Beware of the Upsell With ED Treatments: Why You Should Start with a Urologist For Bigger, Better And More Educated Results

Thanks for tuning in to the Armor Men’s Health Hour Podcast today, where we bring you the latest and greatest in urology care and the best urology humor out there.

In this segment, Dr. Mistry and Donna Lee answer a listener's question about the difference between focused linear compression therapy and shockwave therapy, which treatment might be right for his erectile dysfunction, and how to know which practice offers the highest quality options. Dr. Mistry cautions this listener that any medical practitioner advertising their treatments as "the best" is unethical and can be highly manipulative when something as important as patient health is at stake. In addition, he urges this patient to find a doctor who will give him a thorough evaluation before selling him on any therapeutic tool or process. In this case, that would involve a review of his medical history, medication regimen, lifestyle choices, and other complicating factors that might be contributing to the symptoms he's experiencing (ED). Only then should he be willing to pay out for expensive therapies, some of which aren't even guaranteed. Regarding the difference between focused linear compression and high intensity focused ultrasound (HIFU or "shockwave" or "sparkwave" therapy), Dr. Mistry finds shockwave to be the more effective tool for treating ED. Regardless of your symptoms, your doctor should be willing to try more modest interventions (like lifestyle modifications and/or supplements, medications) before upselling you on the "latest and greatest" procedure. Dr. Mistry:

You know what I was thinking about this weekend was how, like, if we were Star Wars, okay? The galactic empire couldn't have a quirky podcast to describe what they're doing. So the big group can't do this. You know what I'm saying? It just wouldn't make sense. Like it's like Darth Vader getting on the radio and saying,"[inaudible]."

Donna Lee:

"Luke, I am your urologist."

Dr. Mistry:

But Luke Skywalker could do it, and Chewy and Han, you know?

Donna Lee:

And all those fun monster animals at the bar.

Dr. Mistry:

Yeah, all the monsters...We can do that. We're quirky. You know, we're fresh, we're new.

Donna Lee:

I don't think they have a bunch of quirky people at the big one. Probably right.

Dr. Mistry:

We're part of the revolution.

Donna Lee:

Yeah, that's right. The last patient who requested to talk to me, he said that, uh, we really pushed the limit, but he made it makes him laugh. So, there you go.

Dr. Mistry:

Well there you go. Now for those of you that are tuning into the show for the first time...

Donna Lee:

Shame on you. Where have you been?

Dr. Mistry:

...we talk about things, we often talk about things related to erectile dysfunction, enlarged prostate, even some odd topics like difficulty with orgasm. We talk about kidney stones. Our practice is really one that is very holistic based. We have a nutritionist in office. We have pelvic floor physical therapy. We often have people who have, you know, a real atypical problem set. Today we've had, this week in our office, we had multiple people with abdominal bloating. We've had people with abnormal gastric kind of symptoms. Those people have a value and a resource in us, too. And we're very proud to be able to provide services for a wide range of urologic services. There are four physicians, there are five physician assistants. We have two pelvic floor, physical therapists. We have in-house nutrition. And we believe that sleep apnea has some deep impacts when it comes to your logic health. That's another thing that we treat.

Donna Lee:

Right? And I want to remind patients, you know, we'll get you in as quickly as possible, but our mid-levels or our nurse practitioner and PAs physician assistants are highly qualified in urology. I think a lot of our patients are like,"Well, I really want to see a doctor first. And you know, I'm just having recurrent UTIs." But we can certainly get you in front of one or the other providers.

Dr. Mistry:

Oh yes. And, and each one of those PAs has been personally trained by myself. The worst and best year of their life. And probably the year they have to spend with me closely.

Donna Lee:

Or, in Leonora's case, the worst eight years of her life.

Dr. Mistry:

Especially with somebody like Leonora has been with me for so long. It's almost like a carbon copy of seeing me.

Donna Lee:

Well, people actually prefer her over you, so.

Dr. Mistry:

They do. She has smaller fingers, they tell me sometimes. So your questions are what drive the show. Donna, you got any questions for us?

Donna Lee:

I do. I've been engaging with this gentleman back and forth and I let him know, we we've talked about spark wave therapy, shockwave therapy. His question to me was,"Hi, do you offer focused linear compression treatments for men?" So I went on to say,"Yes, we do. Ours is called spark wave. You'll also hear shockwave." I sent him one of the podcasts. We've done several on them. Right? So then he kept coming back to me with really great question. So I thought what a great topic for this show, because his response to me was,"I heard one of your competitors saying they have the quote best, but there's a lack of specific information about the equipment used to properly compare one treatment to another. Just want to be sure."

Dr. Mistry:

That's a great question. It's actually not appropriate for a medical professional to say that they have the best of anything, is actually in our ethics we can't do that.

