They're like, everyday life. It should be in every course.  "},{"startTime":163.2,"endTime":167.04,"body":"But what's important is what do we teach? That's a start, right? Because there's  "},{"startTime":172.32,"endTime":177.12,"body":"so much to cover. So what is essential? What is "},{"startTime":172.32,"endTime":177.12,"body":"important? Whether it's a you know BS student,  "},{"startTime":177.12,"endTime":182.32,"body":"baccalaureate or it's a you know advanced practice, "},{"startTime":177.12,"endTime":182.32,"body":"right? Right. Student and and it's really it's  "},{"startTime":182.32,"endTime":191.2,"body":"application from even pre-birth. Right. So "},{"startTime":182.32,"endTime":191.2,"body":"pre-birth to even end of life to death. I want to first, even "},{"startTime":292.56,"endTime":300.48,"body":"though I did introduce you and and you know  "},{"startTime":300.48,"endTime":306.16,"body":"state your title, can you tell us a little bit "},{"startTime":300.48,"endTime":306.16,"body":"more about your roles and what your position is  "},{"startTime":306.16,"endTime":312.0,"body":"at your current institution? And I'll start with "},{"startTime":306.16,"endTime":312.0,"body":"you Cindy. So my main responsibility here at the  "},{"startTime":312.0,"endTime":318.661,"body":"Georgia Center for Oncology Research and Education "},{"startTime":312.0,"endTime":318.661,"body":"and that is short we'll say Georgia core C O R  E."},{"startTime":318.661,"endTime":323.76,"body":"I am the advanced clinical genomics nurse "},{"startTime":318.661,"endTime":323.76,"body":"practitioner that administers and is responsible  "},{"startTime":323.76,"endTime":328.88,"body":"for the genetic screening program that we do in "},{"startTime":323.76,"endTime":328.88,"body":"partnership with the department of public health.  "},{"startTime":328.88,"endTime":333.44,"body":"So I actually work in public health genomics "},{"startTime":328.88,"endTime":333.44,"body":"and women who are seen within public health  "},{"startTime":333.44,"endTime":338.72,"body":"departments are screened for hereditary breast "},{"startTime":333.44,"endTime":338.72,"body":"and ovarian cancer and if they screen positive  "},{"startTime":338.72,"endTime":345.28,"body":"they're referred to me for genetic testing and "},{"startTime":338.72,"endTime":345.28,"body":"and that's my main responsibility here at Georgia  "},{"startTime":345.28,"endTime":352.24,"body":"CORE. So I did work in clinical practice prior "},{"startTime":345.28,"endTime":352.24,"body":"to this at a hospital system for about 36 years  "},{"startTime":352.24,"endTime":357.92,"body":"and in the last part of my career there did the I "},{"startTime":352.24,"endTime":357.92,"body":"was in a hereditary cancer risk assessment clinic  "},{"startTime":357.92,"endTime":363.12,"body":"there. Okay. Well, thank you. Very interesting "},{"startTime":357.92,"endTime":363.12,"body":"position. And Deborah, can you tell us a little  "},{"startTime":363.12,"endTime":370.8,"body":"bit about your role where you're at the VA? Yeah, "},{"startTime":363.12,"endTime":370.8,"body":"sure. So, I'm full-time associate professor at  "},{"startTime":370.8,"endTime":378.8,"body":"Brigham Young University. I teach genomics to "},{"startTime":370.8,"endTime":378.8,"body":"both undergraduate and graduate students and  "},{"startTime":378.8,"endTime":385.68,"body":"I worked as a women's health nurse practitioner "},{"startTime":378.8,"endTime":385.68,"body":"for several years before obtaining my PhD. And  "},{"startTime":385.68,"endTime":394.24,"body":"there I focused on genomics. And so recently I've "},{"startTime":385.68,"endTime":394.24,"body":"begun working part-time with the San Francisco  "},{"startTime":394.24,"endTime":403.2,"body":"VISN 21 genomics clinic. In there, I primarily "},{"startTime":394.24,"endTime":403.2,"body":"do intake