Speaker 2:Hey everyone, welcome to another episode of the Good Neighbor Podcast. Today we are welcoming back Dr David Abramson, who is one of the best and most renowned plastic surgeons in the area, in the area being northern New Jersey and New York City. Dr Abramson, welcome back to the show. Thank you for having me again. So this month meaning well, it's not quite May, but the May issue of Neighbors Magazine just came out and you have an article again, and this time it's talking about Avalie, and I want to get into what these novel technologies are. Not only that, but Emsculpt Neo, because part of the you know, part of the services, I guess, that you provide. You know there's the traditional plastic surgery that people think about, but then there's also these in-office treatments that can really help. You know people's appearance and, by extension, you know how they feel about themselves. So you chose to write about Abilene. Let's, let's get into that. So what is that?
Speaker 3:So, and I'll just correct the pronunciation it's a belly, but that's okay.
Speaker 3:So it's a treatment for cellulite and you know every woman, or almost every woman, knows what cellulite is and it actually is only a problem really in women. It's just dimpling of the skin, where the skin is basically scarred down to the underlying muscle. So you get these dimples and there's been a million things over the years to try and treat cellulite and none of them have been particularly effective or they've had super long recovery times. So what this technology is, it's what we call minimally invasive, meaning you have to make a little tiny hole to do it, but you can address those dimples by actually cutting those bands. So the way the process works is someone who comes in and we evaluate them, for you know the areas where the cellulite is.
Speaker 3:Usually it's the back of the thighs, sometimes it's the buttocks, and you know we can usually treat most of the areas in a single setting and it is really a one-time treatment.
Speaker 3:So what we do is we mark all the little dimples with little circles and then we'll put the person you know laying on their stomach and we provide some local anesthetic to the area and we can usually also give some gas to help with that discomfort so that they don't have pain while we're doing the procedure and then we literally go in with this device and it slides past the little band and we pull on it and it has a little knife in it and it cuts the band. So once that band's cut it's completely released and the person then afterwards usually wears a little bit of compression stockings for a few. You tell them to wear for a week, but there's usually some bruising for about three or four days. That's moderate, and I tell people no exercise for at least a week to 10 days, but other than that they could go back to work the next day, things like that. It's really not a big problem in terms of the recovery or pain and it does seem to really address cellulite so that the legs and buttocks look much better.
Speaker 2:It's funny, I always thought cellulite was something that you is caused maybe by fat deposits or when you get out of shape, and it's, you know, I didn't realize that it's more about the structure of the muscles. You know, I thought it was something more. That kind of becomes more like a fatty deposit. I had no idea.
Speaker 3:Yeah, it's actually there's. You don't see the fat in that spot because of these tethered areas and when you manage these people I mean, some people used to come in for liposuction for it. Liposuction of anything probably makes cellulite worse. You know, significant weight gain and weight loss do make these things worse as well. So you know, and we are seeing more and more people with significant weight loss with the various GLP-1 drugs and things like that. So I think we're going to see more loose skin and more evident cellulite. But it's important to see someone who can say this is loose skin, this is cellulite, because the wrong treatment is not going to solve the issue. If you really have so much loose skin frequently, the only way to get rid of it is to actually cut out the loose skin. But if you have cellulite, you can address it with a belly and the procedure is easily tolerable in the office and people do very well.
Speaker 2:I mean it's reassuring to know you can have it done in a day and it's not, like you said, liposuction, which seems a lot more invasive. Now you also. There's something else that I mentioned called M-Sculpt Neo. Now, is that another technology, like treatment procedure, that's used for cellulite?
Speaker 3:No, it's for completely different areas. Now, m-sculpt Neo does two things. It has two areas of technology together. One is radiofrequency, which will help reduce fat, and the other is HyFem, which is muscle stimulation variety of different body parts and depending on what area you're treating, you may be trying to reduce fat more, you may be trying to build up muscle more. So the most common area people do is their abdomen, and what happens is is the device does two things.
Speaker 3:One is it goes through a process of this hyphen, which is many, many stimulations to the muscle. So they say it's like the equivalent of doing 20,000 crunches in 30 minutes. So um, and it's a series of treatments. Usually we do a series of about six treatments a week apart, and now it's flexible. No one's schedule is is perfect for every week on the same day and same time, but the treatment should follow between five and 10 days after each other. And it also has a radio frequency part that will help reduce some fat.
Speaker 3:And now I know the concept of 30,000 crunches sounds terrible, like you'll be in so much pain.
Speaker 3:But to be honest with you, I actually did it myself and I did my abdomen, I did my flanks, and the first time I did it I was a little sore the next day for about 24 hours and then I was fine and it definitely does increase your muscle tone.
Speaker 3:I mean, personally, I took pictures of me from the side and, without weight loss, I looked, my stomach looked thinner, just from the muscle, you know, having more strength. Now you can use it. There's various areas. You can use it on the buttocks, and people use that to build up their butt muscles as opposed to having to go do a lot of squats. It can be used on the thighs and it can be even be used on the arms, the biceps and triceps to build muscle. And it can even be used on the arms, the biceps and triceps to build muscle in those areas Because, you know, not everyone has enough time to go to the gym and try and work every area out. But if you can increase the tone of the muscles on the arms, you can make the arms look less flabby and look more youthful as you maybe go into your 40s, 50s and 60s.
Speaker 2:Now, is it just about appearance, or does it actually help improve or augment strength and endurance? I mean, it sounds like something that athletes might, kind of you know, gravitate towards.
