The Surprising Liver Hormone Connection
SPEAKER_00does a sluggish, fatty liver have to do with your reproductive hormones?

SPEAKER_00So we are pulling from two major sources to understand this today. First, we have a cutting-edge 2026 medical review from the International Journal of Molecular Sciences. And they map out this newly defined biological network that scientists are calling the liver testis axis.
SPEAKER_01Which is a huge paradigm shift in how we view the body.
SPEAKER_00Definitely. And second, we're diving into a really fascinating clinical study on using ozone therapy to treat fatty liver disease.
SPEAKER_01Yeah, that study is incredibly promising.
SPEAKER_00So our mission in this deep dive is to untangle this hidden two-way conversation happening right now between your liver and your hormones. SPEAKER_01It really is. So for our conversation, we'll just stick to calling it fatty liver disease. Much easier.
SPEAKER_00Yeah. Let's keep it simple for everyone.
SPEAKER_01Aaron Powell And then on the other side, we have male hypogonadism, which is just the clinical term for chronically low testosterone.
SPEAKER_00Aaron Powell And if you suffered from both of those issues, say a decade ago, you were sent to completely different floors of the hospital.
SPEAKER_01Oh, without a doubt.
SPEAKER_00You saw a hepatologist for your liver, and then maybe weeks later you'd go see a urologist or an endocrinologist for your low testosterone.
SPEAKER_01Aaron Powell Right. And neither specialist was likely looking at the other specialist's notes.
SPEAKER_00Aaron Powell Exactly. But our sources reveal this massive shift. These aren't just two unrelated problems that happen to coexist, you know, just because of age or a bad diet.

SPEAKER_00Aaron Powell So to understand how they talk to each other, we really have to rethink the liver entirely. Because I think most of us just picture the liver as like a giant Britta filter.
SPEAKER_01Right, just a sponge that sits there cleaning the blood.
SPEAKER_00Yeah, exactly. And if it gets fatty, it just gets clogged and slows down. But it's not just sitting there passively failing.
SPEAKER_01Far from it. What's fascinating here is that the liver is actually a highly active endocrine organ.
SPEAKER_00Meaning it makes hormones.
SPEAKER_01Yes. It manufactures and releases its own hormones and signaling proteins that give commands to the rest of the body.
SPEAKER_00Wow. Okay.
SPEAKER_01So when your liver gets bogged down by excess fat and metabolic stress, that normal communication system doesn't just shut off, it morphs.
SPEAKER_00It changes completely.
SPEAKER_01Right. The liver transforms from this stabilizing, detoxifying organ into a primary source of toxic signals.
The Liver As A Hormone Organ
SPEAKER_00And it actually dials back the production of something incredibly important, a protein called sex hormone binding globulin or SHBG.
SPEAKER_01Yeah, SHBG is it's basically the transport taxi for your hormones.
SPEAKER_00Taxis. I like that.
SPEAKER_01Because testosterone cannot just float freely through your bloodstream in large quantities. It is a fat-soluble molecule, and your bloodstream is mostly water.
SPEAKER_00Oh, like oil and water. They don't mix.
SPEAKER_01Exactly. So it needs to get inside one of these SHBG taxis to travel safely to your muscles, your brain, and your tissues.
SPEAKER_00So if the fatty liver stops manufacturing those taxes, you could theoretically have completely normal testosterone production, but the hormones are just stranded.
SPEAKER_01Aaron Powell They're just stuck. They can't get to where they need to go to give you energy or build muscle.
SPEAKER_00Wow. But that's just the passive damage, right?
SPEAKER_01Yeah. That just starves the body of the hormone by stripping away the transport infrastructure. But the active damage is much, much more aggressive.
SPEAKER_00Aaron Powell Tell me about the active damage.
SPEAKER_01Aaron Powell Well, a stressed, fatty liver begins to rapidly manufacture and release inflammatory molecules directly into your systemic circulation.
