Steve Miff: Access to Care Makes No Sense

For most hospitals + health systems, care seems to stop after the patient leaves the hospital. Even with all of the healthcare data and information tracking on patients, we are still unable to meet patients where they are. How can we have all of this data, but still lack the means to provide better access to care?
