Why they happen"},{"startTime":67.001,"endTime":69.461,"body":"and what do they mean for future pregnancies."},{"startTime":70.041,"endTime":72.729,"body":"Sometimes these blood ovums are called"},{"startTime":72.825,"endTime":75.769,"body":"an embryonic pregnancy, meaning no embryo in the"},{"startTime":75.785,"endTime":78.617,"body":"pregnancy. And from the"},{"startTime":78.649,"endTime":81.465,"body":"standpoint of a patient, you would actually never"},{"startTime":81.513,"endTime":84.405,"body":"know that something is wrong. Matter of fact, it's not"},{"startTime":84.453,"endTime":87.041,"body":"uncommon for these not to even be diagnosed"},{"startTime":87.341,"endTime":89.981,"body":"until women come into their OB appointment around 12"},{"startTime":90.021,"endTime":92.893,"body":"weeks. Now, in fertility, we catch them earlier because we're"},{"startTime":92.917,"endTime":95.485,"body":"doing ultrasounds earlier, and so we can catch them"},{"startTime":95.533,"endTime":98.421,"body":"earlier. So some of the things we're going to talk"},{"startTime":98.461,"endTime":100.601,"body":"about here are what causes them,"},{"startTime":101.181,"endTime":103.521,"body":"why do some people misdiagnose them,"},{"startTime":104.141,"endTime":106.957,"body":"and is there any way to prevent"},{"startTime":106.989,"endTime":109.627,"body":"them? Now, to truly understand"},{"startTime":109.789,"endTime":112.679,"body":"the blight of ovum is you need to understand a little bit more"},{"startTime":112.695,"endTime":115.611,"body":"of the science of a blastocyst."},{"startTime":116.031,"endTime":118.638,"body":"A, blastocyst essentially has two"},{"startTime":118.678,"endTime":121.326,"body":"parts. There's a trophectoderm,"},{"startTime":121.422,"endTime":124.262,"body":"which becomes the placenta, and then there's the"},{"startTime":124.31,"endTime":126.818,"body":"inner cell mass which becomes the embryo."},{"startTime":127.398,"endTime":129.894,"body":"So if we look at the analogy we said earlier"},{"startTime":130.086,"endTime":132.998,"body":"about the egg, the"},{"startTime":133.118,"endTime":136.014,"body":"trophectoderm is kind of the shell, the egg in some"},{"startTime":136.046,"endTime":138.942,"body":"ways, and the inner cell mass would be the"},{"startTime":138.95,"endTime":141.718,"body":"embryo. And so a blighted ovum"},{"startTime":141.798,"endTime":144.766,"body":"is when the ICM fails to develop,"},{"startTime":144.902,"endTime":147.902,"body":"but the tropctoderm keeps growing. And"},{"startTime":147.95,"endTime":150.67,"body":"because it keeps growing, it produces acg,"},{"startTime":150.734,"endTime":152.766,"body":"because that's where the ACG comes from, the"},{"startTime":152.782,"endTime":155.75,"body":"tropectoderm. And so now your pregnancy will"},{"startTime":155.774,"endTime":158.406,"body":"be positive. And not only will it be positive,"},{"startTime":158.582,"endTime":161.358,"body":"but all of the symptoms of pregnancy will be there."},{"startTime":161.478,"endTime":164.418,"body":"The nausea, the morning sickness,"},{"startTime":165.038,"endTime":167.71,"body":"the increased reflux and constipation,"},{"startTime":167.814,"endTime":170.606,"body":"pretty much everything. A pregnancy will be the same because the"},{"startTime":170.662,"endTime":173.414,"body":"8 CG that's being made is going to"},{"startTime":173.446,"endTime":176.35,"body":"cause progesterone to go up, and that's going to cause"},{"startTime":176.374,"endTime":179.254,"body":"all the symptoms of pregnancy. Now, what's interesting is a lot"},{"startTime":179.286,"endTime":182.03,"body":"of clinics, when they see a patient,"},{"startTime":182.174,"endTime":184.87,"body":"will call something a blind ovum when it's"},{"startTime":184.894,"endTime":187.83,"body":"not. And what they're looking"},{"startTime":187.854,"endTime":190.84,"body":"at is they're saying, well, the embryo never grew."},{"startTime":190.984,"endTime":193.768,"body":"But that's not a blighted ovum. It's not an early"},{"startTime":193.808,"endTime":196.176,"body":"loss. It's when the embryo doesn't"},{"startTime":196.232,"endTime":199.096,"body":"develop. And one of the first red flags of that"},{"startTime":199.192,"endTime":201.76,"body":"is if there's no yoast sac, when you look at the"},{"startTime":201.784,"endTime":204.6,"body":"embryo at around five weeks, five days, the"},{"startTime":204.624,"endTime":207.232,"body":"yolk sac comes from the icm."