When you clench and grind, you can get an increase in muscle tension. So oxygen desaturations andor apnea events are creating a clenching and grinding. And what your body's trying to do is essentially keep the airway opened up. During an apnea event where you stop breathing, it's trying to tone and open the airway. It's like your body's trying to do CPR itself.
SPEAKER_00If you've ever woken up with a sore jaw, morning headaches, sensitive teeth, or your dentist has told you that you're grinding your teeth at night, you've probably assumed the cause is stress. And while stress can certainly play a role, it's often not the whole story. For some people, jaw clenching and teeth grinding may actually be the body's response to something happening while they're asleep. Something they may not even realize. In today's episode, Dr. Fred Eck helps us understand the surprising connection between jaw clenching, airway health, and sleep. He'll explain why these habits develop, what they may be telling us about your breathing, and why simply treating the symptoms isn't always enough if the underlying cost is being overlooked. Dr. Eck, it's nice to see you again. Let's start with the basics. Most people have heard the term bruxism, but what exactly is it and how common is it?
SPEAKER_01Bruxism is just simply grinding your teeth, sometimes grinding, side to side, sometimes clinching. That's the basic definition of bruxism. It's fairly common. I would say about a third of the population overall does some degree of bruxing or clenching. Now, it is much higher in the area of sleep, probably looking at about 10% of the population overall that grind their teeth. But if you look at the people who have an airweight problem, it's close to about half of those people. So if you have an airweight problem, you are much, much more likely to grind your teeth.
SPEAKER_00Oh, that's interesting. With you, it's been the first time that I relate bruxism to airway. Many people assume stress is that primary cause of teeth grinding. That's what I thought as well. Is that always true or is there more than the story? So what what what is the truth?
SPEAKER_01It's almost never true. Stress can make grinding and clenching worse, but it is rarely the primary cause. Sleep disorders and misaligned teeth are primary causes. Stress can make it worse, and stress can create it in some people, but it's not a primary thing for most people. It just can make it worse. Certain medications can make it worse. Certain medications can actually create grinding, a lot of antidepressants, certain muscle relaxers. So a lot of people don't realize you take a muscle relaxer for your back, a muscle relaxer like Fletra, and that medication is good for your back, but it has a different impact on your jaw muscles. And so it's relaxing your back, but it's making you grind your teeth. So medications can have a play in this as well. Also, recreational stuff, alcohol, caffeine, recreational drugs like cocaine, amphetamines, they can all create grinding as well. So when it comes to treating grinding, clenching, facial pains, TMJ headaches, you got to look at the whole picture. We always talk about this, right? Getting the root cause. What is causing the problem? You have to figure that out. You can't go in with blinders on. You gotta figure out what the cause is. If you figure out what the cause is, you can fix it. And you can fix it pretty easily with almost anybody.
SPEAKER_00And Dr. Ike, you've mentioned that in some patients, jaw clenching may actually be the body's attempt to protect the airway. Can you explain how that works?
SPEAKER_01When you clench and grind, you can get an increase in muscle tension. So oxygen desaturations and or apnea events are creating a clenching and grinding. And what your body is trying to do is essentially keep the airway opened up. So during an apnea event where you stop breathing, it's trying to tone and open the airway. It's like your body is trying to do CPR on itself. You engage when you clench and grind, you engage jaw muscles that stabilize the airway. Our jaw muscles are involved in stabilizing the airway. So if you can create tone to those muscles, you can help to open up the airway or stabilize the airway. So that's why our body can create a grinding habit during airway issues, is because it's trying to stabilize the airway. And it can also, if you tone the muscles, that includes your tongue. Your tongue is not a muscle. Your tongue is a respiratory organ that has eight muscles. So we can help prevent our tongue from collapsing backwards if we create some muscle tone in these muscles. So at the end of the day, you might grind your teeth, you might create sore muscles, you might create headaches, but do you know what it does? It helps you breathe. And at the end of the day, your body cares about one thing more importantly than anything else, and that's breathing. You can live a long time without eating and a long time without drinking. But you can't live a long time without oxygen. So your body will twist, contort, create grinding, muscle tone, whatever it has to do in order to try to establish breathing.
