Hey everybody, welcome back to another episode of the Traveling Therapist Podcast. We have a great guest today. He's not a psychotherapist, but he's in our field or was in our field. We're going to get into that a little bit. But today we're talking about a topic that's on the mind of so many therapists and practice owners: how to build a business that gives you more freedom, not less. And that's what we talk about in this podcast all of the time. So we could be free in our traveling therapist lives. Our guest, Dr. Jamie Schreyer, is a former physical therapy practice owner who built and sold a million-dollar business. I can't wait to hear about that. Now, as the founder of Practice Freedom U, he coaches other practice owners, including therapists, on how to work less, earn more, and live their passion. He's the best-selling author of the Practice Freedom Method and is here to share his playbook for breaking free from the daily grind and building a practice that truly serves your life. Davey, welcome.
SPEAKER_01Oh, Dave. So wonderful to be here.
SPEAKER_00I'm so excited to dive into this topic. A lot of the people that listen to this podcast are entrepreneurial therapists. We already are outside of the box thinkers because we travel, we take our portable practices with us, but this always comes up. Like I'm stuck in the one-to-one model. I'm getting burned out. How am I going to enjoy these places I'm traveling to if I'm just like having to see a million clients every week? So you are a perfect guest for this podcast. I'm really glad to have you here. Thanks for being a guest. Let's just dig into it. You built and sold a million-dollar physical therapy practice. What was the moment you realized that you had built a business that was running you instead of you running it?
SPEAKER_01Oh, that's a great story. When I had my practice, which I had it for 15 and a half years, I can't tell you I ever took a real vacation prior to building it the way I'll share in a minute, in the first four or five years. Like real vacation meaning I'm away, I'm unplugged, I'm not jumping on my emails, I'm not responding to stuff or anything like that. And as we know what happens when we start building and growing our business, the complexity and problems grow with it. So I finally was like, okay, my wife, Colleen, I said, let's let we got to go away for at least a couple days. I'm going crazy. So we go away, we stick family in the back, have a great time, just a weekend, right? Like taking off a half a day Friday was like a big deal.
SPEAKER_00Wow. Yeah.
SPEAKER_01Coming back on Sunday. It's a beautiful fall day. The phone rings. I'm driving, smiling, everyone's smiling. My wife's smile just turns blank.
SPEAKER_00Oh no.
SPEAKER_01And I'm like, what? And she goes, It's my dad. I go, is everything okay? He goes, he just saw the TV. Your building's on fire.
SPEAKER_00And I was like, I just got goosebumps. Oh my God.
SPEAKER_01Kim, without missing a B, I said, Good. I hope it burns to the F and ground.
SPEAKER_00How many times have I said, I wish my business would just burn down to the ground?
SPEAKER_01And it's amazing because when I share that story with people, everybody has what they call the fire story.
SPEAKER_00Yeah.
SPEAKER_01So literally we pull up to the building, and sure enough, they're throwing all my equipment. I had a small PT practice, throwing everything out the window. And here's the interesting part. The reason I said those words is because at the time the business ran me. Yes, I had a staff. Yes, I was actually making decent money comparatively, you know, speaking, working, but I was working hours and hours. I was working days and days. And I just kept being told, that's the way it is, Jamie. You own a business. You gotta do that. I go, I know, but I've been doing this now four years. Five years. I go, I can't see where it's going to magically get better. I started having like anxiety, right? I started, you know, taking anxiety medicine to my wife was gonna kill me. So she said, You better get on something because you are freaking horrible. So this business that I love, this profession that I love, was actually killing me. And it was, you know, you told me like when was the moment? It was that moment that I went, there's one or two things gonna happen here. Number one, I think maybe I'm done with this profession because I had friends that were making lots of money in other professions and they didn't go to the level of schooling I had, they didn't invest the amount of money I did, and they were making a fortune. So my victimness, life isn't fair, came out of me. And I said, maybe I'll do that. And the other thing I thought, I go, Jamie, you have no idea how to run a business. You weren't taught any of this stuff, and you just have a big ego because you're really good at treating people, but you don't know how to build a business of treating and helping people. And I thought about and I said, I'm not gonna give up. And that was the moment I started to invest, not just invest in clinical. I already did that, but invest in my understanding of business. I hired a coach, I uh joined masterminds, I read books more, and I started to apply things in my business. Most of the stuff didn't work. I was really bad at it. But then some things started to work, and then fast forward, I started to create a business that had a great team, good people, good processes in place. I started to understand how to communicate better, how to show up, not being an asshole, but show up being a supported boss. And my business went to about 1.4, 1.5 million, and I was literally in the business, because it was brick and mortar, maybe five hours a week.
