And today I want to talk"},{"startTime":17.936,"endTime":21.368,"body":"about five pillars of trauma informed care and"},{"startTime":21.424,"endTime":25.144,"body":"specifically why it's important to understand for therapists"},{"startTime":25.272,"endTime":29.09,"body":"and, and for clients. I alluded to"},{"startTime":29.21,"endTime":33.042,"body":"in numerous conversations whenever I talk about this, but in the last podcast to"},{"startTime":33.066,"endTime":36.338,"body":"the fact that this phrase is thrown around more and"},{"startTime":36.394,"endTime":40.242,"body":"more. And I want to dispel a myth at the start as I"},{"startTime":40.266,"endTime":43.97,"body":"talk about five pillars of trauma informed care. The first is"},{"startTime":44.01,"endTime":47.65,"body":"that if you are a client and you have trauma or"},{"startTime":47.69,"endTime":51.362,"body":"abuse or some kind of adverse childhood experience in your"},{"startTime":51.386,"endTime":55.028,"body":"life and you go to the website of a therapist and it"},{"startTime":55.084,"endTime":58.9,"body":"says, I specialize in trauma, it's important to"},{"startTime":58.94,"endTime":62.74,"body":"check out what that means. Oftentimes that means that"},{"startTime":62.78,"endTime":66.148,"body":"a therapist will have had a course in trauma,"},{"startTime":66.324,"endTime":69.94,"body":"perhaps during graduate school or maybe afterwards, or it"},{"startTime":69.98,"endTime":73.444,"body":"means that they read a book about it and they didn't even have a course"},{"startTime":73.612,"endTime":77.316,"body":"and that they simply have an interest in that."},{"startTime":77.468,"endTime":81.196,"body":"So there's a difference between someone who works with"},{"startTime":81.268,"endTime":85.1,"body":"trauma, works with abuse, even says that"},{"startTime":85.14,"endTime":88.476,"body":"they specialize in it, and being trauma"},{"startTime":88.588,"endTime":92.38,"body":"informed. And the difference is, as I think you'll hear"},{"startTime":92.42,"endTime":96.06,"body":"in this podcast, pretty profound. And it can make all the difference"},{"startTime":96.14,"endTime":99.58,"body":"in terms of the kind of progress"},{"startTime":99.74,"endTime":103.5,"body":"and outcome that clients have in therapy."},{"startTime":103.58,"endTime":106.93,"body":"It's also very important for therapists to, to understand"},{"startTime":107.63,"endTime":111.11,"body":"because it helps therapists to be proactive in serving their"},{"startTime":111.15,"endTime":114.774,"body":"clients. But it also is a great way of understanding"},{"startTime":114.822,"endTime":118.294,"body":"our colleagues and our peers that when someone says trauma"},{"startTime":118.342,"endTime":121.61,"body":"informed, there's a certain set of assumptions"},{"startTime":122.03,"endTime":125.798,"body":"for what that means. And so these five pillars are taken from the literature,"},{"startTime":125.974,"endTime":129.622,"body":"taken from different ways that the field talks about these, but"},{"startTime":129.646,"endTime":133.462,"body":"I've put them into my own language. And this is really what restoring the"},{"startTime":133.486,"endTime":136.822,"body":"soul is about. As we say we are trauma"},{"startTime":136.886,"endTime":140.438,"body":"informed, number one. Let's discuss each of these. Number"},{"startTime":140.494,"endTime":144.31,"body":"one. Trauma informed care is based"},{"startTime":144.35,"endTime":148.182,"body":"upon an understanding and a taking into account"},{"startTime":148.286,"endTime":151.894,"body":"of a person's life experience on"},{"startTime":151.982,"endTime":155.254,"body":"their present experience. I'll say that again. It's"},{"startTime":155.302,"endTime":158.998,"body":"understanding and taking into account the impact of life"},{"startTime":159.054,"endTime":162.784,"body":"experience on our present experience and specifically"},{"startTime":162.832,"endTime":166.512,"body":"on the central nervous system and the brain. In"},{"startTime":166.536,"endTime":170.112,"body":"other words, our life, our childhood,"},{"startTime":170.256,"endTime":174.096,"body":"the way that we went through, our psychosocial development,"},{"startTime":174.248,"endTime":177.824,"body":"all of that has impacted who we are today, including"},{"startTime":177.912,"endTime":181.52,"body":"how we see the world, how we respond to the world, how we attach"},{"startTime":181.6,"endTime":185.28,"body":"to others in relationship. Now, this"},{"startTime":185.32,"endTime":188.71,"body":"may seem like a really obvious thought forever."