You ventilated perfectly; the chest rises, the waveform looks great, and the oxygen is still low. 

This episode installs the mental model that fixes the mistake everyone makes in hypoxemia:

Oxygenation is not an airflow problem — it’s a matching problem.
Air alone is useless without blood. Blood alone is useless without air. Oxygen only happens when they meet at the same microscopic interface. 

You’ll learn:

  •  The “speed dating” model: ventilation (V) has to meet perfusion (Q)
  •  Why diffusion is distance-limited (air can’t “wander” to find blood) 
  •  The two extreme failures: 
    • Dead space = ventilation without perfusion (high V/Q) 
    • Shunt = perfusion without ventilation (V/Q = 0) 
  •  Why shunt crushes oxygen faster than dead space (healthy alveoli can’t “overfill” hemoglobin) 
  •  The bedside rule that never lies: severe hypoxemia + poor response to 100% O₂ = true shunt
  •  The fix depends on what’s broken: 
    •  Dead space → restore perfusion / stop crushing capillaries 
    •  Shunt → reopen/fill alveoli (PEEP, drain pleural space, clear fluid)