Donna Lee:

And I told them beware.

Dr. Mistry:

Which probably tells you something about whether or not that other clinic's an actual medical professional place.

Donna Lee:

We went into that, too.

Dr. Mistry:

Because, because I tell everybody the same thing: If you want your erections treated, go to a place that treats erections. If you want your car fixed, you know, don't just go to a muffler shop, because maybe your muffler's not bad?

Donna Lee:

Maybe it's the thing next to the muffler.

Dr. Mistry:

You know, who knows? So the point is that because men have a tendency to self-diagnose a lot, they feel like the best and most efficient thing for them to do is just go to the place that has the treatment they think is going to work for them. Just today we had a patient that came to us with erectile dysfunction who had gone for a shockwave already, but nobody had checked his testosterone, nobody had done anything other workup for him. Nobody had tried meds for him. And maybe because he had kind of self-diagnosed how he was going to navigate it, he spent like$6,000, doesn't work, and he wants to know if my shockwave's going to work. Now we could have done shockwave on him. He had checkbook in hand.

Donna Lee:

But we're don't do that.

Dr. Mistry:

But we're not doing that. You know what I'm saying? I have zero desire to take this man's money to do something that's not going to work for him. He needs a broader, more, you know, comprehensive approach. And we're going to fix it. We're going to put them on daily Cialis because it works much better than the on-demand Sildenafil that he was taking. We're going to use intra-urethral agents. We're going to use other agents to try to enhance and improve the quality of his erections and his penile blood flow. And we're going to make sure that he leaves our clinic, you know, a better, you know, more intact person, not just try to sell them, you know, something that not going to work for everybody.

Donna Lee:

$6,000.

Dr. Mistry:

I know we should really raise our price.

Donna Lee:

We should. I think in comparison ours are a little inexpensive.

Dr. Mistry:

There's a guy in town who sends his shockwaves to a plastic surgeon who has like a wellness clinic in the back. And I was like,"That plastic surgeon is not interested in your erections. You know? I mean, they're just trying to sell you just whatever latest machine that they found." Whether it's us or any other urologists, I would highly encourage those listeners of ours to really think about where they're getting their men's health care from. I hear so many ads, just like everybody that listens to the station from men's health organizations that offer testosterone for cheap price or shockwave for this, or, and I was like wondering to myself, I mean, it's, we, we offer every single thing you have much cheaper and and much better.

Donna Lee:

And by a medical doctor.

Dr. Mistry:

And by a physician who's going to care about your whole health, not just somebody trying to sell you the most expensive things. But another thing that this particular patient that, that we saw recently mentioned was that they were given an injection, intracavernosal injection into the penis to get an erection. And they didn't feel like that experience was particularly sound. And, this just leads to, again, uh, my belief that when it comes to rectal dysfunction, your, your provider should have an algorithm. Where are they starting? What are they trying to finish at? Do they offer all the different treatments including an inflatable penile prosthesis if necessary? Do they offer supplements? Are they taking care of your nutrition habits? Are they making sure that you're not doing anything from your medication standpoint, that's affecting your erections, and calling your primary doctor and getting you off that beta blocker? I mean, these are the kinds of steps that you need to make your erections as good as possible because I'll say that I think many medical professionals undervalue how important getting an erection is to men.

Donna Lee:

Hmm. Except for urologists. You're probably right.

Dr. Mistry:

I think even some urologists, you know, I've met some urologists that will under understate, how, how certain treatments or certain surgeries may affect erections, but we try to set expectations really appropriately. And whether you're having sex or not, I mean, being able to function, you know, fully as a man by getting an erection--I think that that's to a lot of guys.

Donna Lee:

That's what life's all about. Isn't it?

Dr. Mistry:

Well, I mean, that's what you feel about when you're 13, right? And there's that book, everything I needed about life I learned when I was in kindergarten. Maybe, maybe...

Donna Lee:

Is there a boner chapter in that?

Dr. Mistry:

Some men don't learn anything till the seventh grade.

Donna Lee:

Oh, my. Well I think he'd be proud of my response to him. I said kind of exactly what you said. I said,"You should definitely Google the groups that offer very little information. And if their clinics are run by medical doctors, of course we recommend you see a urologist because it might not be the perfect fit because a urologist will have lots of tools in the toolbox to address the symptoms."

Dr. Mistry:

And my belief about the difference between linear compression and shockwave really is that linear compression is going to be really just something tapping your penis over and over and over again, which doesn't really have the same impact that a focused ultrasound can have. You could almost think of it like hitting something with a hammer or focusing sunlight with a magnifying glass. They both could destroy, you know, whatever you're trying to destroy, but they do it in different ways and use different energy i n different mechanisms.
