In the first episode, I introduce myself, my background in emergency medicine, and the mission behind this podcast. I also outline the structure of future episodes — including clinical cases, practical tips for new and experienced APPs, discussions on clinician well-being, and real-world challenges in emergency medicine. This episode sets the foundation for a practical, honest, and educational podcast designed to help APPs grow with confidence.
Have you ever had a patient who looked completely stable, and then hours later you realized something important was evolving, or walked out of a ship thinking about the one case you might have approached differently? Emergency medicine is full of moments where we are forced to make decisions with limited information, constant interruptions, and real consequences. This podcast is about those moments.
SPEAKER_01Welcome to Thrive and Survive Podcasts. I'm Robert Arledge, a PA, broadcasting from a small office upstairs in a more than a century-old farmhouse on a working hobby farm, surrounded by chickens, beehives, and a greenhouse. Out here, the work is practical, the lessons learned are real, and that's exactly how we approach emergency medicine. This podcast is designed for advanced practice providers in emergency medicine, particularly those early in their careers, but also experienced clinicians who want to continue refining how they think, practice, and navigate the emergency department. Emergency medicine is one of the most challenging environments in healthcare. We make decisions with limited information, constant interruptions, and significant consequences. The goal of this podcast is to help clinicians develop structured clinical thinking, practical career insight, and a deeper understanding for the systems we work in. This podcast is intended only for practicing clinicians or students currently in medical training. It is not medical advice for the general public. Welcome to the first episode of Thrive and Survive Emergency Medicine APP podcasts. Each episode is typically 8 to 10 minutes and is organized around three core pillars that form the foundation of the Thrive and Survive philosophy. The first pillar is career insight. Emergency medicine is demanding both intellectually and emotionally. In this section, we will talk about clinician well-being, managing stress during shifts, and practical strategies for maintaining longevity in this field. We will also discuss how to build structured clinical thinking pathways when practicing in an environment filled with interruptions, incomplete histories, consultants calling, family meetings needing updates, and multiple patients competing for attention. Developing a reliable decision-making framework is essential for reaching the safest possible disposition for each patient. Equally important, we will talk about how to maintain balance between work and home life and how to avoid the trap of total immersion in your career. The second pillar is system insight. Emergency medicine does not operate in isolation. Clinicians must understand the systems that support patient care. In this section, we will discuss topics such as electronic medical record platforms, AI documentation tools, RVUs, CPT coding, and workflow efficiency. Understanding these systems helps clinicians not only work more efficiently, but also navigate the operational realities of modern emergency medicine. The third pillar is clinical discussion. Each episode we will discuss emergency medicine cases and clinical reasoning. These discussions are based on the combined experiences of the clinicians working in the emergency departments. Sometimes these will be cases from my own accumulation of experience. Other times they will be cases submitted by listeners. Listeners are welcome and encouraged to submit interesting cases for discussion on the podcast. However, there are strict HIPAA guidelines that must be followed. All cases must be fully de-identified. A case submission form will be available on the website so the case can be structured appropriately for submission. The best cases are often the ones that challenge us, subtle findings that ultimately led to significant diagnoses or situations where the case initially seemed benign but resulted in unexpected outcome. In emergency medicine, many of these are the most valuable lessons that you will learn. I often like to say, sometimes you learn by getting burned. Together, these three pillars, career insight, system insight, and clinical insight form the framework for the Thrive and Survive podcast. From time to time, I'll bring on guest clinicians, APPs, physicians, and other healthcare professionals to bring fresh insights and we'll reward experience. Our goal is simple to help emergency clinicians develop skills needed to not only survive in the emergency department, but truly thrive.
