How One Doctor Found $500,000 in Hidden Revenue in 3 Hours
Use Left/Right to seek, Home/End to jump to start or end. Hold shift to jump forward or backward.
What if 3 hours could uncover $500,000 in hidden revenue sitting inside your private practice right now?
That's exactly what happened at the inaugural Private Practice Summit 2026 — and in this episode, Dr. Una pulls back the curtain on the 3 shifts that transformed 100% of the doctors in the room from physicians into Physician CEOs in just 48 hours.
From startup practices to multi-seven-figure operations… insurance-based to direct primary care to telemedicine-only… every doctor walked out a different version of themselves. And the wins weren't theoretical — one doctor identified a $90,000 revenue opportunity she'd been missing, another uncovered a billing code she wasn't using that once applied will create an additional $500,000 in revenue, and another completely restructured her schedule on the spot.
In this episode, Dr. Una breaks down:
- The identity shift every physician must make to scale a profitable practice (and why your strategy will fail without it)
- The 5-step reverse-engineering process that helped doctors uncover six and seven figures of hidden revenue in a single afternoon
- Why the "right room" accelerates your growth faster than any strategy or tactic ever could
- The 3 things billion-dollar CEOs work on first — and why most physicians skip them
- How to know if community is the missing ingredient in your private practice journey
If you've ever felt stuck, isolated, or unsure of what your practice could actually become — this episode is your roadmap.

Well, hello, hello, my friends. Welcome back to another episode of the OnTraM Day podcast. This is actually an interruption of the regular schedule because I wanted to come talk to you about the magic that happened in the private practice summit 2026. It was our first one. We have historically done On-TraM Day Live every single year. Very powerful event, very transformative. So many people can trace the big breaks in their businesses right back to the On-TraMD Live experience. And so this year, you know, I kind of had this feel that I wanted to do an event specifically for doctors and private practices and then go even deeper. And so go beyond the concepts and the frameworks and the workshops and do all of those things in a way that is much deeper. People are able to work in real time. And my suspicion was that people will be able to get even bigger wins. That was that was my hypothesis. And so we did that, and it was it was such an amazing experience. And we're talking 100% of the people in attendance walked away saying that the version of them that was leaving the conference was completely different from the version of them that came to the conference. And we're we're talking as far as them showing up as the CEOs of their private practices and all of that. It was a phenomenal, phenomenal experience. And so there were, when I think about it, there are three things that stood out to me. And, you know, I want to talk to you about those three. And mind you, you know, the room was filled with doctors who came from all over the country. Many of them had never met each other before. It wasn't a room like what I would have when I host events for the entrepreneur business school, where some of these people have been friends for a year, two years, three years, four years, five years. Um, you know, and it I was interested to see how this would play out. And the makeup of the room, we had people from startup um all the way to multi-seven figures in revenue. We had people who had insurance-based practices, direct primary care practices, direct specialty care practices, hybrid practices, telemedicine only practices. And so there was a there's a beautiful variety in the room. And yeah, I was I was really, really excited to see this. But anyway, so when um we got into it, um, I knew that the first thing that I would need to do is to shift everybody's identity. I was kind of clear about that. I've worked with thousands of doctors over the last seven years or so. And so I know a lot of times we think, you know, I want the strategy, I want the tactic, I want all of that, but it's almost never that. It's almost never that. And so the starting point was selling people on the fact that, okay, you're a physician, and that's great. But this new iteration of you is going to require that you are not just a physician, but that you are a physician CEO. And um, so sold them on that concept that you have two hats. Um, I actually told them I had two personalities, but not multiple personality disorder. But you have two hats. There's you the physician, and there's you the CEO. And if your business is missing a CEO, your business is going to suffer, right? And so we went through all of that. And to watch them in real time change from I'm a physician to I am a physician CEO and I'm the CEO of my practice, which is a mini healthcare system. Um, when that clicked for them, I could see it in the room. When it clicked, it was just like light bulbs going up. You can see people shifting in their seats, you could see people holding their heads higher, you could see people ready to make decisions and change things. And it was just, I mean, it was a thing of beauty. And so that was the first thing. And, you know, when you think about what is going on in your business, whether it's a private practice or your coach or you're an author or you host events or whatever that is, you know, the first thing you need to think of is, you know, who do I need to become to become the kind of person who pulls this off? That literally is always where everything starts from. Um, a lot of times people think either mindset stuff is it's woo-woo or it's soft and things like that. And no, it's not woo-woo, and no, it's not soft. And I have had the privilege of being in rooms with people running half a billion dollar businesses, billion-dollar businesses. And I will tell you this, their biggest challenges are usually not tactical or strategic. It's mindset that's what they work on first. And so, you know, so again, seeing the doctors do this in real time, it was, it was, it was really a thing, it was really a thing of beauty. And it's and it shifted the whole room. So within the first three hours, that was a three-hour block, the the room had shifted completely, right? And so, so that was the first thing. And the second thing that happened is, you know, we we kind of got over this discomfort about, you know, talking about numbers, only revenue goals and stuff like that. And it was very interesting. So um we we talked about it. Like we're, you know, we're gonna talk about numbers, we're gonna sit in numbers, we're gonna dig in numbers, we're gonna play around with that. And I just want to invite you to do that. I find a lot of