Before I left for abroad, I spent weeks going back and forth with a health insurance broker trying to choose a plan. It felt impossible at the time. And finally he stopped and asked, What is your biggest fear here? And I told him, I move abroad and something happens. I get really sick and I want to come back. And he said, Then you'll come back. And that conversation really changed everything for me because I realized that the fear wasn't really about health insurance. It was about things not going as planned. And health insurance, the right plan, is exactly what protects you from that. Today I'm walking through everything I learned about health insurance as a physician living abroad for five years in 17 different countries, with 100% of my health care being outside of the US. Health insurance is one of the first practical questions that come up when physicians start seriously thinking about living abroad. And it is one that stops a lot of people in their tracks because it feels like a wall of complexity sitting between where they are and where they want to be. And I want to make it feel much smaller today. Not by oversimplifying it, but by walking you through it the way I wish someone had walked me through it before I left the US. I also want to say something up front. Insurance rules change, right? So what I share today reflects my understanding as of now, but this is a space where you need current and personalized guidance. And I work with brokers who specialize in international health insurance for physicians and expats or U.S. immigrants. And I can connect you to people who can help you navigate your specific situation. With that said, I want to start with the conversation that changed how I thought about health care abroad entirely. When I was preparing to leave the country, I spent weeks going back and forth with a health insurance broker. We were comparing plans and going through options, weighing premiums and deductibles and coverage areas. And as a nomad who didn't feel compelled to settle in any particular country, it felt impossible at the time to choose. Every option raised a new question, every question raised a new fear, and round and round we went. And then finally, the broker stopped and asked me, What is your biggest fear here? And I told him, I move abroad, something happens to my health, I get really sick, and I want to come back to the US. And he said, Then you'll come back. And I was like, Oh, that was it. It's that simple. You know, if something happens and you need to come back, you come back. And what I realized in that moment is that my fear wasn't really about health insurance at all. It was about uncertainty, about things not going as planned, about whether I could reverse the decision if I needed to. And the answer is yes. Any decision that you make about moving abroad is reversible. None of it has to be permanent. And understanding that really changed everything for me. It relieved a lot of pressure and stress. And then the broker explains something that gave me real peace of mind, something I believe most physicians don't know. Under current US law, a permanent move back to the US from a foreign country is considered a qualifying life event. And that means that it triggers a 60-day special enrollment period that allows you to select a health insurance plan through healthcare.gov or your estate exchange outside of the normal open enrollment window. So, what that means in practice, if you move abroad and something happens and you need to come back, your enrollment window for obtaining health insurance in the US opens the day you officially relocate back to the US and you have 60 days to select the plan. You just need the documentation confirming the move. So knowing that really removed my biggest fear. The return option is always there. And that is really what health insurance is for, right? Not just covering the cost of your care, but making sure that you always have options. Now let's talk about the practical transition. What happens to your current coverage when you leave hospital employment, and how do you move from there to a plan that works for your life abroad? When you leave your employer, your employer-sponsored healthcare coverage ends. So the timing of when it ends depends on your employer's specific policy. Some end coverage on your last day of work, others will extend it through the end of that month. And this is worth confirming before you give your notice. One practical detail worth knowing in a previous episode, we talked about timing your exit on the first of the month so that your employer coverage runs through the end of that month. That same principle can apply here. Understanding exactly when your employer coverage ends lets you plan your transition to a new insurance plan with no gap in coverage. You want the new plan to begin the moment the old one ends, or you want there to be some overlap. The real fear in this transition, a lot of times, is not the logistics, it's more of the unknown. You know, will the new plan cover my health conditions? Will the quality of care be what I need? Will I actually be okay? And those are the right questions to be asking, and they are what the rest of this episode is about. When you are choosing health insurance as a physician living abroad, here is the framework I use and share with the physicians I work with. Number one is to identify your coverage needs first. Before you look at any plan, get clear on what matters most to you. Are you looking for emergency-only coverage? Are you looking for comprehensive care that includes routine and preventative services? Do you have pre-existing conditions that need to be covered? Are you planning to stay in one country or multiple? The answers to these questions determine which type of plan makes sense for you. Number two is deciding between international health insurance versus local insurance. So there are two main paths. International health insurance offers worldwide coverage. So it travels with you. If you are moving between countries or spending time in multiple places throughout the year, this is usually the right choice. These plans tend to be more expensive than local coverage, but they provide continuity regardless of where you are. Local health insurance, so purchasing a plant in the country where you will be living, can be more affordable and may provide