You can have three things in every health care system: Those three things are affordability, universality, and quality. However, every health care system can only guarantee no more than two of the three at a time. Because of the current medical system, and their reimbursement rates, physicians have discovered a way to "upgrade" their services so they can maintain the quality of care they, and their patients, want to have.
