Let's begin by having you briefly "},{"startTime":92.4,"endTime":98.16,"body":"describe the study and a little about cognitive  "},{"startTime":98.16,"endTime":107.28,"body":"rehearsal training. Sure. So, first of all, I'd "},{"startTime":98.16,"endTime":107.28,"body":"like to have the disclosure that this was actually  "},{"startTime":107.28,"endTime":114.48,"body":"funded by NLN. We were very happy to have received "},{"startTime":107.28,"endTime":114.48,"body":"an educational research grant to do this project.  "},{"startTime":115.2,"endTime":126.56,"body":"And this this project was trying to focus on the "},{"startTime":115.2,"endTime":126.56,"body":"experience of incivility for both nursing students  "},{"startTime":126.56,"endTime":132.56,"body":"and into their transition into professional "},{"startTime":126.56,"endTime":132.56,"body":"practice, which is unfortunately a known problem  "},{"startTime":132.56,"endTime":139.12,"body":"and one that leads to burnout and attrition, all "},{"startTime":132.56,"endTime":139.12,"body":"of the things that we don't want to happen to new  "},{"startTime":139.12,"endTime":146.16,"body":"nurses. So, a colleague of mine who was embedded "},{"startTime":139.12,"endTime":146.16,"body":"in the health care side as well as in the school  "},{"startTime":146.16,"endTime":154.16,"body":"Dr. Carrie McDermott and I wanted to see if we "},{"startTime":146.16,"endTime":154.16,"body":"could help nursing students learn some scripting  "},{"startTime":154.16,"endTime":160.56,"body":"techniques to counter incivility if encountered "},{"startTime":154.16,"endTime":160.56,"body":"in the clinical setting. And so that's where  "},{"startTime":160.56,"endTime":168.08,"body":"cognitive rehearsal training comes into place. "},{"startTime":160.56,"endTime":168.08,"body":"This work comes out of authors Cynthia Clark and  "},{"startTime":168.08,"endTime":174.64,"body":"Griffin, forget her first name. But their work, "},{"startTime":168.08,"endTime":174.64,"body":"they do a lot of work with incivility. And so  "},{"startTime":174.64,"endTime":180.64,"body":"cognitive rehearsal training, the very first step "},{"startTime":174.64,"endTime":180.64,"body":"of cognitive rehearsal training is to teach people  "},{"startTime":180.64,"endTime":188.48,"body":"how to pause if they are on the receiving end "},{"startTime":180.64,"endTime":188.48,"body":"of uncivil behaviors, which can include verbal,  "},{"startTime":188.48,"endTime":194.72,"body":"non-verbal, facial expressions, those kinds "},{"startTime":188.48,"endTime":194.72,"body":"of things. And so it's just a pause and try  "},{"startTime":194.72,"endTime":200.64,"body":"to get yourself to not react in the moment, "},{"startTime":194.72,"endTime":200.64,"body":"which can just escalate the whole situation  "},{"startTime":200.64,"endTime":208.88,"body":"in negative ways. And then it is to teach people "},{"startTime":200.64,"endTime":208.88,"body":"some scripting that they could potentially use  "},{"startTime":208.88,"endTime":218.4,"body":"to counter very specific actions that they might "},{"startTime":208.88,"endTime":218.4,"body":"receive. So for instance, eye rolling is a pretty  "},{"startTime":218.4,"endTime":227.28,"body":"well-known uncivil behavior. And so if you are a "},{"startTime":218.4,"endTime":227.28,"body":"person who is on the receiving end of an eye roll,  "},{"startTime":227.84,"endTime":234.56,"body":"your response could be to pause and then to "},{"startTime":227.84,"endTime":234.56,"body":"say something to the person to the effect of,  "},{"startTime":234.56,"endTime":241.12,"body":"it looks, you know, from your facial expression, "},{"startTime":234.56,"endTime":241.12,"body":"I am sensing that what I just did, you know,  "},{"startTime":241.12,"endTime":247.52,"body":"didn't meet your expectations. So, can we talk "},{"startTime":241.12,"endTime":247.52,"body":"about that a little bit more so that I can do,  "},{"startTime":247.52,"endTime":252.4,"body":"you know, do what needs to be done to meet your "},{"startTime":247.52,"endTime":252.4,"body":"expectations? So, it kind of calls out the person  "},{"startTime":252.4,"endTime":258.24,"body":"to say, you know, not to let it slide. Calls them"},{"startTime":252.4,"endTime":258.24,"body":"out and then kind of says like, where do we go  "},{"startTime":258.24,"endTime":263.68,"body":"from here, right? So, it's kind of starting "},{"startTime":258.24,"endTime":263.68,"body":"a conversation, right? Thank you for that  "},{"startTime":263.68,"endTime":267.92,"body":"description. Can you tell us a little bit "},{"startTime":263.68,"endTime":267.92,"body":"about how students perceived incivility in  "},{"startTime":267.92,"endTime":273.12,"body":"the clinical setting and what are the results "},{"startTime":267.92,"endTime":273.12,"body":"of the simulation-based learning experience?  "},{"startTime":273.12,"endTime":281.28,"body":"So unfortunately nursing students are one of "},{"startTime":273.12,"endTime":281.28,"body":"the big parties of perceiving uncivil behavior  "},{"startTime":281.28,"endTime":287.28,"body":"and from their perspective they're low man on the "},{"startTime":281.28,"endTime":287.28,"body":"totem pole. They have no power whatsoever in the  "},{"startTime":287.28,"endTime":293.28,"body":"situation. So even teaching them the scripting "},{"startTime":287.28,"endTime":293.28,"body":"you know is is difficult for them to enact. But  "},{"startTime":294.8,"endTime":299.6,"body":"we conducted this study actually during "},{"startTime":294.8,"endTime":299.6,"body":"the pandemic. We started it before we  "},{"startTime":299.6,"endTime":308.0,"body":"were interrupted during it and then resumed it "},{"startTime":299.6,"endTime":308.0,"body":"afterwards. And of course they were they noticed  "},{"startTime":308.0,"endTime":314.0,"body":"increased instability during the pandemic because "},{"startTime":308.0,"endTime":314.0,"body":"obviously everybody was just under tremendous  "},{"startTime":314.0,"endTime":320.24,"body":"amount of stress. But the nursing students "},{"startTime":314.0,"endTime":320.24,"body":"who participated in this study were on their  "},{"startTime":320.24,"endTime":326.64,"body":"independent practicum for their clinical capstone "},{"startTime":320.24,"endTime":326.64,"body":"course. And so they work one- on-one with nurses,  "},{"startTime":326.64,"endTime":335.84,"body":"nurse preceptors. And unfortunately 29 students "},{"startTime":326.64,"endTime":335.84,"body":"so close to 14% of the students out of the study  "},{"startTime":335.84,"endTime":343.36,"body":"reported that they experienced incivility in some "},{"startTime":335.84,"endTime":343.36,"body":"way and that the most common perpetrator was their  "},{"startTime":343.36,"endTime":351.76,"body":"preceptor which is just terrible. So again trying "},{"startTime":343.36,"endTime":351.76,"body":"to teach them some tools but understanding they're  "},{"startTime":351.76,"endTime":358.64,"body":"in a really low power kind of position. So what "},{"startTime":351.76,"endTime":358.64,"body":"we tried to do was embed some incivility in a  "},{"startTime":358.64,"endTime":364.4,"body":"simulation-based learning experience that's this "},{"startTime":358.64,"endTime":364.4,"body":"kind of chaotic multi-patient kind of simulation  "},{"startTime":364.4,"endTime":370.48,"body":"and we had someone portray an uncivil charge nurse "},{"startTime":364.4,"endTime":370.48,"body":"and so that person went into the room and kind of  "},{"startTime":370.48,"endTime":374.96,"body":"got in their face and asked them things like, "},{"startTime":370.48,"endTime":374.96,"body":"do you even know what you're doing and you know  "},{"startTime":374.96,"endTime":383.68,"body":"things like that. And honestly, we didn't record"},{"startTime":374.96,"endTime":383.68,"body":"this objectively in any measure, but what I  "},{"startTime":383.68,"endTime":390.72,"body":"noticed students doing was they were just ignoring "},{"startTime":383.68,"endTime":390.72,"body":"it, which I thought was interesting. So you know,  "},{"startTime":390.72,"endTime":394.88,"body":"some of them did try to implement some of the "},{"startTime":390.72,"endTime":394.88,"body":"scripting, but for the most part they were like  "},{"startTime":394.88,"endTime":399.52,"body":"just ignoring it and going on their way. And "},{"startTime":394.88,"endTime":399.52,"body":"when I asked him about it during debriefing,  "},{"startTime":399.52,"endTime":404.24,"body":"one