A listener wrote to me recently, and one line from her email really stuck with me. She told me that she's been avoiding gluten for nine years, since shortly after her MS diagnosis, because she was absolutely certain that gluten triggers her MS symptoms. Nine years. Nine years of reading every single food label, nine years of skipping the bread basket, and nine years of dealing with that flicker of anxiety that you get when you're not quite sure what the ingredients of a specific food that you might be eating, and you worry that one, one of the ingredients might be harmful. And what really struck me wasn't even her concern or the question about gluten. It was this She wasn't asking if gluten was the problem. She was asking what else she should cut out just in case If you live with MS, then this may sound familiar to you because when symptoms shift from day to day, sometimes hour to hour, you look for something that will help you regain a sense of control. Food feels controllable So today I want to answer LK's question and maybe yours with context, nuance, and evidence. The information I share in this episode may feel at first like I'm taking something away, but by the end, I really hope it feels like I'm giving something back. So let's talk about it Welcome to the MS Bites podcast, where nutrition science meets the lived MS experience, because good nutrition shouldn't be hard to digest. LK from Georgia wrote this email, quote, "I have been avoiding gluten since I was diagnosed because it triggers my MS symptoms. I've also heard that dairy is bad for multiple sclerosis and then of course all things sugar. Can you please give me a list of inflammatory foods that are most likely to trigger MS symptoms?" End quote. LK added that her symptoms are beginning to affect her work and she wants to do everything she can to preserve her ability to continue working. I think we can all understand that concern, as well as her instinct to protect her livelihood. But it's also important to distinguish between what feels protective and what the evidence suggests is protective. Today, we're going to unpack LK's question carefully and kindly, distinguishing what's proven, what's still hypothetical, and what's often marketed to the fear and loss of control that can often accompany an MS diagnosis. We'll talk about what a relapse is and how it's different from day-to-day MS symptoms; why MS symptoms fluctuate; how food is involved in both relapses and symptoms; why foods can feel like triggers even when they're not; and then finally, we'll talk about how to eat in a way that genuinely supports you fear and without unnecessary restriction. Okay, and anyone quietly wondering if you caused your symptoms with something you ate, this one's for you. So before we get started talking about food or the role that food plays in relapses and MS symptoms, we need to define each of these terms because they're two very different experiences that often get mistaken for one another in MS conversations. The first is a relapse. You might also hear it called an exacerbation, an attack, or a flare. a true relapse has specific criteria that must be met to be considered a relapse. A relapse is new neurological symptoms or a significant worsening of old ones that last at least twenty-four hours, occur at least thirty days after a previous attack, and they are not explained by fever, infection, or heat. Relapses are immune activity in the central nervous system. They are active disease. If you had an MRI during a true relapse, your neurologist might see new lesions on your MRI. That's one thing. That's one experience, an MS relapse. The second is day-to-day MS symptoms, things like fatigue, brain fog, pain, stiffness, visual blurring, sensory overload, cognitive dysfunction. These things and others can change or fluctuate from day to day or hour to hour. But that fluctuation is not a relapse. that's because they're not new CNS damage MS symptoms occur because your brain is sending signals along pathways that have already been damaged. Think of it like this. I'm sure you've seen the image of a frayed wire to depict demyelination. I've used that image myself in presentations. you've seen it, and I use it because it's actually an effective visual for this, for this situation. If we think of it like a lamp with a frayed wire, the wiring is compromised, but it still works. Some days, the light shines steadily. Other days, it flickers or it goes out. The wire itself hasn't changed, but the conditions around it might have. So think of it like, That lamp with faulty wiring might be sitting on a table that somebody bumps and then it flickers or it stops sh-- you know, the light stops working. Or maybe there's a power surge. That's because small shifts in the environment can disrupt an already vulnerable connection. MS works in a similar way. If the wiring of the nervous system is frayed, what factors in our en-environment might jostle those fragile pathways and make symptoms more noticeable? I happen to be recording this in July of 2026, which makes me want to mention that heat is a big one. Uthoff's phenomenon is, something that occurs when body temperature rises even slightly. It can cause damaged nerves c-to conduct signals less efficiently. Temperature shifts from things like hot showers, hot weather, exercise, or fever can cause symptoms to worsen temporarily, but then they improve once you cool down Infections can trigger what we call a