But maybe you have a"},{"startTime":163.595,"endTime":166.347,"body":"history of incompetent cervix"},{"startTime":166.459,"endTime":169.331,"body":"and putting back two embryos could be dangerous for you. Now, clearly there are bad"},{"startTime":226.417,"endTime":229.369,"body":"protocols. If someone told me, hey, I take cyanide with"},{"startTime":229.385,"endTime":232.233,"body":"my protocol, I'd say, well, that's pretty dangerous. I don't want you to die."},{"startTime":232.417,"endTime":235.353,"body":"But there are people out there who unfortunately think"},{"startTime":235.377,"endTime":237.977,"body":"they know more than they do. And sometimes they get bad"},{"startTime":238.009,"endTime":240.833,"body":"advice. I remember one patient came in,"},{"startTime":241.017,"endTime":243.609,"body":"freaked out because she was taking"},{"startTime":243.665,"endTime":246.545,"body":"Clomid and we weren't looking with ultrasound."},{"startTime":246.593,"endTime":249.457,"body":"And she said, oh, my God. Everyone in the group says, like,"},{"startTime":249.489,"endTime":252.44,"body":"I should leave you because I could end, up having, like, eight"},{"startTime":252.48,"endTime":255.472,"body":"babies in me. And how it's so dangerous to not be looking"},{"startTime":255.496,"endTime":258.235,"body":"with an ultrasound. And I said to her, you're,"},{"startTime":258.186,"endTime":261.106,"body":"taking a low dose of Clomid. The chance of you having"},{"startTime":261.154,"endTime":263.802,"body":"multiples is so low. That's why OB doctors give out"},{"startTime":263.842,"endTime":266.81,"body":"Clomid because it's very, very safe. The"},{"startTime":266.826,"endTime":269.546,"body":"group wasn't wrong. Yes, there are times it is"},{"startTime":269.594,"endTime":272.172,"body":"dangerous to be doing stimulation and not looking"},{"startTime":272.322,"endTime":274.92,"body":"for the follicles on the ultrasound, but this"},{"startTime":274.976,"endTime":277.688,"body":"situation did not fit that. Another"},{"startTime":277.72,"endTime":280.352,"body":"example I have is where a patient was freaked out"},{"startTime":280.512,"endTime":283.448,"body":"because she saw another doctor, and they said, oh, I"},{"startTime":283.48,"endTime":286.328,"body":"can't believe that the other doctor didn't make you"},{"startTime":286.48,"endTime":289.328,"body":"have genetic care screening. But the patient"},{"startTime":289.36,"endTime":291.92,"body":"was using donor sperm, and that was already tested and had"},{"startTime":291.976,"endTime":294.744,"body":"no genetic care screenings that came back positive."},{"startTime":294.808,"endTime":297.288,"body":"Which means even if she did come back with something"},{"startTime":297.32,"endTime":299.642,"body":"positive, it would have zero effect."},{"startTime":300.022,"endTime":302.75,"body":"Context matters. And so we were so excited"},{"startTime":324.334,"endTime":327.038,"body":"that she was pregnant, and then one day she came in"},{"startTime":327.15,"endTime":330.014,"body":"bleeding heavy. And anytime someone's"},{"startTime":330.038,"endTime":332.918,"body":"bleeding, it's very unusual with a thro transfer because we're giving them"},{"startTime":332.95,"endTime":335.868,"body":"progesterone. So other than, a subrary hemorrhage, it's very"},{"startTime":335.9,"endTime":338.724,"body":"weird to just bleed and lo the pregnancy."},{"startTime":338.908,"endTime":341.564,"body":"And so we always like to check up progesterone level just to make sure we're not"},{"startTime":341.588,"endTime":344.14,"body":"missing something. And her progesterone level was"},{"startTime":344.196,"endTime":346.98,"body":"zero. And I asked her, I go, it doesn't"},{"startTime":346.996,"endTime":349.86,"body":"make sense. Why is your progesterone zero? You're still taking your"},{"startTime":349.876,"endTime":352.824,"body":"progesterone. Correct? And she said to me, oh, well, the"},{"startTime":352.848,"endTime":355.448,"body":"girls online told me that when they did their"},{"startTime":355.48,"endTime":357.848,"body":"transfer, they were able to stop their progesterone right away"},{"startTime":357.92,"endTime":360.532,"body":"because their clin only goes until"},{"startTime":360.912,"endTime":363.36,"body":"the positive pregnancy test. And I"},{"startTime":363.376,"endTime":366.28,"body":"said, yes, but that was a fresh"},{"startTime":366.416,"endTime":369.288,"body":"transfer. And then in your situation, you did a"},{"startTime":369.32,"endTime":371.928,"body":"frozen transfer. And unfortunately,"},{"startTime":372.12,"endTime":374.944,"body":"she lost the pregnancy. I've seen patients"},{"startTime":375.008,"endTime":377.688,"body":"even delay their IVF cycle because they were told they weren't"},{"startTime":377.72,"endTime":380.358,"body":"ready and that they group thinks they should"},{"startTime":380.43,"endTime":382.99,"body":"wait. Others waste money on"},{"startTime":383.046,"endTime":385.462,"body":"supplements that do nothing. And in some"},{"startTime":385.502,"endTime":388.278,"body":"cases, people actually push for unproven"},{"startTime":388.31,"endTime":390.958,"body":"treatments that won't help them."},{"startTime":391.07,"endTime":393.918,"body":"But because someone in the group said it's the only thing that"},{"startTime":393.95,"endTime":396.59,"body":"worked, they wanted to do"},{"startTime":396.606,"endTime":399.282,"body":"it. Now, I appreciate something here."},{"startTime":399.662,"endTime":402.294,"body":"It is extremely stressful going through"},{"startTime":402.318,"endTime":405.19,"body":"fertility. The fear that you are not going to be pregnant"},{"startTime":405.246,"endTime":408.221,"body":"is so strong that, yes, you will"},{"startTime":408.245,"endTime":411.205,"body":"listen to anyone. You will eat rocks if that will"},{"startTime":411.237,"endTime":413.941,"body":"get you pregnant. My point here is not to tell"},{"startTime":413.965,"endTime":416.789,"body":"you that you are not smart for listening to people."},{"startTime":416.909,"endTime":419.821,"body":"You should. You should listen to everyone. Matter of fact, as"},{"startTime":436.425,"endTime":439.337,"body":"doctors, we're pretty boring. And I bet most of the stuff we put"},{"startTime":439.353,"endTime":442.345,"body":"out is pretty boring, like this podcast. I'm sure there are some people"},{"startTime":442.377,"endTime":444.989,"body":"will go, oh, this'so boring's so medical."},{"startTime":445.289,"endTime":448.177,"body":"And I have no doubt that every single person who"},{"startTime":448.193,"endTime":450.857,"body":"gives a vi had great intentions."},{"startTime":450.993,"endTime":453.473,"body":"But that doesn't mean good intentions can't"},{"startTime":453.521,"endTime":456.433,"body":"have bad outcomes. And that's the opposite of what"},{"startTime":467.685,"endTime":469.809,"body":"you want.
