Do you think that's good?"},{"startTime":38.125,"endTime":40.973,"body":"So everyone always wonders if"},{"startTime":40.989,"endTime":43.837,"body":"their ovaries are overachievers or"},{"startTime":43.981,"endTime":46.145,"body":"if they're just clocking in part time."},{"startTime":46.685,"endTime":49.317,"body":"Well, today we're going to crack open these numbers"},{"startTime":49.501,"endTime":52.065,"body":"on egg retrievals in IVF cycles"},{"startTime":52.725,"endTime":55.335,"body":"because understanding these numbers help set"},{"startTime":55.415,"endTime":58.015,"body":"realistic expectations. Now, before we jump into the"},{"startTime":63.495,"endTime":66.359,"body":"numbers, I think it's important to always understand that when"},{"startTime":66.367,"endTime":69.343,"body":"you're looking at data, it may look a lot"},{"startTime":69.399,"endTime":72.383,"body":"different than what you expected. And that's because when"},{"startTime":72.399,"endTime":75.247,"body":"we hear about information from the public, we"},{"startTime":75.271,"endTime":77.735,"body":"get a bit of a bias of"},{"startTime":77.775,"endTime":80.511,"body":"information. Let me give an example I've talked"},{"startTime":80.543,"endTime":83.089,"body":"about before. If you have a Pap"},{"startTime":83.137,"endTime":85.993,"body":"smear and it goes great, you're not going to go run"},{"startTime":86.049,"endTime":89.009,"body":"to the Internet and say, oh my God, best Pap smear ever. Five"},{"startTime":89.057,"endTime":91.929,"body":"star for sure. Unbelievable. Doctor."},{"startTime":92.057,"endTime":94.993,"body":"No, because that doesn't make sense. But"},{"startTime":95.009,"endTime":97.817,"body":"if that doctor is horrible and that pappar was"},{"startTime":97.921,"endTime":100.361,"body":"horrible, you're going to write about that."},{"startTime":100.513,"endTime":103.401,"body":"Because that makes sense to write about. No one wants to hear"},{"startTime":103.433,"endTime":106.409,"body":"about how everything just went normal. Plus, you're going"},{"startTime":106.417,"endTime":109.353,"body":"to sound pretty weird saying you had the five star pap smear."},{"startTime":109.489,"endTime":112.485,"body":"And that's not any different. When it comes to egg numbers,"},{"startTime":112.825,"endTime":115.585,"body":"no one's go going to write about, oh, Yeah, I got"},{"startTime":115.665,"endTime":118.525,"body":"12 eggs. That doesn't sound very good."},{"startTime":118.905,"endTime":121.809,"body":"But the person who had, let's say, 40 eggs, they're"},{"startTime":121.817,"endTime":124.321,"body":"gonna be like, oh my God, I got so many"},{"startTime":124.353,"endTime":127.297,"body":"eggs. And that's great. But in reality,"},{"startTime":127.441,"endTime":130.201,"body":"that's not the normal thing. That is the"},{"startTime":130.233,"endTime":132.753,"body":"bias of reporting. You're only"},{"startTime":132.809,"endTime":135.563,"body":"hearing what's being reported, but you're not"},{"startTime":135.579,"endTime":138.491,"body":"hearing all the reporting. It's no different than when"},{"startTime":138.523,"endTime":141.347,"body":"people think that women who are 50 years old can"},{"startTime":141.371,"endTime":143.93,"body":"have kids. Because again, there's the bias of,"},{"startTime":143.93,"endTime":146.794,"body":"reporting. You have Janet Jackson out there saying she got"},{"startTime":146.826,"endTime":149.666,"body":"pregnant but not telling the whole truth. And so when"},{"startTime":149.722,"endTime":152.594,"body":"all you hear is reporting of one thing, you"},{"startTime":152.626,"endTime":155.626,"body":"start to believe that is the norm. But in reality,"},{"startTime":155.762,"endTime":158.514,"body":"you're not getting the reporting of the people who"},{"startTime":158.546,"endTime":161.198,"body":"weren't doing well. So let's start first"},{"startTime":161.818,"endTime":164.818,"body":"by talking numbers. And let's break this into age"},{"startTime":164.858,"endTime":167.354,"body":"groups. Let's start first. If you're under"},{"startTime":167.386,"endTime":170.17,"body":"30, if you're under 30, your ovaries"},{"startTime":170.234,"endTime":172.878,"body":"are going to feel like they're bustling egg factory."