pseudorelapse, which is when symptoms flare up, but it's not because of new disease activity. Instead, it's old damage becoming more noticeable because your body is under added strain. As the infection clears and your system recovers, those symptoms typically return to baseline and normal. Things like sleep, stress, dehydration, blood sugar swings, physical or cognitive overexertion can also amplify MS symptoms, and sometimes there's no clear reason at all. You just feel awful today and better tomorrow, or vice versa, and you don't know why. MS is is a complex, unpredictable condition. In many ways, the most predictable thing about MS is its unpredictability. Symptom fluctuation is a part of the disease. It's not an indication that you have done something wrong. So now let's take a stab at answering LK's question. Where does food fit into MS relapses and MS symptoms? Just like LK, You've probably seen claims that certain foods trigger MS symptoms. Things like gluten, dairy, sugar are often packaged into long lists of friendly and unfriendly foods. Some of those lists are rooted in broader public health guidelines, and others come from MS-specific dietary protocols. I unpack the difference between those types of lists in a separate episode because it's an important distinction, but also because I know myself. If I let myself chase every interesting rabbit trail into one single episode, we'd be here for days. So in order to prevent that, I'm making it a separate episode, and I'll drop the link to that episode in the show notes. When foods are tested in controlled studies where researchers account for things like sleep disruption, stress, heat sensitivity, and infection, the evidence for specific, quote, "trigger foods" doesn't consistently hold up. That doesn't mean food doesn't matter, because it absolutely does. It's just not in the fear-based way that is sometimes presented to us all online. It's easy to come away from certain websites feeling like a single bite of the wrong food could set your nervous system on fire. I know I've felt that way, and I know many of the people who I work with professionally have had those fears and concerns. What I wanna offer you is something steadier. Food is usually not a landmine. It's not a loaded weapon. Most of the time, food is simply food, part of a larger pattern, not a single catastrophic event. and I hope that feels genuinely reassuring So LK asked for a list of inflammatory foods that trigger multiple sclerosis, and here's the most honest answer that I can give to that question. There isn't one. No specific food has been proven to trigger an MS relapse. Not gluten, not dairy, not sugar, not nightshades, no food. When researchers study diet and MS relapses, they look at long-term eating patterns, how someone eats over months and years, not a single meal. There's no human study showing that eating a specific food in isolation one time causes a new inflammatory lesion in MS. There's no case report, no food challenge trials, no randomized controlled trials. The evidence simply doesn't exist. To put it plainly, there is no validated list of foods that trigger MS relapses. And I know that can feel frustrating if you're searching for something concrete that you can grab onto and feel like you're controlling things. But look at it from the other side. It means that your last relapse was not caused by something that you ate. It wasn't a dietary, quote, "mistake." There wasn't a single food that you could have skipped to prevent that relapse from happening And that's where the evidence stands on food and MS relapses. Symptoms though, that's gonna require a little bit more nuanced or layered conversation. That's because MS symptoms fluctuate for a lot of reasons, and food may intersect with that in a different way than it does with disease activity. When people say food, quote, "triggers their MS," they're usually describing symptom shifts, not relapses. And symptom shifts can happen for a lot of reasons. So instead of asking, does this food trigger my MS symptoms? A more useful question might be, what's actually happening in the body after I eat? If we slow it down, there are really three different layers to consider. Layer one is just normal post-meal physiology. It's what happens in every human body after we eat. Layer two is heat sensitivity, that Uhthoff's phenomenon, Layer three is the question of amplification, whether MS makes ordinary after-meal effects feel more pronounced. So let's take each of these layers one at a time. Starting with layer one, normal post-meal physiology, also referred to as postprandial effects of a meal. Anyone with or without MS who eats a very large high fat or high sugar meal may feel sleepy, sluggish, bloated, or foggy afterwards. If you add a few cocktails, then you can layer in sedation or unsteadiness. That's digestion and metabolism at work. Normal regulated physiological responses, not pathological reactions or proof that a particular food is harmful. It's not multiple sclerosis Layer two involves heat sensitivity, Hot food, hot drinks, and alcohol can actually raise core body temperature slightly. And as I mentioned earlier, even a small temperature increase can temporarily worsen nerve conduction. That's that Uhthoff's phenomenon. That can mean sudden fatigue, blurred vision, worsened spasticity, brain fog, or otherwise amplified symptoms. It's a real documented and physiologically distinct phenomenon, but it's not a relapse