},{"startTime":207.4,"endTime":210.36,"body":"And so if that is gone, but you see the gestational sac,"},{"startTime":210.424,"endTime":213.368,"body":"which is the sufectedoderm, then you know, there could"},{"startTime":213.408,"endTime":216.206,"body":"be the possibility of a blinded ovum. Now,"},{"startTime":216.262,"endTime":219.23,"body":"before you get scared, if that's your situation, keep in mind"},{"startTime":219.374,"endTime":222.054,"body":"that sometimes it's just hard to see the yoles sac."},{"startTime":222.246,"endTime":224.838,"body":"And so what we do is we usually bring people back a week"},{"startTime":224.918,"endTime":227.566,"body":"later, we take another look, and then we go, oh, there it"},{"startTime":227.582,"endTime":230.414,"body":"is. And then we're not as concerned. But that would be the first"},{"startTime":230.486,"endTime":233.414,"body":"sign that it may be a blighted ovum. That's"},{"startTime":233.446,"endTime":236.39,"body":"one of the benefits of fertility, is that"},{"startTime":236.414,"endTime":239.294,"body":"we do early ultrasounds. So by six"},{"startTime":239.326,"endTime":241.498,"body":"weeks, we early know something is wrong."},{"startTime":242.138,"endTime":244.498,"body":"But it's different if you're not going through fertility"},{"startTime":244.538,"endTime":247.41,"body":"treatment or if you wait to do"},{"startTime":247.434,"endTime":250.418,"body":"your first OBCAN until about"},{"startTime":250.458,"endTime":253.322,"body":"12 weeks, what will happen is you'll go in and"},{"startTime":253.33,"endTime":255.93,"body":"they'll notice there's a big sack and no baby in it."},{"startTime":256.114,"endTime":258.478,"body":"And that is extremely difficult"},{"startTime":258.938,"endTime":261.864,"body":"to even comprehend. And, you"},{"startTime":261.896,"endTime":264.588,"body":"feel like the whole time you've been pregnant and you have been,"},{"startTime":264.968,"endTime":267.668,"body":"and you feel like the whole time that baby has been developing,"},{"startTime":268.088,"endTime":270.976,"body":"but there was never a baby. And from your standpoint, there"},{"startTime":270.992,"endTime":273.568,"body":"was no different. You would never have"},{"startTime":273.608,"endTime":276.456,"body":"known that it was an empty sac. So what's"},{"startTime":276.512,"endTime":279.416,"body":"interesting is that the trophctoderm, which"},{"startTime":279.432,"endTime":282.2,"body":"is the placenta, can keep growing and keep"},{"startTime":282.344,"endTime":285.333,"body":"you from bleeding up to about 12 weeks. Now,"},{"startTime":285.283,"endTime":288.259,"body":"around that time, things start to get spotting and"},{"startTime":288.291,"endTime":291.235,"body":"may pass at that point. But the"},{"startTime":291.259,"endTime":294.043,"body":"main point here is that you can even go into an OB scan at 12"},{"startTime":294.083,"endTime":296.845,"body":"weeks and find out there's a blight ovum,"},{"startTime":297.029,"endTime":299.953,"body":"which seems devastating. So"},{"startTime":300.413,"endTime":303.389,"body":"what causes a blighted ovum? Well, as we just"},{"startTime":303.461,"endTime":306.309,"body":"discussed, as long as the ICM doesn't"},{"startTime":306.341,"endTime":308.313,"body":"grow, then that's a blinded ovum."},{"startTime":308.613,"endTime":311.317,"body":"But a lot of times it could be for"},{"startTime":311.445,"endTime":314.309,"body":"other reasons. Meaning it doesn't have to just be an"},{"startTime":314.341,"endTime":317.12,"body":"error. It can be something wrong with the, DNA."},{"startTime":317.54,"endTime":320.386,"body":"So for some reason, for if it's unemloyed"},{"startTime":320.548,"endTime":323.438,"body":"and the embryo is dividing and something goes wrong"},{"startTime":323.534,"endTime":326.262,"body":"because of the chromosomes, that would be a blinded"},{"startTime":326.286,"endTime":329.222,"body":"ovum. If it's due to something, let's say,"},{"startTime":329.246,"endTime":332.182,"body":"for example, just development when embry is"},{"startTime":332.206,"endTime":335.102,"body":"dividing and something goes wrong, that disrupts the formation of the"},{"startTime":335.126,"endTime":337.97,"body":"icm. That again is a blinded ovum."