of the things they said was we weren't "},{"startTime":399.52,"endTime":404.24,"body":"sure if that was on purpose or if that was  "},{"startTime":404.24,"endTime":410.56,"body":"that person's teaching style, which just made "},{"startTime":404.24,"endTime":410.56,"body":"my heart sink. Yeah. Because I'm like, well,  "},{"startTime":410.56,"endTime":417.28,"body":"so now you're saying this idea about encountering "},{"startTime":410.56,"endTime":417.28,"body":"incivility during your educational experience is  "},{"startTime":417.28,"endTime":422.88,"body":"just a thing, an experience, right? It wasn't it "},{"startTime":417.28,"endTime":422.88,"body":"wasn't unexpected like, well, this could possibly  "},{"startTime":422.88,"endTime":428.8,"body":"just be how this person is. Right. Right. So, "},{"startTime":422.88,"endTime":428.8,"body":"yeah. So, we we didn't pursue that line because  "},{"startTime":428.8,"endTime":433.28,"body":"it was so anecdotal, but it was one of the "},{"startTime":428.8,"endTime":433.28,"body":"things that I found so disheartening about it,  "},{"startTime":434.96,"endTime":439.44,"body":"and then otherwise, like I said, they just kind of "},{"startTime":434.96,"endTime":439.44,"body":"went about their business and they didn't really,  "},{"startTime":439.44,"endTime":446.88,"body":"respond. I did a little followup with some seven "},{"startTime":439.44,"endTime":446.88,"body":"people after they had already started professional  "},{"startTime":446.88,"endTime":452.48,"body":"practice. They were about a year in, and I did "},{"startTime":446.88,"endTime":452.48,"body":"one-on-one interviews with them. They all had  "},{"startTime":452.48,"endTime":460.0,"body":"experienced incivility in some manner. Most of "},{"startTime":452.48,"endTime":460.0,"body":"it was coming from nurse colleagues. And most of  "},{"startTime":460.0,"endTime":466.24,"body":"it was very blatant, being yelled at in front of "},{"startTime":460.0,"endTime":466.24,"body":"patients, being questioned in front of patients.  "},{"startTime":466.24,"endTime":471.28,"body":"And so I asked them, you know, did what kind "},{"startTime":466.24,"endTime":471.28,"body":"of strategies did you employ? And most of them  "},{"startTime":471.28,"endTime":479.12,"body":"had some kind of response. A lot of them did the "},{"startTime":471.28,"endTime":479.12,"body":"pausing. They did they couldn't necessarily name  "},{"startTime":479.12,"endTime":487.36,"body":"a cognitive rehearsal strategy but they most of "},{"startTime":479.12,"endTime":487.36,"body":"them said you know I'm pretty sure that training  "},{"startTime":487.36,"endTime":495.04,"body":"had something to do with the fact that I was able "},{"startTime":487.36,"endTime":495.04,"body":"to even be in that situation you know so so that  "},{"startTime":495.04,"endTime":502.8,"body":"that longitudinal piece definitely needs more "},{"startTime":495.04,"endTime":502.8,"body":"focus so how long was the simulation if you don't  "},{"startTime":502.8,"endTime":508.96,"body":"mind me asking the simulation itself myself. We "},{"startTime":502.8,"endTime":508.96,"body":"had students come in for 3 hours and like I said,  "},{"startTime":508.96,"endTime":516.56,"body":"it was kind of a chaotic situation. It's a, it's a "},{"startTime":508.96,"endTime":516.56,"body":"simulation I designed and I've run for years where  "},{"startTime":516.56,"endTime":520.8,"body":"students come in in small groups and then they're "},{"startTime":516.56,"endTime":520.8,"body":"in a room and they have to care for multiple  "},{"startTime":520.8,"endTime":528.16,"body":"patients who all have different things going on. "},{"startTime":520.8,"endTime":528.16,"body":"It's focused main focus is on priority setting.  "},{"startTime":529.2,"endTime":534.72,"body":"And so really trying to get students to implement "},{"startTime":529.2,"endTime":534.72,"body":"priority setting frameworks in order to make  "},{"startTime":534.72,"endTime":543.68,"body":"decisions about patient care. Oh, wonderful. And "},{"startTime":534.72,"endTime":543.68,"body":"so I would assume one of the maybe sub-objectives  "},{"startTime":543.68,"endTime":551.2,"body":"would be to identify how this bullying kind of "},{"startTime":543.68,"endTime":551.2,"body":"behavior impacted their ability to prioritize care  "},{"startTime":551.2,"endTime":557.6,"body":"that we didn't have that as a clear objective, "},{"startTime":551.2,"endTime":557.6,"body":"but it was something we potentially anticipated  "},{"startTime":557.6,"endTime":564.16,"body":"that it could impact. But like I said, most of "},{"startTime":557.6,"endTime":564.16,"body":"the time they just didn't pay attention to the  "},{"startTime":564.16,"endTime":570.48,"body":"charge nurse and just went about what they were "},{"startTime":564.16,"endTime":570.48,"body":"doing, which also isn't a great response either,  "},{"startTime":570.48,"endTime":577.6,"body":"you know, because you need to have the ability to "},{"startTime":570.48,"endTime":577.6,"body":"communicate and collaborate. And so the idea that  "},{"startTime":577.6,"endTime":581.76,"body":"you're just kind of like there's this noise and "},{"startTime":577.6,"endTime":581.76,"body":"you're going to ignore it and kind of go about  "},{"startTime":581.76,"endTime":588.72,"body":"with your business. That's like not a very healthy "},{"startTime":581.76,"endTime":588.72,"body":"or you know kind of professional court comportment  "},{"startTime":588.72,"endTime":596.56,"body":"way of dealing with it either. I did have one "},{"startTime":588.72,"endTime":596.56,"body":"faculty colleague who was playing this uncivil  "},{"startTime":596.56,"endTime":602.56,"body":"charge nurse for me and the the nursing students "},{"startTime":596.56,"endTime":602.56,"body":"didn't know any of the charge nurses that we used  "},{"startTime":602.56,"endTime":607.2,"body":"which is why it was so interesting to think that "},{"startTime":602.56,"endTime":607.2,"body":"they just thought that one person was just that  "},{"startTime":607.2,"endTime":614.32,"body":"was her normal style. But I had one colleague who "},{"startTime":607.2,"endTime":614.32,"body":"was like determined to get a reaction out of the  "},{"startTime":614.32,"endTime":619.36,"body":"students and it was making her like mad that "},{"startTime":614.32,"endTime":619.36,"body":"they wouldn't engage with her in this way. So,  "},{"startTime":619.36,"endTime":626.64,"body":"she kept going in and trying to get them to do "},{"startTime":619.36,"endTime":626.64,"body":"something, but either they just kind of was like,  "},{"startTime":626.64,"endTime":632.24,"body":"you know, we can talk later, which is a which "},{"startTime":626.64,"endTime":632.24,"body":"is a cognitive rehearsal strategy. You know,  "},{"startTime":632.24,"endTime":639.92,"body":"let's talk about this later. This isn't the "},{"startTime":632.24,"endTime":639.92,"body":"right time for it. Or are they just ignored?"},{"startTime":639.92,"endTime":645.76,"body":"What part of the program are these students in? "},{"startTime":639.92,"endTime":645.76,"body":"Oh, I'm sorry. This is final semester. So we have  "},{"startTime":645.76,"endTime":651.52,"body":"a four semester pre-licensure program. Some of "},{"startTime":645.76,"endTime":651.52,"body":"these were first degree, some of them were second  "},{"startTime":651.52,"endTime":656.56,"body":"degree students because we did it in different "},{"startTime":651.52,"endTime":656.56,"body":"semesters. I wonder if some of them seen I mean  "},{"startTime":656.56,"endTime":662.4,"body":"obviously I'm sure most of them seen some kind "},{"startTime":656.56,"endTime":662.4,"body":"of behavior that was incivil at the very least in  "},{"startTime":662.4,"endTime":667.52,"body":"their clinicals up to this point. I wonder if they "},{"startTime":662.4,"endTime":667.52,"body":"saw that react that response to the charge nurse  "},{"startTime":667.52,"endTime":672.8,"body":"is something they are they were modeling from what "},{"startTime":667.52,"endTime":672.8,"body":"maybe they saw in their own clinical experience.  "},{"startTime":673.36,"endTime":679.2,"body":"Could be. It's an it would be an interesting "},{"startTime":673.36,"endTime":679.2,"body":"question to ask. What recommendations do you  "},{"startTime":679.2,"endTime":683.6,"body":"have for a nursing program that doesn't have "},{"startTime":679.2,"endTime":683.6,"body":"this type of training in their curriculum or  "},{"startTime":683.6,"endTime":688.16,"body":"in their simulation who might be interested in "},{"startTime":683.6,"endTime":688.16,"body":"embedding it in there? Where would they start  "},{"startTime":688.16,"endTime":695.28,"body":"and what would you recommend? Anything. I mean "},{"startTime":688.16,"endTime":695.28,"body":"really anything. But the the cognitive rehearsal  "},{"startTime":695.28,"endTime":705.28,"body":"training that we did was Griffith's model and that "},{"startTime":695.28,"endTime":705.28,"body":"is it's didactic. It's not resource intense. It  "},{"startTime":705.28,"endTime":713.12,"body":"just needs an hour of time. There is the piece "},{"startTime":705.28,"endTime":713.12,"body":"about just talking about incivility, you know,  "},{"startTime":713.12,"endTime":718.8,"body":"bringing it into students awareness, talking "},{"startTime":713.12,"endTime":718.8,"body":"about the effects of it, the consequences of it,  "},{"startTime":718.8,"endTime":725.36,"body":"which include attrition, burnout, you know, "},{"startTime":718.8,"endTime":725.36,"body":"job dissatisfaction, all of those things. So,  "},{"startTime":725.36,"endTime":731.84,"body":"you know, really making it part of the students "},{"startTime":725.36,"endTime":731.84,"body":"awareness to know that. So, so bringing that  "},{"startTime":731.84,"endTime":737.04,"body":"information to them and then teaching some of "},{"startTime":731.84,"endTime":737.04,"body":"the scripting, the the pause and then some of  "},{"startTime":737.04,"endTime":744.0,"body":"the scripting and then leaving time for a little "},{"startTime":737.04,"endTime":744.0,"body":"role play. And so these are this is a published  "},{"startTime":745.2,"endTime":752.08,"body":"content like a didactic almost like a little "},{"startTime":745.2,"endTime":752.08,"body":"training session. And so it should be readily  "},{"startTime":752.08,"endTime":758.64,"body":"available. So it doesn't you don't have to have "},{"startTime":752.08,"endTime":758.64,"body":"like this involved simulation that we did role  "},{"startTime":758.64,"endTime":764.08,"body":"play is considered simulation anyway, so you can "},{"startTime":758.64,"endTime":764.08,"body":"do it that way. You could do it in simulation. It  "},{"startTime":764.08,"endTime":769.04,"body":"could be very interesting. Talking to them about "},{"startTime":764.08,"endTime":769.04,"body":"their experiences while they're in clinical would  "},{"startTime":769.04,"endTime":775.92,"body":"be really important to do. Especially helping them "},{"startTime":769.04,"endTime":775.92,"body":"helping to arm them with some strategies for when  "},{"startTime":775.92,"endTime":780.48,"body":"they they do encounter it. Because if they can "},{"startTime":775.92,"endTime":780.48,"body":"figure out how to do it as a nursing student,  "},{"startTime":780.48,"endTime":785.44,"body":"then they're going to be better prepared in "},{"startTime":780.48,"endTime":785.44,"body":"being able to do something about it when they  "},{"startTime":785.44,"endTime":790.72,"body":"enter professional practice. For sure. Well, "},{"startTime":785.44,"endTime":790.72,"body":"it sounds like anything you can do, whether  "},{"startTime":790.72,"endTime":797.84,"body":"that's a postconference debrief or roleplaying "},{"startTime":790.72,"endTime":797.84,"body":"to begin with, is a start to at least get the  "},{"startTime":797.84,"endTime":803.92,"body":"understanding of the concept and and the response "},{"startTime":797.84,"endTime":803.92,"body":"to it. So, no, that's great. I want to thank you,  "},{"startTime":803.92,"endTime":809.36,"body":"Dr. Wands, for joining us today. You know, "},{"startTime":803.92,"endTime":809.36,"body":"this is bullying and incivility has been,  "},{"startTime":809.36,"endTime":814.0,"body":"you know, a topic for several years now and and "},{"startTime":809.36,"endTime":814.0,"body":"unfortunately we're still talking about it and  "},{"startTime":814.0,"endTime":818.8,"body":"still new studies are coming out demonstrating "},{"startTime":814.0,"endTime":818.8,"body":"that's still pretty pervasive and that's a  "},{"startTime":818.8,"endTime":824.64,"body":"little disheartening, right?
