The Return to Play Framework, Part 1: Why Pain-Free Is NOT The Goal
Most athletes, parents, coaches, and even healthcare providers make the same mistake after an injury:
They use pain as the primary marker of recovery.
But being pain-free and being ready to return to sport are not the same thing.
In this episode of The Clinical Leadership Podcast, we explore the Return to Play Framework and explain why pain-free is only one checkpoint—not the finish line. Whether you're working with a youth soccer player, a collegiate athlete, or an active adult, understanding the stages of recovery is critical for reducing re-injury risk and building long-term resilience.
You'll learn how successful return-to-play programs progress beyond symptom relief and focus on restoring capacity, confidence, performance, and sport-specific demands.
In this episode, we discuss:
• Why pain-free is an incomplete measure of recovery
• The difference between healing, rehabilitation, and performance readiness
• The three stages of the Return to Play Framework:
- Return to Participation
- Return to Training
- Return to Competition
• Common mistakes that lead athletes back into the injury cycle
• How clinicians can better communicate expectations to athletes and parents
• Why objective benchmarks matter more than subjective symptoms
What Should Be Happening in the Treatment Room?
In the Return to Participation phase, treatment should focus on protecting the injured area while minimizing the losses that occur during recovery. The goal isn't simply reducing pain—it's preserving muscle mass, maintaining bone density, restoring movement quality, and preparing the athlete for the demands that lie ahead. Every intervention should answer one question:
"How does this move the athlete closer to their sport?"
If you're a clinician, coach, athlete, or parent looking for a more effective roadmap after injury, this episode will help you think differently about recovery and performance.
Because the goal isn't to get athletes pain-free.
The goal is to get them back to doing what they love—better prepared than before they were injured.
