In February 2019, the city of Stockton, California tried something unusual. A group of residents in neighborhoods at or below the city's median income started getting $500 a month, no strings attached, no work requirement, no rules on how to spend it. After the first year, recipients reported less month-to-month income swings, lower psychological distress, more energy, and less physical pain than a control group who received nothing. They were also able to better handle a $400 expense. And contrary to common assumption about guaranteed income, recipients were actually more likely than the control group to move from unemployment into full-time work. This was even before the pandemic hit. Then, a year into the study, COVID-19 arrived. Suddenly, the researchers weren't just tracking income stability. They were watching what happened when a global health crisis disrupted jobs, schools, childcare, and daily routines for everyone. Participants used the extra money for things like bills, food, and caregiving. Recipients continued reporting better physical functioning than the control group all throughout the pandemic, though some of the earlier gaps narrowed. To be clear, this was one city, a few hundred people, and it didn't settle every question about guaranteed income. nudging the other. So what does getting sick actually cost? The obvious answer is appointments, medication, and hospital bills. But also we need to factor in missed wages, fewer hours, transportation, child care, and time spent caring for someone else. And the number gets a lot bigger. In the US, a KFF survey found that about four in ten adults are carrying some kind of health care debt. Credit cards, payment plans, loans from family, bills they just haven't been able to pay off. Health systems vary a lot from country to country, so the shape of that debt looks different depending on where you live. But lost time and interrupted work show up almost everywhere. An illness might last a few weeks. Rebuilding the savings it drained can take a lot longer, especially with interest piling up on a credit card balance in the meantime. So the real cost of getting sick rarely shows up in one single bill. Financial stress can also affect your health before you even get sick. Worrying about rent or bills can wreck your sleep and your focus. It also shapes how you respond to a health issue. It also shapes how you respond when a health issue actually shows up. Someone might put off a doctor's visit because missing a shift means missing a paycheck, or skip a prescription refill because three other bills are due that same week. From the outside, that can look careless, but up close, it usually just means someone weighing bad options against worse ones. Some households have savings, insurance, paid sick leave, or family nearby who can step in and help. Those cushions absorb the hard part. Those cushions absorb the part of the disruption. A week of savings can cover lost income. Paid leave means an appointment doesn't cost you a paycheck. Without those cushions, a short illness can start affecting rent, food, and work almost immediately. So that $500 a month in Stockton worked the same way. It wasn't a fix to everything, but a cushion that gave people more room to handle whatever came up. Maybe you don't have a mortgage or a stack of medical bills yet. That's fine. This connection between health and money still starts young. Sleep and mental health affect how you focus in class and whether you show up at all. Your family's finances shape your access to transportation, food, and extracurriculars. An injury or a family emergency can eat into school time or push you forward. Deloitte's 2025 global survey of over 23,000 Gen Z and millennial respondents across 44 countries found that 48% of Gen Z said they didn't feel financially secure, and the survey found a close relationship between that financial insecurity and overall well-being. This isn't just a someday problem. It's already part of a lot of people's daily lives, even when nobody is naming it out loud. So over the season, we'll dig into medical debt and insurance, mental health, and money stress, sleep, nutrition, chronic illnesses, housing, and working conditions. We'll also zoom out to the systems around all of it: employers deciding who gets paid leave, governments shaping health care and labor law, communities determining who has access to transportation and care in the first place. Stockton started with a simple question. What researchers found reached further than just spending habits. It touched stress, energy, and people's ability to handle a bad week. The study doesn't close the debate on guaranteed income. What it does is make the case for why health and money belong in the same conversation.
