Speaker 1You know it's kind of wild when you think about it ED, erectile dysfunction it's often seen as like hush hush, a personal issue, right, but it's actually a huge global problem.
Speaker 2Yeah.
Speaker 1Affecting hundreds of millions of men.
Speaker 2Absolutely.
Speaker 1And the projections are pretty startling Like we're looking at around a third of a billion patients by next year. Wow, so it's clearly not a small thing.
Speaker 2No, not at all.
Speaker 1And while we all know about the typical treatments right, you got your pills, your injections devices things like that. Yeah, the usual suspects, the thing is, a lot of men find that those just don't really do the trick for them. Right, the results might be kind of meh, or they don't last very long, or the side effects are just a total deal breaker.
Speaker 2You're hitting on a major pain point for a lot of guys.
Speaker 1Absolutely, and this can be particularly frustrating when you're dealing with vasculogenic ED.
Speaker 2Yeah.
Speaker 1Which is the most common physical cause.
Speaker 2Right, it all comes down to blood flow. Exactly the plumbing's not working quite right. And for those men who just haven't had any luck with those standard approaches for vasculogenic ED, the need for something more, you know, something that really works and lasts. That's a huge unmet need.
Introducing Botox For Vasculogenic ED
Speaker 1For sure, and that's actually what we're going to be diving into today. Okay, because we've got a research article that's looking at a completely surprising and innovative method for treating vasculogenic ED that hasn't responded to the typical medications. I'm intrigued you. Ready for this? Yeah, botox injections directly into the penis.
Speaker 2Botox. Wow, that's definitely not the first thing that comes to mind.
Speaker 1Yeah, I know, right, it's like Botox down there. But hold on, it's not some crazy ID that just popped up out of nowhere, right? We're talking about a legit study here, a well-designed one too. One too, it's called Intracavernous Injection of BOTOX, that's 50 and 100 units for treatment of vasculogenic erectile dysfunction. Randomized controlled trial. Got it Catchy title.
Speaker 2It's very descriptive.
Speaker 1Yeah, it gets right to the point. It was done by Elshare and his colleagues and it was published in the journal Andrology in 2021.
Speaker 2Okay, 2021. So pretty recent.
Speaker 1Yeah, fairly new. And what makes it even more interesting is that it was a randomized, double-blind, placebo-controlled trial.
Speaker 2Okay, so the gold standard for research.
Speaker 1Exactly. That means neither the patients nor the researchers knew who was getting the real deal.
Speaker 2Right.
Speaker 1So that eliminates a lot of bias.
Speaker 2It helps ensure that the results are actually due to the treatment itself.
Speaker 1Exactly so. Our mission today is pretty clear cut we want to unpack what this study found about using Botox for this really tough to treat ED.
Speaker 2That was good.
Speaker 1We want to understand, like what are the potential benefits? You know, is this actually a win for these guys, and how do the researchers think this thing is even working?
Speaker 2Yeah, like what's the mechanism of action here Exactly?
Speaker 1And, most importantly, what could this mean? Is even working. Yeah, like, what's the mechanism of action here Exactly? And, most importantly, what could this mean for, you know, the average guy out there who's been searching for answers and hasn't really found anything that works?
Speaker 2Absolutely. I think a lot of men can relate to that.
Breaking Down The Research Study
Speaker 1So let's get into it. Okay let's do it. So, first things first, let's talk about the specific problem that these researchers were tackling.
Speaker 2Okay.
Speaker 1Which is refractory vasculogenic ED.
Speaker 2Right. So, as we mentioned earlier, vasculogenic ED is when the blood flow to the penis is well. It's just not happening the way it should be. Yeah, the plumbing's got a kink in it somewhere.
Speaker 1And this study zeroed in on men who had already tried the usual treatments and nothing worked.
Speaker 2Right.
Speaker 1We're talking about the PDE5 inhibitors. Those are the oral medications that everyone's heard of, exactly like Viagra and Cialis. Yep, those are the big ones, and they even tried direct injections of Trimix, which is a pretty potent combination of medications. It is.
Speaker 2So these were men who were really struggling. They had tried the go-to treatments and still weren't getting the results they needed.
Speaker 1Exactly so. This wasn't just like a mild case of ED. These guys were facing some serious challenges, absolutely, and that really highlights why this alternative approach with Botox is so intriguing.
Speaker 2It is. It's thinking outside the box.
Speaker 1So how do they actually put this to the test?