SPEAKER_00Aaron Powell Like it's actively pumping out poison.
SPEAKER_01Pretty much. We are talking about inflammatory cytokines and a specific stress hormone called FGF-21.
SPEAKER_00Okay, here's where it gets really interesting. Let's picture this functionally. Imagine the liver is a toxic manager at a massive corporation.
SPEAKER_01Aaron Powell I like this analogy. Trevor Burrus, Jr.
SPEAKER_00Right. So this manager is stressed out, overworked, and surrounded by just unmanageable piles of fat. Instead of just doing the job poorly, this manager starts firing off aggressive company-wide alarm emails. Aaron Powell Right.
SPEAKER_01And those toxic emails are the cytokines and the stress hormone.
SPEAKER_00Aaron Powell Exactly. And when those toxic emails reach the reproductive system, specifically the testes, they don't just bounce off, they are opened and read.
SPEAKER_01Aaron Ross Powell And the instructions in those inflammatory emails are just devastating. They travel directly to the LADIC cells.
SPEAKER_00Trevor Burrus And those are the what? The factories?
SPEAKER_01Yeah. There are the specific cellular factories inside the testes responsible for manufacturing testosterone. When the inflammatory cytokines hit the receptors on those LADIC cells, they literally block the genetic transcription needed to make the worm.
SPEAKER_00Wow, they shut the factory down at the genetic level. That's insane.
SPEAKER_01It is. In laboratory studies, this is a dose-dependent response. Expose those factory cells to a little bit of liver inflammation and testosterone production dips. And if you expose them to a lot, expose them to a high concentration of those inflammatory cytokines, and production completely plummets.
SPEAKER_00So if you are listening to this and you've been dealing with unexplained fatigue, maybe muscle loss or a non-existent libido, and your doctor is only looking at your hormone panels, they might be staring at the wrong organ.
SPEAKER_01Absolutely. Your reproductive system might be perfectly capable of working.
SPEAKER_00But your liver is pulling the fire alarm and forcing it to shut down.
SPEAKER_01Exactly. Which brings up a deeply logical, albeit grim, counterreaction from the body.
SPEAKER_00Right. Because if the liver acts like this toxic manager and forces a shutdown of testosterone production, we have to look at what that sudden drop in testosterone does to the liver itself.
SPEAKER_01Aaron Powell Because the body is a closed loop.
SPEAKER_00Wait, wait, I'm stuck on a contradiction
Inflammation That Shuts Down Testosterone
SPEAKER_00here. If the liver shuts down testosterone production, but it needs testosterone for its own health, then the liver is essentially causing its own destruction.
SPEAKER_01It sounds totally backwards, yeah.
SPEAKER_00Why would our biology evolve a self-destruct button like that? I mean, why would the body allow a system to attack the very hormone it needs to survive?
SPEAKER_01It seems entirely counterintuitive until you look at it through the lens of evolutionary survival.
SPEAKER_00Okay, walk me through that.
SPEAKER_01In the short term, the body views heavy inflammation as an acute crisis.
SPEAKER_00Right.
SPEAKER_01Like a massive infection or starvation.
SPEAKER_00Something that's gonna kill you right now.
SPEAKER_01Aaron Powell Right. And in a severe crisis, the body's triage system shuts down reproduction because making babies is a terrible idea when you are starving or, you know, fighting off a tiger. Trevor Burrus, Jr.
SPEAKER_00That makes total sense. You divert all energy to immediate survival.
SPEAKER_01Aaron Powell But a fatty liver isn't an acute crisis, it's chronic.
SPEAKER_00Ah and that is where the evolutionary mechanism completely backfires.
SPEAKER_01Aaron Powell Exactly. The body is stuck in a permanent state of perceived crisis. It shuts down testosterone to save energy, not realizing that by keeping testosterone suppressed long term, it is removing the liver's primary metabolic shield.