SPEAKER_00Oh wow, that's so incredible. Honestly, when you put it that way, like the body's always trying to even those things that seem to be affecting us are in an attempt to help us survive. They're protecting our life, even when they seem they're detrimental to our health, right? And how how did you figure this out as a dentist? Did you know that when you first started and just saw teeth that were pretty worn out by grinding?
SPEAKER_01Well, I and I've treated facial pain, TMJ, and headaches for a long time. And you know me, I'm always trying to get to root cause of something. I don't like masking or treating symptoms. I like to fix the cause. So when you see people that are grinding, I spent a lot of years figuring it out and doing training and coming up with anything and everything that could create the grinding. Because some people, you try to do this and it would work. Some people you try to do this, it wouldn't work. And then you realize you just didn't get to the root cause. So in 2000, when I started treating facial pain and headaches, and you have some people who don't respond to your normal stuff, I just flabbergasted it. I'm like, well, we got to figure this out. And then you realize this connection between an airway problem and bruxism. And if you get bruxism, then you can have facial pain, headaches, TMJ, and all the same stuff. So to me, it was just about a journey to figure out causes. And it turned out airway was just one of those causes.
SPEAKER_00And did you ever think of it as uh did you ever think that the root cause was stress primarily?
SPEAKER_01Or no, I've I've never, because I've just I've never seen a correlation with all the patients I've treated in 27 years. I've never seen this big correlation between somebody who just had stress and no other problems. So show me somebody who just has stress, but does not have a bad bite and does not have an airway problem. How many of those people then grind their teeth andor get facial pain, headaches, TMJ? It's not me. It's always one of these other things that are contributing.
SPEAKER_00And so does everyone who grind their teeth have sleep apnea, or are there other conditions that can contribute to bruxism?
SPEAKER_01Not everyone has sleep disorder if they grind their teeth. As I mentioned, there are some other things that can create a problem. Misaligned teeth, okay, can create grinding in addition to the airway. However, there are certain things we can look at, grinding so bad that you create tori or the extra bone around the teeth that's on the outside of the teeth. They call it exostosis. That's pretty much diagnostic that you have an airway problem if you have that. Grinding is considerably more common in somebody with an airway problem than the general population. Your body responds to lower oxygen levels in many ways. That resistance in breathing and that reduction in oxygen creates all these issues, as we talked about. You can live a long time without water. You cannot live a long time without oxygen. So your body's going to create a response to that. Even people with horrible postures, everything else can all be related.
SPEAKER_00And this also happens when you're awake as well, right? It's not just that the jaw clenching happens when you're asleep.
SPEAKER_01For some people, it's just the nighttime thing when they're sleeping, based on their oxygen levels and everything else. But then there are people who clinch and grind during the day, because think about what happens if our oxygen goes down and we get thrown in fight or flight or sympathetic nervous system mode. Well, you don't just wake up and because you're awake, all of a sudden you're out of this fight or flight mode. It can continue throughout the day based just on what happened at night when you were sleeping.
SPEAKER_00Okay, so what I'm getting is that it is a stress response, but from trying to solve a breathing issue.
SPEAKER_01Yeah, so you, I mean, you can say when your oxygen goes down. So, you know, I had this conversation earlier with somebody today. Stopped breathing 58 point times, 58 times an hour. That was his AHI score on his sleep test. I said, to count as an event, you got to realize it has to last 10 seconds or longer, right? So you have imagine if somebody came into your room 58 times an hour and choked you for at least 10 seconds. That's the impact that this has in the body. So if somebody came into the room and choked you 58 times an hour, you would go into sympathetic nervous system mode. You would go into fight or flight, all of that, right? So that's why you get night sweats, that's why you get fragmented sleep when you can't stay asleep, a big common cause of your insomnia because of that.