SPEAKER_00Oh my gosh, the dream.
SPEAKER_01Five hours a week, making 250,000, 300,000 a year.
SPEAKER_00Amazing.
SPEAKER_01So about 20, 25% profit, taking insurance wasn't even a cash flow.
SPEAKER_00Wow, yeah.
SPEAKER_01And to fast forward where we are today, I just shared with people what I did because I felt the message needed to be heard, because I wanted to inspire people. And unbeknownst to me, people reached out and said, Can you help me? And that's how I got into this world of coaching, executive growth coaching, or whatever you want to call it, because it always seems to be a different name, but really helping people create their version of practice freedom that gives them the money and the time and the purpose-filled business that creates the life that they want. Because to me, Kim, that's what this is about. If you're not happy, what is it about that? So that's what I've been on a mission now for 13 years. Now, as a CEO of practice freedom, you.
SPEAKER_00I love it. Oh my gosh. So back to the building that burned down. What happened? Did you just leave it down or did you rebuild and then later sell the business? I was you tried.
SPEAKER_01I I tried everything that I could.
SPEAKER_00First of all, I learned about divine interventions.
SPEAKER_01Yeah. Somebody up there pressed the reset button, like the old Staples reset button. I pushed it as long as I can because I didn't want to go back. Eventually I realized I was tied into my lease. There was no way out of it. And oh, I also realized that my cousin, who I love dearly, was my insurance agent. I had really underinsured. And I go, Oh, dude, I don't have any money. It was like, I don't know, 50,000 or something. And he goes, Well, who thought your place was gonna burn down? I tried to keep your rates as low as possible.
SPEAKER_00Oh my gosh, really, thanks.
SPEAKER_01I started to learn a lot about things.
SPEAKER_00Wow, that's amazing. Okay, so the cornerstone of everything you do right now is freedom. What does freedom mean to you? And how do you help? You know, I know you work with a lot of mental health clinicians and private practices, group practices, all of that. How do you help like take your vision of what freedom is and then help us figure that part out? And then let's get into after that, like, how do we design a life around it, really?
SPEAKER_01Yeah. When I do talks and workshops and stuff, I have everyone define freedom because it's defined differently for everyone. For me, when someone asked me, what is it that you want? At the time, it was ideally, I don't want to show up to work on Monday. That was freedom. Because the amount of anxiety, cold sweats that I had at two in the morning on Sunday night, every Sunday night, not knowing what the heck was going to happen on Monday, I was like, if I just didn't have to show up on Monday, that would be freedom. Since then, freedom to me is I want to do what I want to do. I want to do it who I want to do it with. I want to do it when I want to do it, and I want to have the money to afford that. That to me is freedom. And I feel like, not that I'm not continuing to evolve and grow because I am, but I feel like I have designed that type of life with the current business that I have, and help design that type of life with other private practice owners.
SPEAKER_00Yeah. Oh, I love it so much. Yeah, we we talk about that a lot on this podcast. And I always like to kind of update my listeners like where I am right now because we move around so much. But literally, what you're talking about as you're saying that, like the schedule and the life I have now is one I only dreamed about, maybe five years ago. You know, we're living oceanfront condo right now, overlooking the gorgeous ocean. Every other week I take off. So I had work three days on and then I'm off for like a week and a half. It's a dream life, you know, but you've got to, you know, and maybe we can get into that. Like, how do you get to that? You know, how did you how do you teach people to get to that? Because for me, it's like I had this vision. I think back now, it's like I never thought I would be able to do this. And it's just kind of evolved into this now, this lifestyle vision and planning like my business around how I want to live, you know. So I love that you're teaching that. Where does somebody start? We define our free will.
SPEAKER_01You def you define you defined it first.
SPEAKER_00Okay.
