SPEAKER_00Hello everyone, this is Dr. Nicole, and you are listening to the Learning Corner. And today we are going to talk about concierge medicine. But not just from the pulpit here, but from frequently asked questions from our very own patients. So I've been getting a lot of questions from patients as we are transitioning into a concierge clinic. And if you have one question from one patient, it is going to happen with almost all patients. So let's dive in today and talk about the old ways of concierge medicine, the new ways of concierge medicine, how it is changing. You're going to have to just let my enunciation go. I'm trying my best, but these concierge enunciations are quite difficult over and over and over. Okay, so today we're going to look at the old ways and the new ways. So the old ways were I pay for my health insurance, and you know, I just can't get in. So I have the money, I'm going to pay for a concierge, medical doctor, and I'm going to have the deductible for my health insurance and use what I can there or my Medicare, whatever that is. Now I pay 10 grand to an MD every year, and I get their phone number, and I can call them whenever I need them, and they charge me an hourly rate on top of that. So that's the old ways. The new ways, this is the larger picture. The new ways, the concept is this. Instead of paying for health insurance, a monthly deductible, and all the other things that cost upwards of 20, 30, 40 grand a year, um, why don't we change that to a high deductible plan where you're paying, I don't know, six grand a year for a family of four? And I'll get into that in a minute. So when we look at the old way, we're gonna be looking at what I just talked about. You're gonna be paying 20, 30, 40 grand a year, you're gonna go into the MD, you're gonna have long wait times, probably a lot of co-pays and all these other things on top of it. Maybe you pay 60% or 80% after your deductible is met, right? That's very common. But what if you actually could change to a high deductible plan? Because this is what's happening, and this is what I want everybody to understand is if you're not getting what you need out of your health insurance, it's not get concierge medicine on top of it. It's restructure your health insurance because you're not using it or don't need it or whatever else to fit that need, and look at getting a high deductible plan and an HSA. So if you look at it that way as a bigger picture, you actually get to look at cost savings. So let's jump into an example from me. I have a family of four. There's two of us that are older than 40, and there's two in our family that are younger than 15, so there's four in our household. We have a high deductible somewhere between 10 and 15,000 a year, and then we pay 500 a month. So it's a hundred percent covered after the deductible is met, up to I can't remember if it's one million, two million, or three million. I'd have to look at the specifics, but it's catastrophe insurance. It's high deductible for people 500 a month. Might be a little less than that. It changes, excuse me, every year. Um, because A, we get older and then you know everybody costs more money. So let's just say that our high deductible, we pay six grand a year for people, and then we pay for any scripts, any doctor's visits, right? Supplements, anything else that we need, we pay out of pocket for. That is what a high deductible plan is. That's what it's set up for. Um, if one of us had an employer that allowed for an HSA match or an FSA match, right? A health savings account or a flex spending account, those are two different things, and you should definitely talk to your financial advisors andor employers andor accountants, CPAs on those, on what's best for your family. But when you can actually get an HSA, it's pre-tax spending dollars. And places like here, integrative concierge, I'm considered my clinic is considered primary care first, then chiropractic, second, then massage, third. So you look at those and go, hmm, I can spend my HSA account here, and I get five grand a year, and this is what I'm gonna spend that on. And if you have another, um, I don't know what the limits are. Again, you're gonna have to talk to your financial people. I am not giving you advice on your finances, I am not a business finance professional, um, just a disclaimer. But that's how you shift the larger picture in the concept is instead of paying for health insurance and concierge medicine, you have to go. What are the pros and cons of shifting to a high deductible catastrophe type insurance? Getting an HSA, is that something that is accessible? And then looking at um the coverage on that. And once you get that concept down, then if you're like, oh man, I was spending$30,000 a year on a family of four, or maybe$40,000 a year on a family of four, and then we had all these deductibles and all these things, so I was actually spending even more than that, um, shifting that and then looking at the cost benefits, uh, that will draw you into going, well, a concierge medicine practitioner or clinic is actually in my wheelhouse, and they see less