},{"startTime":188.79,"endTime":192.182,"body":"Therapists seem to have been talking about, you know, from Sigmund Freud"},{"startTime":192.246,"endTime":195.99,"body":"onward, how your mother treated you or what happened to you when you were little"},{"startTime":196.03,"endTime":199.75,"body":"and that was the cause of your psychological problems. Now"},{"startTime":199.87,"endTime":203.446,"body":"we understand in far greater detail and"},{"startTime":203.518,"endTime":207.35,"body":"with greater scientific foundation that it's not just a"},{"startTime":207.39,"endTime":211.142,"body":"theory or a hypothesis like Freud's id, ego"},{"startTime":211.206,"endTime":214.992,"body":"and superego, but, but it's actually a way that"},{"startTime":215.016,"endTime":218.832,"body":"our life experiences, shapes our body, shapes our"},{"startTime":218.856,"endTime":222.592,"body":"nervous system. We call this in the field epigenetics. So genetics are what"},{"startTime":222.616,"endTime":226.384,"body":"are given to us. And the simple way of"},{"startTime":226.392,"endTime":230.24,"body":"talking about epigenetics is taking what was given to us through"},{"startTime":230.28,"endTime":234.112,"body":"genetic material and then adding in life experience. And how that"},{"startTime":234.136,"endTime":237.632,"body":"life experience on top of our genetics actually reshapes our"},{"startTime":237.656,"endTime":241.13,"body":"genetics and, and gives us a different set of"},{"startTime":241.17,"endTime":244.682,"body":"coping mechanisms than we would have had. And then it also shapes our"},{"startTime":244.706,"endTime":248.522,"body":"genetics in a way where we pass on something generationally that would"},{"startTime":248.546,"endTime":252.266,"body":"have been very different without that adverse or negative"},{"startTime":252.378,"endTime":255.482,"body":"experience. So this idea of life"},{"startTime":255.586,"endTime":258.986,"body":"experience shaping our current"},{"startTime":259.138,"endTime":262.73,"body":"experience and psychological struggles, mental health issues, is"},{"startTime":262.77,"endTime":266.502,"body":"rooted in interpersonal neurobiology. Kurt is a psychiatrist and I love how he says that he"},{"startTime":295.496,"endTime":298.96,"body":"preaches the gospel of neuroscience. Interpersonal"},{"startTime":299.12,"endTime":302.272,"body":"neurobiology is this idea that every"},{"startTime":302.376,"endTime":305.62,"body":"interaction begins to shape"},{"startTime":305.96,"endTime":309.456,"body":"how we are present within ourselves, how our"},{"startTime":309.608,"endTime":313.408,"body":"soothing mechanisms, our attachment mechanisms, our"},{"startTime":313.464,"endTime":316.736,"body":"perception of the world, the safety of relationships, how that's"},{"startTime":316.768,"endTime":320.564,"body":"all shaped. So trauma informed. Pillar"},{"startTime":320.612,"endTime":324.276,"body":"number one, understanding. Taking into account the impact"},{"startTime":324.428,"endTime":328.26,"body":"of life experience on the central nervous system and brain. And somebody might be"},{"startTime":328.3,"endTime":332.052,"body":"saying, does that include positive experience? And"},{"startTime":332.076,"endTime":335.316,"body":"I would say absolutely, both in terms of people"},{"startTime":335.388,"endTime":338.852,"body":"who did not have significant trauma, abuse,"},{"startTime":338.916,"endTime":342.18,"body":"neglect, or adverse experiences, but"},{"startTime":342.22,"endTime":345.8,"body":"also the way that we can have a gained attachment or an"},{"startTime":345.84,"endTime":349.624,"body":"earned secure attachment through positive life experiences."},{"startTime":349.672,"endTime":353.4,"body":"This can move in either direction, but in terms of mental health issues,"},{"startTime":353.52,"endTime":356.984,"body":"not necessarily mental health, wellness or mental health"},{"startTime":357.072,"endTime":360.808,"body":"specifically, the life experiences that we often"},{"startTime":360.864,"endTime":364.536,"body":"deal with in the therapeutic relationship and in the therapy room"},{"startTime":364.608,"endTime":367.5,"body":"are negative or adverse experiences."},{"startTime":368.24,"endTime":371.704,"body":"The second pillar of a trauma informed"},{"startTime":371.752,"endTime":375.368,"body":"approach is that in light of this knowledge"},{"startTime":375.544,"endTime":379.0,"body":"that our life experiences shape our struggles and"},{"startTime":379.04,"endTime":382.488,"body":"our real experience. Today as"},{"startTime":382.544,"endTime":386.312,"body":"therapists, we anticipate that clients"},{"startTime":386.376,"endTime":389.56,"body":"will be activated, dysregulated or"},{"startTime":389.6,"endTime":393.0,"body":"distressed under certain caregiving"},{"startTime":393.16,"endTime":396.912,"body":"situations. We anticipate that, that activation,"},{"startTime":396.976,"endTime":400.528,"body":"distress and dysregulation are going to come up when