SPEAKER_01If people don't have a vision, if people don't have a bigger future, if you don't have a bigger future, then most likely you have a bigger past. And if you have a bigger past, what that means is do you have anybody that you know in maybe currently or in your past you grew up with that just loves talking about the good old high school days? I had friends that when I see them, they'll bring up high school all the time to me. Oh, yeah. And he even admitted, he goes, Jamie, the best years are behind me. I'm just raising my kids doing the best I can. He's in a job that he's been in for 30 years. Like, it's just the way it is. And for people like you and me and your listeners, we first have to have a bigger future, not a bigger past. And we have to have a future that inspires us, that gives us tingles. Because if not, then why are we gonna do this work? Because it's not gonna be easy. So it starts with having that vision. We call it the North Star. Having that North Star, what is it that where is it? I want to go. What does that look like if anything was possible? What would it mean to have that type of life? So you start by putting that picture together. Then we got to come down to so what is it gonna take right now to start moving towards that? Because having a vision, but not actually taking action, that's just hope. Hope is nice, but maybe maybe at church. Hope is not nice as a business strategy, as we've heard many times before. So having that picture of where we want to go and then looking at where we are now, the first thing I ask, like when I'm when I'm even speaking with a potential client, the first thing I want to look at is does your current business model, what you're currently doing, assuming they're already in some type of business, they're not thinking about it or anything like that, they're doing it. Can this model, the way you're doing it, create that? What I found so many times is people will share with me their business model, like their treatment model, how many visits they see or how many, how much they get paid, whether it's cash or insurance or whatever. And I asked them, I go, Well, how did you figure all of this out? And the answers are just, I don't know. I just kind of did what I thought we should do. And a lot of it's so emotionally triggered of, but I felt bad. I want to do quality, I want to do one an hour. Well, one an hour at $70 an hour with what it costs nowadays to deliver care is not a business model that's going to afford any of this. So then that decision then becomes, are you open to modify your business model? But assuming you have a business that is, you know, financially in a good place, maybe it's not at the time, but it's because of other reasons, such as utilization or staffing or whatever the case is, then it's about putting together a plan. And I don't, I stopped buying into business plans a long time ago. I never did a business plan to start any one of my businesses. What I do is what's called 90-day sprints. I do 90-day sprints that are in line with where I want to be in the next 12 to 18 months. That's about as far as I look out. Why? Because the world changes fast. And if we put the three-year plan, can you imagine a three-year plan three years ago?
SPEAKER_00Not with AI, not with the way it's moving.
SPEAKER_01Now that AI is here, six months is the new three-year plan.
SPEAKER_00Oh my gosh. Of how fast.
SPEAKER_01So that's where we begin. We begin with that, and then we get clear on what is the next step to move forward. And of course, as perfectionists and as you know, high achievers, we want to have it all, we want to have it now, but we need to teach ourselves this discipline of progress. We got to make progress. So, what specifically do we need to do structurally in the business? You know, process, systems, hiring, KPIs, all the stuff that we all know about. And what do we have to do personally? Here's the question that I know you have done, whether it was consciously or not. Who do I have to become to accept that I'm the traveling therapist living a dream? But it's not a dream. Tim, what you're living is a reality for you.
SPEAKER_00Right.
SPEAKER_01For someone else, it might be a dream. And the difference between a dream and the reality is the little tiny efforts we make to then start believing this is real. That's how we kind of take this thing that you want and turn it in something that is doable. And that starts to happen.
SPEAKER_00That's amazing. I mean, it makes so much sense when you put it that way because I mean, it is essentially what I had to do to be able to take off every other week. I needed a team, you know. I didn't know how I was gonna get a team slowly over time because I otherwise I would be doing all of that all day long, every single day. So slowly I built a team.