visits and coordinate patient cases as  "},{"startTime":403.2,"endTime":414.88,"body":"they're preparing for diagnosis and testing and "},{"startTime":403.2,"endTime":414.88,"body":"then collaborate with the physicians. And I got to  "},{"startTime":414.88,"endTime":420.48,"body":"say I love genomics and I am so excited that we're "},{"startTime":414.88,"endTime":420.48,"body":"talking about it today. Yes. I don't feel like we  "},{"startTime":420.48,"endTime":425.68,"body":"talk about it enough. So, I'm glad that we're "},{"startTime":420.48,"endTime":425.68,"body":"having this podcast because as someone who was  "},{"startTime":425.68,"endTime":430.56,"body":"a nurse faculty, you know, I don't think there's "},{"startTime":425.68,"endTime":430.56,"body":"enough genomics in our curriculum, at least to the  "},{"startTime":430.56,"endTime":434.64,"body":"extent that it really needs to be. So, let's "},{"startTime":430.56,"endTime":434.64,"body":"dive in. So, that's sort of kind of my first  "},{"startTime":434.64,"endTime":440.32,"body":"question. How would you explain the relevance of "},{"startTime":434.64,"endTime":440.32,"body":"genomics in everyday nursing practice to educators  "},{"startTime":440.32,"endTime":446.08,"body":"who are listening to this podcast who may not "},{"startTime":440.32,"endTime":446.08,"body":"feel comfortable with teaching the content?  "},{"startTime":446.08,"endTime":452.72,"body":"So Deborah and I talked about these questions "},{"startTime":446.08,"endTime":452.72,"body":"beforehand. So I'm going to try to try to give  "},{"startTime":452.72,"endTime":458.24,"body":"some perspective on the first one here. And so "},{"startTime":452.72,"endTime":458.24,"body":"I'm a family nurse practitioner by trade,  "},{"startTime":458.24,"endTime":463.68,"body":"but I've been a nurse since 1983. And so I "},{"startTime":458.24,"endTime":463.68,"body":"have all degrees related to nursing practice  "},{"startTime":463.68,"endTime":469.84,"body":"that you can actually have from an associate to "},{"startTime":463.68,"endTime":469.84,"body":"a doctorate in nursing practice. And I've seen,  "},{"startTime":469.84,"endTime":479.2,"body":"you know, genomics grow from 1983 to now. But "},{"startTime":469.84,"endTime":479.2,"body":"I just finished the AANP online national on  "},{"startTime":479.2,"endTime":484.96,"body":"demand conference and I can't tell you how many "},{"startTime":479.2,"endTime":484.96,"body":"times I heard in these presentations the word  "},{"startTime":484.96,"endTime":493.44,"body":"genomics, epigenetics, pharmacogenomics, genetics "},{"startTime":484.96,"endTime":493.44,"body":"in presentations from women's oral health, which  "},{"startTime":493.44,"endTime":502.96,"body":"they talked about hereditary gingival hyperplasia, "},{"startTime":493.44,"endTime":502.96,"body":"to lipids. And so it's so very important for nurses,  "},{"startTime":502.96,"endTime":510.48,"body":"BSN and graduate level nurses, to have a clear "},{"startTime":502.96,"endTime":510.48,"body":"understanding of this information. You know,  "},{"startTime":510.48,"endTime":517.2,"body":"guidelines for practice follow the evidence that "},{"startTime":510.48,"endTime":517.2,"body":"is out there and so we have a lot of evidence  "},{"startTime":517.2,"endTime":523.84,"body":"and research to support practice with genomics "},{"startTime":517.2,"endTime":523.84,"body":"and pharmacogenomics, but the guidelines take  "},{"startTime":523.84,"endTime":529.68,"body":"a little bit longer, you know, to to be updated "},{"startTime":523.84,"endTime":529.68,"body":"based on that research. And so I think it's just  "},{"startTime":529.68,"endTime":534.48,"body":"very important for nurse to have nurses to have a "},{"startTime":529.68,"endTime":534.48,"body":"strong understanding of these concepts so they'll  "},{"startTime":534.48,"endTime":540.64,"body":"be