},{"startTime":173.338,"endTime":176.274,"body":"And they're going to typically produce around 18 to 20 eggs per"},{"startTime":176.306,"endTime":178.97,"body":"retrieval. Now that doesn't mean all those will be"},{"startTime":178.994,"endTime":181.986,"body":"mature. About 15 to 16 mature eggs are going to be"},{"startTime":182.002,"endTime":184.758,"body":"ready for fertilization in that age group."},{"startTime":185.258,"endTime":188.126,"body":"But things start to change by the time you're in your"},{"startTime":188.182,"endTime":191.158,"body":"early 30s, they're still in great shape. They look"},{"startTime":191.198,"endTime":194.046,"body":"awesome. But now you might only get 15 to 18 eggs"},{"startTime":194.062,"endTime":196.926,"body":"retrieved. And of those we expect about 12 to"},{"startTime":196.942,"endTime":199.814,"body":"14 mature. Now this is when it gets"},{"startTime":199.846,"endTime":202.662,"body":"real. Once you go across age 35,"},{"startTime":202.71,"endTime":205.582,"body":"those numbers start the dip. Women between"},{"startTime":205.63,"endTime":208.278,"body":"35 and 37 usually see around"},{"startTime":208.398,"endTime":211.298,"body":"13 to 14 eggs. About 11 mature."},{"startTime":211.958,"endTime":214.758,"body":"At age 38 to 40, you're typically looking at"},{"startTime":214.798,"endTime":217.738,"body":"around 11 total eggs. With about 9 mature"},{"startTime":218.398,"endTime":221.11,"body":"and over 40, your orders might be putting in"},{"startTime":221.174,"endTime":224.158,"body":"fewer hours. With 41 to 42 year olds"},{"startTime":224.198,"endTime":226.658,"body":"getting around 9 to 10 eggs,"},{"startTime":227.118,"endTime":230.078,"body":"7, 8 mature. And if you're over"},{"startTime":230.118,"endTime":232.798,"body":"42, you might average about 7 to 8 total"},{"startTime":232.838,"endTime":235.218,"body":"eggs. About 6 mature."},{"startTime":235.678,"endTime":238.43,"body":"So yeah, the ovaries after 40,"},{"startTime":238.614,"endTime":241.238,"body":"they kind of like their free time. I tend to tell"},{"startTime":241.278,"endTime":243.738,"body":"everyone that on average"},{"startTime":244.038,"endTime":246.798,"body":"normal amount eggs is going to be somewhere around eight"},{"startTime":246.838,"endTime":249.582,"body":"to 12 mature eggs. Now that"},{"startTime":249.63,"endTime":252.318,"body":"is a bit of a summary of all the age"},{"startTime":252.358,"endTime":255.286,"body":"groups, but that's a general role. When people make"},{"startTime":255.342,"endTime":258.142,"body":"between 12 and 16, I would say"},{"startTime":258.19,"endTime":260.458,"body":"they are above average."},{"startTime":261.038,"endTime":263.622,"body":"And if you are making more than 16,"},{"startTime":263.75,"endTime":266.318,"body":"like 16 to 20, I would call you an"},{"startTime":266.358,"endTime":269.246,"body":"overachiever. Now it doesn't mean people don't get less than"},{"startTime":269.262,"endTime":272.004,"body":"this and that people don't get more than this. We're talking about a"},{"startTime":272.036,"endTime":274.58,"body":"bell shaped curve. Where does the majority of"},{"startTime":274.604,"endTime":277.396,"body":"people fall into? And so I would say"},{"startTime":277.452,"endTime":280.396,"body":"anyone who gets eight to 12 eggs, and if"},{"startTime":280.412,"endTime":283.233,"body":"you're under 38, you have a very high chance of coming"},{"startTime":283.289,"endTime":286.129,"body":"away successful. After 38 to"},{"startTime":286.177,"endTime":288.885,"body":"about