},{"startTime":338.67,"endTime":341.406,"body":"There'even theories out there about trophectoderm"},{"startTime":341.478,"endTime":344.206,"body":"overgrowth where the trophectoderm"},{"startTime":344.238,"endTime":347.134,"body":"may overdevelop so much that suppresses ICM"},{"startTime":347.262,"endTime":350.078,"body":"and that can cause a blind o them. But what"},{"startTime":350.134,"endTime":352.878,"body":"all these factors have in common is the ICM didn't"},{"startTime":352.934,"endTime":355.918,"body":"develop. Now, what's frustrating about it"},{"startTime":355.934,"endTime":358.542,"body":"is you really can't prevent it."},{"startTime":358.726,"endTime":361.35,"body":"And the reason you can't prevent it is because there's multiple"},{"startTime":361.39,"endTime":364.166,"body":"causes and it's not just genetics."},{"startTime":364.278,"endTime":367.15,"body":"Even though a lot of them may be, you can put back a"},{"startTime":367.19,"endTime":369.97,"body":"pgt, a normal embryo,"},{"startTime":370.35,"endTime":373.19,"body":"and still have a bliet ovum. Now, the good news"},{"startTime":373.23,"endTime":375.83,"body":"is most people just go on and have normal"},{"startTime":375.87,"endTime":378.734,"body":"pregnancies after that. Rarely do you see people"},{"startTime":378.782,"endTime":381.59,"body":"come back with another blighted oval. That's not"},{"startTime":381.63,"endTime":384.302,"body":"very common to have that repetitively. Now,"},{"startTime":384.366,"endTime":387.051,"body":"although you can't prevent a blood Oum, 100%,"},{"startTime":387.227,"endTime":390.187,"body":"there are things you can do that may reduce it. Such as"},{"startTime":390.235,"endTime":393.059,"body":"if you do pgta, you're going to at least remove"},{"startTime":393.131,"endTime":395.571,"body":"the chromosomal problems that can lead to"},{"startTime":395.587,"endTime":398.467,"body":"that. But one of the common myths"},{"startTime":398.515,"endTime":401.423,"body":"out there is that a blight ovum might be your"},{"startTime":401.463,"endTime":404.279,"body":"fault. That is absolutely not true. There is"},{"startTime":404.311,"endTime":407.119,"body":"no lifestyle factor that can cause it. It's"},{"startTime":407.151,"endTime":409.783,"body":"essentially a biological event that happened by"},{"startTime":409.823,"endTime":412.799,"body":"chance. Now, as I mentioned, because it's"},{"startTime":412.831,"endTime":415.351,"body":"still making HCG and you're getting all the symptoms of"},{"startTime":415.367,"endTime":418.367,"body":"pregnancy, you feel like everything's good."},{"startTime":418.495,"endTime":421.367,"body":"In reality, it's not. And so the question"},{"startTime":421.415,"endTime":424.367,"body":"is, for someone going through fertility, you're going"},{"startTime":424.375,"endTime":426.681,"body":"to know about it very early. But someone"},{"startTime":426.697,"endTime":429.449,"body":"who's got pregnant naturally and showed up at their"},{"startTime":429.481,"endTime":432.361,"body":"doctors at 12 weeks, they're going to find about it then."},{"startTime":432.457,"endTime":435.293,"body":"And then the question is, what do you do?"},{"startTime":436.073,"endTime":438.985,"body":"How does it go away? And so if you're"},{"startTime":439.049,"endTime":441.713,"body":"someone who, let's say is having spawning at 12"},{"startTime":441.753,"endTime":444.689,"body":"weeks at your, you could theoretically"},{"startTime":444.761,"endTime":447.185,"body":"just wait and let things"},{"startTime":447.289,"endTime":450.025,"body":"naturally miscarry because it's probably going to"},{"startTime":450.049,"endTime":452.701,"body":"happen pretty soon. But the problem"},{"startTime":452.757,"endTime":455.519,"body":"is, if you caught this early, like, a"},{"startTime":455.551,"endTime":457.927,"body":"fertility ultrasound at six"},{"startTime":457.975,"endTime":460.599,"body":"weeks, well, your body's not going to"},{"startTime":460.631,"endTime":463.551,"body":"recognize