Okay, so let's start with the basics. When people hear hormone panel, they often just think, oh, that's a woman thing, or that's for bodybuilders. But that is so not the case.

Not at all. These hormones are present and critical in all bodies, regardless of gender or age. The levels and ratios differ, but the functions are universal. Energy regulation, stress response, bone density, cognition, libido, all of it.

Here's the thing though, the conversation around hormones has been so heavily gendered that a lot of men don't know their estrogen matters, and a lot of women don't know their testosterone matters. It's wild.

Brilliant point. And that knowledge gap has real consequences. Research published in the Journal of Clinical Endocrinology and Metabolism has consistently shown that imbalances in these three hormones, even subtle ones, correlate with outcomes like fatigue, cognitive decline, and metabolic disruption.

So let's define each one quickly. Testosterone.

Go. Right. Testosterone is an androgen, primarily produced in the testes in men and the ovaries and adrenal glands in women. It drives muscle synthesis, bone density, libido, mood stability, and even motivation. And it is not just a male hormone. Y'all, I had mine tested and it was in the basement. My doctor actually said that low testosterone in women is one of the most overlooked causes of fatigue and low motivation.

Estrogen, primarily estradiol in reproductive age adults, supports cardiovascular health, bone protection, brain function, and mood regulation. And then cortisol, which I feel like is the hormone everybody's heard of, but nobody really understands.

Cortisol is produced by the adrenal glands in response to stress, real or perceived. In the short term, it is absolutely essential. It mobilizes energy and sharpens focus. The problem is chronic elevation. Dr. Robert Sapolski at Stanford, his work on stress biology is foundational here. His book, Why Zebras Don't Get Ulcers, is still the best lay explanation of chronic cortisol dysregulation.

Love that book. Okay, so let's dig into the actual panel. When someone gets their blood work done, what are they looking for?

So a standard hormone panel will typically show total testosterone, free testosterone, estradiol, DHEAS, and cortisol. Sometimes with a morning draw specifically for cortisol because levels fluctuate significantly throughout the day.

That timing thing is so important, and nobody tells you that. My first cortisol test was drawn at like 2 p.m. Completely useless data.

Genuinely, cortisol follows a diurnal rhythm. It peaks within 30 to 45 minutes of waking, what researchers call the cortisol awakening response, and then declines through the day. The Cleveland Clinic recommends morning drawers, ideally between 7 and 9 a.m., for accurate baseline readings.

Okay, so now the numbers. What does the average person actually need to understand when they're looking at that panel?

Right, so here's where it gets nuanced. Normal range is actually quite wide and varies by lab, by age, and by the individual. For men, total testosterone is typically listed as roughly 300 to 1,000 nanograms per deciliter. But a man at 310 might feel absolutely dreadful.

Which is such a good point. You can be in range and still feel terrible. That's where free testosterone matters, right?

Exactly. Free testosterone is the biologically active portion, the part your cells can actually use. A significant portion of testosterone is bound to sex hormone binding globulin, or SHBG and albumin, making it unavailable. So your total number can look fine while your free testosterone is genuinely low.

Here's the thing though, this is exactly the conversation to have with your doctor. Not just am I in range, but where am I in that range and how do I feel?

That's the real friction point, isn't it? The practical answer is to come prepared. Write down your symptoms before the appointment. Fatigue at specific times of day, changes in mood, sleep quality, libido, body composition. These are the clinical clues that help a good doctor interpret numbers in context.

And if you feel dismissed, it is absolutely okay to ask for a referral to an endocrinologist, or to seek out a functional medicine doctor who specializes in hormonal health. You are an advocate for your own body.