age 40, you still have a good chance,"},{"startTime":289.185,"endTime":291.601,"body":"but it's possible you may need another"},{"startTime":291.633,"endTime":294.501,"body":"cycle. After 40, there's a"},{"startTime":294.533,"endTime":297.397,"body":"very good chance you'll need at least a second IVF cycle with"},{"startTime":297.421,"endTime":300.117,"body":"eight to 12 eggs. And so when people make more"},{"startTime":300.141,"endTime":302.785,"body":"eggs it just helps them have more chances,"},{"startTime":303.125,"endTime":305.837,"body":"but it doesn't make you abnormal if you don't make that"},{"startTime":305.901,"endTime":308.885,"body":"amount of eggs. Age isn't the only"},{"startTime":308.925,"endTime":311.669,"body":"factor in play here. Let's talk about AMH"},{"startTime":311.717,"endTime":314.293,"body":"levels, anti mallarin hormone."},{"startTime":314.469,"endTime":316.869,"body":"This is basically the ovarian"},{"startTime":316.997,"endTime":319.619,"body":"inventory number. If you have higher"},{"startTime":319.667,"endTime":322.651,"body":"MH levels, it means you probably are going to get more"},{"startTime":322.683,"endTime":325.643,"body":"eggs, while lower MH levels"},{"startTime":325.779,"endTime":328.691,"body":"are going to mean you'probably get fewer eggs at retrieval."},{"startTime":328.883,"endTime":331.619,"body":"AMH is a good predictor of how many"},{"startTime":331.667,"endTime":334.563,"body":"follicles you're going to get in the retrieval. But"},{"startTime":334.579,"endTime":337.483,"body":"there's more factors here. Your angral follicle count, you can"},{"startTime":337.499,"endTime":340.107,"body":"have the greatest EMH there is,"},{"startTime":340.291,"endTime":343.219,"body":"but if there are only four follicles on your ovary,"},{"startTime":343.387,"endTime":346.089,"body":"that's only X you're going to get. You can't get more"},{"startTime":346.137,"endTime":349.113,"body":"eggs than they are there. It doesn't matter what the"},{"startTime":349.129,"endTime":351.953,"body":"image shows. The aimage is just kind of an"},{"startTime":352.129,"endTime":355.097,"body":"association between the number of eggs you'll get"},{"startTime":355.241,"endTime":358.113,"body":"and your AMH level. And if you listend to some of my podcasts in"},{"startTime":358.129,"endTime":360.761,"body":"the past, you know that high FSH levels can also"},{"startTime":360.793,"endTime":363.713,"body":"limit how many follic call you're goingna get because you may not"},{"startTime":363.769,"endTime":366.585,"body":"respond this well. But then there's even a step"},{"startTime":366.665,"endTime":369.633,"body":"further. That's your doctor. Different"},{"startTime":369.689,"endTime":371.955,"body":"doctors, are willing to be more aggressive."},{"startTime":372.415,"endTime":375.255,"body":"And so some doctors give the same protocol"},{"startTime":375.295,"endTime":376.995,"body":"for everyone and that's okay."},{"startTime":377.775,"endTime":380.375,"body":"But some doctors might even"},{"startTime":380.495,"endTime":383.207,"body":"increase the medication doses for some people. And I do"},{"startTime":383.231,"endTime":385.967,"body":"that. Now, we're not trying not to get ovarian"},{"startTime":385.991,"endTime":388.447,"body":"hyperspimulation, but if I know someone's doing"},{"startTime":388.591,"endTime":390.84,"body":"IVF for PGTM M"},{"startTime":391.455,"endTime":394.351,"body":"and I know they're going to lose a lot of embryos because they have a disorder"},{"startTime":394.383,"endTime":397.321,"body":"affects 50% of their embryos, I'm going to push"},{"startTime":397.353,"endTime":400.273,"body":"them to try to make a lot more eggs. I do want to be in that"},{"startTime":400.329,"endTime":403.249,"body":"20 range. Whereas if I know someone's just going through regular"},{"startTime":403.297,"endTime":406.057,"body":"ivf, I may not take that much risk."