we"},{"startTime":400.584,"endTime":403.76,"body":"seek care, because seeking care is"},{"startTime":403.8,"endTime":407.552,"body":"inherently vulnerable. I'll never forget the very first time I"},{"startTime":407.576,"endTime":410.944,"body":"went to counseling as an adult and I walked in and there was a"},{"startTime":410.952,"endTime":414.624,"body":"relatively small waiting room, even though there were like five psychologists"},{"startTime":414.672,"endTime":418.496,"body":"in this practice. And it was probably three"},{"startTime":418.568,"endTime":421.824,"body":"to five people. I don't remember exactly, and maybe there was a couple, but I"},{"startTime":421.832,"endTime":425.574,"body":"remember this would have been the year 1989. And I remember"},{"startTime":425.702,"endTime":428.01,"body":"sitting in this very cramped"},{"startTime":429.55,"endTime":433.142,"body":"waiting room and there seemed to be like an AM radio"},{"startTime":433.206,"endTime":436.902,"body":"station playing in the background that I don't remember if it was music or"},{"startTime":436.926,"endTime":440.422,"body":"talk, but it was like bad quality. And there were these old magazines"},{"startTime":440.486,"endTime":443.622,"body":"there and I didn't want to look to my left or to my right. You"},{"startTime":443.646,"endTime":446.886,"body":"know, I'm looking out of the corner of my eye and I'm thinking, I'm glad"},{"startTime":446.918,"endTime":450.366,"body":"that nobody in here is anybody. I know. It was really"},{"startTime":450.438,"endTime":453.534,"body":"vulnerable. There was a lot of anxiety there."},{"startTime":453.702,"endTime":456.718,"body":"And a trauma informed approach"},{"startTime":456.854,"endTime":460.654,"body":"anticipates that the simple act of picking"},{"startTime":460.702,"endTime":464.494,"body":"up the phone, logging onto the website, showing up at the"},{"startTime":464.502,"endTime":467.902,"body":"counseling office, or in many cases now, today, telehealth"},{"startTime":468.046,"endTime":471.198,"body":"that that is an incredibly vulnerable thing and"},{"startTime":471.254,"endTime":474.206,"body":"therefore that will likely cause"},{"startTime":474.318,"endTime":477.996,"body":"activation. You add to this"},{"startTime":478.068,"endTime":481.48,"body":"fact that then people may be talking about"},{"startTime":481.78,"endTime":485.244,"body":"shameful material or things that they have experienced shame"},{"startTime":485.292,"endTime":488.86,"body":"around. They may be talking about a broken relationship,"},{"startTime":488.94,"endTime":492.652,"body":"an addiction, some kind of struggle in their life that"},{"startTime":492.676,"endTime":496.476,"body":"they're not proud of, and that takes this potential"},{"startTime":496.508,"endTime":500.22,"body":"for activation and it's like throwing gas on that. And"},{"startTime":500.26,"endTime":502.48,"body":"so as I've talked about in another context,"},{"startTime":503.41,"endTime":506.71,"body":"that this need to understand"},{"startTime":507.25,"endTime":511.002,"body":"what's happening in a person's body and the anticipation of"},{"startTime":511.026,"endTime":514.442,"body":"that then actually allows us to create some"},{"startTime":514.466,"endTime":518.31,"body":"interventions and to be proactive in a way where"},{"startTime":518.69,"endTime":522.346,"body":"we're helping to manage people's dysregulation so that they're"},{"startTime":522.378,"endTime":525.978,"body":"not walking into counseling sessions already traumatized"},{"startTime":526.074,"endTime":529.78,"body":"based on what's happening. I"},{"startTime":529.82,"endTime":533.236,"body":"talked about in a previous podcast about going to the dentist"},{"startTime":533.348,"endTime":537.14,"body":"and feeling so vulnerable when the dentist simply took"},{"startTime":537.18,"endTime":540.804,"body":"pictures of my tooth and posted them on the website, telling me that they were"},{"startTime":540.812,"endTime":544.5,"body":"going to do that, but not asking for permission, not having any kind of"},{"startTime":544.54,"endTime":548.132,"body":"process to engage me with that. Another example at the dentist, and you can tell"},{"startTime":548.156,"endTime":551.956,"body":"that I do not like going to the dentist, is how"},{"startTime":552.108,"endTime":555.86,"body":"at my dentist they will come alongside the chair while I'm lying down"},{"startTime":555.9,"endTime":559.438,"body":"and present to me, the information, pictures of X rays or"},{"startTime":559.494,"endTime":563.326,"body":"whatever the diagnosis is, and then tell me how many thousands of dollars it's"},{"startTime":563.358,"endTime":567.022,"body":"going to cost and ask me in that vulnerable position to make a"},{"startTime":567.046,"endTime":570.25,"body":"financial decision before I even get up."