an issue until potentially 12 weeks. And"},{"startTime":463.567,"endTime":466.311,"body":"then the question is, do you wait"},{"startTime":466.367,"endTime":469.295,"body":"six weeks to allow this to continue to grow until"},{"startTime":469.319,"endTime":471.459,"body":"it outgrows, its blood supply and then"},{"startTime":471.491,"endTime":474.443,"body":"miscarries? And so there are some other options"},{"startTime":474.483,"endTime":477.411,"body":"in that situation. One of those options is a"},{"startTime":477.427,"endTime":480.371,"body":"medication called Visis Arostol. And"},{"startTime":480.387,"endTime":483.059,"body":"what that will do is that will get your body to realize something's"},{"startTime":483.091,"endTime":485.283,"body":"wrong and will help it miscarry"},{"startTime":485.403,"endTime":487.835,"body":"naturally. The other"},{"startTime":487.859,"endTime":490.552,"body":"option is what's called a, dnc,"},{"startTime":490.696,"endTime":493.38,"body":"which stands for dilation and curetage."},{"startTime":493.76,"endTime":496.072,"body":"And that will be able to remove the pregnancy tissue"},{"startTime":496.136,"endTime":499.12,"body":"surgically. And that way you have to wait"},{"startTime":499.24,"endTime":502.144,"body":"for things to happen. There is no"},{"startTime":502.192,"endTime":504.808,"body":"wrong or right. You can either do the expectedant"},{"startTime":504.864,"endTime":507.674,"body":"management and allow it to happen natural, the medication"},{"startTime":507.762,"endTime":510.642,"body":"management, or you can do the surgical management."},{"startTime":510.826,"endTime":513.762,"body":"But what's important to understand is, unlike other type of"},{"startTime":513.786,"endTime":516.562,"body":"miscarriages, where the body will recognize it soon,"},{"startTime":516.706,"endTime":519.41,"body":"this is different. The placenta will keep"},{"startTime":519.49,"endTime":522.338,"body":"growing. And because it will keep growing, it"},{"startTime":522.354,"endTime":524.73,"body":"could take weeks, even a couple"},{"startTime":524.77,"endTime":527.746,"body":"months before your body recognizes that something's"},{"startTime":527.778,"endTime":530.746,"body":"wrong. Now the question is, what happens"},{"startTime":530.818,"endTime":533.692,"body":"afterwards? So, depending on what you do, if"},{"startTime":533.716,"endTime":536.556,"body":"you end up doing the expected management or the medical"},{"startTime":536.628,"endTime":539.308,"body":"management, it's probably going to be good to follow it"},{"startTime":539.324,"endTime":542.308,"body":"up. You're going to watch HCG levels go down and"},{"startTime":542.324,"endTime":545.252,"body":"you're going to do a sono histogram to make sure everything did pass"},{"startTime":545.356,"endTime":547.92,"body":"before you go on to other fertility treatments."},{"startTime":548.54,"endTime":551.364,"body":"Now, if it's a situation where"},{"startTime":551.532,"endTime":554.46,"body":"you're doing the surgery, then you"},{"startTime":554.5,"endTime":557.296,"body":"know everything was removed. And so at that point"},{"startTime":557.428,"endTime":560.232,"body":"you can move forward right away. Now, as I"},{"startTime":560.256,"endTime":563.072,"body":"mentioned before, for most people, they'll"},{"startTime":563.136,"endTime":565.92,"body":"experience just one and then go on to have healthy pregnancies"},{"startTime":566.0,"endTime":568.728,"body":"and never have another one. Now, having one"},{"startTime":568.824,"endTime":571.536,"body":"also doesn't mean you're going to have more of them in the future."},{"startTime":571.648,"endTime":574.28,"body":"It doesn't mean that now there's something else wrong"},{"startTime":574.44,"endTime":577.26,"body":"again. It's just usually an error in development."