So tell us a little bit about what got "},{"startTime":103.96,"endTime":108.6,"body":"you interested in this topic and how it led to  "},{"startTime":108.6,"endTime":116.52,"body":"your study. So I have always had a passion for "},{"startTime":108.6,"endTime":116.52,"body":"end-of-life and palliative care as it relates  "},{"startTime":116.52,"endTime":122.56,"body":"to patients and family members and I'm also "},{"startTime":116.52,"endTime":122.56,"body":"aware that as a nurse I was aware of my lack  "},{"startTime":122.56,"endTime":129.56,"body":"of comfort when handling situations that related "},{"startTime":122.56,"endTime":129.56,"body":"to terminal illness and communicating to patients  "},{"startTime":129.56,"endTime":135.72,"body":"about that. And so my goal was to work with an "},{"startTime":129.56,"endTime":135.72,"body":"institution that would allow me to incorporate  "},{"startTime":135.72,"endTime":142.32,"body":"those concepts into the into their curriculum and "},{"startTime":135.72,"endTime":142.32,"body":"so Dr. Lawanda Greene, who at the time was the  "},{"startTime":142.32,"endTime":150.32,"body":"program chair of an adult gerontology acute care "},{"startTime":142.32,"endTime":150.32,"body":"nurse practitioner program. She recognized that  "},{"startTime":150.32,"endTime":155.96,"body":"there was a gap in the graduate curriculum in "},{"startTime":150.32,"endTime":155.96,"body":"her program and there was also a need to meet  "},{"startTime":155.96,"endTime":162.2,"body":"that accreditation requirement. And so from "},{"startTime":155.96,"endTime":162.2,"body":"there we just began to fill that gap that we  "},{"startTime":162.2,"endTime":168.4,"body":"identified in the curriculum. And that's what led "},{"startTime":162.2,"endTime":168.4,"body":"us to implementing that end-of-life simulation  "},{"startTime":168.4,"endTime":174.68,"body":"program into the adult gerontology acute care "},{"startTime":168.4,"endTime":174.68,"body":"nurse practitioner program. That's great. Tell  "},{"startTime":174.68,"endTime":181.88,"body":"us a little bit about the study specifics. Okay. "},{"startTime":174.68,"endTime":181.88,"body":"So a little overview. Yeah or the methodology. "},{"startTime":181.88,"endTime":187.2,"body":"So what we did was we actually the study was "},{"startTime":181.88,"endTime":187.2,"body":"actually conducted at a university in the  "},{"startTime":187.2,"endTime":194.76,"body":"southeast here in Georgia and we identified that "},{"startTime":187.2,"endTime":194.76,"body":"there was a need. And so our steps were to one,  "},{"startTime":194.76,"endTime":201.36,"body":"implement an end of life simulation into the "},{"startTime":194.76,"endTime":201.36,"body":"program and we wanted to use standardized  "},{"startTime":201.36,"endTime":208.68,"body":"patients. This was my doctoral project and so I "},{"startTime":201.36,"endTime":208.68,"body":"could have paid for actors but instead I recruited  "},{"startTime":208.68,"endTime":213.76,"body":"undergraduate nursing students because that was "},{"startTime":208.68,"endTime":213.76,"body":"also going to be an opportunity for them to learn.  "},{"startTime":213.76,"endTime":219.08,"body":"Oh that's great. And so we recruited undergraduate "},{"startTime":213.76,"endTime":219.08,"body":"nursing students to be those standardized actors  "},{"startTime":219.08,"endTime":226.28,"body":"and we put them through a standardized participant "},{"startTime":219.08,"endTime":226.28,"body":"training for them to be prepared for that process.  "},{"startTime":226.28,"endTime":233.92,"body":"And so we prepared our standardized patients "},{"startTime":226.28,"endTime":233.92,"body":"which were the undergraduate nursing students  "},{"startTime":233.92,"endTime":240.56,"body":"and we had our students actually participate "},{"startTime":233.92,"endTime":240.56,"body":"in one group, they participated in a pre-test  "},{"startTime":240.56,"endTime":247.32,"body":"and post-test study that evaluated their comfort "},{"startTime":240.56,"endTime":247.32,"body":"levels when incorporating or when participating  "},{"startTime":247.32,"endTime":253.52,"body":"in that simulation. That's great, thank you. Can "},{"startTime":247.32,"endTime":253.52,"body":"you guys discuss why you chose the end-of-life  "},{"startTime":253.52,"endTime":258.44,"body":"nursing education consortium curriculum as "},{"startTime":253.52,"endTime":258.44,"body":"the intervention and the nursing anxiety and  "},{"startTime":258.44,"endTime":264.16,"body":"self-confidence with clinical decision-making "},{"startTime":258.44,"endTime":264.16,"body":"scale to measure impact? Okay, We're trying  "},{"startTime":264.16,"endTime":268.92,"body":"to explore using psychometrics and tools more "},{"startTime":264.16,"endTime":268.92,"body":"too so I thought this was a great opportunity,  "},{"startTime":268.92,"endTime":278.4,"body":"this article, to kind of discuss that. All "},{"startTime":268.92,"endTime":278.4,"body":"right, sure. So I was Dr. Daly's chair and  "},{"startTime":278.4,"endTime":284.64,"body":"when she brought this idea that her and Dr. Greene "},{"startTime":278.4,"endTime":284.64,"body":"had, we wanted to do it off of a program that was  "},{"startTime":284.64,"endTime":289.48,"body":"already well established, well-founded. "},{"startTime":284.64,"endTime":289.48,"body":"So ELNEC answered that call. You know,  "},{"startTime":289.48,"endTime":295.52,"body":"it's been around since 2000 Dr. Betty Ferrell and "},{"startTime":289.48,"endTime":295.52,"body":"her colleagues with the City of Hope originally  "},{"startTime":295.52,"endTime":299.64,"body":"developed this for the health care professionals "},{"startTime":295.52,"endTime":299.64,"body":"that are already out there practicing  "},{"startTime":300.68,"endTime":307.56,"body":"but however it formulated over time and expanded "},{"startTime":300.68,"endTime":307.56,"body":"into academia and supporting the educational  "},{"startTime":307.56,"endTime":314.6,"body":"efforts of nursing students at various degree "},{"startTime":307.56,"endTime":314.6,"body":"levels. So that is where we started out with okay  "},{"startTime":314.6,"endTime":321.0,"body":"ELNAC, this is going to really fit what we're "},{"startTime":314.6,"endTime":321.0,"body":"trying to establish here. It's a six module  "},{"startTime":321.0,"endTime":327.12,"body":"program. It touches on all those things that Dr. "},{"startTime":321.0,"endTime":327.12,"body":"Daly has talked about that are really hard to  "},{"startTime":327.12,"endTime":333.2,"body":"deal with when you have a patient that's dying "},{"startTime":327.12,"endTime":333.2,"body":"with palliative care. The symptom management,  "},{"startTime":333.2,"endTime":339.32,"body":"the ethical-legal dilemmas that come along, "},{"startTime":333.2,"endTime":339.32,"body":"cultural considerations that vary between  "},{"startTime":339.32,"endTime":346.6,"body":"patients and communicating and dealing with the "},{"startTime":339.32,"endTime":346.6,"body":"grief process not only the patient but their  "},{"startTime":346.6,"endTime":353.44,"body":"significant others as well. So it was a very "},{"startTime":346.6,"endTime":353.44,"body":"robust program training program and Dr. Daly  "},{"startTime":353.44,"endTime":359.64,"body":"was very committed to it and she went through the "},{"startTime":353.44,"endTime":359.64,"body":"program herself and became the train the trainer  "},{"startTime":360.2,"endTime":367.64,"body":"so she was in the perfect position to pass this "},{"startTime":360.2,"endTime":367.64,"body":"education on to her students. That's wonderful.  "},{"startTime":367.64,"endTime":375.04,"body":"And the tool to measure? So the nursing anxiety "},{"startTime":367.64,"endTime":375.04,"body":"and self-confidence with clinical decision-making  "},{"startTime":375.04,"endTime":382.72,"body":"tool I did discover, well, I will say that when "},{"startTime":375.04,"endTime":382.72,"body":"you think about your project you ask yourself,  "},{"startTime":382.72,"endTime":388.32,"body":"what what do I want to get out of this? Snd "},{"startTime":382.72,"endTime":388.32,"body":"I did want to see if that ELNEC portion of  "},{"startTime":388.32,"endTime":396.36,"body":"communication, if that portion of didactic would "},{"startTime":388.32,"endTime":396.36,"body":"be effective and what an impact learning about "},{"startTime":396.36,"endTime":404.08,"body":"communication would have as it relates to anxiety "},{"startTime":396.36,"endTime":404.08,"body":"and self-confidence. And so Dr. Krista White,  "},{"startTime":404.08,"endTime":408.88,"body":"who actually she developed this tool "},{"startTime":404.08,"endTime":408.88,"body":"from Georgetown University's School  "},{"startTime":408.88,"endTime":415.56,"body":"of Nursing and Health Sciences and I did get her "},{"startTime":408.88,"endTime":415.56,"body":"permission to use it but it was the perfect tool "},{"startTime":415.56,"endTime":421.12,"body":"to