Now, I'm not saying that"},{"startTime":128.744,"endTime":131.618,"body":"each transfer will be the same. If, you"},{"startTime":131.642,"endTime":134.106,"body":"have five embryos and two of those"},{"startTime":134.146,"endTime":137.014,"body":"embryos are genetically normal and"},{"startTime":137.07,"endTime":139.694,"body":"three are abnormal, doing"},{"startTime":139.75,"endTime":142.31,"body":"PGT is going to let you sort them"},{"startTime":142.478,"endTime":145.014,"body":"to know which one is the best, two to put back"},{"startTime":145.07,"endTime":147.702,"body":"first and avoid putting back some of the abnormal"},{"startTime":147.734,"endTime":150.386,"body":"ones. However, if you don't have that information"},{"startTime":150.886,"endTime":153.854,"body":"and you just transfer an embryo one at a"},{"startTime":153.87,"endTime":156.814,"body":"time, eventually you will get to"},{"startTime":156.83,"endTime":159.71,"body":"those two embryos, whether it's on the first"},{"startTime":159.758,"endTime":162.752,"body":"transfer, second, or even on the fifth. The difference"},{"startTime":162.784,"endTime":165.784,"body":"is that with pgt, you're likely to get there"},{"startTime":165.84,"endTime":168.728,"body":"faster because you're picking the embryo with the highest chance"},{"startTime":168.752,"endTime":171.576,"body":"of success first. Or in the scenario"},{"startTime":171.616,"endTime":174.424,"body":"with chocolates, you can get to that coconut first"},{"startTime":174.56,"endTime":177.13,"body":"before the jelly. Now, keep in Mind, it doesn't"},{"startTime":177.186,"endTime":180.034,"body":"mean that your chances are better. It might give"},{"startTime":180.058,"endTime":183.018,"body":"you the one that has the higher likelihood of having"},{"startTime":183.09,"endTime":186.082,"body":"success, but it still doesn't guarantee success."},{"startTime":186.202,"endTime":189.174,"body":"This means of those five embryos, if two of them"},{"startTime":189.198,"endTime":192.07,"body":"are genetically normal, whether you do pidgt"},{"startTime":192.126,"endTime":194.95,"body":"or not, you may still not end up with a"},{"startTime":194.966,"endTime":197.814,"body":"live birth, even though two of those embryos were genetically"},{"startTime":197.878,"endTime":200.78,"body":"normal. What we do know is that the, abnormal embryos had"},{"startTime":200.796,"endTime":203.572,"body":"a very, very low chance of effort becoming the live"},{"startTime":203.612,"endTime":206.284,"body":"birth. And you're able to avoid those. So"},{"startTime":206.308,"endTime":209.18,"body":"then no one should use pgt, right? When is it"},{"startTime":209.196,"endTime":212.188,"body":"really helpful then? Well, not exactly. PGT"},{"startTime":212.22,"endTime":214.624,"body":"is incredibly useful. But in more"},{"startTime":214.688,"endTime":217.64,"body":"scenarios where there'a higher chance of having an"},{"startTime":217.656,"endTime":220.552,"body":"deploy embryos, if you are 38 years old"},{"startTime":220.632,"endTime":223.624,"body":"or older, the risk of a chromosomal"},{"startTime":223.688,"endTime":226.232,"body":"abnormal embryo increases significantly. So"},{"startTime":226.272,"endTime":229.208,"body":"PGT can help avoid failed transfers or miscarriages"},{"startTime":229.28,"endTime":232.072,"body":"by identifying the healthy embryos first. Now, it doesn't"},{"startTime":232.112,"endTime":234.704,"body":"mean before age 38 that it can't be"},{"startTime":234.728,"endTime":237.264,"body":"useful. It's just saying when you base it off of the"},{"startTime":237.288,"endTime":240.264,"body":"idea of the risk of having an"},{"startTime":240.288,"endTime":243.134,"body":"anaploid embryo after age 38, more"},{"startTime":243.158,"endTime":246.014,"body":"than 50% of the embryos are abnormal. So there's"},{"startTime":246.038,"endTime":248.726,"body":"a clear benefit to getting to the"},{"startTime":248.814,"endTime":251.598,"body":"eulooid embryo first instead of having to go through"},{"startTime":251.63,"endTime":254.334,"body":"multiple transfers that fail. It would even be cost"},{"startTime":254.398,"endTime":257.35,"body":"effective because you would have to spend money doing multiple"},{"startTime":257.406,"endTime":260.366,"body":"transfers to get to that live birth. But there"},{"startTime":260.374,"endTime":262.95,"body":"are a couple less obvious but really valuable ways"},{"startTime":263.006,"endTime":265.282,"body":"PGT help in other ways."