},{"startTime":406.161,"endTime":408.937,"body":"I'm going to shoot around 16 eggs on them if they're"},{"startTime":408.961,"endTime":411.721,"body":"younger. And this is where I always talk about talking to your"},{"startTime":411.753,"endTime":414.657,"body":"doctor. If you want a lot more eggs, let"},{"startTime":414.681,"endTime":417.641,"body":"them know, hey, I'm okay. Being a little more uncomfortable. I"},{"startTime":417.673,"endTime":420.505,"body":"know I might have a slight risk of ohss, but I"},{"startTime":420.545,"endTime":423.4,"body":"really like to get more eggs. And that's not a reasonable"},{"startTime":423.472,"endTime":426.08,"body":"discussion. Doesn't mean they're going to do it. What's a"},{"startTime":426.112,"endTime":429.048,"body":"reasonable discussion? And don't be afraid you're going"},{"startTime":429.056,"endTime":432.016,"body":"to burn out your eggs. That's not a thing. We"},{"startTime":432.04,"endTime":434.976,"body":"put donors through very high doses and we push them, make lots"},{"startTime":435.0,"endTime":437.324,"body":"of eggs and their eggs are still great."},{"startTime":437.664,"endTime":439.992,"body":"Now, outside of your doctor, then there's going to be"},{"startTime":440.008,"endTime":442.604,"body":"conditions. So if someone is"},{"startTime":443.224,"endTime":446.192,"body":"a polycystic ovarian syndrome patient, they're going to"},{"startTime":446.208,"endTime":448.968,"body":"make more eggs. Because they have more eggs doesn't mean"},{"startTime":448.976,"endTime":451.84,"body":"theygs going to be better. They're just going to make more of them."},{"startTime":452.032,"endTime":454.912,"body":"And if someone with severe diminished ovarian reserve, it"},{"startTime":454.928,"endTime":457.344,"body":"doesn't matter what their age is, they're also going to make"},{"startTime":457.384,"endTime":460.376,"body":"less. So then the question comes up, is"},{"startTime":460.44,"endTime":462.856,"body":"more always better? The point is"},{"startTime":462.96,"endTime":465.712,"body":"no. Sure doesnzen egg sounds great,"},{"startTime":465.888,"endTime":467.964,"body":"but what matters most is quality."},{"startTime":468.624,"endTime":470.856,"body":"It doesn't matter if you get a billion"},{"startTime":470.92,"endTime":473.816,"body":"eggs if only 10 are mature, you only"},{"startTime":473.84,"endTime":476.732,"body":"have 10 capable fertilization. Now, that doesn't mean"},{"startTime":476.748,"endTime":479.676,"body":"there isn't a sweet spot. Studies show that"},{"startTime":479.7,"endTime":482.628,"body":"the optimal sweet spot for retrieval is about 10 to 15"},{"startTime":482.676,"endTime":485.396,"body":"eggs. That's kind of what I was talking about"},{"startTime":485.46,"endTime":488.156,"body":"earlier. Eight to 12 eggs usually is going to get you"},{"startTime":488.18,"endTime":490.924,"body":"pregnant unless you're in the further years of your life."},{"startTime":491.084,"endTime":494.06,"body":"But what about fewer than five? Well, that will show"},{"startTime":494.092,"endTime":496.98,"body":"that you'll have lower chances. What about"},{"startTime":497.012,"endTime":499.956,"body":"having over 30? Well, sounds great, but that"},{"startTime":499.98,"endTime":502.104,"body":"does increase your riskso oarian hyper stimulation"},{"startTime":502.674,"endTime":505.554,"body":"very high. But the real thing is what is"},{"startTime":505.594,"endTime":508.134,"body":"the goal? And the goals get pregnant."