That's one way you'll end up with something in the rethra of a female. But then, I guess you could see this person is probably talking specifically that the patient presented as a blonde female patient.- Yeah. Okay.- Third of systodomy along with deobstructing a cat. Recently I saw a cat on social media have a permanent systodomy with a port that owner drain and kept clean. I wonder if that will become a common option. They do make permanent systodomy in production goats that have bladder stones and obstruction concerns.- I didn't know that was interesting.- Well, I think what he's talking about is the urethrostomy.- Oh.- Right.- In a goat. Like in a male, so like if you had a male dog and he was obstructed and you couldn't relieve the obstruction because of the auspienis, then you might end up just making an open urethrostomy.- Yeah.- That makes sense.- Right. And then just peeing out of that opening, not out of the tip of the penis.- So is it like a PEU surgery?- Yeah.- Or kind of.- Or kind of take away any of the enemy.- Oh, I see.- You're just leaving an opening.- I see.- It's like a salvage procedure.- Yeah.- So maybe that's what.- I kind of remember it from school. But if we look and see all that crud in there, you don't have to. I guess that's the other point is, but I learned coming, working alongside you, that usually those just go hand in hand because eventually you might end up there.- Those are the reason they belong.- Yeah.- Then you have to.- Right.- Because they don't read stones.- Right.- When I pull the UCAS.- Yeah. As soon as you pull it, they're just going to re-abstruct.- Right.- Yeah.- You don't have enough time, I guess, to leave the urethra open long enough such that the diets would dissolve the stones.- Yes.- Because you can't leave the urethra in for weeks at a time.- Right.- I mean, that's a good point.- So, yeah. That's why we end up doing them a lot more frequently than what we used to.- Mm-hmm.- Yep. All right. You guys want to do them with three?- Sure. I got it.- Okay.- Recent WVC DCM update. Any dog on green-free, boutique, exotic and green diets is recommended to have cardiac BNP testing as a screening. And if they do have DCM secondary to diet, there can be improvement/resolution if early in disease with switching diets.- Good to know. So WVC, that must have been a new announcement.- Yeah. And that's smart, though, because what I was reading was that if they get DCM, then you have to start treating it as DCM. So, knowing that there can be improvement/resolution is.- Yeah. Pretty good. I think you have to catch it very early.- Right.- Yeah.- That's the problem is that what you're screening them once a year, yeah. So like what's happening the other 364 days of the, like, you know, what's to say that your probi-MP is in the normal range, right, this month and next month. But yet, you're going to wait entire another year to, so I think that's why they say it's so hard once you find it to reverse it.- Yeah.- Because if you just got lucky and found it right away.- Found it right away.- Yeah.- Maybe.- Yeah, I'm not sure.- Okay. But yeah, that's cool.- We made a video on DCM with grain-free diets in the past few days, Dr. Hamden, she.- She's got all the all-starved out of her.- Yeah, she just did a little blurb about it. And that formulation and quality control is way more important than avoiding ingredients. Also, if we are going to go, if we are going to oversimplify and say, stay away from green-free completely, then a lot of veterinary therapeutic diets fall into that very broad and nonsensific category. So what theoretical novel protein diets are we going to recommend to our patients? You learn a lot as you come out and then all of a sudden there's all these thousands of diets and then you're like, well, crap, there's so many things out there. Well, I do think that we did say that it's not an ingredient. We don't know what it is.- I was saying in general, not about last time.- Okay.- Carry on.- No, no, like because of what the nutritionist is saying. Like I don't want to give the impression that we said you, that it's only these great free diets, like you know what I'm saying?- Yeah.- They don't know what it is. There's no consensus as to what is causing it.- Yeah.- But the thought is to avoid them. So why are they telling us to avoid them? Why are the powers that be saying, right? Listen, you should be telling your clients to avoid them. You're saying, what are we supposed to do then? Well, I'm saying, you're the nutritionist. Tell me what I'm supposed to be saying.- Yeah.- Yeah.