SPEAKER_01And Kim, that's a mental shift too. So I want to, I wanna, I always like to bring it to the audience that when you find someone that has, quote, done it, a lot of times the people are unconsciously, they're unconsciously are unconscious of how they actually did it. They think what they did is all, I did this step, and then I used this strategy, then I bought this software. No, all of that came after. What came first is how you thought about it. You just said, in order for me to have this type of life, the business cannot be totally dependent on me. So that shifted you to be like, so that means I have to find good people. But in order to find good people, I gotta make sure I'm clear on who am I gonna hire, what's your job gonna be, what are their outcomes, how am I gonna be the person that communicates and trains them and supports them and do it in a way that ultimately allows me to have a week or two off every other week. Like all of those thoughts came into play, which then shifted how you went to solve the problem. That right there is the biggest difference. The problem, and this is one of my mentors' favorite line, Kim. The problem, and I'll fill in the blanks, the problem as we see it is never the problem. The problem is how we think about the problem. Wow, that's the problem. When you shift how you think about the problem, which you did, which I did, and so many other people out there, you start shifting about the solutions. When you don't think about problems the right way, which is why I believe coaching is so important, that's why I have a coach because I can't think other than how I think. But when you start shifting how you see something, it turns you to different solutions. And those solutions start to shift your direction. So this isn't an if, this isn't some pie in the sky. I've taken this idea that I thought it couldn't happen for me, and I've welded it down to something actually concrete and actionable. But at the same time, you also have to elevate who you are as a leader. Those two things have to happen: structure of the business and leadership of growth. You've had both. It would be impossible for you to be with you where you are if you didn't.
SPEAKER_00I totally agree with you, although I would never put it that way, but you're absolutely right. And that has been a journey, even trying to figure out how to be a leader versus you know, a private, solo private practice owner, you know, that I did for so long. It's been such a journey to like like I don't know how to manage people, I don't know what SOPs are, you know.
SPEAKER_01Like, oh what a great journey. So you and I are excited here. There's people looking at me just be like, why the hell are they so excited about? Yeah, because it's a journey. Yeah, I don't have it all figured out. I feel like at age 56, yeah, I'm just figuring out some stuff. Like I've been doing this 30 years in business. I'm more excited now than I can ever remember myself. But that right there happens when you start getting out from underneath the heaviness and the dredge and the drudge that you've surrounded yourself in. That's a shift in how you show up, and it's a shift how your business is structured. But it's possible for sure.
SPEAKER_00So let me ask you something you write about, and I saw that you you talk about sometimes, is the growth trap. How is this connected in to private practice owners and the growth trap with what we're talking about, scaling, getting out of that one-to-one that kind of thing?
SPEAKER_01The biggest growth trap happens what we affectionately call the overwhelmed operator phase. Typically, you start out your business as a solopreneur.
SPEAKER_00Right.
SPEAKER_01Now, if that's what you want to do only, that's fine. You're gonna, you know, maybe have some support, depending on what it is, but that's gonna be the business. Can you can you make decent money doing it? Sure. But ultimately you're gonna trade time for money, which is fine. As you start to grow, assuming you want to grow, as your business grows, as revenue grows, well, you have to get people to help you and do that. So now you have people coming. Well, people don't show up with batteries already included, they don't think like you, they don't come early and stay late, like they show up however they show up.
SPEAKER_00True. It's like you're not gonna work 18 hours a day like I do. I don't get it. Oh yeah.
SPEAKER_01So now they show up. You show up with your job that you had when you were we call a committed clinician, solopreneur, but now you're adding more stuff on your plate because you now have to manage other people. And you don't like managing other people. So your stress goes up, your number of hours you're working goes up, your income may increase a little bit, may not, but what happens is you start getting into this trap because the business now becomes more and more dependent on you. You see, the more you're required in your business, the more the business is dependent, which then the heavier it feels on you, which is the harder it comes to ever get out of it. And most people, and I've done some research and studies, most people do not cross that million-dollar mark, do not ever get out of this overwhelming stage. And it's because working harder is no longer the answer. And that's the hardest part. So I just, you know, talk about that as this growth trap. And there's other traps along the way in your business, but that's like the biggest one.
SPEAKER_00I feel like you're in my head right now because I literally before we were in here, I was walking on the beach, right? But I was texting my friend and I was like, how is it that I continue to hire more people and I still have more work to do? Like, I don't understand. So, what you're saying right now like is really resonating with me because it's like, I mean, yes, I've got every other week off, but I'm cramming so much into like work. Work time so I can have the other time. It's like burnout city, you know, just like trying to do it all in like three days. Right.
SPEAKER_01You're just shifting time.
SPEAKER_00The workload help is not being answered.
SPEAKER_01Well, well, the answer, I mean, what's what's interesting with the answer is the answer is always about like if you and I are working together, I would say, where is the business dependent on you on areas that you really don't like? Like I focus on always the owner. Anytime we're working with anyone, the focus is always on where are you feeling angst? Where are you feeling friction? Because whatever you're doing in that moment, chances are it's something you don't particularly like to do.