ready when the guidelines are updated. Thank "},{"startTime":534.48,"endTime":540.64,"body":"you. Deborah, do you have anything to add to that  "},{"startTime":540.64,"endTime":550.4,"body":"question? Genomics is important because as you "},{"startTime":540.64,"endTime":550.4,"body":"said it applies to everyday nursing. I think that  "},{"startTime":550.4,"endTime":557.36,"body":"the biggest misconception is that genomics is "},{"startTime":550.4,"endTime":557.36,"body":"a specialty and it is not. It applies to every  "},{"startTime":557.36,"endTime":565.36,"body":"nurse at every level and in every setting of "},{"startTime":557.36,"endTime":565.36,"body":"practice. I was just very curious about  "},{"startTime":565.36,"endTime":575.92,"body":"the how do we kind of like give specific, concrete "},{"startTime":565.36,"endTime":575.92,"body":"examples of how genomics is really transforming  "},{"startTime":575.92,"endTime":580.64,"body":"patient care? So kind of like sometimes we "},{"startTime":575.92,"endTime":580.64,"body":"talk about yes genomics pharmacogenomics  "},{"startTime":580.64,"endTime":587.44,"body":"they're, they're so abstract. So can you give "},{"startTime":580.64,"endTime":587.44,"body":"one specific example for how genomics can  "},{"startTime":587.44,"endTime":598.64,"body":"really create or enhance patient outcome? I can. "},{"startTime":587.44,"endTime":598.64,"body":"Yeah. That's what you do every day, right? I mean  "},{"startTime":598.64,"endTime":604.48,"body":"I can tell you there is a very cheap genetic "},{"startTime":598.64,"endTime":604.48,"body":"screen and that's a family history and taking a  "},{"startTime":604.48,"endTime":610.16,"body":"three generation pedigree. And when you look at a "},{"startTime":604.48,"endTime":610.16,"body":"family history and you look at three generations,  "},{"startTime":610.16,"endTime":615.36,"body":"you can start to pick up patterns within that "},{"startTime":610.16,"endTime":615.36,"body":"family and you can determine that this family may  "},{"startTime":615.36,"endTime":621.12,"body":"be at increased risk for a certain disease process "},{"startTime":615.36,"endTime":621.12,"body":"or for cancer. And I think it's just, you know,  "},{"startTime":621.12,"endTime":626.48,"body":"that's one thing right there where you can make a "},{"startTime":621.12,"endTime":626.48,"body":"really huge impact on patient care when you look  "},{"startTime":626.48,"endTime":633.2,"body":"at that family history and look for those patterns "},{"startTime":626.48,"endTime":633.2,"body":"that are oftentimes, you know, may be hereditary  "},{"startTime":633.2,"endTime":639.36,"body":"and certainly related to genetics. Yeah, and "},{"startTime":633.2,"endTime":639.36,"body":"that ties up very well with what we do as nurses,  "},{"startTime":639.36,"endTime":645.44,"body":"right? Assessments. But yeah, I'm just imagining "},{"startTime":639.36,"endTime":645.44,"body":"like, okay, how I'm going to do assessments and  "},{"startTime":645.44,"endTime":651.28,"body":"get that like three generation pedigree, "},{"startTime":645.44,"endTime":651.28,"body":"right? So, I think those are the practical  "},{"startTime":651.28,"endTime":656.48,"body":"aspects of care that I have in mind. But, but "},{"startTime":651.28,"endTime":656.48,"body":"thank you for that answer. And and I want to bring  "},{"startTime":656.48,"endTime":662.88,"body":"up something that happened just last week. So last "},{"startTime":656.48,"endTime":662.88,"body":"week I went to see my health care provider for my  "},{"startTime":662.88,"endTime":668.24,"body":"yearly exam and it was interesting because soon "},{"startTime":662.88,"endTime":668.24,"body":"as I sat down I had to do this questionnaire and  "},{"startTime":668.24,"endTime":672.96,"body":"I didn't really know what it was and it was all "},{"startTime":668.24,"endTime":672.96,"body":"about the genetic history, my family history. So,  "},{"startTime":672.96,"endTime":677.6,"body":"I filled out the questions like who had cancer, "},{"startTime":672.96,"endTime":677.6,"body":"what age, all these things. And then I it said  "},{"startTime":677.6,"endTime":684.16,"body":"that I was at high risk for what I didn't pass "},{"startTime":677.6,"endTime":684.16,"body":"the screening test, whatever it was. And then they  "},{"startTime":684.16,"endTime":688.64,"body":"offered me a genetic screening test. And she gave "},{"startTime":684.16,"endTime":688.64,"body":"me this brochure and and you know, she asked me if  "},{"startTime":688.64,"endTime":695.04,"body":"I wanted to participate in this extensive genetic "},{"startTime":688.64,"endTime":695.04,"body":"testing and it had a list of all these different  "},{"startTime":695.04,"endTime":698.48,"body":"things that they were going to test me for. And "},{"startTime":695.04,"endTime":698.48,"body":"you know, she wasn't sure if insurance was going  "},{"startTime":698.48,"endTime":703.52,"body":"to cover it, but they would, you know, get back to "},{"startTime":698.48,"endTime":703.52,"body":"me and tell and once they looked at my insurance  "},{"startTime":703.52,"endTime":708.64,"body":"and then I could decide which test I wanted to "},{"startTime":703.52,"endTime":708.64,"body":"include. I remember BA1 was on there, BA2. I  "},{"startTime":708.64,"endTime":712.8,"body":"can't remember all of them. That was new to me. "},{"startTime":708.64,"endTime":712.8,"body":"Is this something now that we're going to start  "},{"startTime":712.8,"endTime":718.72,"body":"seeing commonly in when we go to get our physical? "},{"startTime":712.8,"endTime":718.72,"body":"So this is what's happening now with in the state  "},{"startTime":718.72,"endTime":724.0,"body":"of Georgia, which I'm as far as I know is the "},{"startTime":718.72,"endTime":724.0,"body":"only state that has this public health genomics  "},{"startTime":724.0,"endTime":730.96,"body":"program where women who come in are screened with "},{"startTime":724.0,"endTime":730.96,"body":"a family history tool for hereditary cancer. But  "},{"startTime":730.96,"endTime":738.0,"body":"this is also happening at mammography centers. So "},{"startTime":730.96,"endTime":738.0,"body":"there's a hospital system in southwest Georgia who  "},{"startTime":738.0,"endTime":742.56,"body":"screens women who when they come in for their "},{"startTime":738.0,"endTime":742.56,"body":"mammograms with a tool to determine if they're  "},{"startTime":742.56,"endTime":748.4,"body":"appropriate for genetic testing for hereditary "},{"startTime":742.56,"endTime":748.4,"body":"breast cancer. So this is becoming part of the  "},{"startTime":748.4,"endTime":753.36,"body":"regular process that women and men go through when "},{"startTime":748.4,"endTime":753.36,"body":"they're going in for their health care and talking  "},{"startTime":753.36,"endTime":759.28,"body":"to their health care providers. and Raquel as far "},{"startTime":753.36,"endTime":759.28,"body":"as how do you do it a three generation pedigree  "},{"startTime":759.28,"endTime":765.52,"body":"you know oftentimes this stuff has to be embedded "},{"startTime":759.28,"endTime":765.52,"body":"within EHRs and they often times are and they're  "},{"startTime":765.52,"endTime":772.4,"body":"getting better the the electronic health records "},{"startTime":765.52,"endTime":772.4,"body":"are getting better at documenting family history  "},{"startTime":772.4,"endTime":779.44,"body":"and that kind of like leads me to another "},{"startTime":772.4,"endTime":779.44,"body":"question about like misconceptions that nurse  "},{"startTime":779.44,"endTime":785.04,"body":"educators have about genomics and its role in "},{"startTime":779.44,"endTime":785.04,"body":"nursing, right? Because earlier we said, \"Oo,  "},{"startTime":785.04,"endTime":791.6,"body":"it sounds so specialized, right?