},{"startTime":570.55,"endTime":574.158,"body":"And that is something where a person who"},{"startTime":574.214,"endTime":577.95,"body":"has trauma is likely to either go into a fight or flight"},{"startTime":578.03,"endTime":581.422,"body":"mode, which might mean that I just want to get out of here or I"},{"startTime":581.446,"endTime":585.07,"body":"want to. I become angry and I'm not going to make a wise decision or"},{"startTime":585.11,"endTime":588.784,"body":"I shut down. And in my instance with me being"},{"startTime":588.872,"endTime":592.448,"body":"a nice guy and I want to not ruffle any feathers and I"},{"startTime":592.504,"endTime":595.872,"body":"actually wanted to say, no, I need some time to think about this. But in"},{"startTime":595.896,"endTime":599.632,"body":"the event of the experience at the dentist, I was"},{"startTime":599.656,"endTime":603.504,"body":"just like, okay. And then afterwards I became resentful about that. So the"},{"startTime":603.512,"endTime":606.32,"body":"classic thing is that if that had given me an"},{"startTime":606.36,"endTime":610.208,"body":"opportunity to be empowered to say, let me think"},{"startTime":610.264,"endTime":614.012,"body":"about this, or we'd like to take a moment, have"},{"startTime":614.036,"endTime":616.812,"body":"you get up out of the dental chair and we have this little office where"},{"startTime":616.836,"endTime":620.524,"body":"you can sit down on the side and the lighting is kind of dim and"},{"startTime":620.532,"endTime":623.996,"body":"there's comfortable chairs. And we're going to have you come back for another"},{"startTime":624.068,"endTime":627.9,"body":"appointment that may be inconvenient, but addressing that need in"},{"startTime":627.94,"endTime":631.644,"body":"a proactive way, that would be a trauma informed dental"},{"startTime":631.692,"endTime":635.292,"body":"office. Okay. So in addition to anticipating"},{"startTime":635.356,"endTime":638.876,"body":"the distress and activation of clients when they're"},{"startTime":638.908,"endTime":642.572,"body":"seeking care and treatment, the third pillar of trauma"},{"startTime":642.636,"endTime":646.204,"body":"informed care is that at restoring the soul, we are seeking"},{"startTime":646.252,"endTime":650.028,"body":"to provide a connection with clients"},{"startTime":650.204,"endTime":653.9,"body":"that is safe, that is stable and secure. And I"},{"startTime":653.94,"endTime":656.88,"body":"want to talk about each one of these individually."},{"startTime":657.22,"endTime":660.972,"body":"Safe is not just physically safe. So"},{"startTime":660.996,"endTime":664.652,"body":"we want to make sure, you know that there's no ice outside of our"},{"startTime":664.676,"endTime":668.34,"body":"building. In our particular building. That's sometimes the case. And I have to let"},{"startTime":668.5,"endTime":671.508,"body":"the building owners know that, hey, they need to come and shovel or put down"},{"startTime":671.564,"endTime":675.204,"body":"salt or sand or something like that. That's important. But I'm talking"},{"startTime":675.252,"endTime":679.076,"body":"about an emotional safety that then, if that safety"},{"startTime":679.108,"endTime":682.948,"body":"is not there, triggers something physically in their body. And now we're"},{"startTime":682.964,"endTime":686.052,"body":"not talking about a physical injury, but a physical"},{"startTime":686.116,"endTime":689.3,"body":"reaction or physical dysregulation or"},{"startTime":689.34,"endTime":692.612,"body":"activation, which takes us back to that very first point"},{"startTime":692.796,"endTime":696.35,"body":"of understanding, understanding and taking into account the fact that this"},{"startTime":696.39,"endTime":699.598,"body":"activation will happen. We try to prevent"},{"startTime":699.774,"endTime":703.55,"body":"unnecessary dysregulation and activation which makes"},{"startTime":703.59,"endTime":706.398,"body":"a person feel unsafe. And then we try to"},{"startTime":706.534,"endTime":710.302,"body":"proactively create an environment"},{"startTime":710.406,"endTime":713.774,"body":"where that doesn't happen as well. But essentially safety is"},{"startTime":713.942,"endTime":717.71,"body":"that it feels safe and it feels good and"},{"startTime":717.75,"endTime":721.342,"body":"it feels right for me to be here in my body"},{"startTime":721.526,"endTime":725.23,"body":"at this moment. And I'll often do"},{"startTime":725.27,"endTime":729.07,"body":"work with clients where, and this is almost cliche now in some"},{"startTime":729.11,"endTime":732.942,"body":"circles, but where we'll ask people to stop, to take a"},{"startTime":732.966,"endTime":736.622,"body":"couple of breaths and to pay attention to what's happening"},{"startTime":736.686,"endTime":740.382,"body":"in