excellent coverage for the time that you are in that specific country. And so many countries have robust private insurance markets with access to high-quality care at a fraction of U.S. costs. So if you are settling in one place for an extended period, a local plan may be the more practical and affordable option. I myself use international health insurance as a nomad and have found that having access to the best doctors all around the world suits me and my lifestyle. Number three is to consider your pre-existing conditions. So if you have pre-existing conditions, look carefully at how each plan handles them. Some international plans will cover pre-existing conditions from day one. Others have waiting periods, some exclude them entirely. This is a non-negotiable item to investigate before choosing a plan, and it is exactly the kind of detail a broker who specializes in coverage will know how to navigate. Number four is to consider emergency and evacuation coverage. You want to look specifically for plans that include emergency and evacuation coverage in the event of a serious illness or an injury. This benefit covers transportation to the nearest appropriate medical facility or repatriation back to the US if needed. This is the practical coverage that addresses the fear that I described in the beginning of this episode. And it is the plan that says if something happens, you have options, including coming home. Another piece that my international insurance has is that if I have an emergency abroad, they will also fly one of my family members out to be with me. So having different contingencies in your insurance plan is really good. Okay, number five is to consider your network of providers. Understand where you can receive care under your plan. Many international plants have networks of preferred providers in major cities around the world. Some will include telehealth services. So if you are planning to spend time in more remote areas, you want to verify coverage there specifically. And in my personal experience, access to providers in most of the countries I've lived in has been surprisingly straightforward. In many places, I've been able to communicate with my doctors directly or via WhatsApp or text. And the ease of access has been one of the genuinely pleasant surprises of healthcare abroad. Number six is understanding the full cost picture. So monthly premiums are only one part of the cost, but you want to look at deductibles, co-pays, and out-of-pocket maximums, just as you do with your insurance coverage within the US, many international plans will offer tiered options and you choose the level of coverage based on your needs and your budget. So a broker can help you compare the full cost picture across plans rather than just the headline premium. I will say that compared to folks who self-insure in the US, meaning that they don't have an employer-sponsored plan, my annual healthcare insurance premium, which covers me around the world, costs less than their US only coverage plan. Just so you can get an idea of the cost difference. And even with looking at plans, let's say, for example, within Mexico, a private insurance plan that just covers healthcare in Mexico compared to my international plan, which covers me around the world. There's probably about a $2,000 difference, which depending on your budget and how much you're willing to pay for healthcare, that can mean a lot of money to you. That can mean not much. It just depends on the amount of flexibility that you want when it comes to your healthcare options. That's just to give you an idea of what those differences and costs may be. I want to spend a moment on this because it's a fear that I think can sometimes sit underneath some of the practical questions about coverage and costs. Will the care actually be good enough? And I have received, you know, all of my health care outside of the US for over five years across 17 countries. So that's every preventative and routine appointment, every dental visit, urgent care, including necessary medical procedures. And I say this as someone who is entirely U.S. trained from kindergarten right through both of my fellowships. The best care that I have ever received has been outside of the US. More time given to the appointment, more direct access to providers, more patient-centered care. In many places that I live, I reach out to my doctor directly via WhatsApp or text. Getting an appointment does not take weeks. The process of actually receiving care is easier, faster, and in most cases, more affordable. And that is not a direct criticism of U.S. Medicine. You know, I trained in it and I respect it from that point of view, but it is an honest reflection of what I've experienced receiving health care in Latin America and across Europe. Excellent healthcare exists all over the world. And in many places, the private healthcare market, which is what most US immigrants access, is exceptional. And accessing it is one of the practical realities of location-independent life that is often much easier than physicians expect. So knowing this, not just believing it, but having lived it is one of the most liberating parts of a location-independent life. So the fear of being without good care is something natural that I think can come up before you leave the country. But the reality is almost always very different. And I want to close with something that I think matters beyond just the practical insurance questions. The US healthcare system is undergoing significant changes right now. Eligibility requirements, Medicaid enrollment, coverage provisions, these are all shifting in ways that affect many people. And I'm not going to get political about it, but I will say this. Having access to healthcare systems beyond the US is not just a practical necessity for physicians living abroad. It is a form of security. Knowing that excellent, affordable, accessible care exists in the world and that you have the ability to access it is its own kind of freedom. No matter where you live in the world right now, having options matters. Dependence on any single system, whether it's your employer, your country's health care, or your country's political decisions about coverage, it carries risk. Location independence is not just about where you live or where you work, it's also about having choices in every aspect of your life.