people. And maybe you want to see one like me, integrative concierge, that does chiropractic care, genetic-specific meal plans, all the lab testing that you get your one supplement solution from. So your labs are dictating your nutrients in your supplements, specifically somebody who does massage in-house, those are the things that you have to start looking at is the larger picture concept is it is not health insurance and concierge, it's health insurance or concierge. You always need a high deductible plan or a catastrophe, a go-to plan. It's always recommended to make sure that if I don't know, anything weird happens that's not covered from an auto accident or is not covered from anything else that you're insured for is covered and you don't lose your assets. So the larger picture is just that. Your conceptual mind has to go, hmm, it's not health insurance plus concierge, it's high deductible HSA that pays my concierge. Hmm, good thought. Um, after you replace your health insurance with a high deductible plan and hopefully are able to get a match for an HSA from your employer or match yourself if you are self-employed, then you use your concierge doctor me, unless you're wealthy and you want to do both. Where money doesn't matter, I'm just gonna pay for both, and it doesn't matter, I don't even care. Um, but that's how it used to be is you pay for health insurance because it was health insurance, it paid for things. You you got the care that you wanted and you didn't pay all these other deductibles, and it was cost benefit. Um, and then you paid concierge medicine, but it's not that way anymore. Somewhat thanks to COVID, there are good things that come from bad things. When you look at the evolution from even five years ago to now with concierge medicine, I mean, there's a lot more private clinics coming out. Um, me as a doctor of chiropractic, functional medicine background, functional neurology background, um, musculoskeletal background. I'm getting into concierge so that I can invest in patients. See patients come through to the end feeling their best, being able to advocate for themselves, having a small number of 50. Um, that's where I want to be right now is 50 patients. Um, that is something that never really was happening before. So the new way is bringing practitioners like me on board, going, hey, now I can actually package this up. People's HSAs can pay for this, everybody's getting on a high deductible. Um, this is now kind of the new way of health care. You actually get seen, right? So oftentimes people say, Wow, I love that you have appointments available. And it's more of an immediate access, right? I love that you have short lead times when I need an appointment. I love that you can actually advise me on supplementation. You can order labs, you can order x-rays, you can make referrals to other doctors, and I follow up, right? It's it's basically those things. It's more meaningful and efficient in our conversations. Um, preventative care is what I'm looking for, and that is what you help me with. Looking at blood work and different genetic testing and helping interpret and connecting all of them together because you have the time to do it. Then you put in exercise, then you change diet, you know, physical therapy, stretching, muscle therapy all together. That's where all my patients, they're never like, oh, where's the value? No, it's always, well, how can I get from paying, you know,$20,000,$30,000,$40,000 a year in health insurance to get to pay for you? How do I get from there to paying for you? And what does that look like? So the overall big picture isn't value, it's well, how do I do this? How do I look at things? And you look at your health insurance and go, okay, if I did a if I did a switch to a high deductible plan, what does that look like for my family? And then does my employer match me for HSA or can I open an HSA? And what does that look like? Because there's health savings and health benefits from doing that alone. Um, then you start going, wow, okay, well, now I can use my HSA and I can pay for my annual private membership. Yes, sign me up. That's where I need to get the mindscape of the old way to the new way. We need to, we need to understand and morph with the time. We need to evolve with the time, right? So the last five years has been a huge change. Think about it. When COVID came, nobody was able to really be face to face during all the restrictions. So telemedicine, we started doing this whole landscape of telemedicine, then the laws changed, then you know, things things were unrestricted, then they got restricted. But needless to say, now we have availability via telemedicine, places like um or his.com, hers.com, these are the norm, right? Going to Amazon.com and seeing an MD or a nurse to write you a script is as simple as wow, I have an Amazon.com membership, and it's X amount per year to have the availability to actually talk to a doctor. And oh my goodness, the scripts are cheap and they're delivered right to my door. So that takes away the office visit that you used to have to go see your MD. So you're driving to