If the idea of pain relief sounds like heat, needles, or “just live with it,” this conversation offers a different angle: light. We’re talking about cold laser therapy, a gentle, non-invasive treatment we use to help calm inflammation, support tissue healing, and speed up recovery, especially when irritated nerves or stubborn joints keep people stuck in the same pain loop.

We walk through what cold laser therapy is, why the specific laser we use is designed for true therapeutic penetration, and how it works at the cellular level by increasing ATP, the energy your cells use to repair. We also get practical about the patient experience: what it feels like (most people don’t feel anything), what you need to do to prep (very little besides getting to bare skin), and how long a session takes (typically 10 to 15 minutes).

Then we dig into the real-world cases where we see the strongest response, including FDA-approved use for carpal tunnel, inflamed discs, sciatica, neck and back pain tied to nerve irritation, and chronic shoulder or knee problems that have people considering surgery. We also cover contraindications, how cold laser differs from other light-based treatments, and what a typical plan looks like, often starting with a six to twelve visit program depending on how acute or chronic the condition is.

If back pain, sciatica, carpal tunnel, or joint pain is slowing you down, listen now and share this with someone weighing injections or surgery. Subscribe, leave a review, and tell us what condition you want us to break down next.

Welcome And Natural Care Focus

SPEAKER_01

Welcome to the Back Talk Podcast, where we talk about your back so it doesn't talk back to you. Hosted by Dr. Peter Paducci, Clinic Director of American Back Center. Each episode explores real solutions for back pain using non-invasive, non-surgical care designed to help you avoid spinal surgery. Helping people throughout the greater Pittsburgh area feel better naturally, without the hardware or the drama.

What Cold Laser Therapy Is

SPEAKER_02

Today we will discuss a light-based therapy that helps calm inflammation, support healing, and boost natural recovery. Welcome, I'm Julie Schwenzer, co-host and producer with Dr. Peter Paduzzi, the clinic director at American Back Centers. Dr. Padozzi, it's so great to be with you. If we could dive in, what is cold laser therapy?

SPEAKER_00

So cold laser therapy, and specifically what we use is the MR4 laser from microlight. And it's one of a kind. It really has a great therapeutic benefit, increasing what's called ATP into the cells, which is a form of energy, allowing the area to heal much quicker than it normally would without

What It Feels Like And Setup

SPEAKER_00

the laser. So we use it on shoulders, knees, really any uh part of the body that needs healing and inflammation reduction. Uh it works exceptionally well for acute where you have a lot of inflammation and healing needs to be done. And even chronic conditions where things are just not uh healing like they should, you know, chronic shoulder issues, as I mentioned, knee issues. We use it for the spine as well. So people that come in with uh inflamed discs, inflamed nerves, we can use it on uh an inflamed carpal tunnel, median nerve, we can use it on an inflamed sciatic nerve and a hot disc per se as well.

SPEAKER_02

What does it feel like? How gentle is it?

SPEAKER_00

Well, it's very gentle. You don't really feel anything. It's a cold laser, so you you don't even feel heat. Um you you feel the uh if we turn the sound on, you can hear the beeping. Uh basically that's just helping us with the cycles and telling us where the patient is, but you really don't feel anything. And uh it's the laser that's penetrating into the area producing the results that we want.

SPEAKER_02

And how do you prep for something like that? And how long can sessions last?

SPEAKER_00

There's really no prep. Um, we just need skin, so we can't go through clothing. Um so we just need to get to the bare skin of the area. And um the sessions usually last, they're they're very quick, only about 10 to 15 minutes max.

SPEAKER_02

Actually, it sounds like an amazing alternative, so some other options. And uh, can you expand too on how it uh the therapy actually interacts with the tissues beneath the skin?

Best Conditions And Disc Protocols

SPEAKER_00

So, I mean, you have to have the uh the enough penetration to actually do something into the uh tissue. So once you get into the area, like I said, it's producing that ATP, that energy in the cells to promote healing and reduction of the inflammation within the cells. So, I mean, it's getting in there, affecting it with the laser um uh energy and uh producing uh positive results for the patient in many areas. And we do use it with our disc uh protocols now as well for patients with herniated disc, and it's now allowed us to treat more difficult cases, cases that patients have uh resorted maybe to epidural injections. Now they don't have to do that because we're getting that more severe inflammatory condition and helping patients with that.