SPEAKER_00Yeah, now that it explains all the anxiety related to sleep issues, all the mood changes and irritability and everything that happens when you don't sleep. Because when you just say you're just not sleeping well, it doesn't really paint the picture that you just painted for us. Like someone is actually coming and stopping your breathing. That's like a left life-threatening situation for your body.
SPEAKER_01That anxiety doesn't stop just because you wake up, it continues through the day.
SPEAKER_00And what are some warning signs that the jaw clenching may be related to sleep breathing issue rather than just stress?
SPEAKER_01So, how do you have to look for signs of breathing disorders? And we've talked about that in previous episodes, right? The simple things that we can look at on somebody and do you have these things? And the more of these things that you have, the more likely that you have a breathing disorder. Tori, the extra bone that's built around your teeth, recession of your dungs, a collapsed bite, a scalloped tongue, which means you bite down for 10 seconds and open, do you got those little marks around your tongue? That is highly predictive of having an airway problem. And that's a 10-second test, a narrow palate or upper jaw, or the donic issues. Your jaws didn't grow, so you have no room for your teeth. Okay. Look at somebody's medical history. If I see people who are on more than one medication for blood pressure, they usually have an airway issue. Uh, AFib. 87% of people with AFib have a breathing disorder. Getting up three times a night to pee, bed wetting too late in years as a child. There's so many things we can look at in a medical history that's caused by airway problems that we can kind of start to predict whether this person has a breathing issue or not.
SPEAKER_00And why is it related, the the pe the night urination, frequent night urination? How is that related? Because I've I've heard that that happens when blood sugar is high. So, like it happens a lot for diabetic people, but how is it related to it?
SPEAKER_01There are several things that can create it, but there's a hormone that's produced that makes your body resist the urge to go pee. And so that's what allows us to sleep all night without having to get up to go pee. Well, what happens when we have a sleep disorder is oftentimes we get fragmented sleep. So we talked about deep sleep and REM sleep in a couple episodes, right? And the importance of deep sleep is that is the stage of sleep at which we regulate hormones. So beyond how we feel, that's the stage which we regulate hormones. All things, including this, is controlled by hormones. So if we get fragmented sleep, we don't spend enough time in deep sleep, we cannot control the hormones that control a lot of things. And that is the relationship to this, that is the relationship to diabetes and blood pressure, cortisol, et cetera.
SPEAKER_00And Dr. Eck, in terms of tonsils, how is that related?
SPEAKER_01Well, tonsils can have that's one of those things when we talk about things that we can look at that might indicate that you have a breathing disorder. Large tonsils is just one of those things. Just because you have large tonsils doesn't mean you have a breathing disorder, but you're more likely to. And if you combine that with you have any of those other things we talked about, then you probably have a breathing disorder.
SPEAKER_00But is it because of the mouth breathing that tonsils get enlarged, or the fact that you have enlarged tonsils reduce the space in the airway? Like what?
SPEAKER_01It's like the chicken and the egg. People argue about this day in and day out. On our next episode, by the way, with that brilliant ENT from Australia, we'll be talking about this a lot because there are people out there that believe that you can do certain things, breathing exercises, myofunctional therapy, they call it, and some other things to reduce naturally, quote unquote naturally, the tonsils without needing to do surgery. And then there's the other side that says, don't waste time doing that, just get them out of there. Everybody has their own opinion on what they think about that, but there are things that can cause, obviously, your tonsils to enlarge. Your tonsils are enlarged for a reason. And if we want a quick solution, removing them is a quick solution, right? If we're willing to give us some time and try some other things, myofunctional therapy, laser treatments, etc., then you can get the reduction in size over time. Will they get back to normal? Some people yes, some people no.
SPEAKER_00Interesting. So we'll have to wait to the next episode to get more in-depth in that topic. But so many patients are given, going back to the jaw uh and the clenching, many patients are given a night guard to protect their teeth. So what does a night guard do well and what um are its limitations?