your MD, you sit in the waiting room, you pay your copay, you go in, you get charged anywhere from a hundred to six hundred dollars for the visit, and they say, Oh, well, you have an ear infection, and here's your script, go down to the pharmacy and fill it. Five minutes, ten minutes, whatever it is. Maybe they order some labs, I don't know. But the visit, that fee gets wiped away. Now the script gets cheaper because it's more available, and guess what? The cost savings goes on to you. So that's where things are really, really changed. They are changing, they are evolving, they have already evolved. Um, so when you're looking at the question of, because I commonly get this question, well, I'm really concerned about getting scripts. Okay. So we just talked about that. There's one way to do it, and that's that way. But guess what? The best thing about integrative healthcare is when we look at your labs and we look at your history and intake, and we go through literally your genetics, your micronutrients, your exposure to things, maybe your DEXA scan and body composition, when we look at all that, we get to go, hey, let's treat this, this, this. I shouldn't say that. Let's support, I don't treat anything in my office. I support body functions. And when I support them, your body gets fixed, which is amazing. So people's cholesterol goes down and they have to go and get off usually their medication, right? We all know what that's talking about. But, anyways, so when you support deficiencies, you should listen to my deficiency podcast if you're curious about it, or have a conversation with me. Deficiencies cause disease. So we support deficiencies based on your labs and what your body is doing or not doing, what your blueprint is from your genetic tests, what your micronutrients say about you that gives you the specific one supplement support. These are the things that I'm confident in saying if we support you, you're gonna get off your scripts. Usually depression goes away, anxiety goes away, IBS goes away, cholesterol comes into where it should be, it starts working for the body, hormones, so bioidentical hormones start decreasing and/or eliminating. You can go on and on and on. Obviously, I can't grow back limbs, that's for God. But if you're looking at treating deficiencies, I should say sorry, I don't treat anything. Supporting deficiencies, when we support the body with the nutrients it needs specifically, it starts going back online. And so I'm not as concerned about scripts, but I know a lot of people are like, what are we gonna do about these and why? But oftentimes what happens is the patient, you comes into the clinic going, oh my gosh, I've been trying to get off this statin drug or this high blood pressure medication or diabetes, metaphor, whatever that is, bioidentical hormones, and because it's expensive, I don't always feel right, and it's just hit and miss. So when we support that in our concierge medicine clinic, right? In our integrative concierge clinic here, those scribes start getting less and less and less. You don't usually need to see the MD as much. Um, that's where it's kind of cost effective in multitudes of ways. Because here in my clinic, I like to see lesses more. I want to see you eating food before taking supplements. I want to see you exercising before taking prescription drugs. I want to see the advocacy um being taught to you so you can start going, oh, Dr. Nicole said this. I'm gonna start trying that because that actually makes sense. And when I do that, I actually sleep good or I poop good or whatever that is. So the old way of doing things is is really no longer. And that's where we have to mentally go, okay, but what is the new way? The new way is high deductible plan, or if you have a health insurance plan that pays for everything, that's fantastic. A high deductible plan, possibly an HSA to pay for concierge medicine, and not just concierge medicine, but concierge medicine that deals with genetic testing, labs, imaging, everything but drugs and surgery, because drugs and surgery is covered by I don't know, your high deductible plan, uh your HSA, um car accidents, uh scripts such as hisandhers.com or Amazon.com. Sorry about that, guys. I thought I'd turn that off. Anyways, so that's where I really, really, really am excited to go through these um frequently asked questions, is people, everybody has this question. I have been getting this is the biggest question I've been getting for the past month is but it's expensive and how do I get there? So that's the concept is it's not expensive that the problem is, it's not the value, it's how do I shift my money and my concepts to embrace this new way, right? So the old way, let's see here. I always put notes down so I don't miss anything. Okay, so let's go through okay, the old way, we already talked about that. The new way, high deductible, HSA, online telemedicine for scripts, concierge with an integrative doctor, aka me, an integrative concierge. Um so I think that's about it on today's facts, frequently asked questions. So if you have questions, continue to write them to me.