SPEAKER_00Okay, let's look at how that shield normally works. Under healthy conditions, testosterone actively promotes fatty acid oxidation in the liver.
SPEAKER_01Right. It helps the liver burn fat for fuel.
SPEAKER_00And it also blocks a specific genetic switch in your liver called SREBP1C.
SPEAKER_01Which is a very important switch.
SPEAKER_00Yeah, because when this switch is flipped on, it commands your liver to take the excess carbohydrates you eat and convert them directly into new fat tissue.
SPEAKER_01And testosterone normally keeps a heavy lid on that switch.
SPEAKER_00So as the liver's toxic emails shut down the testosterone factories, the liver loses its own shield. Without testosterone keeping that switch turned off, the liver just starts packing on even more fat.
SPEAKER_01And that unchecked fat creation doesn't just stay in the liver either.
SPEAKER_00Where does it go?
SPEAKER_01It spills over, leading to systemic weight gain, particularly visceral fat. You know, that hard, deep belly fat that wraps around your organs.
SPEAKER_00Well, that's the worst kind.
SPEAKER_01It really is. And inside that specific type of belly fat hides an enzyme called aromatase.
SPEAKER_00Oh, I've heard of this. And aromatase actively converts what little testosterone you have left into estrogen, right?
SPEAKER_01Yep. It acts like a biochemical vacuum. It pulls the remaining androgens out of your system and turns them into estrogen, creating a severe hormonal imbalance.
SPEAKER_00Wow.
SPEAKER_01And that excess estrogen combined with rock bottom testosterone commands the liver to store even more fat.
SPEAKER_00So let me get this straight. The liver attacks the testes, the testes stop making testosterone, the liver loses its shield and gets fatter. Yep. The resulting belly fat turns the tiny bit of remaining testosterone into estrogen, which makes the liver even fatter, which causes the liver to produce even more inflammation, attacking the testes even harder.
SPEAKER_01Aaron Powell That's it. It is a self-perpetuating downward spiral. The organs are basically locked in a mutual destruction loop.
SPEAKER_00Wow. So if insulin resistance and toxic fats have fundamentally locked the system into a death spiral, diet and exercise alone often aren't enough to break the circuit, are they?
SPEAKER_01Usually not, no. The system is actively fighting your attempts to heal.
SPEAKER_00But to figure out how to stop it, we have to look at what pulled the trigger on this cycle in the first place.
SPEAKER_01Right. Where the fire started.
SPEAKER_00Exactly. And the 2026 medical review points to two underlying saboteurs that strike both organs simultaneously.
SPEAKER_01Yeah. To understand the origin of the fire, you have to look at insulin resistance and lipotoxicity.
SPEAKER_00Let's start with insulin resistance because the review says it is the pathophysiological bridge connecting the liver and the testes.
SPEAKER_01Aaron Powell Right. And you know, we hear about insulin resistance constantly, usually just as a precursor to type 2 diabetes.
SPEAKER_00Yeah,
The Downward Spiral Of Fat And Estrogen
SPEAKER_00you hear it all the time. But its effects are tissue specific. In the liver, insulin is supposed to tell the cells to pull sugar out of the blood and burn it for energy.
SPEAKER_01Aaron Powell Right. And when the liver cells become resistant to that message, they just refuse to burn the sugar.
SPEAKER_00Aaron Powell So the body panics, produces even more insulin, and that tidal wave of insulin forces the liver to convert all that unused sugar into fat.
SPEAKER_01Aaron Powell Exactly. But insulin resistance in the testes does something completely different.
SPEAKER_00Okay, let's unpack this. What happens down there?
SPEAKER_01Down there, high levels of insulin actively break the chemical signaling chain that tells the LADIG cells to produce testosterone.
SPEAKER_00Like cutting a telephone wire.
SPEAKER_01Yeah, exactly. The brain sends the signal, but the insulin resistance creates static on the line. The factory just never gets the work order.