Speaker 2Well, they recruited 176 men aged between 40 and 70, all of whom had this non-responsive vasculogenic.
Speaker 1ED A good sample size.
Speaker 2Yeah, and then they randomly assigned these men into three groups.
Speaker 1Okay.
Speaker 2The first group received a single intracavernous injection of 100 units of Botox.
Speaker 1Okay.
Speaker 2The second group got a single injection of 50 units of Botox.
Speaker 1Okay, so two different doses yeah.
Speaker 2And the third group, that was the control group. They received a placebo, which was just a simple saline injection.
Speaker 1Okay, and you mentioned earlier that this was a double blind study right.
Speaker 2So neither the participants nor the medical team administering the injections knew who was getting what Exactly, and that's really important for keeping things objective.
Speaker 1It eliminates bias.
Speaker 2Exactly. You don't want anyone's expectations influencing the results.
Speaker 1So that level of rigor in the study design really makes the findings more credible.
Speaker 2Absolutely.
Speaker 1So how long did they track these men for, and what were they measuring?
Speaker 2They followed them for six months.
Speaker 1Six months OK.
Speaker 2Yeah, and they used a whole bunch of different tools to assess their erectile function.
Speaker 1OK.
Speaker 2This included what we Like, questionnaires and rating scales.
Speaker 1Got it.
Speaker 2So one of the tools they used was the SHIM score, which stands for the sexual health inventory for men.
Speaker 1Okay.
Speaker 2SHIM score. It's a pretty comprehensive questionnaire that covers different aspects of erectile function. You know, like, how often they have erections, how firm they are, things like that Makes sense. They also use the erection hardness score, which is a simple rating scale where men rate the firmness of their erection.
Speaker 1OK, so that one's pretty straightforward.
Speaker 2Yeah, it is. And then they looked at the sexual encounter profile questions which get down to the nitty gritty Things like were they able to achieve penetration and was intercourse successful?
Speaker 1So really focusing on the outcomes that matter most to the individual.
Clinical Results and Effectiveness Data
Speaker 2Right, because, at the end of the day, that's what it's all about.
Speaker 1Exactly.
Speaker 2And they also used the global assessment score, which captured how satisfied both the patient and their partner were with the treatment.
Speaker 1Oh, ok, so they got both perspectives.
Speaker 2Yeah, it's important to get the whole picture. And then, on top of all those subjective measures, they also used penile Doppler ultrasound.
Speaker 1OK, so that's more of an objective measurement.
Speaker 2Right, it's an actual physiological measurement.
Speaker 1What does that tell you?
Speaker 2Well, it allows us to see the blood flow within the penis. Okay, we can look at things like the diameter of the main arteries and how fast the blood is flowing during different phases of an erection.
Speaker 1Wow, so they really took a thorough approach to this study.
Speaker 2They did. They wanted to get a comprehensive understanding of what was happening.
Speaker 1So, after all that, what were the main findings?
Speaker 2Well, here's where things get really interesting.
Speaker 1Okay, I'm all ears.
Speaker 2The study found that both the 50-unit and the 100-unit Botox groups showed significant improvement Across all those subjective measures we just talked about the SHIM score, the erection hardness score, the sexual encounter profile questions and the global assessment score.
Speaker 1So they were doing better in all areas.
Speaker 2Yep, and this was compared to the placebo group, which, as you might expect, didn't really show any significant changes.
Speaker 1Okay, so the Botox was actually making a difference was actually making a difference.
Speaker 2It seemed to be, and the most interesting part is that the biggest improvements were generally seen around the three-month mark, after just that one injection.
Speaker 1Oh wow. So just one shot and they were seeing real improvement.
Speaker 2That's right, and for a group of men who had previously been unresponsive to even the most common and potent ED treatments, that's a pretty significant finding.
Speaker 1It is. It gives them hope.
Speaker 2Absolutely. It opens up the possibility of intimacy again.
Speaker 1And what did that translate to in real life? Were these men able to be intimate again?
Speaker 2Well, the study reported that around 40% of the men in both Botox groups were able to achieve erections that were firm enough for successful sexual intercourse.
Speaker 1Wow, that's huge.
Speaker 2It is especially considering that these men had pretty much given up hope. Right, they had tried everything else and nothing worked 40%.
Speaker 1That's the number that's going to get people's attention.
Speaker 2Definitely.
Speaker 1Now you mentioned that both doses showed improvement. Was there a clear winner between the 50-unit and the 100-unit groups?
Speaker 2So in the first few months after the injection there wasn't a statistically significant difference between the two groups.
