This show is kind of the brainchild of Donna Lee and myself to really highlight why we think men's health issues deserve special attention. Really, to encourage people to ask questions and really get into topics that you may not either do on your own, or don't feel comfortable seeing a doctor for. A lot of times, people didn't even know that certain conditions could be treated like, you know, orgasmic issues or certain times of erectile dysfunction. A lot of times younger guys feel kind of out of place if they feel like they're having a problem that they think they're too young for. And sometimes the older guys feel like, you know, these things are normal parts of aging.

Donna Lee:

Right, and too proud to go.

Dr. Mistry:

And too proud to go. And so all of these things are wonderful topics for us to tackle. Our practice is unique. We have a nutritionist on staff, we have a pelvic floor, physical therapist on staff, we have sex therapy on staff. That's in addition to the four amazing doctors, the three amazing nurse practitioner and PAs. We do in-office obstructive sleep apnea testing because that condition and urologic conditions have such great overlap. And when you come in, it is our goal to take care of you from head to toe, wven though our focus is really just in the middle.

Donna Lee:

Just the little middle part--or the big middle part. Sorry guys.

Dr. Mistry:

That's right. You don't say"little" in our office.

Donna Lee:

That word's never used.

Dr. Mistry:

That's right.

I wrote somebody up the other day for using the word"little" around you.

Dr. Mistry:

Oh, that's awesome. Patient engagement and patient questions are awesome. And I think that we had a question about one of the procedures and specialties that we have here. They call it the penile pump, and I'd love to, why don't you tell the audience the question and then let's go into the details?

Donna Lee:

Audience? Well, we've grown from having one listener to an audience?

Dr. Mistry:

Yes. Thank you for listening, Michael.

Donna Lee:

So this patient sent in after last week's show:"Dr. Mistry, I heard you on the radio show talking about a doctor who is with your practice and specializes in the surgery for the penis pump." First, he wanted the doctor's name, but he also, this patient went on to ask about the different types of pumps and how he can find more information from us.

Dr. Mistry:

That's great. And so what we're talking about here is the doctor's name first in our practice is Dr. Yang, which I think is a great name for a urologist.

Donna Lee:

It really is. It's really kind of like"Mistry," but" Yang" is more specific.

Dr. Mistry:

That's correct. You know, what kind of doctor Dr. Yang is? He takes care of your Yang. Dr. Yang, he did his medical school at UTMB, which is apparently a medical school in Galveston. I didn't know they had anything else in Texas besides Baylor, but...then he did his residency in Chicago...

Donna Lee:

As did you.

Dr. Mistry:

...at UIC. I went to college in Chicago.

Donna Lee:

Oh.

Dr. Mistry:

I stayed at Baylor for mine.

Donna Lee:

Oh, that's right. The amazing Baylor.

Dr. Mistry:

That's right. He then left and went to Tampa and did a fellowship in just advanced surgery for erectile dysfunction. It may not seem like that could take a whole year, but you know, if somebody is going to handle that part of your body...

Donna Lee:

They better take a year.

Dr. Mistry:

...they better take a year. And you want them to take special care of that body part.

Donna Lee:

The ding-a-ling?

Dr. Mistry:

That's right. So at Baylor, in fact, a guy named Brantley Scott at Baylor, where I trained right, invented...

Donna Lee:

...the penis?

Dr. Mistry:

...the inflatable penile prosthesis. We were trying to find a way to overcome erectile dysfunction. This is before there was medicines or anything, so if you couldn't get it up, you were out of luck. What he invented was a silicone device that's implanted completely internally with a pump that goes into the scrotum, two cylinders that fill with fluid in the penis, and then a reservoir that holds the fluid in the abdomen. Everything is completely concealed. You can't tell that somebody has one of these things.

Donna Lee:

Even the pump?

Dr. Mistry:

Even the pump.

Donna Lee:

Underneath?

Dr. Mistry:

That's correct. You cannot tell.

Donna Lee:

It's not like a third testicle?