because no new damage has occurred, and symptoms resolve once temperatures normalize, usually within minutes to hours Layer three asks the question, does MS amplify normal after-meal effects? It is a biologically plausible idea that MS could amplify normal post-meal fatigue because a a nervous system that's already working harder may feel metabolic shifts more intensely. But this hasn't been tested in controlled studies comparing people with MS to healthy controls, so it remains plausible, not proven. Now let's take a step back and notice something important. None of these three layers that we just went through involve food directly triggering immune activity in the brain or spinal cord. Not normal digestion, not heat sensitivity, not amplified symptoms Those are physiological responses. They are not immune system attacks. And that brings us to the word that I really think is sitting underneath LK's question, and that's inflammation Oh, where do I begin on inflammation? Um, inflammation is one of the most overused words in modern nutrition. Online, it gets attached to everything, and I mean everything. Uh, gluten, dairy, sugar, seed oils. It's as if your immune system is on standby, ready to launch an attack if you even think of consuming those foods. But inflammation doesn't really work that way. It's an immune response to a perceived threat. For a food to be truly perceived as a threat or to be truly inflammatory in the way people imply, it would need to reliably activate the immune system as a danger signal when it's consumed. That makes sense, right? And that does happen. In specific diagnosable allergies, think shellfish or peanuts, for example, the body recognizes that food or those foods as dangerous. And when a person with an allergy consumes them, even a trace amount of peanut or shellfish can activate IgE antibodies, trigger mast cells, and set off an inflammatory cascade that tightens the throat, drops blood pressure, and can become life-threatening within minutes without epinephrine. Tiny exposures matter. The reaction is immediate and real, and is potentially deadly. Celiac disease is related, but a little bit different. It is, however, just as real. Where the, shellfish and peanut allergies are an acute response to a food that is recognized as dangerous, in the case of celiac disease, repeated gluten exposure triggers an autoimmune attack on the small intestine, causing measurable tissue damage over time Both are immune-mediated responses. The biology is clear, and the reaction is not optional. In other words, if somebody who has a documented shellfish allergy decides while they're on vacation that they would like to indulge or have a cheat day and have some oysters Their body is going to respond in the same way that they would if they weren't on vacation. Their, um, immune system is going to trigger mast cells, set off an inflammatory cascade that tightens their throat, drops their blood pressure, and a life-threatening situation will develop immediately without epinephrine. But for someone without a food allergy or celiac disease, food is not recognized as a threat. It's recognized as food. In other words, a slice of cheesecake may cause a temporary rise in blood sugar and a brief increase in inflammatory markers as a part of normal metabolism. That response is transient, and it happens in everyone, and the body eventually returns to baseline. That's physiology, not pathology. The negative health effects do not come from consuming an isolated slice of cheesecake. it can begin to have an impact on your health when it's repeated over time. That's because what contributes to sustained inflammation isn't a single food, but a dietary pattern that gradually worsens metabolic health over months and years. It's a slow shift in how your body processes and stores energy. If someone consistently consumes more energy or calories than they burn, the excess energy is stored as visceral fat. That visceral fat tissue releases inflammatory signaling molecules, insulin resistance can emerge, blood sugar control worsens, low-grade inflammation becomes more persistent and more chronic. It's not about a single ingredient causing an immediate inflammatory cascade. It's about what happens when certain eating patterns are repeated over time, and that's an important distinction. Having ice cream, cookies, soda, hot dogs, especially if you're at Wrigley Field, or bacon in your diet does not automatically create an inflammation problem. However, when those foods make up the foundation of your diet and more nourishing foods are regularly crowded out, then metabolic health can gradually shift in an unfavorable direction. insulin sensitivity can decline, visceral fat can accumulate, low-grade inflammation becomes more persistent. Again, it's not about an isolated food choice, it's about what your overall pattern looks like across months and years. And there's one more distinction related to inflammation that really matters, and while we're talking about inflammation, I'm gonna take a minute and share that. When we talk about, and I'm air quoting here, "inflammation," we're often talking about very different biological processes, and because the internet has lumped them together, we discuss them as if they're all the same thing. This creates confusion and unnecessary, like, fear around food. an allergic response to food, is acute and rapid, it's potentially life-threatening. Metabolic inflammation develops