SPEAKER_00Yeah.
SPEAKER_01The other chance is it's probably not critical for you to do it within the company. And the other thing is it's probably not a high revenue producing activity. So all three of these things are happening, yet, for whatever reason, you're continuing to do it. It could be because you got a problem with letting go and control. It could be with, but I'm a therapist. That's what a therapist is supposed to do. So we call that identity. Or it could be a belief issue, like a false belief. Well, you know, as the owner, this is this is what owners are supposed to do. So you're fighting those deeper things inside of you. It's not necessarily because you don't know what to do. I've never met anybody when I ask them the right questions, they know what they do, but for whatever reason, something's holding them back from it. So that's what I would say to you. I would say, all right, well, why don't you just list the stuff you do in a given week or two weeks? Let's just list all that stuff you're doing. Then put them in just two categories. One, activities that you like that give you energy, and two, activities that you don't particularly like or don't care for that take energy. Give energy or take energy. If it's in between, it's take energy. Then, how about we start easy? What is one activity? We we refer to it as a low energy activity, one activity that you don't particularly enjoy doing that takes energy. Who else could do this thing? And in answering that, you would have to answer what is the activity, what is the outcome that the activity should produce? Who is going to own that activity? What are the steps of that activity? And how do you measure if it's done well?
SPEAKER_00Yeah.
SPEAKER_01Do we talk about what is the outcome or purpose of the activity? Who's going to own it, the steps, and the outcome, or the uh the measurement of it?
SPEAKER_00Like yeah, yeah, the result or whatever. Yeah.
SPEAKER_01Whatever, the KPI.
SPEAKER_00Whatever. Yeah, yeah. I know what you're saying.
SPEAKER_01And what I did, and this was my secret sauce, and I'm doing it now in this business. I've been doing it for 13 years, but every week, every month, I'm always asking myself, because I want to grow. Like I want to grow. I want to go personally, I want to go professionally, I want to grow in my business, income, and everything, but I don't want to grow and not have the life I want. So that's always the thing it's measured against. And I'm always asking, what is Jamie doing? I talk to myself in third person. What is Jamie doing that does not really give Janie energy? That's someone else who actually enjoys that type of fit. And what is the resisting point that I'm feeling? I'll give you a real thing. I'm gonna pull back the curtain yesterday and today. Now, my business right now, in full transparency, we have a you know $1.3 million coaching business, which it's amazing. I'm like bewildered because most coaching businesses will never get over $100,000. So we're I know we're in rarefied air, but I also know there's a lot more people that need help. And my business is going through somewhat similar to what your business is going through. I'm hitting a ceiling that Jamie's still doing more than I really should do and want to do in my business, even though I have lots of flexibility. Like if you look at my calendar, you'd be like, Jamie, half your calendar is empty. I get, I get that, but it's still there's things that I'm doing that I don't want to do. And I just sat down and journaled around it, thought about it, got on Chat GPT, played around with it, and I went, What am I resisting? And I'm just so open and honest with myself. And I went, I don't know, just I don't know if I want to pay someone to do it. I don't know if I want to let go of this. Say, and I wrote up a just playing, a job description of what they could take off my plate, who would be the right person? And I'm reading it, and all of a sudden my body went, God, that would be awesome if I had that kind of person.
SPEAKER_00Oh my God. I'm doing that right today.
SPEAKER_01It wouldn't cost really much at all. Couple thousand bucks, maybe.
SPEAKER_00Yeah. But compared to what it does to me, like the emotional toll, right? Like as you're talking, I know exactly what it is. I'm like, I'm with you.
SPEAKER_01Anything does invoices for our coaches, it doesn't take that long, but every month I send out the email. Every month, there's always a coach or two that doesn't give back their invoice, and I got to follow up on me, go, is this what the CEO does?
SPEAKER_00Yeah.
SPEAKER_01I go, This is ridiculous. And I have two other people on my team. One handles most of my social media, and the other is like my catch-all assistant. Well, damn, her schedule is so full. If I give her any more, she's just gonna drop something, which is kind of what's happening. And now, right now, I took action on that. And I am meeting with a person tomorrow that is gonna help me find that person.