utilize when trying to determine, hey, "},{"startTime":415.56,"endTime":421.12,"body":"are students comfortable when communicating  "},{"startTime":421.12,"endTime":427.16,"body":"with patients about terminal illness? Do they "},{"startTime":421.12,"endTime":427.16,"body":"have the self-confidence and will it change and "},{"startTime":427.16,"endTime":432.44,"body":"make an impact in their anxiety levels? So it was "},{"startTime":427.16,"endTime":432.44,"body":"the perfect tool to measure what we were trying  "},{"startTime":432.44,"endTime":438.56,"body":"to discover. And you didn't find any changes "},{"startTime":432.44,"endTime":438.56,"body":"in anxiety levels or correlation between years  "},{"startTime":438.56,"endTime":444.0,"body":"of experience and self-confidence. Do you have "},{"startTime":438.56,"endTime":444.0,"body":"any explanation for why you didn't see that or  "},{"startTime":444.0,"endTime":450.48,"body":"why that may be? I thought it was curious. Yeah. "},{"startTime":444.0,"endTime":450.48,"body":"Well I will say that what the study identified  "},{"startTime":450.48,"endTime":457.28,"body":"was I think what we all kind of understood that "},{"startTime":450.48,"endTime":457.28,"body":"there is a certain level of anxiety that one may  "},{"startTime":457.28,"endTime":462.84,"body":"experience when they're discussing terminal "},{"startTime":457.28,"endTime":462.84,"body":"illness and end of life to patients. So to  "},{"startTime":462.84,"endTime":470.44,"body":"not have a change in their anxiety levels, it "},{"startTime":462.84,"endTime":470.44,"body":"made sense to us. We understood that finding. "},{"startTime":470.44,"endTime":476.28,"body":"We also were able to identify that this just goes "},{"startTime":470.44,"endTime":476.28,"body":"along with our purpose here that there needs to  "},{"startTime":476.28,"endTime":482.0,"body":"be more training. And not just graduate "},{"startTime":476.28,"endTime":482.0,"body":"programs, undergraduate programs and even "},{"startTime":482.0,"endTime":487.92,"body":"when they become nurses to continue that training "},{"startTime":482.0,"endTime":487.92,"body":"because regardless of the amount of years of  "},{"startTime":487.92,"endTime":494.8,"body":"experience that you have there is a level of "},{"startTime":487.92,"endTime":494.8,"body":"discomfort that you may have if you are not  "},{"startTime":494.8,"endTime":502.72,"body":"comfortable or trained to communicate effectively "},{"startTime":494.8,"endTime":502.72,"body":"with those family members and patients. Yeah,  "},{"startTime":502.72,"endTime":509.44,"body":"I enjoyed that statistic, thinking that with more "},{"startTime":502.72,"endTime":509.44,"body":"experience perhaps maybe there was more comfort  "},{"startTime":510.36,"endTime":516.84,"body":"or less anxiety and when you're dealing with "},{"startTime":510.36,"endTime":516.84,"body":"something like end of life it seems that there's  "},{"startTime":516.84,"endTime":521.76,"body":"still some discomfort there even those who may be "},{"startTime":516.84,"endTime":521.76,"body":"seasoned or experienced in this area. And is it  "},{"startTime":521.76,"endTime":527.36,"body":"the nature of the environment they're working in "},{"startTime":521.76,"endTime":527.36,"body":"or is it because they still maybe this training  "},{"startTime":527.36,"endTime":534.12,"body":"that you were talking about would be helpful. I "},{"startTime":527.36,"endTime":534.12,"body":"would say when we looked at the participants and  "},{"startTime":534.12,"endTime":540.4,"body":"where they worked a lot of them were emergency "},{"startTime":534.12,"endTime":540.4,"body":"room nurses or ICU nurses and still they felt  "},{"startTime":540.4,"endTime":546.64,"body":"a sense of anxiety and they felt a sense of "},{"startTime":540.4,"endTime":546.64,"body":"discomfort. And so that again shows that that  "},{"startTime":546.64,"endTime":555.2,"body":"environment is known to have an increased possible "},{"startTime":546.64,"endTime":555.2,"body":"level of ... they experience terminal illness or  "},{"startTime":555.2,"endTime":560.8,"body":"death a little bit more frequently than someone "},{"startTime":555.2,"endTime":560.8,"body":"who may work in a different setting. And they  "},{"startTime":560.8,"endTime":570.04,"body":"too still were uncomfortable with the topic. What "},{"startTime":560.8,"endTime":570.04,"body":"are your next steps? So actually and Dr. Greene  "},{"startTime":570.04,"endTime":576.84,"body":"can talk more on this, although this study was "},{"startTime":570.04,"endTime":576.84,"body":"done with graduate nurse practitioner students,  "},{"startTime":576.84,"endTime":582.92,"body":"Dr. Greene and I have actually been working with "},{"startTime":576.84,"endTime":582.92,"body":"her undergraduate nursing students at Fort Valley  "},{"startTime":582.92,"endTime":591.24,"body":"implementing end of life standardized participant "},{"startTime":582.92,"endTime":591.24,"body":"simulations into her curriculum. And I think that  "},{"startTime":591.24,"endTime":596.24,"body":"when we first started talking about this I knew "},{"startTime":591.24,"endTime":596.24,"body":"that this was a big deal because we were looking  "},{"startTime":596.24,"endTime":602.64,"body":"at the competencies for adult gerontology care "},{"startTime":596.24,"endTime":602.64,"body":"nurse practitioners who primarily work in the  "},{"startTime":602.64,"endTime":610.44,"body":"acute care setting, critical care, and they see a "},{"startTime":602.64,"endTime":610.44,"body":"lot of death and dying. But that like Dr. Daly was  "},{"startTime":610.44,"endTime":616.96,"body":"saying, the comfort with talking to people about "},{"startTime":610.44,"endTime":616.96,"body":"end of life care and who should do it - should it  "},{"startTime":616.96,"endTime":622.04,"body":"be a chaplain talking to the family, should it be "},{"startTime":616.96,"endTime":622.04,"body":"the nurse, should it be the physician, the nurse  "},{"startTime":622.04,"endTime":627.64,"body":"practitioner? And these students need that type, "},{"startTime":622.04,"endTime":627.64,"body":"like we would saying, that type of training. I  "},{"startTime":627.64,"endTime":633.44,"body":"feel like it needs to start at the undergraduate "},{"startTime":627.64,"endTime":633.44,"body":"level. When we did an initial run with this a  "},{"startTime":633.44,"endTime":639.48,"body":"couple months ago with our BSN students we had one "},{"startTime":633.44,"endTime":639.48,"body":"students say, well, we usually have a nurse to do  "},{"startTime":639.48,"endTime":644.24,"body":"this. I'm like, that's the role you're stepping "},{"startTime":639.48,"endTime":644.24,"body":"into! Yes. And whether you like it or not,  "},{"startTime":644.24,"endTime":650.0,"body":"you're gonna see a lot of death and dying with all "},{"startTime":644.24,"endTime":650.0,"body":"these different disease processes that are going  "},{"startTime":650.0,"endTime":657.04,"body":"on in the light of Covid it's ... you're going to "},{"startTime":650.0,"endTime":657.04,"body":"see it and it's just a matter of time when you see  "},{"startTime":657.04,"endTime":666.24,"body":"it. so being prepared to talk to families and "},{"startTime":657.04,"endTime":666.24,"body":"to just they say more than I understand because  "},{"startTime":666.24,"endTime":671.16,"body":"you really don't. You don't understand. You may "},{"startTime":666.24,"endTime":671.16,"body":"can sympathize with them if you've been through  "},{"startTime":671.16,"endTime":675.76,"body":"similar situations but you don't understand "},{"startTime":671.16,"endTime":675.76,"body":"where they're coming from. so that listening,  "},{"startTime":675.76,"endTime":682.92,"body":"being empathetic, and knowing what not to say that "},{"startTime":675.76,"endTime":682.92,"body":"may trigger somebody to go into a tail spin is a  "},{"startTime":682.92,"endTime":689.4,"body":"big deal. starting early and getting them on "},{"startTime":682.92,"endTime":689.4,"body":"board with this would be great. I think that's  "},{"startTime":689.4,"endTime":693.32,"body":"wonderful. I think all undergraduate programs "},{"startTime":689.4,"endTime":693.32,"body":"should have some simulation built in that's  "},{"startTime":693.32,"endTime":697.8,"body":"about end of life or palliative care. I think it's "},{"startTime":693.32,"endTime":697.8,"body":"very helpful for the students. We used to do it at  "},{"startTime":697.8,"endTime":702.6,"body":"a university I worked at previously and I know "},{"startTime":697.8,"endTime":702.6,"body":"we're talking about doing something like that in  "},{"startTime":702.6,"endTime":710.08,"body":"our new undergraduate curriculum that's beginning "},{"startTime":702.6,"endTime":710.08,"body":"this fall. So go ahead. I was just gonna say also  "},{"startTime":710.08,"endTime":716.84,"body":"I'm doing a interdisciplinary collaboration "},{"startTime":710.08,"endTime":716.84,"body":"project with the respiratory therapy program  "},{"startTime":716.84,"endTime":724.68,"body":"and we're going to actually try, we are going "},{"startTime":716.84,"endTime":724.68,"body":"to actually implement terminal weaning with our  "},{"startTime":724.68,"endTime":730.16,"body":"respiratory students and incorporating the role "},{"startTime":724.68,"endTime":730.16,"body":"that nurses have and getting them comfortable with  "},{"startTime":730.16,"endTime":736.68,"body":"collaborating during that end of life process and "},{"startTime":730.16,"endTime":736.68,"body":"so I'm excited about that as well. It just shows  "},{"startTime":736.68,"endTime":743.12,"body":"the passion that we all have when it comes to end "},{"startTime":736.68,"endTime":743.12,"body":"of life. Dr. Roberts and her simulation experience  "},{"startTime":743.12,"endTime":748.52,"body":"giving us the guidance to follow the simulation "},{"startTime":743.12,"endTime":748.52,"body":"guidelines accurately to make sure we meet our  "},{"startTime":748.52,"endTime":755.6,"body":"outcomes. We definitely have a desire to implement "},{"startTime":748.52,"endTime":755.6,"body":"this in our own individual programs and let it be  "},{"startTime":755.6,"endTime":761.68,"body":"something that's continuous. As Dr. Greene stated, "},{"startTime":755.6,"endTime":761.68,"body":"it has to start with our undergraduate students,  "},{"startTime":761.68,"endTime":765.8,"body":"making sure that they're comfortable and "},{"startTime":761.68,"endTime":765.8,"body":"they we start with simulation because that's  "},{"startTime":765.8,"endTime":773.64,"body":"a safe environment for them to begin with such a "},{"startTime":765.8,"endTime":773.64,"body":"difficult situation and difficult conversation. So  "},{"startTime":773.64,"endTime":778.56,"body":"is it accurate to say we should be expecting "},{"startTime":773.64,"endTime":778.56,"body":"some new research data to come out from  "},{"startTime":779.4,"endTime":785.0,"body":"your team? It's accurate to say you'll see one our "},{"startTime":779.4,"endTime":785.0,"body":"names, each of our names on some form of research  "},{"startTime":785.0,"endTime":789.84,"body":"related to either end of life simulation or "},{"startTime":785.0,"endTime":789.84,"body":"palliative care. That is true. Great. I'm  "},{"startTime":789.84,"endTime":796.04,"body":"looking forward to that. What's your takeaway "},{"startTime":789.84,"endTime":796.04,"body":"messages you want to leave the audience with?  "},{"startTime":796.04,"endTime":803.92,"body":"I definitely will highly encourage all nursing "},{"startTime":796.04,"endTime":803.92,"body":"institutions, nursing schools to implement end  "},{"startTime":803.92,"endTime":810.2,"body":"of life and palliative care into their curriculum "},{"startTime":803.92,"endTime":810.2,"body":"somehow. ELNEC was the perfect way to do that. It  "},{"startTime":810.2,"endTime":817.28,"body":"covers six domains related to end of life nursing "},{"startTime":810.2,"endTime":817.28,"body":"education consortium and I just highly recommend  "},{"startTime":817.28,"endTime":823.36,"body":"all programs to find a way to implement end of "},{"startTime":817.28,"endTime":823.36,"body":"life into the curriculum and add simulation to  "},{"startTime":823.36,"endTime":830.48,"body":"give that real world safe experience on how to "},{"startTime":823.36,"endTime":830.48,"body":"effectively communicate during those difficult  "},{"startTime":830.48,"endTime":839.16,"body":"times. And another component to that is looking at "},{"startTime":830.48,"endTime":839.16,"body":"spirituality and how that plays a role into this  "},{"startTime":839.16,"endTime":845.48,"body":"as well because a lot of people depend "},{"startTime":839.16,"endTime":845.48,"body":"on their spirituality. I don't like to  "},{"startTime":845.48,"endTime":850.72,"body":"use religion because religion is just a set "},{"startTime":845.48,"endTime":850.72,"body":"of principles that a group of people follow,  "},{"startTime":850.72,"endTime":856.28,"body":"but everybody has some type of spirituality so "},{"startTime":850.72,"endTime":856.28,"body":"that needs to be incorporated as well and how  "},{"startTime":856.28,"endTime":862.56,"body":"it plays a role into end of life care, advanced "},{"startTime":856.28,"endTime":862.56,"body":"care planning, palliative care, so that's what  "},{"startTime":862.56,"endTime":868.36,"body":"I'm working on right now.