Speaker 3:I mean, you know it is helping build some muscle. It's not. You know, it may be more of a toning than strength per se. There are definitely have been athletes that have added this to a workout regimen to make things look better on top of what they're doing. But so there's definitely a variety of benefits to it. Like when we use it on the biceps though we use only the high fem, we don't use the radio frequency because nobody really has fat over their biceps, so you don't want to take fat out in those spots. You want to focus that on building muscle.
Speaker 3:You know you may have a person who's trying to build muscle on their buttocks, so you don't want to take fat out in their buttocks, whereas you may have someone else who's trying to actually maybe tone up their butt but actually get rid of some of the fat. So these things are adjustable and you know we can tailor the treatment to the person and what they're looking for. And when you look at this device, there's actually different hand pieces. So you know there's ones that go on the abdomen, there's ones that go on the flank, then there's other ones that might be used on the arms and legs, and they get done in pairs so you can actually do both flanks at the same time, or both buttocks at the same time, or both arms at the same time. It's actually entertaining to watch a video of someone who's having their breastfed biceps done, because when it contracts their bicep it actually will contract their arms, so their, their arms will flop up and down and, uh, you know, from the table. So it's it's a little bit entertaining to watch. So right.
Speaker 2:So how many treatments do you normally, uh, or how many sessions or treatments does one normally have?
Speaker 3:It's usually a series of of six sessions for a given body part. Okay, Um, you know we'll do, and frequently people do more than one body part. They might do like the abdomen and flanks together, or they may have the abdomen and something else. It really depends on their anatomy. And each treatment of an area is about 30 minutes. So you know it's not really a big deal. You know, each time someone does it, we do adjust it. It tends to get a little bit stronger in terms of the percentage of high fem that we use, because you want to bring it out to what the person can tolerate, you know, in terms of their comfort, but giving them the most result.
Speaker 2:Right, right. And then so you're saying six sessions. What are they about? A week apart?
Speaker 3:Yes, about a week apart, and if someone is happy with the result and they want to build more, we usually tell them to wait about three months and then they can do another set if they want.
Speaker 2:And how long does it usually last? Like the noticeable effects?
Speaker 3:You know, it's kind of hard to tell because it depends on how active someone remains. But, probably you could maintain by doing something once a year, you know a set a year, wow Assuming you do something and just don't become completely sedentary between. This can't be the only form of physical activity.
Speaker 2:No, no, it's supposed to be, you know, to supplement, uh, not to replace. So I'd imagine you know you're going on a beach vacation in a month or so and maybe this is one reason why people might come in to see'd imagine you know you're going on a beach vacation in a month or so and maybe this is one reason why people might come in to see you. Or you know they're getting ready for the class reunion or their daughter's wedding or whatever it is, and maybe they're already active. But there's just this one part, and they want to see more immediate results than what all the hours in the gym are giving them. Is that a typical or not? Maybe a typical, but an example of some of your clients.
Speaker 3:Yes, I think that there's usually people who have a body area focus, they want to improve, and they also aren't somebody necessarily looking for surgery. Now, this tends to treat things very different than surgery, because you're building muscle. So I think there's some significant differences, but it is addressable and you do see a result in about six weeks. So if you plan, maybe pre-summer, I'm going to start to do this. So you know, maybe have a little less stuff hanging off the side of the bathing suit. You know, when I put my shorts on and things like that.
Speaker 2:Right yeah, it's been a long winter and you know, with that bathing suit you're when I put my shorts on, things like that Right yeah, it's been a long winter and you know, you're like what happened, man. Now that's interesting and I had also imagined you know out in Hollywood when it comes to award season. It was funny I think it was the Golden Globes, or Nikki Glaser made a joke about all the people using a Zen pic that were in the room, but there was quite a few people in that room also doing Emsculpt and other procedures and side effect wise, it doesn't sound like there are quite as few or quite as many side effect or risks involved, correct?
Speaker 3:No, that's been shown to be very safe. I mean, the original Emsculpt machine came out about eight or 10 years ago, so this was a second generation and they've actually you know, they do continual upgrades in terms of software and things like that, so it's very safe. I mean, we haven't encountered any issues with someone having it in terms of any side effects. You know more than someone maybe being sore for two days instead of one day.
Speaker 2:But Right, right. Well, I think this is a good time because it's, you know, coming to the end of April, we're getting into the summer season and, as he said, if you know you need to work out I mean work a plan six weeks in advance. This is the time to start. So what's the best way for people to contact you to schedule a consultation?

My nurse actually used that, uses the machine. The aveli is done by me. So even though it's a uh, even though it's a minimally invasive procedure, I do that myself and it that does take longer. It's probably an hour, could be a little, could be an hour and a half, depending on you know how much cellulite you have and how many areas. I mean. The more you have, the longer it takes, because it just takes a certain amount of time to numb it up and it takes a certain amount of time to actually address each band.
Speaker 2:Got it, got it. And do you have multiple locations? Or is your main office that you perform this in Englewood, or is your main office that you perform this in Englewood?
Speaker 3:I do this in Englewood. The Emskull machine is too big to move On a valley. If someone was coming to me in my city office I could do it, but it's just easy enough to one time. You come to the office and I also have the Pronox machine, which gives you some nitrous and makes you comfortable as well. So I think it's good to have you know, make people comfortable and then they can still drive home themselves afterwards.