slowly and circulates at low levels throughout the body. An MS relapse involves immune cells crossing the blood-brain barrier and creating localized inflammation inside the central nervous system. It's the lesions that we see on an MRI. Each of these processes that involves inflammation are biologically distinct. They involve different tissues, different pathways, and different mechanisms. So when inflammation, there again, I'm air quoting, when inflammation is used as a catch-all term online, it can blur distinct biological processes into a single alarming concept, and then it generally takes that next step and overstates the role of food in driving each of them. So if you've been carrying guilt or fear about something that you ate related to an MS relapse or an MS symptom, you can set that down. Food matters, but it's not a landmine. It's not a relapse trigger, and it's definitely not a moral test. Food matters in patterns that we build over time in ways that support or doesn't support our overall health. So the patterns that support our overall health should definitely be the focus. at this point, you might be thinking, "Um, if foods don't trigger MS, why do I always feel worse after eating them?" You're definitely not imagining that. The patterns that you notice are real, and if you tell me that you feel worse after you eat gluten or insert other food here, I believe you. But a more useful question upon reflection anyway might be what's actually driving that experience? And I say this because it's really easy to land on one explanation especially when it feels simple and actionable. But when food becomes the primary lens, every symptom starts to look diet related. It's like that old adage, when all you have is a hammer, everything looks like a nail. So before we conclude that a specific food is the culprit of us feeling crummy after a meal, it helps to widen the frame a bit, Because there are several explanations that often fit just as well or maybe even better Number one, a digestive issue that is not MS. Lactose intolerance, FODMAP sensitivity, or simple indigestion can cause bloating, fatigue, and brain fog. That's physiology, not a flare. Number two, normal energy swings. That big meal, the sugar spike, maybe you didn't eat enough, or maybe you skipped food all day, leaving you feel tired and foggy. When you live with MS, it's easy to interpret those normal fluctuations as disease activity, but not every energy dip is a relapse or even MS at all number three, the nocebo effect. You've probably heard of the placebo effect, which is when you expect to feel better after a treatment, even if it has no active ingredient, you feel better simply because you expect it to help. A nocebo is the flip side of that. If you expect a food to harm you, your body can mount a stress response. The symptoms that you feel are real, but they're driven by a mind-body loop, a stress response, not new inflammatory damage, not MS Number four, coincidence and confirmation bias We've already established that MS fluctuates. It's unpredictable. But our brains, are wired to find patterns. So if your symptoms feel worse after you have a dinner consisting of pizza and ice cream, your brain links the two. But what you don't notice are all the times you've eaten pizza and ice cream and felt fine, or maybe felt lousy after an, a dinner that consists of grilled salmon and a kale salad. Because once you believe a specific food is a trigger, your brain starts collecting supporting evidence and quietly discarding the rest Number five, the control factor. MS is unpredictable. MS bites, really. It sucks. A restrictive diet or a list of food rules that you can diligently adhere to can offer a sense of agency. it's you taking action. It's you doing something. However, if you feel better on a structured plan, it may be because you're eating more intentionally, choosing nutrient-dense foods more consistently, or simply feeling empowered, and those benefits are all real. However, they don't necessarily prove that the food you eliminated was the cause of a relapse or a symptom in the first place Number six is mixed messaging, and this is a tough one because, we live in an extremely challenging information environment here in July of 2026. But it, it's important to realize that mixed messaging doesn't only come from the internet. It often starts with the healthcare system itself Dietitians are not consistently included on the MS care team. So conversations about nutrition are often left to well-meaning providers in other disciplines, neurologists, primary care physicians, therapists, PT, OT, speech pathologists, each offering their own individual perspective in an attempt to answer your nutrition-related questions. And to be clear, disagreement is not the problem. Thoughtful debate is healthy. Multiple sclerosis is a complex neurological condition, and nutrition science is complex, and evidence in the field of nutrition continues to evolve But a problem arises when very different nutrition recommendations are presented as if they carry equal levels of evidence. Conventional nutrition, just like conventional MS care that you may get from your neurologist, relies largely on clinical trials, reproducible data, and established standards of evidence. Alternative medicine approaches such as functional, integrative, holistic medicine, or naturopathy rely heavily on nutrition interventions, and these providers may present their recommendations