A hospital stay can “fix” the diagnosis and still leave someone weaker than before. We sit down with Jonathan Gasso, owner of PARC Home Care and Hospice, to get practical about what happens next and how families can build the right support so an older parent can stay home longer, safer, and with more confidence.
We talk through what skilled home health care actually looks like in real life: physical therapy and occupational therapy that focus on everyday function, skilled nursing for medication changes, wound care, catheter care, infusion care, and speech therapy for swallowing and communication challenges after stroke. We also clear up confusing discharge terms like SNF, rehab, and skilled nursing facility, and explain why “hospital atrophy” is real and why it matters when someone comes home.
Then we shift to one of the hardest crossroads families face: when progress plateaus and comfort becomes the goal. Jonathan explains how hospice eligibility works, why the “six months” guideline is widely misunderstood, and why having a coordinated hospice team can be a relief not only for the patient, but for the entire family. We also reflect on how faith, forgiveness, and spiritual care can shape the end-of-life journey with more peace and less fear.