SPEAKER_01I hate night guards. I treated facial pain, headaches, and TMJ for so long, and that is almost never a treatment for me. We've talked many times in the past about masking symptoms versus treating a problem, right? First of all, traditional mouth guards that most people do today, that's a over a hundred-year-old technology. That's over a hundred-year-old mind thinking. We know better today. But yeah, that's still the prevalent thing out there. Uh a mouth guard does nothing to stop grinding, and in fact, if it does anything, it increases grinding. The idea behind it is you just put a piece of plastic between your teeth, so when you're grinding, you're wearing plastic instead of your teeth. That's the idea behind it. It doesn't fix anything, it doesn't stop grinding. And you know how much I hate just masking problems and not treating the cause. Okay? A mouth guard does nothing but to treat the cause. Now, with that said, there are some very unique mouth guards, sleep appliances that are out there that we can utilize that can actually improve grinding, can actually decrease grinding. So obviously if we treat the airway issue, improve oxygenation, grinding gets better. But even making, let's say, a unilateral bite block, so instead of a uh mouth guard that goes all the way around your mouth, okay, you only bite on one side, that can decrease your grinding ability by half. But that's not the old school way, that's not what most people do. If you do a sleep study with somebody with a mouth guard, traditional mouth guard, do you know their sleep test is going to be worse most of the time with their mouth guard in place?
SPEAKER_00Why why is that?
SPEAKER_01Because we are almost always fighting for space for our tongue when we're talking about an airway patient. And you put a big old bulky mouth guard in there, and guess what? You have less room for your tongue. So they're usually worse.
SPEAKER_00Okay. And that's that traditional mouth guard that you're speaking of, right? So my understanding in the is that there is some mouth guard for protecting the teeth. That's just like merely I'm not gonna say aesthetics, because I guess it does have the function of like protecting the teeth, which are important, but it does nothing for the sleep disorder and hairway issue, right? And so Correct.
SPEAKER_01It does nothing to stop traditional mouth guards do nothing to stop grinding in general.
SPEAKER_00Okay. Okay.
SPEAKER_01There is a way that you can design them with certain bites, etc. That you can improve the grinding, but that's not the kind of mouth guard that most people do.
SPEAKER_00Okay. I see. And you offer the the more modern ones. Yes. Okay. Have you seen patients whose jaw pain or teeth grinding improved after treating an underlying airway or sleep disorder?
SPEAKER_01Oh, it's very common. I mean, we have people who come to see us for the sole purpose of grinding TMJ headaches, and if they have an airway disorder, then you treat that and that gets better. Or we use one of these more modern, unique style mouth guards that help to turn off the grinding. Because you can make a mouth guard that will reduce the grinding, reduce the muscle tone.
SPEAKER_00And those mouth guards, I'm guessing, they do have space for for the tongue, right? Because you mentioned that.
SPEAKER_01Yeah, they're designed to open the bite. They're designed to only block on one side. They're designed to uh do everything it can to improve room for your tongue and reduce your body's ability to clinch or grind. If you treat airway, you improve oxygenation. Decrease in oxygenation creates headaches. So if oxygen goes down, you get this misflux of CO2 and oxygen. And so your body's trying to balance all this out. So it creates vasodilation, vasoconstriction of the blood vessels, expands the blood vessels, shrinks the blood vessels, trying to get everything in homeostasis, right? Get everything in balance. And if all that's happening in your brain, blood vessels expand, blood vessels shrink. That's why if you wake up with morning headaches, it can very much be related to your airway, because it's that body trying to do that, and that creates a lot of your morning headaches. And that's from the oxygen desaturations. The oxygen desaturation also creates bruxism. So bruxism creates soreness in our muscles, which can then lead to headaches. So oxygen going down can lead to headaches multiple ways.
