And as you can imagine,"},{"startTime":76.208,"endTime":78.48,"body":"if you don't ovulate, you can't get"},{"startTime":78.528,"endTime":81.29,"body":"pregnant. So in today's episode of, talk"},{"startTime":81.33,"endTime":83.71,"body":"about fertility Tuesday, we're going to talk about"},{"startTime":84.45,"endTime":87.226,"body":"how do you know if you're ovulating? Can you"},{"startTime":87.258,"endTime":90.026,"body":"know? And if you suspect you're not,"},{"startTime":90.098,"endTime":93.098,"body":"how can you find this out? So to understand this, you have to"},{"startTime":93.114,"endTime":96.058,"body":"understand what a menstrual cycle is. But before the embryo can"},{"startTime":154.588,"endTime":157.396,"body":"implant into the uterus, the lining"},{"startTime":157.428,"endTime":159.76,"body":"needs undergo luteinization."},{"startTime":160.5,"endTime":163.446,"body":"And that occurs in the luteal phase. And"},{"startTime":163.518,"endTime":166.246,"body":"after you ovulate, the sac that"},{"startTime":166.278,"endTime":168.822,"body":"released the egg now becomes a"},{"startTime":168.846,"endTime":170.95,"body":"production factory for the hormone"},{"startTime":170.99,"endTime":173.91,"body":"progesterone. And we call this a"},{"startTime":173.95,"endTime":176.518,"body":"corpus luteum. This then"},{"startTime":176.574,"endTime":179.438,"body":"secretes progesterone, which prepares the"},{"startTime":179.454,"endTime":182.382,"body":"uterine lining for a potential pregnancy. Now"},{"startTime":182.406,"endTime":185.102,"body":"this corpus luteum has a kind of time"},{"startTime":185.166,"endTime":188.126,"body":"clock to it. It lasts for about twelve to 14"},{"startTime":188.238,"endTime":190.856,"body":"days and then it collapses."},{"startTime":191.048,"endTime":193.56,"body":"And at that point the progesterone level then drops"},{"startTime":193.6,"endTime":196.18,"body":"completely. So"},{"startTime":196.6,"endTime":199.28,"body":"if you don't get pregnant, the corpus"},{"startTime":199.32,"endTime":201.896,"body":"lilium will collapse, the progesterone level will"},{"startTime":201.928,"endTime":204.848,"body":"drop and then you will get a period."},{"startTime":205.024,"endTime":208.008,"body":"What causes the period to come is"},{"startTime":208.024,"endTime":210.736,"body":"the shedding of the lining from the loss of"},{"startTime":210.768,"endTime":213.2,"body":"progesterone. So you have to first have"},{"startTime":213.24,"endTime":215.178,"body":"progesterone, then remove"},{"startTime":215.234,"endTime":218.146,"body":"progesterone to be able to get the lining to shed"},{"startTime":218.178,"endTime":220.826,"body":"itself. And the purpose of shedding is to"},{"startTime":220.858,"endTime":223.306,"body":"prepare them for the next month for potential"},{"startTime":223.378,"endTime":226.29,"body":"pregnancy. If you are fortunate enough to become"},{"startTime":226.37,"endTime":229.09,"body":"pregnant, then what will happen is when the"},{"startTime":229.13,"endTime":231.546,"body":"embryo implants into the uterine"},{"startTime":231.578,"endTime":233.258,"body":"lining, the"},{"startTime":233.314,"endTime":235.778,"body":"syncidiotrophoblastic cells will then"},{"startTime":235.834,"endTime":238.586,"body":"start releasing hcg,"},{"startTime":238.698,"endTime":241.03,"body":"human chorionic gonadotropin."},{"startTime":241.61,"endTime":243.714,"body":"This signals to the corpus"},{"startTime":243.762,"endTime":246.642,"body":"luteum to keep making"},{"startTime":246.706,"endTime":249.65,"body":"progesterone. That's because progesterone is"},{"startTime":249.69,"endTime":252.57,"body":"usually created through lh luteinizing"},{"startTime":252.61,"endTime":255.51,"body":"hormone, but hcg will mimic that hormone"},{"startTime":255.81,"endTime":258.762,"body":"and so it rescues the corpus luteum."