confusion starting about an hour ago."},{"startTime":39.11,"endTime":42.75,"body":"His blood pressure on"},{"startTime":39.11,"endTime":42.75,"body":"presentation is blood"},{"startTime":97.9,"endTime":100.46,"body":"pressures that you mentioned"},{"startTime":100.46,"endTime":103.9,"body":"to 230/140, and evidence"},{"startTime":100.46,"endTime":103.9,"body":"of end organ damage"},{"startTime":103.9,"endTime":105.25,"body":"in multiple systems."},{"startTime":105.25,"endTime":108.99,"body":"So he has evidence of CNS"},{"startTime":105.25,"endTime":108.99,"body":"involvement with his delirium,"},{"startTime":108.99,"endTime":111.617,"body":"the kidney damage is evident"},{"startTime":108.99,"endTime":111.617,"body":"by his elevated creatinine"},{"startTime":111.617,"endTime":114.44,"body":"and proteinuria, pressure."},{"startTime":193.13,"endTime":195.13,"body":"So they have a change in the set point"},{"startTime":195.13,"endTime":196.97,"body":"of the cerebral autoregulation."},{"startTime":196.97,"endTime":199.78,"body":"Reducing the blood pressure"},{"startTime":196.97,"endTime":199.78,"body":"rapidly to normal levels"},{"startTime":199.78,"endTime":202.76,"body":"for the regular population"},{"startTime":199.78,"endTime":202.76,"body":"often leads to hypoperfusion"},{"startTime":202.76,"endTime":205.43,"body":"and can lead to watershed"},{"startTime":202.76,"endTime":205.43,"body":"cerebral and"},{"startTime":212.342,"endTime":214.75,"body":"much more easily titrated"},{"startTime":212.342,"endTime":214.75,"body":"than oral therapy,"},{"startTime":214.75,"endTime":217.32,"body":"so I would go with an IV"},{"startTime":214.75,"endTime":217.32,"body":"option over an oral option."},{"startTime":217.32,"endTime":220.37,"body":"Oral options in this setting"},{"startTime":217.32,"endTime":220.37,"body":"really would be inappropriate."},{"startTime":220.37,"endTime":222.762,"body":"So of the choices that you"},{"startTime":220.37,"endTime":222.762,"body":"listed answer C. is what"},{"startTime":222.762,"endTime":225.35,"body":"I would go with, the"},{"startTime":222.762,"endTime":225.35,"body":"20 mg of IV labetalol,"},{"startTime":225.35,"endTime":227.76,"body":"and then I'd start a continuous IV drip."},{"startTime":227.76,"endTime":229.82,"body":"And thinking about some"},{"startTime":227.76,"endTime":229.82,"body":"of the other options,"},{"startTime":229.82,"endTime":232.69,"body":"immediate release nifedipine,"},{"startTime":229.82,"endTime":232.69,"body":"it is particularly potent,"},{"startTime":232.69,"endTime":235.16,"body":"but it's associated with a"},{"startTime":232.69,"endTime":235.16,"body":"higher risk of myocardial"},{"startTime":235.16,"endTime":237.15,"body":"infarction when used in this setting."},{"startTime":237.15,"endTime":240.32,"body":"[Dr. Wiener] Okay, so four of"},{"startTime":237.15,"endTime":240.32,"body":"the options mentioned drugs,"},{"startTime":240.32,"endTime":243.238,"body":"you excluded the three"},{"startTime":240.32,"endTime":243.238,"body":"oral drugs and you chose"},{"startTime":243.238,"endTime":246.69,"body":"the parenteral therapy,"},{"startTime":243.238,"endTime":246.69,"body":"what about plasmapheresis?"},{"startTime":246.69,"endTime":248.63,"body":"This patient has shistocytes,"},{"startTime":248.63,"endTime":250.74,"body":"does that make me think that"},{"startTime":248.63,"endTime":250.74,"body":"this patient has some other"},{"startTime":250.74,"endTime":253.58,"body":"disorder besides just simple hypertension?"