SPEAKER_00Amazing.
SPEAKER_01That's self-awareness in line with where you want to go, having radical honesty with yourself, getting clear, and then most importantly, which I know you've done, is take action on it.
SPEAKER_00Yes. I and hearing you put it into a process is so helpful because I mean, I've literally done that for almost everything. It's evolved into that same pro. Like, I hate this. Oh, I gotta find somebody to do it, find somebody, and it's like such a relief, but there's always like the next thing. So I totally hear you, and just talking about it today, I'm like, I totally know what that thing is. I gotta figure it out. I gotta figure out another solution because it's bogging me down. So tell us about practice freedom you and how you help people do this, what you're talking about right now.
SPEAKER_01Yeah, so I know there's people out there that they have something bigger that they want, whatever that is, but it's something significant. And they, yes, we have AI now, yes, we have all that stuff out there, but they're like you and me. They're an entrepreneur with a lot of ideas and a lot of energy. And yeah, what they need is direction, they need what I like to refer to as a thinking partner. AI can't be a thinking partner. It can give you what you give it, it can give it back. But we need someone that's able to kind of hold space, push back. AI will never push back, push back when needed to help you create this plan and help you along this journey of actually doing it and making sure that the plan is always aligned with what's going to give you the life that you want. I don't have a cookie cutter thing. Yes, we have a a way, a process, and how this is done, but it's so unique depending on who's doing it and where they're starting and what they're about. But the result is always the same. When you are able to have a plan in place and have someone supporting you, because you know, our confidence is always questioned, and you take the right actions at the right things, and you recognize what the problems are. Not only does your business grow financially, profitably, but your life becomes better. Your team is better. You become a better version of yourself because that's part of the work that gets you to that level. It's not all just structure outside of you, this system's in place, we hired this person. Those good people in your business, they're only gonna stay if you are capable of working with them. The business rises to the level of you. So what we're doing at PFU for those type of people that have those bigger visions, that have those upper, you know, million dollar and multimillion dollar, or maybe like in your case, multiple different revenue stream businesses, to have someone in your corner is like the unfair advantage. We are that type of company. That's what we do because that's the type of person that I was fortunate if I went through a few coaches, but I was fortunate enough to find, and I realized I bet there's other people that need that type of coach. So I developed this coaching company over the years to really focus on those types of people. We even teach you how to create an AI business coach because there's a lot of things you can do and not pay anybody to do, a lot of material stuff. So that's not really what coaching is. It's much more of a higher and kind of the deeper level and making that connection and helping people navigate through the twists and turns and ultimately creating the life that they want and love.
SPEAKER_00That is awesome. So, where do we find you out in the world?
SPEAKER_01Yeah, so there's a couple things. One, we have what's called a practice freedom assessment, and we'll give you the link and everything. So this is kind of a, you know, you talk about self-awareness. This is a self-awareness assessment. It's 24 questions, it only takes a few minutes. But what it does is help create self-awareness of where you are compared to how much your business is dependent on you. So it allows you to kind of assess one through five. Once you're done with that assessment, then you can schedule or apply to schedule a review. So with me or one of our coaches, we'll review what you wrote down and at the very least help you get a clear picture and plan of what you need to do and how to get there. Then, of course, if it's something you want to help with, then we can talk about how we could possibly help you. But first is let's get clear where you are and where you want to go and what has to happen. Then, if it makes sense, we can talk about whether we're the right people to help you get there. Or if there's another recommendation that we can make, we can make. And that is, you know, for your audience, that is no charge. And it is highly valuable to get that type of perspective and awareness around your business. Because I ask you all different types of questions.
SPEAKER_00Yeah, I'm gonna do it. I already saw the link for it. It's gonna be under the show to you guys.
SPEAKER_01It hits everything in your business. But what I like doing that before just meeting with someone blind, I like doing that because it gives me an understanding of where you're coming from without having to ask you 24 questions.
SPEAKER_00Right, right. You're already like therapists, right? Like, let's do the intake assessment before you even come in. Like, let's skip that question. It's the same thing.
SPEAKER_01So then when you and I are talking, we're gonna gather some other information so then we can make a proper assessment and then come up with a plan, just like we do in therapy. It's always the SOAP. I've adapted that to how we coach and guide.