doctors won't even set goals. And these are doctors who've been in business for a long time, won't even set revenue goals that they truly own because they don't want to have the disappointment of not hitting it, or they're overwhelmed by all the challenges. So they just make an executive decision that things like that are not possible and things like that. And, you know, I I held space. We did this, we were at this for hours. I was I held space. I was like, okay, people, come on, we're gonna do it, right? And that whole process of setting your goals and reverse engineering your goals, all of those are things that make people nervous. A lot of times they don't want to touch it, and all of that. And there's always a statement I make that when you start looking at the numbers, when you start owning your goal, when you start doing these things, you become strategic. Because sometimes people will think that, you know, I'm not that kind of person. I'm like, you have no idea what kind of person you are. Until you're around the right mentorship accountability, accountability, and community, you have no clue. And so it was so fun for people to start working the numbers and I walk them through it's it's it's a five-step process to reverse engineer your revenue goals. And it was interesting for them to that, because you know, after it all, you know, we we were at this for a while. After it all, I said, okay, I wanted people to come up and share like the big aha's that came from them working this process of looking through their numbers. And I found people who are like, you know, a doctor got up, she was like, oh my goodness, like my schedule is not optimized for the things that actually allow me to serve people in the way I want to serve them and earn more. And she had she is so fun. She literally emailed her scheduler and it's like, from this week, this is the way I want my schedule to look. So optimizing for that. Um, and you know, she she changed things around and is going to have a significant, huge impact on her bottom line. So that was one doctor. Another doctor was like, oh my goodness, like my best performing product, I'm not even necessarily offering it. Like if I can start offering it and 10% of my patients decide to use it, that's an extra$90,000 to my bottom line. That's$90,000. And then one of the fun ones, another doc came up and said, Oh my goodness, there's this code that I could be using that I hadn't been using. And I don't even have to be involved in this part of the service and all of that. And it'll add another$500,000 to my revenue. I want you to think about half a million. She discovered from doing this. And for every one of them, yes, I taught them the concepts and how to do it and all of that, but the exact strategy, they came up with it. And they came up with it because once you start looking at your numbers, you become very strategic. That's that's what happens in that way, right? So walk them through that five-step process. Again, this was something I didn't rush it. We probably were at it for about three hours, and I was like, let's go. But I mean, three hours to 90,000, three hours to 100,000, three hours to 500,000. And for many of them, for most of the room, it was their first time of doing it. And they keep doing it, they keep getting the gift of accountability and all of that. They become pros at this. What happens to their practices? And at this time, I'm seeing like the things that I talk about, the things that I see happen in EBS, the things that you know I've seen doing workshops and all, to just watch it happen in place, in person, in that short amount of time was just so good. I am telling you, yes, can private practice be challenging or not even can it be? Is it? It is. It is, but there are so many opportunities for us to make our private practices better that if we take it, everything changes. And that's the truth. And so the third thing that really got me was um the power of community. And historically, you know, when I started working with doctors, people were like, man, Dr. Una, I don't want to be a part of a group. I just want to work with you one-on-one and all of that. And I say, I understand that, I understand why you would say that. But I am telling you that the pathway to the most radical change is a community. A community, not any community, but a community of people who are doing what you're trying to do, a community of people who are growing, a community of people who are advancing, a community of people that, you know, some of them are ahead of you, so you can see what is possible. Someone, some of them are um are people that you're ahead of, like on your journey, and you can be a gift to them. Well, they're also a gift to you because you have they have more of uh uh a growth mindset and they have more of more enthusiasm about doing the work, typically than the person who's been at it a while for a longer time. So they inspire you to stay on top of it. So so I always I was like, you need community, you need community. And I remember in the beginning, you know, people didn't quite believe me, but after we had had the entre MD business school for a few months, um, people would walk back to me and say, you know, and they say, Oh my goodness, Dr. Una, you're amazing. And everything, but oh my goodness, the community. When I would interview people, they're like, Yeah, Dr. Una is amazing and all, but oh my goodness, the community, right? Now, I want you to understand this community is a little different because the community of people, you know, again, who didn't necessarily know each other before this event. Some of them did. And if they did, they knew one or two people, they didn't know the whole room. And watching the community do what the community does, like the right community, again, a thing of beauty. So somebody will say, Oh, I need this kind of support. And somebody's like, Oh, I have you. I'll con I know the person who's in charge of this in Georgia. I'll connect you to the person. Somebody else will get up to ask a question, and the somebody else is like, wait a minute, your practice is here. I'm there too. I've been looking for a doctor like you. I will get you busy. It was beautiful. People generously sharing of their expertise, their experiences, their connections, people exchanging phone numbers, people connecting people to other people, people encouraging each other, people noticing who was struggling and noticing who needed extra support. And at the end of the two days, every doctor walked away, empowered, and not just a rah-rah empowered, but empowered in this beautiful way that they're inspired, they're able to see further as far as vision-wise, their expectation is so much higher. In fact, there's a doctor who walked up to me and she said, you know, my dad was in private practice, and I saw a version of private practice