},{"startTime":253.58,"endTime":255.17,"body":"[Dr. Handy] Well, so"},{"startTime":253.58,"endTime":255.17,"body":"malignant hypertension"},{"startTime":255.17,"endTime":257.11,"body":"or hypertensive emergency can cause"},{"startTime":257.11,"endTime":259.07,"body":"an intravascular hemolytic anemia,"},{"startTime":259.07,"endTime":262.2,"body":"so that is not abnormal for"},{"startTime":259.07,"endTime":262.2,"body":"the case that's presenting,"},{"startTime":262.2,"endTime":263.9,"body":"but in terms of plasmapheresis,"},{"startTime":263.9,"endTime":265.799,"body":"that would be warranted"},{"startTime":263.9,"endTime":265.799,"body":"if this were a case of"},{"startTime":265.799,"endTime":268.59,"body":"thrombotic thrombocytopenic"},{"startTime":265.799,"endTime":268.59,"body":"purpura or TTP,"},{"startTime":268.59,"endTime":270.01,"body":"but that's unlikely in this case"},{"startTime":270.01,"endTime":271.87,"body":"and he has normal platelets as well."},{"startTime":271.87,"endTime":273.79,"body":"[Dr. Wiener] Great, studies"},{"startTime":298.14,"endTime":301.57,"body":"that frequently include"},{"startTime":298.14,"endTime":301.57,"body":"subanalyses by race and sex,"},{"startTime":301.57,"endTime":303.67,"body":"but we know that there's"},{"startTime":301.57,"endTime":303.67,"body":"a lot of variability"},{"startTime":303.67,"endTime":306.17,"body":"within these groupings,"},{"startTime":303.67,"endTime":306.17,"body":"not only with race,"},{"startTime":306.17,"endTime":310.62,"body":"but also with social constructs"},{"startTime":306.17,"endTime":310.62,"body":"and socioeconomics and even"},{"startTime":310.62,"endTime":314.13,"body":"variability in how some of"},{"startTime":310.62,"endTime":314.13,"body":"these determinants are defined."},{"startTime":314.13,"endTime":317.44,"body":"How do you use these data"},{"startTime":314.13,"endTime":317.44,"body":"from these large studies,"},{"startTime":317.44,"endTime":319.46,"body":"if at all, in determining the 130/80"},{"startTime":334.13,"endTime":336.34,"body":"and hypertensive"},{"startTime":334.13,"endTime":336.34,"body":"emergency as in this case,"},{"startTime":336.34,"endTime":338.89,"body":"is defined as an acutely marked elevation"},{"startTime":338.89,"endTime":341.64,"body":"in blood pressure with"},{"startTime":338.89,"endTime":341.64,"body":"target organ damage,"},{"startTime":341.64,"endTime":344.74,"body":"so fairly far advanced vascular disease."},{"startTime":344.74,"endTime":346.33,"body":"Epidemiology studies, as you point out,"},{"startTime":346.33,"endTime":348.73,"body":"have consistently shown that hypertension"},{"startTime":348.73,"endTime":350.62,"body":"disproportionately affects blacks,"},{"startTime":350.62,"endTime":353.73,"body":"and in particular, black males"},{"startTime":350.62,"endTime":353.73,"body":"who also have higher rates of"},{"startTime":353.73,"endTime":357.23,"body":"complications, such as"},{"startTime":353.73,"endTime":357.23,"body":"myocardial infarction, stroke,"},{"startTime":357.23,"endTime":358.6,"body":"and sudden death."},{"startTime":358.6,"endTime":361.77,"body":"But as you also highlighted,"},{"startTime":358.6,"endTime":361.77,"body":"Charlie, race is a really poor"},{"startTime":361.77,"endTime":364.63,"body":"surrogate for the biological"},{"startTime":361.77,"endTime":364.63,"body":"basis of any disease"},{"startTime":364.63,"endTime":366.64,"body":"or response to therapy."