- Do you know what I'm saying? Okay, so what is it? But we did because I was like, did she not listen to that?- You know, funny that because I saw that clip pop up of me saying, of us saying, stay away from grain-free diets. And I was like, huh, I wonder if that's going to ruffle some feathers because it's just going straight to that. And if people don't listen to the full episode, then they're not going to know why we're saying that, right? It did say it. So that's why though--- Yeah, we hardly ever get to pick the brain of a veterinary nutritionist.- Yeah, we really don't. We don't know why, so just--- Don't do it.- Avoid it, right?- Yeah.- Like, why would we say at your own risk?- The thought of the point is, what are you going to use for novel protein diets then? Because you don't have grain in it.- Yeah.- Or I think is what, and that is true. I mean, hills, real canine, you know, they have novel protein diets. So what are we supposed to use then?- Right. We essentially are putting them on a grain-free formula.- Mm-hmm.- Right?- Is it completely grain-free?- Dup can sweet potato.- And it's completely grain-free.- Right?- Yeah.- And then the blue alligator.- Right, so then she's saying, okay, but I don't think, my point is that I don't think that they know what's causing it, which is true because there's no consensus about what's causing it. There may be some other ingredient in those diets that we aren't aware of that's causing the DCM.- It's just the--- So that's hence where avoid those diets come from because you don't know.- And these diets have not been implicated in development of DCM because the FDA put out that entire--- Yeah, it's going to say there are certain brands. There's a whole chart that says, which ones were--- Whereas these ones, what she's talking about, the novel protein diets from hills, real canine, have not been implicated in the development of DCM, which is I think why people said there's something else in the ingredient list in these boutique and these exotic diets that aren't in these other main production company diets that are causing DCM.- Which is to her point of quality control, why that's so important too.- Right.- Okay, so vital vet nutrition, if you're listening, please call us so you can come on a podcast and then we can pick your brain and you can help us be smarter.- Yeah, maybe they have more research that we don't know yet or something. No, we had a shout out on social media about a cat that had eaten a foreign body that was,- That was a dog.- That was a dog, sorry.- I just figured it was like, my case is a cat.- I don't know.- But the did it. But that Courtney and I were talking about before the podcast started. She sent a picture in of the dog who ate all of these hair man's, right, basically, like gobs and gobs of them.- They were up 15.- Throughout 15 and then still had to have surgery to remove another 55.- 55, yeah, 55.- 55 bald.- A grand total of 70 hair ties.- I don't even own 70 hair ties.- Wow.- Do they just keep buying more because they couldn't find the good one?- We got the cat that I saw that we ended up doing surgery on, ended up with like 20 of the rubber hair ties. And his stomach was so hard you could feel it on the outside of his attitude.- Wow.- When you were palpating his abdomen, yeah, it felt corrugated.- Wow.- It was the straight, and so it was distended when we went in to do surgery. I mean, it was just full and he just pulled them out.- I'm so lucky, my cats are not like that.- Yeah.- They were knock on wood.- No, I know.- This is a wood.- I'll be honest that first, I saw the picture. I just, I didn't read it. I just saw the picture and I was like, oh, it's just, it's an obstruction of parasites. Because that's also a thing that they got started from adult parasites.- Yeah, yeah.- Yeah, yeah.- And the hard thing too when, reason why I was going to bring up cats is that it's hard to make a vomit. Like they don't usually vomit. You can try, but they're kind of like--- We talked about that last podcast.- Yeah, we just bonded around.- Yeah, yeah.- Well, they'll do these patients. They like sat on the spinning chair and spun around and--- Try to make a vomit.- Yeah, yeah.- Didn't work.- Cats don't, yeah, cats don't.- They just don't like to.- Nope.- So anyway, Devon, thank you for sending that.- Yeah. What are your thoughts on the new monoclonal parvo antibodies? What's your clinic go to parvo treatments? At B--- B rat.- B rat.- B rat.- B rat.- B rat.- B rat.- Okay, I love the monoclonal antibody stuff. I love that they're coming out with it. Not just this drug, all of it.