},{"startTime":258.866,"endTime":261.415,"body":"So instead of it, collapsing in those twelve to 14"},{"startTime":261.503,"endTime":264.487,"body":"days, it continues. And that's"},{"startTime":264.519,"endTime":267.471,"body":"why we associate when someone, let's say, is"},{"startTime":267.511,"endTime":270.447,"body":"pregnant and gets bleeding, we associate"},{"startTime":270.479,"endTime":273.447,"body":"with something wrong with the pregnancy, because if something happens to the"},{"startTime":273.479,"endTime":276.247,"body":"baby, the ecg level will drop."},{"startTime":276.399,"endTime":279.279,"body":"And if the ecg level drops, then that can cause"},{"startTime":279.295,"endTime":282.055,"body":"the progesterone levels to drop. Now, an interesting"},{"startTime":282.103,"endTime":284.751,"body":"caveat. If you are doing a frozen"},{"startTime":284.791,"endTime":287.639,"body":"embryo transfer and you are giving injectable"},{"startTime":287.695,"endTime":290.351,"body":"progesterone, even if there is"},{"startTime":290.391,"endTime":293.175,"body":"bleeding, it's probably not from the pregnancy,"},{"startTime":293.223,"endTime":296.127,"body":"meaning the baby, because you're giving the progesterone, they"},{"startTime":296.159,"endTime":298.827,"body":"can't drop. Only in a"},{"startTime":298.899,"endTime":301.707,"body":"natural pregnancy or with a pregnancy through"},{"startTime":301.779,"endTime":304.731,"body":"ovulation can the pregnancy mean"},{"startTime":304.771,"endTime":307.499,"body":"the baby acg levels drop to"},{"startTime":307.555,"endTime":310.291,"body":"cause bleeding, because then the acg"},{"startTime":310.331,"endTime":313.291,"body":"levels drop, the progesterone level drop, and that"},{"startTime":313.331,"endTime":316.187,"body":"causes bleeding. So the important thing to understand"},{"startTime":316.299,"endTime":319.267,"body":"here is that the follicular phase is to"},{"startTime":319.299,"endTime":322.203,"body":"develop the follicle, which also develops the thickness"},{"startTime":322.227,"endTime":325.103,"body":"of the lining. The luteal phase is to"},{"startTime":325.127,"endTime":327.651,"body":"get the lining prepared for implantation."},{"startTime":328.071,"endTime":330.975,"body":"And only when the uterine lining is"},{"startTime":331.023,"endTime":333.423,"body":"given progesterone for a period of time and then"},{"startTime":333.487,"endTime":336.063,"body":"remove does someone"},{"startTime":336.207,"endTime":339.071,"body":"withdraw the blood from the uterus,"},{"startTime":339.151,"endTime":341.861,"body":"causing them, period. And that causes immenses."},{"startTime":343.019,"endTime":345.891,"body":"now, it doesn't have to be natural. You can give"},{"startTime":345.931,"endTime":348.925,"body":"someone progesterone. So if I give you proverae"},{"startTime":348.963,"endTime":350.969,"body":"or prometrium or injectable"},{"startTime":351.025,"endTime":353.953,"body":"progesterone for about seven to ten"},{"startTime":354.017,"endTime":356.297,"body":"days, and then I remove that"},{"startTime":356.409,"endTime":358.513,"body":"hormone, you will also get"},{"startTime":358.537,"endTime":361.337,"body":"amenses. This is very similar to even"},{"startTime":361.369,"endTime":364.313,"body":"how birth controls work. So, birth controls give"},{"startTime":364.337,"endTime":367.065,"body":"you both estrogen and progesterone,"},{"startTime":367.233,"endTime":370.049,"body":"and you take it for, let's say, 21"},{"startTime":370.105,"endTime":372.633,"body":"days. Then you stop it. Taking sugar"},{"startTime":372.657,"endTime":375.021,"body":"pills, the progesterone levels drop,"},{"startTime":375.471,"endTime":378.231,"body":"you have another menses. But,"},{"startTime":378.231,"endTime":380.597,"body":"this menses is not the same menses as"},{"startTime":380.629,"endTime":383.485,"body":"ovulation. We call this a hormonal"},{"startTime":383.573,"endTime":386.429,"body":"withdrawal bleed. Basically, this is just"},{"startTime":386.485,"endTime":389.437,"body":"distinguishing it from a hormonal"},{"startTime":389.469,"endTime":392.453,"body":"withdrawal bleed, from ovulation. By ovulating,"},{"startTime":392.517,"endTime":395.001,"body":"you make progesterone, and then it disappears."