\" But what "},{"startTime":785.04,"endTime":791.6,"body":"do educators think in in your experience  "},{"startTime":791.6,"endTime":799.68,"body":"and what misconceptions have you encountered from "},{"startTime":791.6,"endTime":799.68,"body":"educators about genomics and its role in nursing,  "},{"startTime":799.68,"endTime":805.52,"body":"right? Yeah. So as I mentioned, I think that the "},{"startTime":799.68,"endTime":805.52,"body":"biggest misconception is that it is a specialty  "},{"startTime":805.52,"endTime":813.84,"body":"and does not apply to whatever area they teach. "},{"startTime":805.52,"endTime":813.84,"body":"And you know this bears out in research when  "},{"startTime":813.84,"endTime":823.52,"body":"nurses are asked is genetics important? Yes. Is "},{"startTime":813.84,"endTime":823.52,"body":"genetics used? Are you using it? No. So how can  "},{"startTime":823.52,"endTime":828.64,"body":"something be important if it's not practiced? "},{"startTime":823.52,"endTime":828.64,"body":"I think what nurse educators sometimes don't  "},{"startTime":828.64,"endTime":835.12,"body":"realize is the extent to which the things they "},{"startTime":828.64,"endTime":835.12,"body":"are doing are informed by genetic principles  "},{"startTime":835.12,"endTime":843.84,"body":"and genomic principles. And I think that if we "},{"startTime":835.12,"endTime":843.84,"body":"learn to call those out, then we will be helping  "},{"startTime":843.84,"endTime":849.2,"body":"students feel comfortable that they understand "},{"startTime":843.84,"endTime":849.2,"body":"these principles. Yeah. and and tagging along with nursing "},{"startTime":1076.48,"endTime":1087.44,"body":"education and our roles as nurse educators. So  "},{"startTime":1087.44,"endTime":1094.0,"body":"what strategies or teaching methods have you "},{"startTime":1087.44,"endTime":1094.0,"body":"found most effective in helping our learners  "},{"startTime":1094.0,"endTime":1103.12,"body":"or our students connect genomic concepts to "},{"startTime":1094.0,"endTime":1103.12,"body":"clinical situations? Yeah. So my my favorite  "},{"startTime":1103.12,"endTime":1110.72,"body":"way of teaching is to explain a principle and "},{"startTime":1103.12,"endTime":1110.72,"body":"then provide a resource and give them practice.  "},{"startTime":1110.72,"endTime":1118.24,"body":"And to do that you need to have case studies and "},{"startTime":1110.72,"endTime":1118.24,"body":"examples. But I'll teach my students for example  "},{"startTime":1118.24,"endTime":1123.68,"body":"principles of inheritance. That's very basic "},{"startTime":1118.24,"endTime":1123.68,"body":"genomic science. Something's autosomal dominant,  "},{"startTime":1123.68,"endTime":1130.72,"body":"right? And we like to make our pundit squares, "},{"startTime":1123.68,"endTime":1130.72,"body":"but Punnett squares are about genetic inheritance,  "},{"startTime":1130.72,"endTime":1137.92,"body":"not necessarily about phenotype, meaning how "},{"startTime":1130.72,"endTime":1137.92,"body":"it translates into the body, right? And so what  "},{"startTime":1137.92,"endTime":1145.12,"body":"you'll find is that students need to understand "},{"startTime":1137.92,"endTime":1145.12,"body":"things like incomplete penetrance or variable  "},{"startTime":1145.12,"endTime":1153.52,"body":"expressivity or de novo mutations. Teach these "},{"startTime":1145.12,"endTime":1153.52,"body":"principles and then show them how they can look  "},{"startTime":1153.52,"endTime":1159.36,"body":"up information about those principles online. Then "},{"startTime":1153.52,"endTime":1159.36,"body":"give them a case study and ask them what would you  "},{"startTime":1159.36,"endTime":1165.44,"body":"what would you tell your patient?