their body. And I'm surprised, even after kind of"},{"startTime":740.406,"endTime":743.822,"body":"having this approach and working in this for over 10 years,"},{"startTime":744.006,"endTime":747.832,"body":"how many people are talking about material in"},{"startTime":747.856,"endTime":751.064,"body":"their past that's not, quote, unquote, major"},{"startTime":751.152,"endTime":754.888,"body":"trauma. It's not particularly intense. At least they"},{"startTime":754.944,"endTime":758.44,"body":"wouldn't identify it as such in the moment. But as they tune into their"},{"startTime":758.48,"endTime":761.896,"body":"body, they're aware of feeling tightness in their chest,"},{"startTime":762.088,"endTime":765.66,"body":"a clenched jaw, tension in their forehead,"},{"startTime":766.08,"endTime":769.912,"body":"tension in their hips, butterflies in their stomach, clammy"},{"startTime":769.976,"endTime":773.674,"body":"hands. And then oftentimes we'll just talk about that"},{"startTime":773.832,"endTime":777.51,"body":"will stop and I'll check in and I'll say something"},{"startTime":777.63,"endTime":781.35,"body":"like, if you could give a color"},{"startTime":781.51,"endTime":785.334,"body":"to that tension in the middle of your chest, what would"},{"startTime":785.422,"endTime":789.222,"body":"that color be? And they might say something like blue"},{"startTime":789.286,"endTime":792.998,"body":"or yellow or red. And I'm not trying to attribute a certain"},{"startTime":793.054,"endTime":796.886,"body":"meaning to any of those colors, but whatever that color is, that actually helps"},{"startTime":796.918,"endTime":800.614,"body":"them to articulate what they might not have words to. And then I might say,"},{"startTime":800.702,"endTime":804.17,"body":"if you were to give a texture to that particular"},{"startTime":804.63,"endTime":807.95,"body":"tightness in your chest, what would it be? Oh, that's easy. It would feel like"},{"startTime":807.99,"endTime":811.742,"body":"sandpaper. Okay. If you were to give it a voice like a"},{"startTime":811.766,"endTime":815.534,"body":"cartoon bubble, and that cartoon bubble with words from the Sunday"},{"startTime":815.622,"endTime":819.406,"body":"funnies had words in it, what would it say? Oh, that's"},{"startTime":819.438,"endTime":823.23,"body":"easy. Or maybe in some instances, they would take a couple of minutes and"},{"startTime":823.27,"endTime":826.798,"body":"not know exactly, but they'd close their eyes and just pay attention,"},{"startTime":826.974,"endTime":830.552,"body":"kind of looking at that hypothetical imaginary cartoon bubble, and"},{"startTime":830.576,"endTime":834.232,"body":"they'd say, I'm in trouble, or you can't"},{"startTime":834.296,"endTime":837.96,"body":"go here. In other words, it's a part of them that is"},{"startTime":838.0,"endTime":841.432,"body":"embodied and that's physiological, that feels"},{"startTime":841.496,"endTime":845.224,"body":"unsafe. And this safety is so important"},{"startTime":845.312,"endTime":848.44,"body":"for people to feel because it's the foundation of"},{"startTime":848.48,"endTime":851.976,"body":"trust. Without people feeling seen and without"},{"startTime":852.048,"endTime":855.186,"body":"people feeling feeling safe,"},{"startTime":855.298,"endTime":858.69,"body":"trust cannot possibly develop. And without a sense of"},{"startTime":858.73,"endTime":862.098,"body":"security, trust cannot possibly develop. And of course,"},{"startTime":862.234,"endTime":865.826,"body":"therapy, no matter how mechanical or"},{"startTime":865.978,"endTime":869.23,"body":"treatment driven it is, trust is just"},{"startTime":869.53,"endTime":873.19,"body":"essential for the therapeutic process. Stability."},{"startTime":873.69,"endTime":877.426,"body":"We provide and seek a connection that is safe, stable, and secure"},{"startTime":877.458,"endTime":881.02,"body":"for clients. Let me talk about stability. Stability would"},{"startTime":881.06,"endTime":884.32,"body":"have other words like reliable"},{"startTime":884.66,"endTime":888.508,"body":"and predictable. I'm going to be there"},{"startTime":888.644,"endTime":892.412,"body":"for you over the prescribed amount of time. So"},{"startTime":892.436,"endTime":896.22,"body":"if a person is in weekly Counseling. The stability is"},{"startTime":896.26,"endTime":899.644,"body":"that the therapist is not always changing their schedule"},{"startTime":899.692,"endTime":903.5,"body":"around, that the therapist is not always running a half an hour late so that"},{"startTime":903.54,"endTime":907.014,"body":"the client shows up 15 minutes early, sits in the waiting room, and then"},{"startTime":907.132,"endTime":910.834,"body":"half hour in, they're wondering, is my therapist even here? Are they running behind?"