},{"startTime":395.341,"endTime":398.269,"body":"In a situation with medication, you"},{"startTime":398.285,"endTime":401.269,"body":"are taking the progesterone because you're not making it. And"},{"startTime":401.285,"endTime":404.285,"body":"then the withdrawal of that progesterone when you stop it is what causes the"},{"startTime":404.293,"endTime":406.789,"body":"uterine lining to shed, causing the"},{"startTime":406.845,"endTime":409.561,"body":"menses. So, now that we defined"},{"startTime":409.901,"endTime":412.429,"body":"how a menstrual cycle works"},{"startTime":412.605,"endTime":415.521,"body":"and what causes a woman to get a menses,"},{"startTime":416.061,"endTime":418.721,"body":"the question is then, how can you bleed"},{"startTime":419.141,"endTime":422.053,"body":"and not be ovulating? And so this could"},{"startTime":422.077,"endTime":424.997,"body":"be other causes of bleeding. What"},{"startTime":425.029,"endTime":427.869,"body":"could happen is when the lining of the uterus builds up"},{"startTime":427.925,"endTime":430.809,"body":"and gets thicker and thicker and thicker, it"},{"startTime":430.825,"endTime":433.817,"body":"can eventually bleed. Think"},{"startTime":433.849,"endTime":436.697,"body":"of it as it can't support its own weight, so the"},{"startTime":436.729,"endTime":439.297,"body":"lining is building up on top of itself, and"},{"startTime":439.329,"endTime":442.221,"body":"eventually it's too much, and it comes out."},{"startTime":443.161,"endTime":445.617,"body":"But there can also be times when, even"},{"startTime":445.689,"endTime":448.641,"body":"monthly, you're getting a period. And that's because"},{"startTime":448.681,"endTime":451.633,"body":"there's hormonal fluctuations that are occurring. And even"},{"startTime":451.657,"endTime":453.591,"body":"though you're not, ovulating, those hormonal"},{"startTime":453.631,"endTime":456.327,"body":"fluctuations can be causing"},{"startTime":456.439,"endTime":459.341,"body":"a the bleeding to be occurring even"},{"startTime":459.381,"endTime":461.361,"body":"though it's not due to progesterone."},{"startTime":461.941,"endTime":464.773,"body":"Now, the name category"},{"startTime":464.837,"endTime":467.501,"body":"we call this is anovulation, meaning you're not"},{"startTime":467.541,"endTime":470.165,"body":"ovulating. This is the most common cause for"},{"startTime":470.213,"endTime":473.133,"body":"someone to think they're getting"},{"startTime":473.157,"endTime":476.013,"body":"their period every month, but they're not. So this is where"},{"startTime":476.117,"endTime":477.561,"body":"either you have significant"},{"startTime":478.901,"endTime":481.677,"body":"things like polycystic ovarian syndromes or hormonal"},{"startTime":481.709,"endTime":484.375,"body":"imbalances or something that causes your body"},{"startTime":484.533,"endTime":486.431,"body":"to not release an egg anymore,"},{"startTime":487.331,"endTime":490.323,"body":"but because that lining is thickening up over"},{"startTime":490.347,"endTime":493.123,"body":"the month, because you're still making estrogen in the follicular"},{"startTime":493.187,"endTime":495.907,"body":"phase. But you never go in the luteal phase."},{"startTime":496.059,"endTime":499.019,"body":"Eventually, you just get shedding of the lining, and"},{"startTime":499.035,"endTime":501.851,"body":"you get a period. Now, most of the time,"},{"startTime":501.891,"endTime":504.843,"body":"this comes in very odd times, meaning"},{"startTime":504.947,"endTime":507.859,"body":"it's not every single month. You might go a couple months without a"},{"startTime":507.875,"endTime":510.587,"body":"period. Then you might get a period. Might be"},{"startTime":510.619,"endTime":513.579,"body":"45 days and 35 days. And usually that's how you"},{"startTime":513.595,"endTime":516.426,"body":"can figure those ones out. But the thing is, no"},{"startTime":516.458,"endTime":519.418,"body":"one really is checking their menstrual cycle that well,"},{"startTime":519.514,"endTime":522.375,"body":"most women go, okay, well, I got a period in July. I, got a"},{"startTime":522.391,"endTime":525.367,"body":"period in August. They don't think twice about it, but they really"},{"startTime":525.407,"endTime":528.327,"body":"looked at, they would notice it's changing all the time. The"},{"startTime":528.367,"endTime":531.223,"body":"second situation is where it feels like it's perfectly on"},{"startTime":531.255,"endTime":534.187,"body":"time, but then when you check it, it's still fluctuating."