(Summit Pointe 75) Understanding Case Management

One of the significant roles at Summit Pointe is that of our colleagues who are case managers.

Case managers work with some of our Calhoun County adult neighbors who may need some assistance to achieve their life goals. Many times, these neighbors are living with a severe mental illness or a intellectual or developmental disability.

Our colleague Elizabeth is the Director of Community Based Services - Specialty Programs and talks in this episode about how the case management team is helping individuals change their lives and achieve their goals.
