Chapter 122 - Part C: How a Reversible Problem Becomes an Irreversible One
Use Left/Right to seek, Home/End to jump to start or end. Hold shift to jump forward or backward.
In this BoardsCast episode, we continue Tobias Chapter 122 — Pinna and External Ear Canal with the most important shift in chronic otitis thinking:
The infection didn’t destroy the ear. The inflammation did.
Early otitis externa is a soft tissue problem—red, swollen, painful, but still structurally recoverable. But when the trigger persists (allergy, mites, foreign body, keratinization disorder), the ear doesn’t just stay inflamed. It remodels.
This episode walks through the irreversible sequence:
Inflammation → swelling → hyperplasia → fibrosis → stenosis → mineralization
And once you reach that point, the “bugs” are no longer the primary problem. The canal becomes a sealed, humid incubator that can’t ventilate, can’t drain, and can’t receive medication. At that stage, the anatomy itself is the perpetuating factor—often progressing to otitis media/interna and neurologic disease.
Key takeaway: When anatomy becomes the disease, medicine starts losing.