},{"startTime":534.527,"endTime":537.135,"body":"Now, the thing that's really interesting is the period is"},{"startTime":537.191,"endTime":540.167,"body":"very accurate. Most women can"},{"startTime":540.207,"endTime":542.387,"body":"tell you the day they're going to get their menses."},{"startTime":543.198,"endTime":546.11,"body":"The first time I heard this, I couldn't believe it. They could literally tell"},{"startTime":546.134,"endTime":549.126,"body":"me the day they're going to get it. That is what"},{"startTime":549.142,"endTime":551.95,"body":"we would consider usually normal. Now, it doesn't mean"},{"startTime":551.974,"endTime":554.926,"body":"there's not some flexibility here. And someone can be two"},{"startTime":554.982,"endTime":557.598,"body":"days late or one day late, but if you"},{"startTime":557.638,"endTime":560.508,"body":"are five days or, more fluctuating between your"},{"startTime":560.548,"endTime":563.196,"body":"cycle day, there could be an issue there."},{"startTime":563.372,"endTime":566.3,"body":"Now, where a lot of women get a little confused is"},{"startTime":566.324,"endTime":569.31,"body":"they think when they're on the birth control pillow, that that's"},{"startTime":569.334,"endTime":572.206,"body":"a menses that is not. So, a lot of times I'll talk to"},{"startTime":572.222,"endTime":575.214,"body":"people, say, well, it says here your menses were every month. They're like, yeah, it's been"},{"startTime":575.246,"endTime":578.182,"body":"perfect. But I say to them, but haven't you been on birth control?"},{"startTime":578.23,"endTime":581.062,"body":"And they're like, oh, yeah, I've been on birth control. That"},{"startTime":581.11,"endTime":583.91,"body":"is not a menses. That's just a withdrawal bleed"},{"startTime":583.974,"endTime":586.07,"body":"from a hormonal removal of"},{"startTime":586.094,"endTime":588.726,"body":"progesterone. The same thing. If you're taking"},{"startTime":588.782,"endTime":591.63,"body":"progesterone, such as provera or"},{"startTime":591.654,"endTime":594.414,"body":"prometrium every month, that does not mean"},{"startTime":594.446,"endTime":597.27,"body":"you're ovulating. You're just giving yourself the"},{"startTime":597.294,"endTime":599.878,"body":"hormone that caused the withdrawal bleed."},{"startTime":600.038,"endTime":602.654,"body":"So then how do you determine"},{"startTime":602.806,"endTime":605.534,"body":"if you need to get this worked up? Well, in"},{"startTime":605.566,"endTime":608.47,"body":"general, this really matters for people when they're trying"},{"startTime":608.494,"endTime":611.486,"body":"to get pregnant. So if you're trying to get pregnant and"},{"startTime":611.502,"endTime":614.486,"body":"your cycles are not regular, you want to see a"},{"startTime":614.502,"endTime":617.446,"body":"fertility doctor or OBGYN within the first six"},{"startTime":617.502,"endTime":620.486,"body":"months. But if"},{"startTime":620.502,"endTime":623.048,"body":"your periods are every month and you"},{"startTime":623.088,"endTime":625.896,"body":"start charting them and start noticing they're not"},{"startTime":625.992,"endTime":627.668,"body":"as accurate as you thought,"},{"startTime":628.088,"endTime":630.748,"body":"potentially you may be one of those people"},{"startTime":631.048,"endTime":633.96,"body":"who may not be ovulating. Now, the way you can"},{"startTime":633.984,"endTime":636.792,"body":"test for this are a couple ways you can"},{"startTime":636.84,"endTime":639.744,"body":"do what are called ovulation predictor kits. And these"},{"startTime":639.776,"endTime":641.668,"body":"do check the hormone LH."