},{"startTime":265.622,"endTime":268.608,"body":"One is diagnostic clarity. When the"},{"startTime":268.64,"endTime":271.6,"body":"transfer doesn't work with the untested embryo, the first"},{"startTime":271.656,"endTime":274.104,"body":"assumption is always, well, it's probably"},{"startTime":274.128,"endTime":277.04,"body":"abnormal. But when you transfer a"},{"startTime":277.056,"endTime":280.016,"body":"EULID embryo that did'implant you know, the"},{"startTime":280.064,"endTime":283.032,"body":"issue isn't going to be the genetics"},{"startTime":283.112,"endTime":285.888,"body":"of the embryo. Now the emer could still have problems,"},{"startTime":286.0,"endTime":288.808,"body":"but at least it's not the genetics. And what that"},{"startTime":288.84,"endTime":291.416,"body":"does is it forces us to look at other potential"},{"startTime":291.464,"endTime":294.46,"body":"problems like the uterus, the timing"},{"startTime":294.656,"endTime":297.42,"body":"or other factors. And so you're able to"},{"startTime":297.476,"endTime":300.34,"body":"speed up the journey to success because instead of wasting"},{"startTime":300.396,"endTime":303.356,"body":"cycles guessing, you're able to narrow"},{"startTime":303.484,"endTime":306.484,"body":"the issue earlier. Sometimes it can also"},{"startTime":306.508,"endTime":309.332,"body":"be cost effective. So if you're at a clinic"},{"startTime":309.452,"endTime":312.132,"body":"that has a very high cost of embryo"},{"startTime":312.172,"endTime":315.028,"body":"transfer, then PGT may be useful."},{"startTime":315.14,"endTime":318.14,"body":"If it costs $8,000 to do a transfer, but"},{"startTime":318.156,"endTime":320.182,"body":"the PGT testing only costs"},{"startTime":320.332,"endTime":323.242,"body":"3200, then even one fell transfer"},{"startTime":323.322,"endTime":325.662,"body":"would make PGT cost effective."},{"startTime":326.122,"endTime":328.902,"body":"We see this a lot when people are using surrogates."},{"startTime":329.202,"endTime":332.05,"body":"It's very expensive to use a surrogate so you"},{"startTime":332.066,"endTime":334.698,"body":"don't want to waste money trying transfers that won't"},{"startTime":334.73,"endTime":337.442,"body":"work. So it's pretty common now to do PGT"},{"startTime":337.482,"endTime":340.37,"body":"testing. So that way, when you put the embryo in the circuit, you have"},{"startTime":340.386,"endTime":343.106,"body":"the highest chance of success, which will reduce your"},{"startTime":343.154,"endTime":346.034,"body":"overall cost of using a circuit. Another"},{"startTime":346.058,"endTime":349.022,"body":"situation is for people who are banking embryos for the"},{"startTime":349.062,"endTime":351.374,"body":"future. Let's say you're"},{"startTime":351.478,"endTime":354.47,"body":"37 and you want to freeze embryos, but you don't even want"},{"startTime":354.486,"endTime":357.382,"body":"to use them until you're like, 41 or 42."},{"startTime":357.542,"endTime":360.162,"body":"Or maybe you want to build a big family. So you know that"},{"startTime":360.542,"endTime":363.358,"body":"you need to get all the embryos now. Well, if you don't"},{"startTime":363.39,"endTime":366.042,"body":"test them, you really don't know what you have."