This Frayme Podcast episode was created as part of our Knowledge Mobilization (KMb) Fellowship Program


In this episode, we sat down with Candice Anderson and Samantha Laxton from the Health Standards Organization (HSO), as well as Frayme Knowledge Mobilization Fellow Julia Parrott to discuss their collaboration on the Pathways Toolkit to Help Advance People-Centred Integrated Care. This toolkit is designed to help teams develop context-driven and evidence-based integrated care pathways in partnership with their community. Julia and her Fellowship partner Abeer worked alongside HSO throughout the duration of the Fellowship Program to conduct a case study of the toolkit. As Julia and Abeer’s work progressed, they developed a care pathway for parents and caregivers whose children access services through the Prince Edward Island Rape and Sexual Assault Centre.