},{"startTime":642.448,"endTime":645.144,"body":"Now, LH can be elevated in people with"},{"startTime":645.176,"endTime":648.0,"body":"polycystic ovarian syndrome. And so when you do"},{"startTime":648.024,"endTime":650.616,"body":"these kits, it's very important that you look that it's"},{"startTime":650.672,"endTime":653.668,"body":"negative, negative, negative, and then becomes positive"},{"startTime":654.418,"endTime":657.41,"body":"and stays positive for a day or two and goes down. That seems"},{"startTime":657.434,"endTime":660.37,"body":"to be more accurate than if you just see"},{"startTime":660.394,"endTime":663.25,"body":"it composite. The first time you do it, you don't know was it always"},{"startTime":663.314,"endTime":666.29,"body":"elevated or did it go up? But the"},{"startTime":666.314,"endTime":669.186,"body":"other thing is, whenever it says you did ovulate,"},{"startTime":669.242,"endTime":671.998,"body":"you should be getting a period in approximately two weeks."},{"startTime":672.378,"endTime":675.37,"body":"If you check the test and says you ovulated, but you don't get a period"},{"startTime":675.434,"endTime":678.426,"body":"for three weeks, then you know that wasn't really ovulation."},{"startTime":678.522,"endTime":681.514,"body":"Because ovulation always occurs in that twelve to"},{"startTime":681.546,"endTime":684.422,"body":"14 days, meaning you"},{"startTime":684.43,"endTime":686.938,"body":"get your period twelve to 14 days after the ovulation."},{"startTime":687.438,"endTime":690.406,"body":"And so if that doesn't happen, you're either pregnant or"},{"startTime":690.542,"endTime":693.478,"body":"that was an ovulation. One of the most accurate"},{"startTime":693.518,"endTime":696.278,"body":"ways is to check a progesterone level on the"},{"startTime":696.318,"endTime":699.214,"body":"21st day of your cycle. Now, this assumes you have a"},{"startTime":699.246,"endTime":702.11,"body":"28 day cycle. If you have a"},{"startTime":702.174,"endTime":704.718,"body":"35 day cycle, then you want to check on day"},{"startTime":704.758,"endTime":707.67,"body":"28, basically midway between"},{"startTime":707.734,"endTime":710.41,"body":"that two week timeframe, from when you ovulate"},{"startTime":710.534,"endTime":713.21,"body":"to when you get your menses. As long as that"},{"startTime":713.234,"endTime":716.158,"body":"progesterone is above four, you are ovulating."},{"startTime":716.458,"endTime":719.346,"body":"Now, you'll hear people out there talk about progesterone levels should"},{"startTime":719.362,"endTime":722.25,"body":"be above ten. That is not true. That"},{"startTime":722.274,"endTime":725.274,"body":"is something completely different. You just want to make sure that"},{"startTime":725.306,"endTime":727.426,"body":"it's above four to at least verify you're"},{"startTime":727.482,"endTime":730.178,"body":"ovulating. There are also some great apps out"},{"startTime":730.218,"endTime":732.858,"body":"there that use your hormones, such as urine"},{"startTime":732.898,"endTime":735.72,"body":"hormones, check body temperature and try to"},{"startTime":735.744,"endTime":738.436,"body":"determine this as well. And they're pretty accurate as well."},{"startTime":738.592,"endTime":741.436,"body":"But in the end, the only thing that is absolutely going to"},{"startTime":741.452,"endTime":744.324,"body":"be accurate is checking a progesterone level to verify"},{"startTime":744.356,"endTime":746.988,"body":"if you ovulate it. So, in summary,"},{"startTime":747.148,"endTime":749.808,"body":"if you get perfect 28 day cycles"},{"startTime":750.468,"endTime":753.412,"body":"and your ovulation is exactly twelve"},{"startTime":753.46,"endTime":755.636,"body":"to 14 days from when you get your"},{"startTime":755.652,"endTime":758.5,"body":"menses, then you probably don't have an"},{"startTime":758.524,"endTime":761.5,"body":"issue. But if your periods are"},{"startTime":761.524,"endTime":764.49,"body":"irregular or I. If, like"},{"startTime":764.514,"endTime":767.314,"body":"I said, your period is coming longer"},{"startTime":767.346,"endTime":769.678,"body":"than those twelve to 14 days,"},{"startTime":770.178,"endTime":772.754,"body":"or if you've ever been diagnosed with"},{"startTime":772.786,"endTime":775.762,"body":"polycystic ovarian syndrome, this could be an issue.
