Katie breaks down the difference between growth and expansion, and why so many clinic owners increase the “size of the shoe” (assets and overheads) before they’ve fully maximised the “size of the foot” (utilisation, conversion, retention, diary structure, and revenue per square foot). She shares how she’s been challenging business decisions through the lens of simplification, including the reality that more rooms don’t automatically mean more profit, and a busy team doesn’t necessarily mean a high-performing business.

She also explains why revenue stops being the score as you scale, and why the real framework to understand is assets → revenue → profit → cash. Because you can have a high-revenue clinic that’s permanently skint, and a smaller clinic that produces strong cash, less stress, and a better life. The key is learning which numbers actually show whether the machine is working, rather than relying on feelings like “we’re busy” or “the diary feels full.”
