In this segment, Dr. Mistry and Donna Lee answer two listeners' questions. The first question comes from a 75 year old man who recently masturbated for the first time in years only to find that his ejaculate was brown. Dr. Mistry explains that discolored semen is usually an indication of an infection such as prostatitis, and that if the only symptom is the discolored semen, the infection has likely passed. In this case, the patient should clear out the old semen until the discoloration is gone. If there is fresh blood in the semen, or if there are additional symptoms such as urinary urgency or frequency, incomplete bladder emptying, or lower back pain, then a course of antibiotics may be required to clear the infection.

The second question addressed in this segment is from a 50 year old who developed Peyronie's disease and erectile dysfunction after a prostatectomy. He asks Dr. Mistry about the effectiveness of shockwave or spark wave therapy for his condition. Dr. Mistry explains that having severe ED after prostatectomy along with Peyronie's usually means that getting an inflatable penile prosthesis is your best option. When you put in the rigid silicone cylinders that fill up your penis, you can model the penis over the cylinders and straighten out the effects of Peyronie's. This will give you back length and adequate girth, and the product itself gives you tremendous spontaneity. For men who aren't interested in surgical solutions, Dr. Mistry describes a combination of therapeutic modalities that could help reduce symptoms of ED and Peyronie's. He recommends Xiaflex injections for the Peyronie's disease, intracavernosal injections for getting robust erections, and then spark wave or shockwave therapy to help regenerate the erectile bodies to make them healthier.