Dr. Mistry:

It kind of looks like...but I mean, unless somebody is really feeling down there, and at that point, you've already won anyway.

Donna Lee:

It's touch me!

Dr. Mistry:

That's right. What you do is when you when you press on this pump, it inflates the cylinders and gives you a very satisfying erection. Active intercourse feels the same, your orgasm feels the same, and you're very satisfied. It has a satisfaction rate over 90%!

Donna Lee:

As is she.

Dr. Mistry:

Would you believe that it's covered by insurance?

Donna Lee:

That is so upsetting.

Dr. Mistry:

When I was at the VA training...

Donna Lee:

Breasts are not covered but penises are covered.

Dr. Mistry:

...when I trained at the VA, when I was a resident, we actually had a patient who came in that said that his wife just activates it at night, while he's sleeping.

Donna Lee:

Stop it. Oh, party.

Dr. Mistry:

I think we call that sexual assault nowadays, but back then it was funnier.

Donna Lee:

He's not complaining.

Dr. Mistry:

That's right. He was not complaining. It takes it to a new level of just sit there and just lie there.

Donna Lee:

That's so funny.

Dr. Mistry:

That penile prosthesis, it can also help lengthen the penis. So spinal cord injury patients like it because it helps keep the, if they're having urinary issues where they were a bag over their penis all the time, or patients with limited mobility, having that constant girth in there can also help. They make three different kinds. One is called a malleable prosthesis in which it's always kind of erect. So then...

Donna Lee:

It's kind of off-putting for a woman, though, isn't it?

Dr. Mistry:

No.

Donna Lee:

Is that a banana in your pocket?

Dr. Mistry:

It's just a lot easier to deal with. And then you just bend it up and down, like some of our listeners may remember Gumby, you know? You can bend it up and down and out of the way. So although it may sound like...

Donna Lee:

I thought you were going to say Beckham. You said Gumby.

Dr. Mistry:

Gumby. It may sound like...Beckham?

Donna Lee:

"Bend it Like Beckham."

Dr. Mistry:

Oh, okay. Well, there you go. I can acceptthat. That use of malleable prosthesis is something that's a wonderful option for people with limited dexterity of their hands. It's easy to put in. It's very effective from day one and there's no mechanical transfer of fluid. So the reliability of it's very good. The more natural feeling version of these are going to be the inflatable ones, and there's two piece and three piece. Two piece ones do not require any kind of extra reservoir, but the standard three piece IPP or inflatable penile prosthesis would be what we would consider kind of the standard of care. It takes about an hour to put in. As I mentioned, Medicare covers it most all insurances cover it. And it, you know, negates the need for pills or shots or vacuum erection devices. And it results in an incredibly satisfying kind of erection.

Donna Lee:

What's the recovery time?

Dr. Mistry:

It takes about a week to recover and then you can start using it in six weeks, once everything is kind of healed up. And I think that unfortunately, a lot of men think of this kind of thing as the last possible thing they would ever consider. But by that time you're already so frustrated, and you're so...you know, that the idea of having to go through surgeries is such a monumental thing. And you may not get that much use out of it. I mean you'd probably get a lot more use out of it if you're a 55 year old diabetic man whom[inaudible] doesn't work anymore, like this is when you should consider it, or the taking of the medicines causes side effects. Because the further you go down in life where you're not having satisfactory functional performance, the less likely you are to pursue it, and just kind of live with whatever you have. And couples that get out of the habit of, you know, sexual intimacy, I think that they, it's hard to get back into the habit. Usually that's something that kind of fades over time.

Donna Lee:

True that.

Dr. Mistry:

True that. And so if...

Donna Lee:

Sorry, Michael.

Dr. Mistry:

If getting your erectile function, you know, better controlled, and when you come to our practice, you're going to get absolutely top notch care with a fellowship-trained surgeon, and you're going to get nutritional care. We're going to make sure your other medical conditions are under control, like your diabetes and high blood pressure. We're going to make sure you're surgically optimized from a nutrition standpoint.
