Why Stopping Acne Meds Can Trigger Worse Breakouts

You finally quit the acne medication you were told would “fix” your skin and somehow your breakouts come back worse. We walk through why post-pill acne and post-medication acne rebound is so common when the root cause was never addressed. I explain the core idea we use in naturopathic acne treatment: your skin is a mirror, and acne is a sign of a deeper pattern inside the body. We connect the dots between chronic inflammation and common underlying drivers like gut imbalance, detoxification bottlenecks, insulin issues, and infections that can quietly worsen while a medication suppresses symptoms.
Using the birth control pill as the main example, we talk about what it does to hormone production, why suppressing androgens can temporarily clear skin, and how nutrient depletion can build over time including zinc, magnesium, and B vitamins. We also cover how the gut microbiome can shift in an unfavorable direction and why that matters for skin health. Then we unpack the “androgen rebound” effect after stopping, when androgen activity and receptor sensitivity surge and create the perfect storm for oil production, clogged pores, and painful flare-ups.

SPEAKER_00Welcome to Naturopathic Beauty's Clear Skin Sessions, where we heal your acne from the inside out. Hello, beauties, and welcome to the Clear Skin Sessions. I'm so happy that you are here. My name is Dr. Stacy Shillington. I'm a naturopathic doctor. I've been healing acne for 20 years already. I can't believe it's been that long, but I love doing it. So I never get I never get sick of talking about acne, which is quite interesting to me. But this episode is all about why you come off of acne medication and your acne gets worse. And I see so many patients that this happens to. So I really can't wait to dive into this. But this podcast is for you. The reason I do this podcast is because I want you to understand you do not have to live with acne anymore. The old way of healing acne, which is going to see your dermatologist doing medications, trying different medications because certain medications don't work, struggling for years, that's old, that's over. There's a new way to heal acne, and that is by understanding what's actually going on in your body and healing that. When you heal your body, your skin is going to clear. That is just the way the body works. I've been doing this for 20 years. I can't emphasize this enough. So if you've been struggling with acne, I want you to know we have to understand why you have acne first and heal your body, then your skin is going to clear. So let's go back and talk about acne medications for a minute. And this is important because I see so many patients that they finally find me after they've been struggling with acne and medications for years. Often these patients have gone through everything. They've been on antibiotics, they've been on spironolactone, they've been on the birth control pill, they've even been on acutane. And when they come to see me, their skin is worse than it's ever been. Not only is it inflamed, but it's dry, it's flaking, it's dehydrated, it's dull. And they have a ton of other symptoms. They may have headaches, they may have PMS, they may have constipation, they might have bloating, maybe insomnia, anxiety. These women are frazzled, they're not feeling good, they're not looking like themselves, and they are like, what is going on? Because I have done everything right. I've tried everything my doctor has suggested, and here I am, years later, feeling worse than ever before. So let's talk about this. Let's talk about why this is happening. And I understand personally why this is happening because it happened to me. So when I was in my late teens, I left home, I went to university, and my skin started breaking out. And I'm not surprised that happened because I started eating poorly, I didn't sleep, I partied, I drank alcohol, and my skin responded to that. So I went and I saw the campus doctor. He gave me the birth control pill. I was on the birth control pill for 10 years and I didn't even think about my skin because my skin was perfectly clear. The birth control pill perfectly suppressed my symptoms. But when I went to go off the birth control pill in my late 20s, my skin broke out like I had never even seen before. My acne was probably about 50 times worse than it ever was when I was in my teens or my early 20s. And I was shocked. I wasn't expecting that. I went off the birth control pill because I was starting to embrace a lot of yoga and natural living. And I did not want to take synthetic hormones anymore. I wanted to have a natural cycle. So that's why I went I went off the birth control pill and I was shocked. I had no idea what to do. I went to see my doctor. He suggested Accutane. That was not going to happen because that's not where I was heading in terms of my lifestyle. So I tried everything. I went to the library and took books out and researched acne. But you've got to remember, this was 20 years ago. We barely had the internet. We definitely didn't have Instagram. So I was on my own trying to figure this out. Finally, I went back to school to become a naturopathic doctor because I just was not solving it on my own. I didn't have the resources. So I went back to school, became a naturopathic doctor. The rest is history. I've been healing acne for the last 20 years. So what happened? Why did my skin flare up so horribly after being on the birth control pill for 10 years? And why does acne flare up so badly after women have been on this the antibiotics or spironolactone or even acutane? So I want to explain why this is happening. The number one reason is because your skin is a mirror and it shows you what's going on within your body. So when you have acne, your acne is a sign, but there's a deeper imbalance within your body that needs to be healed. If you do not heal that deeper imbalance, that imbalance is going to remain there. And for many people, it gets worse and worse as time goes on. It doesn't get better. So that's what happened to me. When I went on the pill, when I went on the pill for my acne, looking back now, I know I had a bad candida infection, systemic candida that was being aggravated by my lifestyle. When I went on the pill, it did not solve that infection. In fact, that infection continued to get worse over time. And I see this happening with patients often. They tell me about their history, they tell me what was going on in their life when their acne flared and they went on the pill. And often I can trace it back to either poor detoxification, some type of infection, some type of gut imbalance, maybe an insulin issue. But because those issues were not solved and instead they used a band-aid medication to suppress their acne, these issues got worse over time. So when they finally come off the medication, things are worse and they have to deal with the root cause at that point to get their acne to resolve. And often instead of doing that, because people don't know to do that, and doctors are not guiding them