Home Health---
Home Health is Not Home Care. Home Health provides clinical or skilled needs.
Home Health could be a Nurse or Therapy, such as Physical, Occupational etc.
Patient  is recovering in the home from a broken hip or a stroke.
Should you go to Skilled Nursing or Home Health? That is a decision for each Patient and their family. 
Home Health is not in the Home 24 hours a day. You will also need Custodial Care, which is Home Care or a family member.
Home Health is set up via your Doctor. 
How is Home Health Paid? it is paid by Medicare and possibly some private insurance.
You can also receive Home Health in an Assisted Living.

Hospice---
Quality and Comfort Care at end of Life. 
When do you start the conversation about Hospice? When there is 6 months or less, life expectancy.
Hospice is not in the Home 24 hours a day.  Someone else must be in the Home with the Patient. Home Care or the family.
A Doctor must refer the Patient to Hospice.
What happens after 6 months, can Hospice Care last years. Yes it can.
Can you Graduate from Hospice Care? Yes you can! 
Hospice Care can happen in the Home, the Hospital, Assisted Living, Skilled Nursing, etc.
Medicare Pays for Hospice!! Medicaid will also pay for Hospice.

Kendra Tietz, has been a nurse for almost 20 years. I started my career as a Neuro nurse, transitioning to my passion which is hospice 17 years ago. I am currently the hospice clinical manager and have a masters degree in Nursing Leadership and Management. In my opinion, hospice is the best nursing field, but I am biased! Hospice nurses are the navy seals of the nursing profession. They exhibit some of the highest levels of critical thinking required to guide patients to end of life. I am proud to be a nurse and even more proud to be considered a hospice nurse.

 Dani Barnes, has worked with St. Luke’s Hospice for a little over 9 years now. For years, I oversaw all of our clinician’s schedules and referral intake. I was able to form a close relationship with our social workers and case managers. I have gained an immense amount of empathy and respect for what they do every day. I moved into Community Outreach for our hospice and home health program.
