I'm Simon and I'm joined by my co-host, Amy. Hello everyone. We've got a fascinating topic today, exploring the long-term benefits of postoperative physical activity following joint arthroplasty using accelerometers to measure activity levels. There's a lot to look into, so let's get started.
The main aim of this study was to investigate the association between postoperative physical activity and mortality following hip and knee arthroplasty.
The authors conducted a retrospective cohort study utilizing the UK Biobank dataset, which contains health information from around half a million UK volunteers. The study included 1,769 patients and found that [00:01:00] higher levels of postoperative physical activity were linked to reduced long-term mortality.
They assessed physical activity using wrist-worn accelerometers and examined various metrics such as daily step count, light physical activity, and moderate to vigorous physical activity.
These physical activity metrics were derived using a machine learning algorithm tailored for wrist-worn accelerometer data. The study found that adjusted daily step count, light physical activity, moderate to vigorous physical activity, and peak cadence were all significantly associated with reduced all-cause mortality.
One intriguing aspect is the follow-up of patients for up to 20 years postoperatively using Cox regression models to assess survival probability. This long-term follow-up is essential for understanding the sustained benefits of physical activity.
Indeed. One of the key findings was that higher postoperative physical activity was associated with reduced all-cause mortality between the 10 and 20 year follow-ups. For example, a higher daily [00:02:00] step count had a hazard ratio of 0.92, indicating a significant mortality benefit.
Furthermore, the study highlighted variations based on the type of surgery. Patients who underwent hip resurfacing and unicompartmental knee arthroplasty demonstrated lower mortality rates than those who had total joint replacements. This suggests that the type of procedure can also influence long-term outcomes.
That's a crucial point. The study emphasised that the less radical procedures of partial knee arthroplasty and hip resurfacing may offer some reduction in mortality, possibly due to higher postoperative activity levels following these less invasive procedures.
There are some limitations to the study as well, which the authors acknowledge. For instance, there was no data on preoperative physical activity, so comparisons between pre- and postoperative activity levels can't be made. Additionally, the cohort was predominantly white, which might limit the generalisability of the findings. True, but despite [00:03:00] these limitations, this research still provides valuable insights into the benefits of promoting physical activity post-surgery.
It aligns with broader literature suggesting that regular physical activity protects against chronic health conditions and comorbidities.