},{"startTime":508.474,"endTime":511.13,"body":"And so it doesn't matter if you get 12"},{"startTime":511.162,"endTime":513.994,"body":"eggs, 20 eggs or even 30"},{"startTime":514.034,"endTime":517.002,"body":"eggs. What matters is are"},{"startTime":517.018,"endTime":519.41,"body":"you going to be able to get pregnant. And that's the good"},{"startTime":519.442,"endTime":522.21,"body":"news. When we talk about that sweet spot of 10 to"},{"startTime":522.242,"endTime":525.05,"body":"15, it just means getting a viable embryo to"},{"startTime":525.082,"endTime":527.97,"body":"transfer. And so if you get a viable embrber the"},{"startTime":528.002,"endTime":530.818,"body":"transfer and you only had, let's say, eight eggs,"},{"startTime":530.906,"endTime":533.866,"body":"who cares? You're still in the same spot as everyone else"},{"startTime":533.93,"endTime":536.882,"body":"because now you have a chance. In the end, if"},{"startTime":536.898,"endTime":539.762,"body":"you're the guy who bought 12 lottery tickets or"},{"startTime":539.858,"endTime":542.682,"body":"the guy who bought one, if the guy who bought one"},{"startTime":542.738,"endTime":545.722,"body":"won the lottery, he's not embarrassed that he didn't get a"},{"startTime":545.738,"endTime":548.506,"body":"bunch of lottery tickets because he still won the"},{"startTime":548.53,"endTime":551.242,"body":"lottery. And that should be your goal, winning the"},{"startTime":551.258,"endTime":554.214,"body":"lottery, not worrying about how many tickets you got."},{"startTime":554.754,"endTime":557.722,"body":"Sure, you might feel better than your chances because"},{"startTime":557.738,"endTime":560.226,"body":"you got more tickets, but the guy only bought one and"},{"startTime":560.25,"endTime":563.019,"body":"one, he's still happy. Now, here's some"},{"startTime":563.067,"endTime":565.683,"body":"interesting information. If you look at it"},{"startTime":565.739,"endTime":568.627,"body":"globally and wonder, do we make more eggs"},{"startTime":568.651,"endTime":571.515,"body":"or less eggs than other countries? What you're going to find is we actually"},{"startTime":571.555,"endTime":574.338,"body":"make more than most countries now. it's because as Americans,"},{"startTime":574.378,"endTime":576.754,"body":"we're better. I mean, of course that's"},{"startTime":576.786,"endTime":579.65,"body":"why, but in reality, no, we're not"},{"startTime":579.674,"endTime":582.634,"body":"better. It's because of different practices of fact that"},{"startTime":582.666,"endTime":584.794,"body":"some countries, they even do mild"},{"startTime":584.866,"endTime":587.842,"body":"ivf, purposely retrieving fewer eggs"},{"startTime":587.93,"endTime":590.714,"body":"and so if you're someone who's going to another country to maybe do"},{"startTime":590.746,"endTime":593.522,"body":"IVF because it's less cost, keep in mind that"},{"startTime":593.57,"endTime":596.57,"body":"they might only make six to eight eggs. And so although it may"},{"startTime":596.594,"endTime":599.57,"body":"be less cost, you also actually low your chances by"},{"startTime":599.594,"endTime":602.41,"body":"not getting as many eggs. In other countries, such"},{"startTime":602.434,"endTime":605.338,"body":"as in Israel, where a lot of young people are doing ivf, they"},{"startTime":605.378,"endTime":608.362,"body":"actually get more eggs because there's more people who are younger doing"},{"startTime":608.37,"endTime":611.172,"body":"an ivf. It's a little bit different there that people"},{"startTime":611.22,"endTime":613.86,"body":"go into IVF a lot sooner because it's part of"},{"startTime":613.964,"endTime":616.568,"body":"the medical system there. In the end,"},{"startTime":616.868,"endTime":619.604,"body":"ovaries age everywhere, but"},{"startTime":619.636,"endTime":622.268,"body":"practices vary widely, giving us"},{"startTime":622.388,"endTime":624.956,"body":"egg specations that differ"},{"startTime":625.052,"endTime":628.004,"body":"globally. So in the end, what"},{"startTime":628.036,"endTime":630.972,"body":"you should remember today, yes, there is a"},{"startTime":631.02,"endTime":633.74,"body":"general number of eggs that most people get, but in"},{"startTime":633.764,"endTime":636.644,"body":"reality, it's probably not what you