},{"startTime":366.782,"endTime":369.414,"body":"It would be like the situation of getting a box of"},{"startTime":369.438,"endTime":372.342,"body":"chocolate, and you learned that the Willy Wonka factory"},{"startTime":372.422,"endTime":374.974,"body":"is closing down and you are never going to"},{"startTime":374.998,"endTime":377.958,"body":"get another box of chocolates. And you're"},{"startTime":377.99,"endTime":380.438,"body":"not sure if your box has those coconut"},{"startTime":380.47,"endTime":383.326,"body":"chocolates. And you're freaking out if you don't look at the"},{"startTime":383.334,"endTime":385.814,"body":"chocolates now. You don't know if you need to buy another box of"},{"startTime":385.838,"endTime":388.79,"body":"chocolates. And potentially, if you wait too long and"},{"startTime":388.806,"endTime":391.447,"body":"you finally open the box and find you don't have them, it's too late."},{"startTime":391.607,"endTime":394.145,"body":"The factory has, shut down. So by doing"},{"startTime":394.185,"endTime":397.177,"body":"PGT in this situation, you're able to tell which chocolates"},{"startTime":397.201,"endTime":400.153,"body":"are caramel, which are coconut, and which"},{"startTime":400.249,"endTime":403.065,"body":"are not worth saving, such as the jellies."},{"startTime":403.965,"endTime":406.065,"body":"Additionally, it's somewhat empowering"},{"startTime":406.805,"endTime":409.237,"body":"when the time or future fertility might be"},{"startTime":409.261,"endTime":411.925,"body":"limited. Knowing what you have in the freezer"},{"startTime":412.085,"endTime":414.785,"body":"isn't just helpful, it's empowering."},{"startTime":415.165,"endTime":418.109,"body":"There's another part of fertility that we don't talk a"},{"startTime":418.117,"endTime":421.015,"body":"lot about, and, that's the emotional toll of"},{"startTime":421.047,"endTime":423.967,"body":"going through fertility. Even today, I had a patient"},{"startTime":424.063,"endTime":426.983,"body":"who knows we have more kids, and they"},{"startTime":426.991,"endTime":429.795,"body":"were asking me should they do another retrieval"},{"startTime":429.947,"endTime":432.719,"body":"or do a transfer with our last embryo from the other clinic?"},{"startTime":433.059,"endTime":435.955,"body":"And they've had two failed transfers. What I told them"},{"startTime":435.987,"endTime":438.635,"body":"is having a pregnancy"},{"startTime":438.827,"endTime":441.635,"body":"helps reduce a ton of stress. And so"},{"startTime":441.707,"endTime":444.519,"body":"I lean towards the transfer because"},{"startTime":444.819,"endTime":447.755,"body":"going through another retrieval isn't going to help reduce"},{"startTime":447.787,"endTime":450.719,"body":"the stress. At this moment, she's young enough that she can wait,"},{"startTime":451.139,"endTime":453.955,"body":"but once she has that baby in her hand, a lot of that"},{"startTime":453.987,"endTime":456.283,"body":"stress goes away. But each one of those"},{"startTime":456.331,"endTime":459.083,"body":"failures causes stress and keeps"},{"startTime":459.131,"endTime":461.719,"body":"giving hopelessness"},{"startTime":462.379,"endTime":464.159,"body":"towards the future of having a child."},{"startTime":465.419,"endTime":468.315,"body":"Even when IVF is fully covered, as it is"},{"startTime":468.347,"endTime":471.139,"body":"in countries like France and Belgium, many"},{"startTime":471.179,"endTime":473.827,"body":"patients stopp treatment after failed transfers"},{"startTime":474.003,"endTime":476.923,"body":"and not because of the cost, but because of the emotional"},{"startTime":476.971,"endTime":479.779,"body":"exhaustion. In France, a study found that"},{"startTime":479.859,"endTime":482.413,"body":"26% of couples discontinued"},{"startTime":482.461,"endTime":485.333,"body":"IVF after just one unsuccessful"},{"startTime":485.381,"endTime":487.725,"body":"cycle, despite not having to pay for"},{"startTime":487.757,"endTime":490.621,"body":"anything. In Belgium, another"},{"startTime":490.645,"endTime":493.181,"body":"study showed that 50% women cited"},{"startTime":493.285,"endTime":496.245,"body":"psychological burden as the main reason for stopping"},{"startTime":496.277,"endTime":499.093,"body":"treatment. Each failed transfer"},{"startTime":499.141,"endTime":501.809,"body":"isn't just a medical event, it's a loss."