},{"startTime":366.64,"endTime":369.74,"body":"There's growing recognition of"},{"startTime":366.64,"endTime":369.74,"body":"the inappropriate use of race"},{"startTime":369.74,"endTime":372.29,"body":"in diagnostic or therapeutic decisions"},{"startTime":372.29,"endTime":374.62,"body":"as it is a self-defined construct"},{"startTime":374.62,"endTime":377.23,"body":"and less likely biological in basis"},{"startTime":377.23,"endTime":379.4,"body":"than socioeconomic in origin."},{"startTime":379.4,"endTime":381.57,"body":"And really what we're thinking"},{"startTime":379.4,"endTime":381.57,"body":"about here in this discussion"},{"startTime":381.57,"endTime":384.17,"body":"is race-based medicine as a reflection"},{"startTime":384.17,"endTime":386.49,"body":"of the structural racism that's permeated"},{"startTime":386.49,"endTime":389.77,"body":"our medical care system"},{"startTime":386.49,"endTime":389.77,"body":"for the past 400 years."},{"startTime":389.77,"endTime":393.75,"body":"Socioeconomic disparities that"},{"startTime":389.77,"endTime":393.75,"body":"have grown out of that global"},{"startTime":393.75,"endTime":396.434,"body":"structural racism are much"},{"startTime":393.75,"endTime":396.434,"body":"more likely to contribute"},{"startTime":396.434,"endTime":399.33,"body":"to the prevalence of hypertension"},{"startTime":396.434,"endTime":399.33,"body":"and its complications,"},{"startTime":399.33,"endTime":401.35,"body":"including hypertensive emergencies"},{"startTime":401.35,"endTime":403.59,"body":"among self-identified blacks."},{"startTime":403.59,"endTime":407.14,"body":"So rather than using a"},{"startTime":403.59,"endTime":407.14,"body":"race-based decision algorithm in"},{"startTime":407.14,"endTime":408.78,"body":"pursuing therapeutic goals,"},{"startTime":408.78,"endTime":411.49,"body":"my view is to really tailor"},{"startTime":408.78,"endTime":411.49,"body":"the treatment to a patient's"},{"startTime":411.49,"endTime":413.58,"body":"individual hypertensive phenotype."},{"startTime":413.58,"endTime":415.2,"body":"We will touch more on that in a little bit"},{"startTime":415.2,"endTime":417.16,"body":"I think in one of the later questions."},{"startTime":417.16,"endTime":420.02,"body":"And most important, to develop"},{"startTime":417.16,"endTime":420.02,"body":"a trusting relationship"},{"startTime":420.02,"endTime":422.02,"body":"with the patient either directly,"},{"startTime":422.02,"endTime":424.28,"body":"or through another member of the care tremendous"},{"startTime":435.07,"endTime":437.93,"body":"progress in improving the rates of"},{"startTime":437.93,"endTime":439.97,"body":"complications from hypertension,"},{"startTime":439.97,"endTime":442.64,"body":"but heart disease is still the"},{"startTime":439.97,"endTime":442.64,"body":"most common cause of death in"},{"startTime":442.64,"endTime":444.94,"body":"the United States, and"},{"startTime":442.64,"endTime":444.94,"body":"we know that adherence"},{"startTime":444.94,"endTime":447.263,"body":"to oral medications like"},{"startTime":444.94,"endTime":447.263,"body":"you started to mention in"},{"startTime":447.263,"endTime":449.41,"body":"the outpatient setting can be challenging."},{"startTime":449.41,"endTime":451.89,"body":"How do you think we can"},{"startTime":449.41,"endTime":451.89,"body":"improve adherence with therapy?"},{"startTime":451.89,"endTime":454.262,"body":"And what do you see as"},{"startTime":451.89,"endTime":454.262,"body":"the major goals over"},{"startTime":454.262,"endTime":457.25,"body":"the