},{"startTime":577.84,"endTime":580.696,"body":"Now, if you are having multiple of them, then"},{"startTime":580.728,"endTime":583.604,"body":"the question I would ask is one, is it being"},{"startTime":583.652,"endTime":586.468,"body":"diagnosed incorrectly? I've seen"},{"startTime":586.564,"endTime":589.356,"body":"many charts I review that will say they had"},{"startTime":589.388,"endTime":592.332,"body":"blighted ovums. And I see a yolk sac on it, and that's not"},{"startTime":592.356,"endTime":594.548,"body":"aighted ovum. That's an early"},{"startTime":594.604,"endTime":597.428,"body":"miscarriage. So it's important that gets diagnosed"},{"startTime":597.484,"endTime":600.164,"body":"correctly. But the other thing is, if it's happening"},{"startTime":600.212,"endTime":602.988,"body":"multiple times, it could be an"},{"startTime":603.044,"endTime":605.876,"body":"issue with the chromosomes. And that's where PGT may"},{"startTime":605.908,"endTime":608.5,"body":"come in. Because there might be a reason you're having those"},{"startTime":608.58,"endTime":611.264,"body":"very, very early miscarriages before even the"},{"startTime":611.272,"endTime":614.2,"body":"am develops. Personally, I have never"},{"startTime":614.24,"endTime":616.672,"body":"seen this, but I know theoretically it could"},{"startTime":616.696,"endTime":619.36,"body":"happen. But most of the time when I see someone with"},{"startTime":619.4,"endTime":622.352,"body":"recurrent blight ovum, when I go into the data and look"},{"startTime":622.376,"endTime":625.168,"body":"at it, they weren't blight ovums. No"},{"startTime":625.184,"endTime":627.816,"body":"matter when this happens, whether it's six weeks or 12"},{"startTime":627.848,"endTime":630.072,"body":"weeks, it is going to be"},{"startTime":630.176,"endTime":632.9,"body":"emotionally difficult and it should be."},{"startTime":633.2,"endTime":635.936,"body":"Losing a pregnancy, even without an embryo,"},{"startTime":636.008,"endTime":638.462,"body":"is still a loss. But what makes this so"},{"startTime":638.526,"endTime":640.65,"body":"difficult is the confusion."},{"startTime":641.03,"endTime":643.774,"body":"Because when youe told theres no baby"},{"startTime":643.822,"endTime":646.59,"body":"there but youve been feeling the pregnancy,"},{"startTime":646.75,"endTime":648.85,"body":"its something you couldnt even fathom."},{"startTime":649.87,"endTime":652.766,"body":"When you see that pregnancy test and you"},{"startTime":652.798,"endTime":655.694,"body":"know that everything is potentially going well, it"},{"startTime":655.702,"endTime":658.446,"body":"is confusing to then be told theres no"},{"startTime":658.478,"endTime":661.422,"body":"baby and you can start looking"},{"startTime":661.446,"endTime":664.398,"body":"for answers thinking maybe I did something wrong. And"},{"startTime":664.414,"endTime":667.302,"body":"there is a lot of frustration and a lot of times people dont even"},{"startTime":667.326,"endTime":670.278,"body":"believe the diagnosis. And thats why we usually will do another"},{"startTime":670.294,"endTime":673.182,"body":"ultrasound to make sure that you feel comfortable knowing"},{"startTime":673.366,"endTime":676.238,"body":"that this is truly the diagnosis. I think it is"},{"startTime":676.294,"endTime":679.006,"body":"important that if you do come across this, there are"},{"startTime":679.078,"endTime":681.39,"body":"support groups out there, counseling and other"},{"startTime":681.43,"endTime":684.406,"body":"things that can help the healing process. If you had to go"},{"startTime":684.438,"endTime":687.334,"body":"through this. A blighted ovum can be a confusing"},{"startTime":687.382,"endTime":690.238,"body":"and heartbreaking experience. But that is not the end of"},{"startTime":690.254,"endTime":692.546,"body":"your ferity journey. As I"},{"startTime":692.578,"endTime":695.578,"body":"mentioned, most women who experience one go on to"},{"startTime":695.594,"endTime":698.498,"body":"have healthy pregnancies. So if"},{"startTime":698.514,"endTime":700.95,"body":"you've been through this, you'not alone."},{"startTime":701.49,"endTime":704.362,"body":"You would be surprised at how many people have had blighted"},{"startTime":704.386,"endTime":707.266,"body":"ovums. There are support groups out there and there are"},{"startTime":707.298,"endTime":710.202,"body":"people you can talk to if you did have one. I'm"},{"startTime":710.226,"endTime":712.53,"body":"sorry there is never a good"},{"startTime":712.57,"endTime":715.554,"body":"miscarriage but I know this one is very difficult when people"},{"startTime":715.602,"endTime":718.39,"body":"have it.