Speaker 1Oh, interesting. So it seemed like the lower dose was doing just as well.
Speaker 2Yeah, initially it did.
Speaker 1But we know that with a lot of ED treatments the big question is how long does it last?
Speaker 2That's the million dollar question, right.
Speaker 1Exactly so. What did the study reveal about how long the Botox effects lasted?
Speaker 2Well, they found that even a single injection had a pretty long duration of action. Okay, which is already great news, but here's where things get even more interesting.
Speaker 1Okay.
Speaker 2At the six-month follow-up, the 100-unit group showed statistically significantly better results than the 50-unit group. Oh, okay, so while both doses worked well initially, the higher dose seemed to provide a more sustained effect over time.
Speaker 1OK. So if you're looking for longer lasting results, the higher dose might be the way to go.
Speaker 2That's what the data suggests.
Speaker 1And what about those objective measurements? The penile Doppler ultrasound Did that back up what the men were reporting?
Speaker 2It did. The Doppler ultrasound showed significant improvements in the Botox groups compared to the placebo group.
Speaker 1Okay, so what exactly did they see?
Speaker 2Well, they looked at things like the diameter of the cavernosal arteries and the speed of blood flow during different phases of an erection, and they saw improvements in both of those areas.
Speaker 1So there were actual physiological changes happening.
Speaker 2Exactly, it wasn't just a placebo effect.
Speaker 1Right.
Speaker 2There were real, measurable changes in blood flow.
Speaker 1That's great because it validates what the men were experiencing Right.
Speaker 2It shows that their subjective experiences were actually reflected in their physiology.
Speaker 1Exactly, and you mentioned something earlier about an increase in penile length.
Speaker 2I did. That was a pretty surprising finding it was.
Speaker 1I mean, who would have thought that Botox could potentially make the penis longer?
Speaker 2Right, it's not exactly what you think of when you think of Botox.
Speaker 1What's the explanation for that?
Speaker 2Well, the researchers think that it might be due to the Botox relaxing the smooth muscles within the penis.
Speaker 1Okay.
Speaker 2So when those muscles are more relaxed, it could allow for a slight increase in length. It's interesting. It's still just a hypothesis, but it's an interesting one.
Speaker 1For sure, and I'm sure some men would be quite happy with that side effect.
Speaker 2I'm sure they would.
Speaker 1So we're seeing improvements in erectile function, potential for increased length, but how is Botox, a neurotoxin, actually doing all of this in the penis?
Speaker 2That's the big question, right? How does this all work?
Speaker 1It's not working the same way as the traditional ED medications, is it? No, it's not, so what's going on?
How Botox Works For ED
Speaker 2Well, the researchers have proposed a few different mechanisms of action based on what we already know about how Botox works.
Speaker 1Okay.
Speaker 2So, first of all, botox is known to block the release of certain neurotransmitters, specifically acetylcholine and norepinephrine.
Speaker 1And what do those do?
Speaker 2Well, they're involved in the sympathetic nervous system, which is the part of your nervous system that gets activated when you're stressed or in fight or flight mode.
Speaker 1Okay.
Speaker 2And one of the things the sympathetic nervous system does is constrict blood vessels.
Speaker 1Okay.
Speaker 2Including the blood vessels in the penis. So by blocking the release of these neurotransmitters, Botox may be helping to prevent that constriction.
Speaker 1Okay, so it's basically taking the brakes off.
Speaker 2In a way, yes.
Speaker 1And that allows more blood to flow into the penis.
Speaker 2Exactly.
Speaker 1Okay, that makes sense.
Speaker 2And there's another possible mechanism at play too.
Speaker 1Yeah.
Speaker 2The researchers think that Botox might be increasing the expression of certain substances in the penis. Like what Like vascular endothelial growth factor or vegf? Okay and cd31 okay, I'm not familiar with those well, they're basically substances that promote the growth of new blood vessels oh okay, so it's not just a quick fix.
Speaker 1It could actually be improving the overall vascular health of the penis. That's the idea. That's really interesting. It is, and they also found that the effects ofascular health of the penis.
Speaker 2That's the idea.
Speaker 1That's really interesting.
Speaker 2It is, and they also found that the effects of Botox in the study seem to be working through pathways that are different from nitric oxide.
Speaker 1Okay, and why is that significant?
Speaker 2Well, nitric oxide is the main signaling molecule that's targeted by drugs like Viagra and Cialis. So this suggests that Botox might be effective for men who haven't responded to those traditional medications, because it's working in a totally different way.