},{"startTime":910.922,"endTime":914.642,"body":"And then they start to ask questions like, am I important? Do I"},{"startTime":914.666,"endTime":918.466,"body":"matter to my therapist? Et cetera. Because they fill in the blank when that knowledge"},{"startTime":918.498,"endTime":922.29,"body":"is not there. We get outside of our window of tolerance,"},{"startTime":1283.122,"endTime":1286.896,"body":"as we've talked about so much on this program. And the trauma informed"},{"startTime":1286.928,"endTime":1290.64,"body":"therapist knows what to do in that instance. It's not"},{"startTime":1290.68,"endTime":1294.432,"body":"a panic. It's not a, oh, my gosh, we have to fix"},{"startTime":1294.496,"endTime":1297.68,"body":"this. But it's also realizing that our"},{"startTime":1297.72,"endTime":1301.456,"body":"dysregulation is our body communicating to"},{"startTime":1301.528,"endTime":1305.296,"body":"us. It's our body saying, there is a threat. It's"},{"startTime":1305.328,"endTime":1309.072,"body":"our body saying, I'm feeling uncomfortable. There is a great"},{"startTime":1359.966,"endTime":1363.59,"body":"wisdom of the body, the way that God created us"},{"startTime":1363.71,"endTime":1366.98,"body":"fearfully and wonderfully, and the way that our"},{"startTime":1367.02,"endTime":1370.388,"body":"souls include in this Hebrew"},{"startTime":1370.484,"endTime":1373.828,"body":"idea of the rich, rich"},{"startTime":1374.004,"endTime":1377.7,"body":"understanding of soul that was more than the Greek idea of"},{"startTime":1377.74,"endTime":1381.572,"body":"the mind, the emotions and the will. The Hebrew idea of soul"},{"startTime":1381.636,"endTime":1384.772,"body":"was one that was an embodied idea that our soul"},{"startTime":1384.916,"endTime":1388.628,"body":"includes our bodies. And therefore, if our body"},{"startTime":1388.764,"endTime":1392.292,"body":"has experienced trauma, yes, that affects our mind,"},{"startTime":1392.396,"endTime":1396.202,"body":"yes, it affects our emotions, yes, it affects our choices, our volition, our"},{"startTime":1396.226,"endTime":1399.466,"body":"capacity to choose, but it affects our body, and then that"},{"startTime":1399.538,"endTime":1403.11,"body":"affects everything else. Now, some of these"},{"startTime":1404.53,"endTime":1407.978,"body":"tools, techniques and protocols to help"},{"startTime":1408.034,"endTime":1411.706,"body":"manage dysregulation can be simple but highly"},{"startTime":1411.738,"endTime":1415.53,"body":"effective things like breathing techniques. Now, there are"},{"startTime":1415.57,"endTime":1419.162,"body":"many people whose level of dysregulation, especially in"},{"startTime":1419.186,"endTime":1422.998,"body":"instances where there's complex trauma, oftentimes"},{"startTime":1423.174,"endTime":1426.822,"body":"to breathe and to relax and to begin to kind of"},{"startTime":1426.846,"endTime":1430.422,"body":"drop into our center that is so terrifying to the"},{"startTime":1430.446,"endTime":1434.198,"body":"brain because they've lived for so long with a hypervigilance and"},{"startTime":1434.254,"endTime":1438.054,"body":"a guardedness that to do deep breathing sends a signal to the"},{"startTime":1438.062,"endTime":1441.702,"body":"brain that says, oh, our defenses are down. Now we have to"},{"startTime":1441.726,"endTime":1445.51,"body":"ramp up even more. So for therapists that might be listening to this and"},{"startTime":1445.55,"endTime":1449.302,"body":"wondering, why doesn't breathing work with this person? It may be that"},{"startTime":1449.326,"endTime":1453.006,"body":"they need a lot of work to build up to that, so that they."},{"startTime":1453.078,"endTime":1456.862,"body":"They can then use the breathing on their own, but that their"},{"startTime":1456.886,"endTime":1460.622,"body":"nervous system is too dysregulated and too hijacked in"},{"startTime":1460.646,"endTime":1464.286,"body":"certain moments for breathing to allow them to relax. Other kinds"},{"startTime":1464.318,"endTime":1467.886,"body":"of interventions might be somatic awareness"},{"startTime":1468.078,"endTime":1471.422,"body":"and visualizing and focusing on different parts of the body."},{"startTime":1471.526,"endTime":1475.342,"body":"Relaxation exercises. There's a tool that"},{"startTime":1475.366,"endTime":1478.542,"body":"I use with clients regularly, and somebody by the name of"},{"startTime":1478.566,"endTime":1482.244,"body":"Weitzman, probably a psychologist or social worker or therapist way back,"},{"startTime":1482.382,"endTime":1485.992,"body":"created this, and therefore it came to me multiple years ago, called the"},{"startTime":1486.016,"endTime":1489.704,"body":"Weitzman Relaxation Protocol. And instead of"},{"startTime":1489.792,"endTime":1493.528,"body":"just walking people through a visualization, it gives a"},{"startTime":1493.584,"endTime":1497.304,"body":"set of phrases that rotate back and forth."