Moises Pérez-Martínez is a member of Advocate Community Providers (ACP)’s leadership team, work with the largest healthcare network in the city (ACP), which was formed with the support of Gov. Cuomo’s Delivery System Reform Incentive Payment (DSRIP) program which is in the beginning stages of launching. The DSRIP goal is to strengthen the Medicaid safety net through preventative care, higher health literacy, better overall health outcomes and lower costs. ACP is responsible for over 100,000 lives in Manhattan, many of them concentrated in Harlem and Chinatown, which has the highest concentration of “hot spots” in Manhattan and nearly a quarter across other boroughs. ACP is the largest and only network created from DSRIP that is physician-run – with over 2,000 physicians – and nearly 700,000 patients citywide. https://somoscommunitycare.org/