as equivalent options to conventional recommendations, but they have not been subjected to the same level of testing, oversight, or long-term evaluation. When that distinction is not made explicitly, it creates confusion. And confusion, especially around something as personal as food, can push you towards the most confident voice rather than the most rigorously tested one. And into that uncertainty and confusion steps the wellness industry. The wellness industry doesn't simply respond to confusion, it thrives on it, and it benefits from it. And that is because clear, confident answers are compelling. Whereas nuanced, evidence-limited explanations, not so much. And that's because what you are really looking for when you're googling your question is a simple, actionable hammer, not a nuanced discussion that starts with, quote, "Evidence is mixed and it depends." But in nutrition, it depends isn't a retreat from certainty, it's an acknowledgement of complexity. In fact, it's the starting point of any credible answer to a nutrition question. That's because nutrition interventions have to be contextualized beyond your MS diagnosis, to the individual that you are, to your eating pattern, your medical history, the obstacles you encounter, and so on. The recommendation to, quote, "Cut these seven foods and you'll feel better," end quote, is simple. It's actionable, and it certainly can feel incredibly appealing. It is important to know, though, that those oversimplified claims collapse all the nuance, all the context that would surface in an honest it depends conversation into a single ineffective hammer, and that's not an accident. It's a marketplace dynamic. That's because the wellness marketplace is very good at selling hammers So LK, let me answer you directly. You asked for a list of inflammatory foods that trigger multiple sclerosis. There isn't one. No specific food has been shown to cause MS symptoms or relapses, but that doesn't mean that food doesn't matter. It just means that food matters differently than the internet often suggests. It's less about fearing a single ingredient and more about the overall pattern that you build over time. The strongest evidence supports a generally healthy way of eating, sometimes referred to as a Mediterranean pattern, that's built around a variety of vegetables and fruits, whole grains, legumes, fish, olive oil, nuts and seeds. Balanced and flexible, able to adapt to the realities of living with multiple sclerosis. Not a miracle, but a foundation The effects are gradual and supportive. Food works as a team. It's, it's synergistic. No single ingredient is the hero, and no single ingredient is the enemy. is important to build meals around what supports you, n-not what you're afraid of. Be careful not to under-nourish yourself in pursuit of eating, air quote, "clean" or any other rigid and arbitrary standard of dietary purity. Eliminating entire food groups without thoughtful replacements can worsen fatigue and brain fog, sometimes mimicking the very symptoms you're trying to prevent. The basics matter more than the bans. Eat enough, eat regularly, build balanced and varied meals, stay hydrated. These steady habits, while boring, will influence energy and long-term health far more reliably than eliminating one food ever will. If you truly suspect that a food is having an impact on you in one way or another, resist the urge to immediately eliminate half of your diet. Get curious instead. The steady truth under all of this is that people with multiple sclerosis do not need an MS-specific diet. You need a sustainable, nourishing way of eating that supports your body as it is now. MS symptoms fluctuate because MS is a complex immune-mediated disease, not because of a single inflammatory food or meal Aside from that allergy situation, food is not a threat to your nervous system. It's fuel. It's the building blocks for everything in our body. It's daily care. You did not cause your disease with something that you ate, and you deserve to eat without fear. The goal is not dietary perfection, it's resilience, flexibility, and nourishment. It's a way of eating and living that's bigger than MS All right. Okay. We have covered a lot of ground here, and I wanna bring this full circle. You emailed me wanting to do everything right to protect your health and your ability to keep working. That instinct is a strength. My hope is that this conversation helps you to anchor that strength in evidence, not fear. Here is the core truth. No foods have been proven to trigger MS symptoms or relapses, and a generally healthy way of eating supports your long-term health. Both are true. MS fluctuates because MS fluctuates, not because of your food, not because of one meal, and not because you failed Perfect eating will not control this disease or how it shows up in your life. But a balanced, varied, and flexible approach to eating will help you to give your body the building blocks it needs to function at its best, even if best has a different meaning after an MS diagnosis. MS is complicated. Eating well with MS doesn't have to be. Eat in a way that supports you, adjust when needed, and please leave room for joy. This week, I hope you will make something you enjoy and eat it without guilt or fear. That isn't negligence, it's living. But what I hope I've given you instead is something steadier. Not control over MS, but control over how you nourish yourself while you're living with MS.