- Got it.- I love all of it, every single bit of it.- We haven't had to use it yet, no?- No.- No.- It's not parvo season.- Right.- Wait, are we talking about panacool?- No.- That's a great time to talk about this.- Okay, okay.- But the monoclonal antibody for parvo is C, I think that is it, K9 parvo monoclonal parvo C and A.- It's almost parvo C and A.- What is it called?- C-P-M-A.- Okay.- Because it's on like conditional approval.- Yeah.- So--- I guess I was thinking pinkery titles when I read that.- Yeah, so you can order it, but that will be, I think that will be the go to treatment.- I do too. I think we just had the rep talk to us about it and just hearing, I don't remember the stats and I don't want to say the wrong thing, but basically it was impressive how they treated in their studies with just that and then I don't, again, don't remember the timeline, but they were able to improve and do fine without any additional treatment.- And I think the cool thing is that if you have a lot of times, unfortunately, when we're talking about treating parvo cases, it's so expensive.- Yeah.- And so being able to, the people that are having difficulty from a financial perspective, affording treatment for their dog, their puppy, being able to even just a shot in the dark, like you're giving this one the zit. Like I'm gonna spend my money on this and keep my fingers crossed. And those patients, I think, will be able to help a lot better than what we currently were.- I guess so too.- So like our previous treatment was a combination of hospitalization with IV fluids, antibiotics, anti-naudia medication, sometimes pain medication.- Right.- That's the same thing.- Am I missing anything?- Well, if it's, you can also do outpatient as the case of the severe.- Is there anything else that?- No, I'm just thinking of the feeding, but usually I've never had to do any like tube feeding or anything.- Yeah.- But you still have to get them feeling well enough to eat.- So it's just like one IV injection?- Yeah.- How long is it supposed to take to take effects?- Well, it binds right to the site, so it keeps the problem from entering the cell. So you have to also get it in a, you don't want it at the right time.- Right. Like if your dog has been sick for four days, I don't think that's gonna be an ideal situation.- Yeah, it would be great for a litter of puppies where a dog tested par of it positive, and then you've got these other puppies. You'd be giving it to all of them.- Yeah.- You know? And so I think that's, that's, at least when I was reading is that it binds the site, keep the par of IVs from going into the cell.- Okay.- So if a puppy comes in and they've been vomiting and having bloody diarrhea, do you give it to them? Or does it just depend on case by case?- I think you'd still test for par of O.- Like, and then if it, if the dog super super sick you wouldn't recommend it?- No, I think they would still do it.- I think it's just more.- I think it's more of my applications.- And I gotcha.- Yeah.- You know, it's more likely too, because you already have, because with par of O virus, you're not, when we're treating them, historically when we've treated them, we're not treating the virus. We're treating the second of symptoms that the virus is causing, and we're letting the virus run its course.- That's not true.- Right? So that's already happening in that dog that's three to four to five days into the virus, all those secondary things are happening. So giving the drug, the monoclonal antibody, which is to keep the virus from doing what it's supposed to do, is the trains are down the track.- I see.- So you can't, it's hard to slow it down. So I think that's why they say, the sooner you can give it, the better off you are, you know, with those patients. But would I still use it?- Probably.- Yeah.- Because not gonna hurt anything.- Yeah.- Yeah.- You know.- Interesting.- Yeah.- I wonder how much it'll be.- It's, and it's for eight weeks and older, not that I wouldn't use it off label in somebody under eight weeks.(laughs) It's supposed to be eight weeks and older. It's actually not cost prohibitive.- Good.- So, I mean, it's not like $50, don't get me wrong.- Right.- But I think, if it works, it's better than $70 or $70.- Not like panic-well.- I'm sorry.- It's not like panic-well. Like panic-well is like,- Panic-well, or?- Panic-well, or?- Also multiple doses.- Yeah.- You open, you have to use the vial and it's three to take it up.- So, you have to give it a couple times in a row.- Yeah, you can just.- It's not one in a row.- Yeah.- Yeah.