SPEAKER_00So a state of chronic high insulin is hammering both organs simultaneously, just using different mechanisms.
SPEAKER_01Yes. And then we have lipotoxicity, which honestly sounds like a hazard warning on a chemical plant.
SPEAKER_00Right. It really does. But it is a super accurate description of what is happening at the cellular level.
SPEAKER_01Definitely. Lipotoxicity occurs when toxic lipid byproducts, specifically a type of fat molecule called a ceramide, begin accumulating inside your nonfat tissues.
SPEAKER_00Like your liver cells and your reproductive cells, where they absolutely do not belong.
SPEAKER_01Right.
SPEAKER_00So think of ceramides not just as a generic sludge, but as microscopic insulating foam.
SPEAKER_01Oh, that's a great visual.
SPEAKER_00Yeah. If you spray expanding foam into the intricate gears of a machine, everything just grinds to a halt. When these ceramides build up inside your tests, they essentially foam up and block the doors to your mitochondria.
SPEAKER_01The mitochondria being the energy grids of the cells.
SPEAKER_00Right. And by blocking those doors, the ceramides completely cut off the delivery of cholesterol.
SPEAKER_01Which is huge.
SPEAKER_00It is. Because we always hear about cholesterol as a bad thing, but it is the raw material, the fundamental building block that your cells absolutely must have to manufacture testosterone.
SPEAKER_01No cholesterol entering the mitochondria means no testosterone production, period. The foam just ruins the machinery.
SPEAKER_00It's incredible. And in the liver.
SPEAKER_01In the liver, that same toxic foam causes liver cells to literally undergo apoptosis.
SPEAKER_00Which is programmed cell death, right?
SPEAKER_01Yeah, they literally self-destruct. Which triggers the immune system to swoop in, creating even more of that systemic inflammation we talked about earlier.
SPEAKER_00Aaron Powell Man, we've mapped out a highly depressing, deeply entrenched biological trap here.
SPEAKER_01It is pretty intense.
SPEAKER_00The liver testis axis is locked, fueled by insulin resistance and this microscopic toxic foam. If we just stop the deep dive right here, it would be a pretty bleak day for you listening.
SPEAKER_01Yeah, don't worry, we won't leave you hanging. Right.
SPEAKER_00Because we need a biological wedge to interrupt that inflammation and force the system to reset. Which brings us to the clinical study on ozone therapy.
SPEAKER_01Because when a biological loop is this reinforced, you often need a physiological intervention that fundamentally alters the biochemical environment.
SPEAKER_00The body needs an outside shock to break the static.
SPEAKER_01If we connect this to the bigger picture, the intervention in this study is a protocol called ozone autologous blood reinfusion treatment.
SPEAKER_00Okay. Now, for the ordinary person listening, the idea of drawing your blood out, mixing it with ozone gas, and putting it back in sounds intense, almost like science fiction.
SPEAKER_01It does sound out there for sure.
SPEAKER_00So what is actually happening to the blood cells when you do this? How does ozone physically change the blood?
SPEAKER_01Aaron Powell Well, the mechanism is actually a deeply logical application of what we call hormesis.
SPEAKER_00Aaron Powell And hormesis is the idea that a small controlled stress triggers a massive healing response, right? Like exercise. Trevor Burrus, Jr.
SPEAKER_01Exactly like exercise. Ozone is a highly reactive molecule. It's O3, meaning it has three oxygen atoms instead of the usual two.
SPEAKER_00Okay.
SPEAKER_01When you expose a small amount of the patient's drawn blood to this ozone gas in a sterile closed loop, the ozone reacts instantly with the lipids in the blood cell membranes.
SPEAKER_00Aaron Powell Wait, so the ozone doesn't actually go back into the patient's?
SPEAKER_01No, not at all. The ozone gas is gone in seconds. What it leaves behind are chemical messengers called lipid peroxides. Oh, I see. Exposing the blood to ozone is basically like running a massive
Insulin Resistance And Toxic Ceramides
SPEAKER_01fire drill for those cells. It induces a tiny, very calculated amount of oxidative stress.
