By now most of you have probably heard of ECMO, and if you tuned into my Intro to ECMO podcast, you should have a pretty solid understanding of what it is and how it helps oxygenate our patients when traditional methods fail. Well, what about severe ARDS refractory to standard treatments where we need super-lung-protective volumes and minute volumes with Vt's down near 3-4mL/kg? In the past we would be hesitant to approach these levels due to hypercarbia and other issues. But enter ECCO2R, which is like a mini-version of ECMO but instead of adding oxygen to the blood outside the body, we are actually able to clear pH and PaCO2 so that their baby/injured lungs can handle the super-low minute volumes.