},{"startTime":502.589,"endTime":505.237,"body":"It's hope deferred, and that could be"},{"startTime":505.253,"endTime":508.221,"body":"devastating. This is where PGT can play"},{"startTime":508.245,"endTime":511.235,"body":"a crucial role. By helping us identify"},{"startTime":511.267,"endTime":513.199,"body":"the most viable embryos up front,"},{"startTime":513.499,"endTime":515.923,"body":"PGT can reduce the number of"},{"startTime":515.971,"endTime":518.283,"body":"unsuccessful transfers for your"},{"startTime":518.331,"endTime":521.159,"body":"failures in fewer emotional setbacks."},{"startTime":521.539,"endTime":524.199,"body":"And that can help patients stay the course"},{"startTime":524.699,"endTime":527.643,"body":"to be able to have success. So after that,"},{"startTime":527.691,"endTime":530.171,"body":"then you would think everyone should do pgt."},{"startTime":530.315,"endTime":533.147,"body":"Well, not necessary. But there are plenty of"},{"startTime":533.163,"endTime":535.603,"body":"times when PGT isn't"},{"startTime":535.651,"endTime":538.357,"body":"needed. It's not unreasonable to do it,"},{"startTime":538.493,"endTime":541.089,"body":"but it's really not needed. For example,"},{"startTime":541.469,"endTime":544.269,"body":"if you're under 35 and you have lots of high"},{"startTime":544.309,"endTime":547.237,"body":"quality embryos and no history of miscarriage, you"},{"startTime":547.253,"endTime":549.917,"body":"might just be fine trampfering one at a time without testing."},{"startTime":550.053,"endTime":552.973,"body":"And if you're planning on using all your embryos anyways, your"},{"startTime":553.021,"endTime":555.653,"body":"cumulative pregnancy rate will be the same."},{"startTime":555.821,"endTime":558.685,"body":"It just takes a little longer to get there. As we start"},{"startTime":558.717,"endTime":561.621,"body":"this podcast, PGT does not"},{"startTime":561.725,"endTime":564.601,"body":"make your embryos better. It just"},{"startTime":564.705,"endTime":567.337,"body":"increases the chances of picking the best"},{"startTime":567.393,"endTime":570.081,"body":"one in the beginning. From a"},{"startTime":570.105,"endTime":572.561,"body":"cumulative standpoint, if you have a normal"},{"startTime":572.585,"endTime":574.913,"body":"embryo, you will eventually get to it."},{"startTime":575.081,"endTime":577.905,"body":"So if you're not doing pgt, don't"},{"startTime":577.937,"endTime":580.155,"body":"worry, you're not missing, on some magic"},{"startTime":580.187,"endTime":583.091,"body":"enhancement. Your embryos are what"},{"startTime":583.115,"endTime":586.027,"body":"they are, and if you're patient and you're"},{"startTime":586.043,"endTime":588.979,"body":"willing to transfer one at time, you'll likely reach"},{"startTime":589.019,"endTime":591.611,"body":"the same destination. It just might take a little bit"},{"startTime":591.635,"endTime":594.195,"body":"longer. Now, I'm not saying not to do"},{"startTime":594.227,"endTime":596.531,"body":"pgt, as you probably heard me say. Another"},{"startTime":596.555,"endTime":599.483,"body":"podcast, if it's free, I'd recommend everyone"},{"startTime":599.531,"endTime":602.499,"body":"to do it because I do think there are some benefits. But I"},{"startTime":602.539,"endTime":605.067,"body":"also don't want people to do it when they really don't need"},{"startTime":605.083,"endTime":607.827,"body":"it. The chances of actually having a baby with a"},{"startTime":607.843,"endTime":610.659,"body":"chromosomal abnormal T by not testing embros is"},{"startTime":610.699,"endTime":612.883,"body":"very small, which I talked about in a