SPEAKER_00Oh, and this is a little bit off the topic of uh jaw clenching, but it came up with what you were saying. Does numbness in the morning or while you're sleeping have anything to do with airway? Is it a symptom or no?
SPEAKER_01So you mean like in your fingers and toes and that kind of stuff?
SPEAKER_00Yeah, or your hands.
SPEAKER_01Yeah, so the Mayo Clinic several years ago came up with a statement that said peripheral neuralgia. There's problems in our end of our hands and arms and feet and legs. Uh peripheral neuralgia, one of the most common undiagnosed causes of peripheral neuralgia is sleep apnea.
SPEAKER_00Oh, wow.
SPEAKER_01We affect our arteries, our inflammation in the body, our body's ability to respond, oxidative stress, all of these things are related to sleep apnea.
SPEAKER_00Wow, it it it's so incredible for me that sleep is one of the main causes of every other health problem. And I know we've said this before, you know, in our conversations, but how is it possible that it's so untalked about? How is it possible that it I don't know, I feel like there would there there needs to be campaigns about sleeping and, you know, treatings.
SPEAKER_01There needs to be funding on sleep treatment and well, unfortunately, we're gonna talk as we talked about, we're gonna talk about this a little bit next week as well. Uh it's it's the healthcare system. I see a concierge doctor. Everything is about prevention and correction and treatment and not masking or managing just symptoms. But that's not the normal healthcare system. And the way the normal part of healthcare works is we treat symptoms. So we write a lot of prescriptions, we do physical therapies, we do everything to mask the symptoms, we don't get to root cause. So medicine as a whole does not have an interest in getting to root cause. Because if you get to root cause and fix the problem, guess what you don't get to do anymore? Make money managing symptoms. It's sad. It's so sad, but that is the life we live in. And the average patient would rather rather than do what it takes to get fixed, go to the gym to lose weight, they'd rather go to a counter where they can get it filled. So unfortunately, the the medical system is also just following the desires of people.
SPEAKER_00Yeah, like you mentioned, it's a it's a systemic thing. Um, because people are so overwhelmed with with information as well. Like they don't really know how to discern information. They don't really know what to believe. So I think that's why it's really important that you're speaking about this in in in this podcast and because there's so much information out there. Like, you know, we spoke about the supplements in an earlier episode, and everyone wants to s everyone wants to sleep well. We know it's a foundation of health. Everyone wants to breathe well. Um, like you mentioned, we can't, you know, we can't spend any time without breathing. Like that's like the foundation of our the true foundation of our lives. But we're so confused as to what is important nowadays, right? Uh and I guess um yeah.
SPEAKER_01Bad sleep has just become normalized. Oh, I'm stressed. I have a new baby. I we make an excuse why bad sleep is considered okay. For health, it's not considered okay. If you want to be healthy and you want to live long, if you don't want to have heart problems, you don't want to have dementia, then you gotta pay attention to your sleep. And we just make an excuse every day as to why it's okay. I didn't sleep well last night.
SPEAKER_00Yeah, I I don't know. This is I w I wish I wish we had a system that prioritized the most important things and like you said, go to the root cost rather than trying to fit fix problems that might be too complex to fix once they're so developed. And like you said, that's why it's so important when you have spoken about kids, you know, and um understanding that say kids lead to broken adults. Like we have an opportunity to become aware of how important is uh sleep is and we can uh start to notice that in our children and and uh treating ourselves as adults so that we can give that example to the kids as well to take care of their sleep and you know, prevent, like you mentioned, uh not only cognitive dysfunctions, but just disease later on.
SPEAKER_01Yeah, it's it's uh you me the sooner you can start, the better, but it's never too late. And we just had a consult today, she was I don't know, 78, a spouse with dementia, and she's like, I mean, is it too late? It's never too late. I mean, it's it's never too late to try to improve your health. And anything you can do today to improve not only your years, but the quality of those last several years is important.
SPEAKER_00Yeah. And if someone listening wakes up with jaw pain, headaches, or notices signs of teeth grinding, what what should their first step be?