thought. Ask yourself that"},{"startTime":636.676,"endTime":639.62,"body":"question. Did you think that the sweet spot would be 10"},{"startTime":639.644,"endTime":642.124,"body":"to 15? Did you think"},{"startTime":642.156,"endTime":645.044,"body":"that most people will come away successful"},{"startTime":645.076,"endTime":647.428,"body":"with that number? Or did you think you need around"},{"startTime":647.508,"endTime":650.404,"body":"25? Many times I've heard from people saying,"},{"startTime":650.516,"endTime":652.82,"body":"well, this woman, she got 24 eggs and only got two"},{"startTime":652.844,"endTime":655.644,"body":"embryos. And that's the fear, because you probably"},{"startTime":655.716,"endTime":658.612,"body":"only heard the number of eggs they got. But what"},{"startTime":658.62,"endTime":661.428,"body":"you didn't know is that 24 eggs they got, maybe"},{"startTime":661.468,"endTime":664.406,"body":"only 16 of them were mature. And that's really the dumb number you"},{"startTime":664.422,"endTime":667.326,"body":"need to be looking at. How many mat eggs do you get?"},{"startTime":667.462,"endTime":670.374,"body":"That's why when I talk to patients, I don't even really tell them"},{"startTime":670.406,"endTime":673.126,"body":"how many egs are re going to get. I said to them, you're probably going to get around"},{"startTime":673.182,"endTime":676.11,"body":"15 mature eggs. And then they get nervous, say, well, what if those"},{"startTime":676.134,"endTime":678.75,"body":"aren't mature? And say, no, those are the mature eggs."},{"startTime":678.894,"endTime":681.894,"body":"I tell them, don't worry about how many eess you get. It doesn't matter. What"},{"startTime":681.926,"endTime":684.606,"body":"matters is how many mature you get, because those are the ones"},{"startTime":684.702,"endTime":687.63,"body":"who can be fertilized. There's no question there's going to be"},{"startTime":687.654,"endTime":690.254,"body":"differences. People with different AMHs, Andra"},{"startTime":690.286,"endTime":693.122,"body":"follicle counts, even stimulation protocols are"},{"startTime":693.17,"endTime":695.594,"body":"going to be different. But egg quality"},{"startTime":695.706,"endTime":698.274,"body":"trumps pure quality every time."},{"startTime":698.426,"endTime":701.33,"body":"Trust me, I have proof. My wife was never"},{"startTime":701.354,"endTime":704.25,"body":"a quantity person. She didn't make a lot of eggs, but when she made"},{"startTime":704.274,"endTime":707.25,"body":"them, she produced that golden ticket. So"},{"startTime":707.274,"endTime":709.906,"body":"the point is, averages are just"},{"startTime":710.002,"endTime":712.706,"body":"averages. Your story might be different."},{"startTime":712.882,"endTime":715.386,"body":"It only takes one golden egg to change"},{"startTime":715.442,"endTime":718.29,"body":"everything. So although lots of liary tickets make us"},{"startTime":718.314,"endTime":721.194,"body":"feel better and feel like we have a better chance of winning"},{"startTime":721.306,"endTime":724.194,"body":"in the end, the winning ticket is what makes us"},{"startTime":724.226,"endTime":727.22,"body":"win. I, know it's scary when you don't make as many eggs because it"},{"startTime":727.252,"endTime":730.1,"body":"feels like it won't work. But I can assure you 8 to 12"},{"startTime":730.132,"endTime":733.116,"body":"eggs is normal. And if you have less than that, don't"},{"startTime":733.14,"endTime":735.932,"body":"be worried. It doesn't mean you don't have that winning ticket,"},{"startTime":736.028,"endTime":738.924,"body":"it just means statistically it might be a little bit lower."},{"startTime":739.084,"endTime":741.476,"body":"So keep those egg expectations"},{"startTime":741.62,"endTime":743.824,"body":"real and don't stop trying."},{"startTime":744.244,"endTime":746.964,"body":"Hopefully this podcast was helpful for you.