},{"startTime":1497.352,"endTime":1500.74,"body":"And this is just an example from. Is it possible"},{"startTime":1501.36,"endTime":1504.38,"body":"to. Can you. So, for example,"},{"startTime":1505.12,"endTime":1508.78,"body":"over about 10 minutes in the course of this relaxation"},{"startTime":1509.1,"endTime":1512.82,"body":"protocol, I would say, is it possible for you to"},{"startTime":1512.86,"endTime":1516.692,"body":"picture a red balloon off in the distance in front of"},{"startTime":1516.716,"endTime":1519.8,"body":"you, floating gently in front of the sky?"},{"startTime":1520.3,"endTime":1523.68,"body":"And then you wait about five seconds, and then you say,"},{"startTime":1524.14,"endTime":1527.508,"body":"can you picture a wisp of"},{"startTime":1527.564,"endTime":1530.8,"body":"cloud rising up in front of you?"},{"startTime":1531.34,"endTime":1535.12,"body":"And you'd wait about five seconds and you'd say, is it possible"},{"startTime":1535.76,"endTime":1539.576,"body":"for you to gently focus your attention on your right"},{"startTime":1539.648,"endTime":1543.08,"body":"earlobe, Wait five seconds."},{"startTime":1543.24,"endTime":1546.92,"body":"Is it possible for you to gently focus your attention on"},{"startTime":1546.96,"endTime":1550.408,"body":"your left earlobe? I will use that after I've"},{"startTime":1569.354,"endTime":1573.162,"body":"gotten to know a person. But that's something that. It's a"},{"startTime":1573.186,"endTime":1576.938,"body":"visualization. It may or may not involve breathing, but"},{"startTime":1576.994,"endTime":1580.266,"body":"it. It involves giving"},{"startTime":1580.458,"endTime":1583.866,"body":"clear intention of focus. And what's"},{"startTime":1583.898,"endTime":1587.67,"body":"happening is that it's allowing the person to settle by"},{"startTime":1587.71,"endTime":1591.542,"body":"talking in a calm way, by having certain images that are"},{"startTime":1591.566,"endTime":1594.45,"body":"relaxing like a balloon floating in the"},{"startTime":1595.31,"endTime":1598.934,"body":"front in front of them, but then also by the mindfulness"},{"startTime":1598.982,"endTime":1602.566,"body":"and the attention that's given to focusing on your right earlobe"},{"startTime":1602.678,"endTime":1606.454,"body":"or focusing on the weight of your right arm upon"},{"startTime":1606.502,"endTime":1610.31,"body":"your lap. That mindfulness plus the relaxing images"},{"startTime":1610.39,"endTime":1613.862,"body":"says to the brain, that threat that you thought was going to"},{"startTime":1613.886,"endTime":1617.672,"body":"overwhelm you, that was such a danger. We've got this."},{"startTime":1617.776,"endTime":1621.176,"body":"It's okay. Other kinds of tools might include"},{"startTime":1621.328,"endTime":1624.824,"body":"tapping. I'm on video for this, so I'm going to model"},{"startTime":1624.872,"endTime":1628.552,"body":"that butterfly tapping, where people will sit with their"},{"startTime":1628.576,"endTime":1632.248,"body":"feet on the ground, close their eyes and breathe. And they might tap back and"},{"startTime":1632.304,"endTime":1636.06,"body":"forth. Bilateral stimulation on the outer edge of the bicep"},{"startTime":1636.4,"endTime":1640.168,"body":"While they're picturing a safe place or a place of peace with"},{"startTime":1640.224,"endTime":1643.17,"body":"all the different sensory input that they can,"},{"startTime":1643.95,"endTime":1647.29,"body":"they might tap on their knees, just behind their kneecaps,"},{"startTime":1647.63,"endTime":1651.382,"body":"doing what we call resource tapping. Prior to the quarantine and"},{"startTime":1651.406,"endTime":1654.982,"body":"the pandemic, this was all something that was generally done in a"},{"startTime":1655.006,"endTime":1658.742,"body":"counselor's office. And at some point into treatment, when"},{"startTime":1658.766,"endTime":1662.55,"body":"it felt appropriate, people were given these tools and they could do them at home."},{"startTime":1662.67,"endTime":1666.038,"body":"But once we all went online during the quarantine and people couldn't go to"},{"startTime":1666.094,"endTime":1669.428,"body":"therapist's office and they were dealing with trauma and such anxiety"},{"startTime":1669.614,"endTime":1673.272,"body":"from the pandemic, therapists began to use these online,"},{"startTime":1673.456,"endTime":1677.112,"body":"and we've come to realize just how effective they are."