Speaker 1Oh I see it's not just another Viagra knockoff.
Speaker 2Not at all. It's a completely different approach.
Speaker 1So it's offering a whole new route to improving erectile function. That's right, that's really exciting.
Speaker 2It is, and there's one more mechanism I want to mention.
Speaker 1Okay.
Speaker 2The researchers also think that Botox might be decreasing the tone of the small resistance vessels in the penis.
Speaker 1Okay.
Speaker 2Which would lead to an increase in resting blood flow.
Speaker 1Okay.
Speaker 2And it might also be reducing the muscle tone in the erectile tissue itself.
Speaker 1So it's basically making everything more relaxed.
Speaker 2Exactly, and that relaxation allows for better blood flow and engorgement.
Speaker 1Okay, so it seems like Botox is working on multiple levels to improve erectile function.
Speaker 2It does.
Speaker 1That's pretty amazing.
Safety Profile and Future Research
Speaker 2It is, but, of course, with any medical treatment, especially one that's as new and different as this, we have to talk about safety.
Speaker 1Absolutely, that's a top priority.
Speaker 2So what did the study find?
Speaker 1Yeah, were there any side effects?
Speaker 2Well, the good news is that the study reported a very low rate of adverse events in the Botox groups only about 4%. Oh, that's good. And the side effects that were reported were mostly mild and localized to the injection site.
Speaker 1Like what.
Speaker 2Things like temporary penile pain or minor bruising.
Speaker 1Okay, so nothing too serious, Right.
Speaker 2And there were a few cases of prolonged erection. That happened during the Doppler ultrasound exam.
Speaker 1Oh.
Speaker 2But those resolved on their own.
Speaker 1That's good.
Speaker 2And it's worth noting that there was one case of priapism.
Speaker 1Which is.
Speaker 2It's a prolonged and potentially painful erection.
Speaker 1Oh, okay.
Speaker 2And this happened in the 100-unit group at the six-month follow-up.
Speaker 1Okay.
Speaker 2But it was successfully treated with medication. Okay so overall, a pretty good safety profile. Yes, especially when you compare it to some of the other injection therapies for ED.
Speaker 1Okay, how so?
Speaker 2Well, traditional intracavernous injection therapy can sometimes have higher rates of side effects.
Speaker 1Like.
Speaker 2Pain at the injection site.
Speaker 1Okay.
Speaker 2Or the development of plaque or scar tissue within the penis, oh wow, and even penile curvature.
Speaker 1So Botox seems to have a better safety profile.
Speaker 2In some ways, yes. And another interesting thing about this study they actually observed an increase in penile length in some of the participants and no new cases of penile curvature were reported.
Speaker 1So that's a plus.
Speaker 2It is and, importantly, the study also reported no systemic side effects from the Botox injections.
Speaker 1Okay, so it seems like a pretty safe and well-tolerated treatment.
Speaker 2It does.
Speaker 1But of course, as with any research, there are always limitations, right. So what did the researchers themselves point out as areas where further investigation is needed?
Speaker 2Well, one thing they acknowledged was that their study focused specifically on men who had vasculogenic ED and hadn't responded to other treatments.
Speaker 1Okay.
Speaker 2So they said that future research should definitely include men with ED due to other causes.
Speaker 1Right, because ED can have a lot of different underlying causes.
Speaker 2Exactly. It's not a one size fits all situation.
Speaker 1So they need to see if this works for other types of ED as well.
Speaker 2Exactly, and they also said that we need longer follow up periods to really understand how long the benefits of Botox last.
Speaker 1Yeah, Because this study only followed the men for six months.
Speaker 2That's right.
Speaker 1So we need to know if it lasts for a year or two years or even longer.
Speaker 2Absolutely.
Speaker 1So this study is a really exciting first step.
Speaker 2It is. It opens up a whole new avenue of treatment for ED.
Speaker 1But we need more research to confirm these findings and to see how it works for different types of ED? Definitely. So what would you say is the main takeaway from our deep dive into this Botox research?
Speaker 2I'd say the main takeaway is that this was a well-designed study, yeah, and it provided some pretty compelling evidence that Botox could be a safe, effective and relatively long-lasting treatment option for men with vasculogenic ED who haven't had success with other therapies.
Speaker 1It's a potential game changer.
Speaker 2It could be.
Speaker 1It's definitely a surprising and innovative approach.
Speaker 2It is. Using Botox to treat ED is not something you hear every day.