SPEAKER_00It panics the cells just enough to wake them up.
SPEAKER_01Exactly. The blood cells respond to this mild stress by dramatically upregulating their own antioxidant and anti-inflammatory defenses. They start churning out healing enzymes.
SPEAKER_00And then what happens?
SPEAKER_01When that supercharged, defensively primed blood is reinfused back into the patient, those enzymes circulate straight into the liver and begin aggressively cooling down the inflammation.
SPEAKER_00Aaron Powell Let's look in how the researchers actually measured this effect. They took 36 hospitalized patients suffering from severe fatty liver disease. Right. Half of them received the standard care, you know, traditional liver protective drugs, but the other half received those standard drugs plus this ozone blood treatment.
SPEAKER_01Aaron Powell And they ran extensive blood panels before and after to track the specific metabolic markers driving the liver testis axis.
SPEAKER_00One of the biggest findings was the effect on leptin.
SPEAKER_01Oh, leptin is a massive player in this web. It is a hormone produced by your fat cells. And its primary job is to tell your brain hey, we have enough energy stored, you can stop eating, and you can start burning fat.
SPEAKER_00But when you have a fatty liver and high inflammation, you develop leptin resistance. The signal just gets lost.
SPEAKER_01Yeah, your brain thinks you are starving even when you have excess fat stored in your liver and your belly.
SPEAKER_00That's crazy.
SPEAKER_01And because the brain thinks you are starving, it locks away the fat stores and completely refuses to burn them.
SPEAKER_00But the clinical study found that the ozone treatment successfully reduced serum leptin levels, for AIDS.
SPEAKER_01Yes. It's significantly improved that leptin resistance compared to the control group.
SPEAKER_00Wow. So by clearing the static on the leptin line, the ozone therapy effectively resets the body's fat-burning thermostat.
SPEAKER_01Exactly.
SPEAKER_00The brain finally hears the message that it has plenty of energy and it allows the body to start mobilizing and clearing out that stored liver fat.
SPEAKER_01And it went even further than just leptin. The researchers also measured an inflammatory marker called prostaglandin F2A.
SPEAKER_00And what happened with that?
SPEAKER_01In the patients receiving the ozone therapy, levels of this inflammatory marker dropped significantly. The research has concluded that ozone therapy has a legitimate, measurable effect on cooling the systemic inflammation that drives fatty liver disease.
SPEAKER_00This is amazing. If we tie this back to our toxic manager analogy, the ozone therapy is essentially walking into the corporate office, calming the manager down, and cutting the internet line so they stop sending out those toxic company-wide alarm emails.

SPEAKER_01If the liver stops flooding the bloodstream with inflammatory cytokines and stress hormones, the Ladig cells in the testes can finally breathe.
SPEAKER_00The genetic block is lifted.
SPEAKER_01Yes. They come back online and start manufacturing testosterone again.
SPEAKER_00And because it is a two-way street, once that newly produced testosterone hits the bloodstream, it acts as the metabolic shield the liver has been missing.
Ozone Therapy And The Hormesis Mechanism
SPEAKER_01Aaron Powell Exactly. It blocks that genetic switch, preventing the liver from making new fat, and it helps the liver burn off the remaining fat.
SPEAKER_00Aaron Powell You reverse the momentum entirely. You turn a downward self-destructive loop into an upward healing loop. Intervening at the liver rescues the testes, and the restored function of the testes helps permanently heal the liver.

SPEAKER_01Yeah, it might not be the engine at all.
SPEAKER_00We began this deep dive looking at a broken system through the old medical lens. You know, the comforting illusion that a problem in one location means the broken part is sitting right there in plain sight.
SPEAKER_01But the human body is not a collection of isolated organs waiting to be treated independently by different specialists.
