Amy receives a concern from a listener that their mother in law has been showing signs of possible cognitive changes.  There are also concerns that these changes may be related to their alcohol intake.  The family does not see the traditional route of approaching these concerns working out and Amy provides another path, one that everyone should take regardless of the situation to ensure that someone's affairs are in order as they age.