SPEAKER_01Always what I talk about. Get to root cause, find out what's causing it. All right. So for us, that's a competitive evaluation of what's going on. We look at your bite, your TMJ, your airway, usually get a sleep test done and try to figure out what the root cause is. So we're not just going in treating symptoms. You know, a lot of people who grind, clinch, get facial pain, headaches, they're doing the Botox, they're wearing the mouth guards, uh, they're taking medications and muscle relaxers and steroids. They're doing everything they can to mask the symptoms rather than get to the root cause. So get to the root cause, fix the problem, and make the problem go away. Otherwise, you're masking the problem your whole life. And there will come a time, by the way, one day, when your problem progresses and whatever you're doing to mask it won't work anymore. And now you're such a complicated case. So much more time, so much more money, so much more energy. Fix it. The sooner you do it, the easier it is.
SPEAKER_00And like you mentioned the Botox, right? That only is relaxing the bruxism, but at the same time, can it be dangerous just because the clenching happens as a protection mechanism, right? As an attempt to open up the airway. So if you have this relaxant or this toxin that is relaxing the muscles, then it affects your airway because now you don't even have that mechanism that it's trying to open up the airway.
SPEAKER_01Yeah, if your cause is airway, you can absolutely make that worse. I mean, there are people who take sleeping aids to sleep or sleep in pain medications or muscle relaxers because of their clenching and grinding and headaches, and it 100% can make your airway issues worse. It can absolutely make your sleep test worse. And one day, you know, you're taking those that Botox injections to control your symptoms, but yet your problem is still there. So your problem is still progressing. Whether that problem is an airway problem, whether that problem is a TMJ problem, your problem is progressing. You don't feel it because you're masking the pain with the Botox. But then one day you're gonna wake up and guess what's gonna happen? The Botox ain't working. That's because your problem has now got to the next level and the Botox can't handle it. And now you're more complicated, now you're more expensive. Everything is just exponentially worse now. So we gotta quit masking problems and solve root costs.
SPEAKER_00Like you said, it's never too late, but it is easier, and it's just easier to to treat things now rather than later.
SPEAKER_01The sooner it is, the easier it is. Okay.
SPEAKER_00And what's the one thing you wish more people understood about the relationship between jaw clenching, airway health, and sleep?
SPEAKER_01Most of the time they're all related. So most of the time, it's a matter of, again, getting to root cause, they're related. Which one is the primary in your case? Um now, if it's an airway issue, then obviously that's the primary for most people. If you do an airway evaluation, 3D image of your airway, your airways look all good, sync test is all good. Well, now we got to figure out the other root causes, misaligned teeth, bad bites, cause clenching and grinding, headaches, etc., then you address that. So you have you got to go through the diagnostic process and figure out what the what the problem is. So you gotta evaluate airway, you gotta evaluate bite, and you need a 3D image that essentially shows the jaws, the TMJ, and the airways and sinuses.
SPEAKER_00Do you do all those tests simultaneously, or would you first do uh the sleep tests and the airway evaluation and see how that comes up?
SPEAKER_01We always do the 3D imaging because that one 3D image allows us to see all of that. And it gives us an idea of which one is most likely the problem. And so after the 3D images and medical history review, we can get an idea of which one is the most likely culprit. And then that's what we look at first. And if we think it might be airway, then we always start with that because in addition to these things grinding, clutching, pain, headaches, there's a huge life-threatening issue with having a sleep disorder. So if that might be a potential cause, we always just start with that. And once that's ruled out, then we can look at everything else.
SPEAKER_00Absolutely. Like you said, um, breathing first. Good breathing is the most important, then look at the rest. Um, Dr. Eck, we've reached the end of our episode. Is there anything else you would like to leave the audience with before we go?

SPEAKER_00Jaw clenching and teeth grinding may seem like isolated dental issues, but in some cases, they can be important clues about what's happening while you sleep.