SPEAKER_02

And you touched on some of the conditions or you know, the feelings that people might have that respond best to cold laser therapy. Can you name some of the or expand on that, like the top ones, the the most um, you know, where you've seen the patients really react the best?

SPEAKER_00

Yeah, I mean the most common ones we use for carpal tunnel, carbal tunnel, it's very effective. Uh this laser specifically is FDA approved for carpal tunnel treatment. Um, obviously the sciatic nerve, because we're seeing a lot of uh patients coming in with sciatica in disc problems. So whether you have a disc problem in the neck, a hot disc in the back, uh it works exceptionally well for reducing that inflammation and aiding in quicker healing as well. Uh, and then just patients coming in contemplating shoulder or knee surgery. We've had a lot of great uh results with uh tissue regeneration and reduction of that uh acute inflammation uh or chronic when they have a chronic knee or uh shoulder problem.

Contraindications And Comparing Lasers

SPEAKER_02

Is there anybody you don't recommend using cold laser therapy?

SPEAKER_00

Um there are some contraindications. Uh if you obviously if you had an active cancer on some type of uh skin condition, we wouldn't obviously do the laser over those areas. Uh if someone was pregnant, uh we wouldn't do the laser, you know, in that vicinity as as well. Um those are the typical contraindications, but otherwise, anyone with an acute or chronic uh issue in their body, whether it's knee, shoulder, spine, uh, carpal tunnels we mentioned, uh even tennis elbow or golfer's elbow, medial or lateral epochondylitis works exceptionally well with this because we're reducing the inflammation with the laser in the tendon and the surrounding structures and allowing the area to heal quicker.

SPEAKER_02

And how does cool laser therapy compare to other light-based treatments, like like what you've covered before, invisored therapy?

SPEAKER_00

Yeah, the invisored is more for weight loss. Um, this is totally different. This is more for pain reduction. Uh, so it's a different frequency, different type of laser. Um there's different classes of laser, class three and class four. A lot of it's based off of the effectiveness and the depth of penetration of the laser. So, yeah, so this is more specific for pain reduction and tissue healing as opposed to weight reduction with the Invisoret.

Treatment Plans Results And Getting Started

SPEAKER_02

So depending on what the patient has, do they see improvement much quicker or does it take a lot longer depending on the condition?

SPEAKER_00

Yeah, I mean, obviously, if somebody has a more chronic, uh, they've had the condition for a longer period of time, it's typically going to uh take a little bit longer. We typically have treatment programs consisting of six to twelve treatments we start with. Uh most patients see uh adequate, if not uh exceptional, results after a six or twelve treatment uh uh treatment course. And that's typically what we recommend to start with.

SPEAKER_02

How long have you been using cold laser therapy there?

SPEAKER_00

I've been using it for uh just over five years, the cold laser therapy, and we've had great results with it. Many patient referrals of people that have gone through uh the treatment sessions and have you know done well and told their family members, friends, and everyone they know that there's alternatives to surgery, namely and uh taking medication and steroids.

SPEAKER_02

And for people looking for relief that have been waiting a long time, like how long can they expect when they come in to see if they're a good candidate for it? And then when can they like come back and actually experience the collaser therapy?

SPEAKER_00

Yeah, typically when people come in, well, we do a standard consultation no matter what condition they have. Um, if we don't need any further imaging, like an X-ray, of course we can do the X-ray right here in the office or an MRI. In many cases, we encourage people to bring those in if they have current imaging. And based off of that, we of course do a physical examination. And in many cases, we can treat them the same day, and it's quite possible they could get relief the very same day as well.

SPEAKER_02

Okay, well, Dr. Paduzzi, thank you for breaking all this down so clearly. It sounds like a great option for treatment. Well, thank you.