where he didn't necessarily understand this concept. So he wasn't building something he could exit from. And, you know, that's the only model I knew. And I didn't think it was for me. And now she sees this whole other model, this whole other possibility. And she's like, whoa, right? And so, yes, did they get the inspiration? Yes. Have their have their visions been expanded and expanded forever? Yes. And so that happened. And it's not just the inspiration, but also the know-how. Like now that you know this, this is how you pull that vision off. We're not gonna leave this vision as a hope, we're not gonna leave it as something, you know, like it would be nice to get. No, no, no. We understand exactly what to do to pull it off. And and and then they also got the opportunity um to be a part of this, you know, going on. Like the weekend doesn't have to end with the weekend. And so when I looked at all of this, it it it mirrors, in essence, what we do in the entream d business school week to week. So they gotta experience it over two days, but this is what happens every week. So for instance, you know, the identity work, um, you know, in the beginning we did it when we did it, but now I'm so committed to it that a week every month, so the entream d business school, we meet um every week, every Wednesday at 7 p.m. Eastern. And it's not random, right? So one of those sessions every single month, we call the champion's mindset, and it's all about working the identity, right? Working the identity. Like who do you need to become to pull off the vision that you have for your practice? And we work on becoming that. We address limiting beliefs, we tackle all of that stuff so the doctors are free to run. And that's why the that's the reason why they keep evolving, whether it is an amount of revenue, whether it's in the the kind of team members, whether it's the size of the team member and the things that are possible for them. They are building buildings, they are opening second locations, they are opening second practice types, they are hosting phenomenal events, they are getting on the biggest stages. All of those things start with the identity shift. So we have a signature session called the Champion's Mindset where we deal with that. And every single month, one of the sessions is on revenue. And I did this because I noticed as physicians, it's not, and it's not just physicians, I mean entrepreneurs in general. Um, you know, we don't really want to talk about revenue and numbers and all of those kind of things. We'd rather talk about serving the people. And so every single month, we have one session that is the RevCon. So revenue conference, and we talk about numbers. This is my goal. This is a strategy I'm using to get there. This is what is getting in my way. How do I tackle this? Like we keep, we go through that. So not only are they troubleshooting their revenue, but they're watching others troubleshoot their revenue. So, you know, they're very strategic about how do I shift what is happening with my revenue. We don't roll over and play dead with revenue. And so that's another session that we have every single month. And I am telling you, people make big breaks there because once you start looking at it, how do I create it? How do I reverse engineer it? Everything changes. The third thing, um, you know, that the other piece of it that happened in a conference that happens every single week is the community. You're you're connected with people, you can ask whatever question. There's no type, don't think there's a stupid question, and people come out and support you. You can talk about challenges that you're having out in your business, and people will come out and either either give you their expertise, they will give you their experience, or they will give you their connections, right? Um, I'm in the group responding, people others in the group responding, and you have the opportunity to give and to receive. And people are so generous. Somebody will come out and say, okay, I hit a million in revenue, this is how I did it. I hosted an open house with 400 people in attendance, this is how I did it. I went through this challenge, this is how I overcame it. Um, I had this situation, I had my office manager just resign, and this is how I turned it around. And so it's almost like you're getting this nonstop education and inspiration and connections and all of these things. And it is, again, so beautiful. And so for the people who came for the conference, for the summit, I said, okay, and you know, of course the on-trained business school is not for everybody, right? It is for the doctor who is, I think first and foremost is coachable, um, who says, okay, you know, I don't know any everything. Um, I am open to new ways of doing things. I'm open to trying the things that I learned. Um, you know, and so someone who's coachable. And it's also really great for someone who's committed, committed to their goals. Like, I am going to hit this. This is the kind of business I'm going to build. And I just need, you know, support. I need help. I need direction on what to do. Because what we do in the entrepreneurial business school is you get world-class mentorship and you get the gift of accountability and you get community, which are three things designed to help you to thrive. Right. And so people, so we are, we are committed. When I when I sign off my emails, I say, I'm a thousand percent committed to your success. And um, and I mean it, it's not the cute way of ending an email. And so for the entrepreneur business school, we are committed to helping you, a thousand percent committed. So we just need you to be a thousand percent committed to your own goals, and then that's what creates what we call a match made in heaven. And so we gave the doctors the opportunity to come join us, and so we have a new cohort that's forming in the entrepreneur business school and we're super pumped. And so now what they experienced is not something reserved for a summit. Like they have it at the summit, they have it every week, they have it ongoing, and they just they accelerate their growth in ways that don't make sense. You know, and I'll tell you what I told them as well, which is you know, there is a price for inaction. There's a there's a price for not choosing the acceleration path. And I'm not saying that to Joe Trip anybody into the entrepreneurial business school or anything. I'm just saying, like when we have the opportunity, we take advantage of the opportunity if it is a good fit for us, meaning we're coachable and we're committed. Right. And so, and so, you know, um at the time of this, if you're listening to this when it just goes live tomorrow, so which is the the 19th of May, um, we have our Entre MD Business School Open House. I would love for you to come join us. It's entreamd.com forward slash open house.