},{"startTime":1677.296,"endTime":1681.096,"body":"So tapping either on the thighs, behind the kneecap, or"},{"startTime":1681.168,"endTime":1684.728,"body":"on the arms like this, these are two of the most popular while"},{"startTime":1684.784,"endTime":1688.488,"body":"visualizing a safe place or a place of"},{"startTime":1688.544,"endTime":1692.248,"body":"peace. And then there's a little more advanced technique where you can actually gently focus"},{"startTime":1692.304,"endTime":1695.592,"body":"your attention on the distressing or unpleasant"},{"startTime":1695.656,"endTime":1699.31,"body":"sensation in particular, and to tap and"},{"startTime":1699.35,"endTime":1703.006,"body":"to bring a sense of focus and relaxation to that."},{"startTime":1703.158,"endTime":1706.542,"body":"EMDR is another way to help people regulate their"},{"startTime":1706.566,"endTime":1710.094,"body":"emotions. There's other ways, different forms of"},{"startTime":1710.182,"endTime":1713.342,"body":"mindfulness, but all of this comes under emotional"},{"startTime":1713.406,"endTime":1717.166,"body":"regulation, which is something that you can experience in relationship"},{"startTime":1717.198,"endTime":1720.91,"body":"with another. And the benefit of that is so much"},{"startTime":1720.95,"endTime":1724.798,"body":"of our social engagement system as it's spoken about"},{"startTime":1724.854,"endTime":1728.426,"body":"in polyvagal theory, which is not something I'm going to go into"},{"startTime":1728.498,"endTime":1732.314,"body":"now. But our social engagement system is the very first"},{"startTime":1732.402,"endTime":1736.202,"body":"way that our bodies were created to regulate and to"},{"startTime":1736.226,"endTime":1739.338,"body":"get back into a window of tolerance and to feel safe,"},{"startTime":1739.514,"endTime":1743.178,"body":"secure, and in a place of peace. And all that simply means"},{"startTime":1743.314,"endTime":1746.618,"body":"think of the infant or the little child. The little"},{"startTime":1746.674,"endTime":1749.11,"body":"child is standing up"},{"startTime":1750.21,"endTime":1753.898,"body":"less than a toddler, but more than a new infant. Let's just say that they're"},{"startTime":1753.994,"endTime":1757.484,"body":"20 months old and they're holding onto the coffee table and"},{"startTime":1757.572,"endTime":1761.292,"body":"starting to say some gibberish baby words, and they fall and they"},{"startTime":1761.316,"endTime":1765.084,"body":"hit their head on the corner of the couch and the parent rushes"},{"startTime":1765.132,"endTime":1768.924,"body":"to them and that baby, before"},{"startTime":1769.012,"endTime":1772.78,"body":"they'll even break into tears or a scream, they'll often"},{"startTime":1772.9,"endTime":1776.54,"body":"look for that adult and look into their face and to see whether"},{"startTime":1776.58,"endTime":1780.3,"body":"that adult is horrified, terrified, and that baby, without the"},{"startTime":1780.34,"endTime":1783.692,"body":"cognitive construct is looking and attuning to the"},{"startTime":1783.716,"endTime":1787.384,"body":"adult as to whether the adult has them. Now, the"},{"startTime":1787.392,"endTime":1791.128,"body":"baby might still cry, but that social engagement system"},{"startTime":1791.184,"endTime":1794.92,"body":"is. We're always looking to others to find"},{"startTime":1794.96,"endTime":1798.392,"body":"out if things are okay or if we're okay. And we could probably all take"},{"startTime":1798.416,"endTime":1802.136,"body":"a moment and give examples of from grade school onto,"},{"startTime":1802.248,"endTime":1805.112,"body":"you know, a talk that you recently gave at work where we were kind of"},{"startTime":1805.136,"endTime":1808.92,"body":"looking around, checking out whether people were affirming and nodding our head"},{"startTime":1808.96,"endTime":1812.79,"body":"and tracking with us or not, at which point our heart might"},{"startTime":1812.83,"endTime":1816.438,"body":"start to race. That's enough about training,"},{"startTime":1816.614,"endTime":1820.438,"body":"specialized training. And very often people would leave the"},{"startTime":1842.926,"endTime":1846.326,"body":"office very dysregulated, very"},{"startTime":1846.398,"endTime":1850.23,"body":"activated, and then if they were in an intimate relationship, they would go home and"},{"startTime":1850.27,"endTime":1853.99,"body":"they would have these either horrible meltdowns or they would get"},{"startTime":1854.03,"endTime":1857.766,"body":"depressed or they would engage in an addiction because they had to find"},{"startTime":1857.838,"endTime":1861.654,"body":"some way to get back to a place of peace, calm and"},{"startTime":1861.742,"endTime":1865.11,"body":"grounding. The fifth pillar of trauma"},{"startTime":1865.19,"endTime":1868.06,"body":"informed care is simply this."},{"startTime":1868.68,"endTime":1872.224,"body":"It's the fact that some of these emotional"},{"startTime":1872.272,"endTime":1875.84,"body":"regulation tools are what's done in the"},{"startTime":1875.88,"endTime":1879.504,"body":"session.
