If you're a woman over 40 with joint pain, unexplained weight gain, fatigue, or an autoimmune diagnosis — this episode connects dots that most doctors never connect.

Dr. Diana Girnita, double board-certified rheumatologist and founder of Rheumatologist OnCall, sits down with Dr. Mirela Titianu, menopause and lifestyle specialist, for a frank conversation about what actually happens to women's bodies during perimenopause and menopause — and what the latest science says about treating it.

They cover the real risk profile of HRT in 2025, why the decade-old fear of hormone replacement no longer holds up, how estrogen loss raises the risk of autoimmune flares, and why GLP-1 medications like Ozempic and Mounjaro may be doing far more than helping women lose weight.

IN THIS EPISODE:
— Why joint pain, brain fog, and fatigue cluster together at menopause
— How estrogen protects the immune system — and what happens when it drops
— The updated 2025 evidence on HRT and autoimmune disease
— Transdermal vs. oral estrogen: why route of delivery changes the risk
— The FDA's 2025 removal of the black box warning on vaginal estrogen
— GLP-1 medications as an anti-inflammatory tool, not just weight loss
— Microdosing GLP-1 in autoimmune patients already on biologics
— Myth-busting: HRT and cancer risk, aging and weight, Ozempic as "cheating"
— What to do right now if you can't access a specialist

This episode is essential listening for any woman navigating menopause with rheumatoid arthritis, lupus, psoriatic arthritis, Sjögren's syndrome, or any other autoimmune condition — and for anyone who cares for them.

More info about Dr. Diana Girnita, MD PhD

Dr. Diana Girnita: Many women, when they hit 40 years old, start noticing more joint pain, fatigue, weight gain, brain fog, and even get newly diagnosed with autoimmune diseases. Are these separate problems? Or are these signs and symptoms of the same problem? In this episode, I have invited an expert, Dr. Mirela Titianu, a menopause and lifestyle specialist.

Mirela Titianu: Thank you so much for having me.

Dr. Diana Girnita: I will start this conversation about menopause and perimenopause. What are the common signs and symptoms of perimenopause in women? Should we expect only hot flashes?

Mirela Titianu: Not at all. Hot flashes are common, but many women first notice irregular periods, sleep disturbances, anxiety, irritability, brain fog, fatigue. Others can have palpitations, headaches, vaginal dryness. It's the whole body transition.

Dr. Diana Girnita: Many women experience joint pain and stiffness. Why is this happening during menopause?

Mirela Titianu: Estrogen has anti-inflammatory effects. As it declines, inflammation can increase, leading to joint pain and stiffness.

Dr. Diana Girnita: Some women gain weight during menopause. What is the main reason for that?

Mirela Titianu: This is common in menopause. We gain weight mainly in the abdominal area because estrogen drops, leading to more visceral fat and loss of muscle mass.

Dr. Diana Girnita: Can you explain what happens during menopause with our immune system when estrogen levels start dropping?

Mirela Titianu: Estrogen helps regulate the immune system. When it declines, the immune system becomes more pro-inflammatory, contributing to symptoms like fatigue and joint pain.

Dr. Diana Girnita: Is it true that women have an increased risk of developing autoimmune diseases during menopause?

Mirela Titianu: Yes, there is a higher risk or unmasking of autoimmune conditions during this transition.

Dr. Diana Girnita: Can menopause worsen the symptoms of someone with an existing autoimmune disease?

Mirela Titianu: Definitely. Hormonal changes can trigger flares or worsen previous symptoms.

Dr. Diana Girnita: Let's discuss solutions for women experiencing these issues. What is the real risk when prescribing HRT today?

Mirela Titianu: The fear largely comes from older data. Today, we know hormone replacement therapy within 10 years of menopause can lower the risk of dying from all causes.

Dr. Diana Girnita: Who is the ideal candidate for HRT, and who should avoid it?

Mirela Titianu: We treat symptoms like hot flashes and sleep disturbances. For women with breast cancer, we try to avoid estrogen therapy.

Dr. Diana Girnita: Is transdermal estrogen safer than oral therapy?

Mirela Titianu: Transdermal therapy is safer for women with a history of blood clots and has less impact on triglycerides.

Dr. Diana Girnita: What is the current data about the use of HRT in patients with autoimmune diseases?

Mirela Titianu: Estrogen has immunomodulatory effects, helping women with autoimmune diseases feel better.

Dr. Diana Girnita: Will HRT be helpful for patients with autoimmune diseases?

Mirela Titianu: Yes, estrogen is FDA approved for preventing osteoporosis and helps with cardiovascular risk.

Dr. Diana Girnita: What exactly is vaginal estrogen cream?

Mirela Titianu: Vaginal estrogen cream is applied directly to the vagina and helps with dryness and urinary issues.

Dr. Diana Girnita: Is vaginal estrogen safe to be given long term?

Mirela Titianu: Yes, it's safe even for elderly women and can prevent UTIs.

Dr. Diana Girnita: Can vaginal estrogen be a solution for patients with Sjogren's disease?

Mirela Titianu: Yes, it increases blood flow and helps with inflammation.

Dr. Diana Girnita: What is the solution for weight gain during menopause?

Mirela Titianu: Hormone replacement therapy can help with metabolism and insulin sensitivity.

Dr. Diana Girnita: Are GLP-1 medications just for weight loss?

Mirela Titianu: GLP-1 analogs have anti-inflammatory properties and help with autoimmune disease activity.

Dr. Diana Girnita: Should GLP-1 be considered for patients with uncontrolled autoimmune disease?

Mirela Titianu: Yes, especially if the patient has a BMI over 27.

Dr. Diana Girnita: What would you recommend to women struggling to find a specialist?

Mirela Titianu: Be your own advocate and look for a menopause specialist.

Dr. Diana Girnita: Let's bust some myths. HRT causes cancer in most women. True or false?

Mirela Titianu: Not true. HRT does not cause cancer.

Dr. Diana Girnita: Menopause is just hot flashes. True or false?

Mirela Titianu: No, there are many symptoms like brain fog and muscle pain.

Dr. Diana Girnita: Weight gain is just because of aging. True or false?

Mirela Titianu: It's due to hormonal changes.

Dr. Diana Girnita: GLP-1 is cheating. True or false?

Mirela Titianu: It's a great peptide with many benefits.

Dr. Diana Girnita: Vaginal estrogen is dangerous. True or false?

Mirela Titianu: No, it's 100% safe.

Dr. Diana Girnita: Thank you for helping us understand these connections. How can people contact you?