in that direction, they jump from medication to medication, thinking that they've got to use a medication to solve their acne or skin care or a treatment or something, but not understanding that real work needs to be done to heal their body and to heal their skin. So that's the number one reason why acne comes back even worse when medication is stopped. Now, there's another reason as well, and this does apply to all the different acne medications. Usually, these medications, they're band-aid solutions. They don't address the root cause. They usually have side effects within the body, and these side effects aggravate the root cause of acne. I'm not going to go into every single acne medication in detail and explain it, but I am going to use the birth control pill as an example because this is one medication that women are on a lot to manage their acne. And they don't always understand what the birth control pill is doing to their bodies. So when you take the birth control pill, basically you are shutting off your natural hormone production. Your natural hormones are being replaced with synthetic hormones. So hormone production is suppressed in your body, and this includes the suppression of androgens. Androgens are the hormones that are directly responsible for creating acne in your body. So it makes complete sense. While you're on the birth control pill, your skin is much better because those androgens have been suppressed. But there's other things that the birth control pill is doing as well. It's also depleting your nutrients. The birth control pill will deplete zinc, magnesium, and B vitamins. And the birth control pill is also notorious for damaging the skin or the gut microbiome. So, you know, it's going to reduce the number of beneficial microbes in your gut and increase the number of pathogenic microbes in your gut. So there's definitely things going on that are not beneficial when you're taking the birth control pill. So just putting everything together, you're on the pill, your initial root cause is getting worse the longer you're on the pill, plus, nutrients are being depleted, plus your gut microbiome is being compromised. And then when you come off the pill, there's something called an androgen rebound that happens. So we, when you come off the pill, you're taking away those synthetic hormones. The body has to start producing its hormones on its own. And what we see is a surge in androgen production. And we also see androgen receptors being very sensitive. So what this means is that you are going to have a lot of androgen activity in your body all at once. This is going to lead to increased oil production. It's going to lead to skin exfoliation not happening optimally, and this is going to turn into really bad acne. I see it all the time. So it's almost like coming off the birth control pill leaves a perfect storm for acne to happen. And this happens with the other medications as well. The mechanism of action isn't the same, but you know, it's the same idea. It's the same concept that these medications, they're not healing the body. They're actually making things worse for you. So what do you do if you're in this situation? Maybe right now you're on medication and you're terrified of coming off because you know that you're very prone to post-pill acne or post-medication acne. So something can absolutely be done. You're not in a hopeless situation. But what you need to do is you need to start healing your body now, before you come off medication. So often I work with patients for three months before they come off the pill or other medications because we have to heal their gut. We have to replace nutrients. We have to start working on any infections that might be present. We have to support detox. And then once things are calm, then we come off the medication. And the post-acne, the post-acne is not that bad. You might have a little bit here and there, but it's not going to be the onslaught that so many women experience. So don't lose hope if you're on medication and you're really scared to come off. There's absolutely something we can do to manage that. So what if you've already stopped your medication and your skin is just raging? Again, not to worry. The body is so resilient. We can actually heal the body. It wants to be healed. The body wants to be healed and balanced. So what we need to do, we need to understand what's going on in your body. It's not going to be the same for everybody. Often I see a lot of candida. I've been talking about that, but sometimes there's mold, sometimes there's parasites, sometimes there's H. pylori, sometimes there's a really big detox bottleneck. Sometimes there's a toxic accumulation. Sometimes it's a lack of iron, ferritin's low. There's so many different scenarios that could be happening. You need to do testing to understand what's going on in your body because acne is a complex medical condition. It's complex because it involves many different organ systems in the body. We need to understand what's going on. And then we need to put together a specific protocol for you. Blanket protocols, some of them can be really helpful to get you to a place, a certain place, but to really get that complete healing, you we need to understand what's going on for you individually. Because if your skin is flaring up, there's something happening. It's not just, it's not random. It's not random. Acne, acne is a really serious sign of inflammation in the body. This is not something that should be ignored. And it's just not something that should be suppressed because, you know, what manifests as acne now could manifest as something more serious 20 years from now. You have to think long term when it comes to your health. Acne is like a signal. It's like, whoa, guys, something's not right. You know, please help us now. Like, we really need to be healed. If you ignore that signal for like 10 years or 20 years, that could turn into something really serious. So I don't want to scare you, but I want to impress upon you that the body is communicative and we need to take responsibility for our health. We need to understand what's going on with our bodies and learn how to heal our bodies ourselves. And this is one thing I love doing is I love teaching women how to heal their bodies. So once they're they've completed working with me, if they see certain signs or symptoms, they know what to do about them. So that's very empowering for sure to understand how to heal your own body. So that is the explanation of why your acne gets worse when you come off medication. And it's not because you're doing anything wrong. You're not. You've followed the advice of your physicians, and you know, that's the person that you're supposed to trust to make sure that your health is optimized. But acne has just been treated incorrectly for so, so many years. We need to change the way that we think of treating acne. We need to think of acne as a sign that something's not right in our body and we need to heal our body. It's acne is not a medication problem, not at all. It's a pattern within the body.
