93,"endTime":494.19,"body":"and delivering medicines."},{"startTime":494.19,"endTime":496.65,"body":"So when we talk about value, for example,"},{"startTime":496.65,"endTime":498.36,"body":"which I talk about quite often,"},{"startTime":498.36,"endTime":501.21,"body":"it's about focusing on"},{"startTime":498.36,"endTime":501.21,"body":"sustainability of medicines"},{"startTime":501.21,"endTime":502.68,"body":"across the system,"},{"startTime":502.68,"endTime":504.57,"body":"not just looking at the pound sign."},{"startTime":504.57,"endTime":506.49,"body":"So it is not a race to the bottom"},{"startTime":506.49,"endTime":507.9,"body":"for the cheapest molecule,"},{"startTime":507.9,"endTime":510.09,"body":"it's about how do we get the"},{"startTime":507.9,"endTime":510.09,"body":"biggest bang for our buck?"},{"startTime":510.09,"endTime":512.16,"body":"How do we use that value base"},{"startTime":512.16,"endTime":513.36,"body":"in getting the most of the medicines"},{"startTime":513.36,"endTime":515.55,"body":"that we have to spend on the medicines?"},{"startTime":515.55,"endTime":518.42,"body":"It's also focusing on"},{"startTime":515.55,"endTime":518.42,"body":"safety, and optimisation,"},{"startTime":518.42,"endTime":520.08,"body":"in this wider sense."},{"startTime":520.08,"endTime":522.96,"body":"So how do we avoid harm in clinical risk,"},{"startTime":522.96,"endTime":525.66,"body":"at the same time reduce"},{"startTime":522.96,"endTime":525.66,"body":"unwanted
