Compassionate Conversations: What Palliative Care Taught Me About Living

What does it mean to talk honestly about the hardest things? In this solo episode, Dr. Sonny Miles puts on her palliative care hat to explore the conversations most of us avoid — and why avoiding them costs us more than having them. Board certified in internal medicine, hospice and palliative medicine, and integrative medicine, Dr. Miles has spent over a decade sitting with people at the edges of serious illness. Here she opens the door to what actually happens in those rooms.
This episode begins by clearing up a common misconception: palliative care is not the same as hospice. Ideally it is an interdisciplinary team supporting the whole person — body, mind, and spirit — from the time of diagnosis onward. From there, Dr. Miles walks through the specific kinds of conversations she has as a physician: how long to continue treatment, what interventions someone would or wouldn't want, and the one question underneath all the others — what does quality of life actually mean to you? She makes the case that this definition is deeply individual and changes over time, which is exactly why these conversations need to happen more than once.
The second half turns toward how these conversations are held. Dr. Miles explores the role of nervous system regulation and safety in medical settings, the impact of medical trauma, and why she asks permission before delivering hard news. She names how grief-illiterate our culture is — how we have language for death and divorce but not for the loss of function, autonomy, or the role we once held in our family. The episode closes on resilience: not spiritual bypassing, but the practice of honoring grief while continuing to live, love, and laugh. Part two will explore how healers stay well while doing this work.