prior"},{"startTime":612.931,"endTime":615.555,"body":"podcast. So when you're younger,"},{"startTime":615.707,"endTime":618.655,"body":"doing PGT to prevent a child with down syndrome"},{"startTime":618.827,"endTime":621.687,"body":"really isn't necessary. Now, it's easy for me to"},{"startTime":621.703,"endTime":624.319,"body":"say that because I haven't had a child"},{"startTime":624.479,"endTime":627.319,"body":"with a chromosome abnormality. but if you're someone who did,"},{"startTime":627.439,"endTime":630.087,"body":"that might be very fearful for you, or maybe you had the family"},{"startTime":630.143,"endTime":632.535,"body":"member. So again, it's not unreasonable to do"},{"startTime":632.567,"endTime":635.271,"body":"pgt, but the reason you shouldn't do"},{"startTime":635.295,"endTime":637.855,"body":"PGT is because you think it makes the embryos"},{"startTime":637.927,"endTime":640.775,"body":"better that it does not. So let's"},{"startTime":640.807,"endTime":643.687,"body":"recap. PGT doesn't fix your"},{"startTime":643.703,"endTime":646.597,"body":"embryos, it just helps us find the best ones"},{"startTime":646.653,"endTime":649.541,"body":"faster. It doesn't improve the number"},{"startTime":649.565,"endTime":652.381,"body":"of embryos that are normal out of the cohort, it"},{"startTime":652.405,"endTime":655.229,"body":"just improves your per transfer success"},{"startTime":655.269,"endTime":658.125,"body":"rate. And that if you end up transferring"},{"startTime":658.237,"endTime":661.205,"body":"all your embryos, the cumulative chances of pregnancy"},{"startTime":661.277,"endTime":664.141,"body":"is going to be the same as if you just transferred the"},{"startTime":664.165,"endTime":666.445,"body":"PGT only embryos that were"},{"startTime":666.477,"endTime":669.285,"body":"euo. But the difference is you"},{"startTime":669.317,"endTime":672.291,"body":"reduce the failed attempts because you"},{"startTime":672.315,"endTime":675.251,"body":"have tested embryos. The group that this is going to help the"},{"startTime":675.275,"endTime":677.995,"body":"most is going to be women over age 38"},{"startTime":678.147,"endTime":680.819,"body":"or people banking embryos for later. And for"},{"startTime":680.859,"endTime":683.479,"body":"anyone needing clarity when transfer failed,"},{"startTime":683.779,"endTime":686.627,"body":"we do know that people with recurrent prenancy loss"},{"startTime":686.803,"endTime":689.539,"body":"will also benefit from this since they may have a lot of"},{"startTime":689.579,"endTime":692.467,"body":"abnormal embryos or even translocations that can"},{"startTime":692.483,"endTime":695.379,"body":"be helped by doing phgt. So you can put back the healthier"},{"startTime":695.419,"endTime":698.247,"body":"embryos, but the most important thing"},{"startTime":698.423,"endTime":700.943,"body":"is it helps reduce emotional toll of repeated"},{"startTime":700.991,"endTime":703.967,"body":"disappointment. I wish we had IVF covered in"},{"startTime":703.983,"endTime":706.735,"body":"our country, but we don't. But if you are"},{"startTime":706.767,"endTime":709.647,"body":"fortunate enough to be able to do pgt, don't let anyone make"},{"startTime":709.663,"endTime":712.575,"body":"you feel bad for doing it. And also don't"},{"startTime":712.607,"endTime":715.503,"body":"feel like you have to do it. If you're concerned"},{"startTime":715.671,"endTime":718.647,"body":"about the embryo being hurt through the biopsy or"},{"startTime":718.703,"endTime":721.647,"body":"concerned about stories you heard about abnormal"},{"startTime":721.663,"endTime":723.671,"body":"embryos bec become normal embryos, then don't do"},{"startTime":723.695,"endTime":726.515,"body":"pgt. Technically you know that eventually"},{"startTime":726.587,"endTime":729.451,"body":"you will get there by transferring the embryos. It's just"},{"startTime":729.475,"endTime":732.31,"body":"it may take a lot longer.
