SPEAKER_01Y'all, let me tell you something that genuinely rattled me. My neighbor Carol, fit woman, goes to yoga three times a week, normal weight, looks the picture of health. She had a routine physical, and her doctor called her back in. Not because of cholesterol, not because of blood pressure, because of something called C reactive protein. She had never heard of it, neither had I, honestly.
SPEAKER_00Right, and that is exactly the kind of story that keeps me up at night, professionally speaking. Because here's the uncomfortable truth. You can have a completely normal BMI, you can exercise regularly, and still be carrying a level of internal inflammation that quietly, significantly raises your risk of cardiovascular disease, diabetes, even certain cancers.
SPEAKER_01And today we are unpacking all of that. Chronic low-grade inflammation, what it actually is, why this one blood marker called C reactive protein matters more than most people realize, and what you can actually do about it. Whether you are 35 and juggling toddlers or 65 and wondering what your annual blood work is really telling you.
SPEAKER_00This one is genuinely important. Let's get into it.
SPEAKER_01The redness, the heat. That's acute inflammation, right? That is your body doing its job.
SPEAKER_00Exactly. Acute inflammation is appropriate. It is your immune system showing up, doing the work, resolving the problem. Chronic low-grade inflammation is something entirely different. It is like your immune system is stuck on a low simmer, constantly, without an obvious cause or resolution.
SPEAKER_01And the sneaky part is you cannot feel it. There is no swollen ankle, no fever. Carol felt fine.
SPEAKER_00Right. Which is precisely why researchers began looking for measurable markers. And C reactive protein, or CRP, became one of the most reliable. It is a protein produced by the liver in response to inflammation. The higher your CRP, the more systemic inflammation is present in the body.
SPEAKER_01So the liver is essentially the canary in the coal mine here.
SPEAKER_00Nicely put, and a landmark study published in the New England Journal of Medicine, led by Dr. Paul Ridka at Harvard, showed that elevated high-sensitivity CRP, the more precise version of the test, was a better predictor of cardiovascular events than LDL cholesterol in apparently healthy women. That was genuinely paradigm shifting when it came out.
SPEAKER_01Wait, better than cholesterol? That is the thing everyone obsesses over.
SPEAKER_00In that particular cohort, yes. And Dr. Rydke went on to develop something called the Reynolds Risk Score, which incorporates high-sensitivity CRP alongside traditional cardiovascular risk factors. It reclassified a significant number of people as higher or lower risk than standard models suggested.
SPEAKER_01Here's the thing though, this is where it gets uncomfortable for people who feel like they are doing everything right. Because we are talking about a risk factor that operates independently, independently of your BMI, independently of whether you exercise.
SPEAKER_00And the research supports that. A study published in the Journal of the American College of Cardiology found that metabolically healthy individuals, meaning normal weight, good blood pressure, normal glucose, could still carry elevated CRP and face meaningfully higher mortality risk. It separates the looking healthy from the being healthy conversation.
SPEAKER_01That is a hard pill to swallow. And I think that is important for our listeners because a lot of us, especially as we get older, use our weight as a proxy for health. If the number on the scale is okay, we assume everything is okay.
SPEAKER_00Which is understandable but potentially misleading. And this connects to something called the obesity paradox in research, where some individuals with higher BMI but low inflammation actually fare better than lean individuals with high inflammation. It has generated considerable debate, but it underscores the point. Inflammation is its own variable.
SPEAKER_01Okay, so I am thinking about our sandwich generation listeners right now. People in their 40s and 50s managing aging parents, managing teenagers, running on fumes, probably stress eating at 10 p.m. What does chronic stress do to CRP?
SPEAKER_00It is a significant driver. The relationship between psychological stress and systemic inflammation is well documented. Work by Dr. Sheldon Cohen at Carnegie Mellon University showed that chronic psychological stress disrupts the body's ability to regulate inflammation at the cellular level. Specifically, it affects glucocorticoid receptor sensitivity, which means the body loses some of its natural break on the inflammatory response.
SPEAKER_01So, caregiving stress, work stress, the kind that never fully turns off, that is not just exhausting your mind. It is literally showing up in your blood.
SPEAKER_00Measurably, yes.

SPEAKER_00Brilliant. Now let's talk about what raises CRP beyond stress. Because the list is important and some of it will surprise people. Hit me! Poor sleep is one of the strongest modifiable drivers. A meta-analysis published in Sleep Medicine Reviews found a consistent association between short sleep duration and elevated inflammatory markers, including CRP and interleukin 6. And we are talking about sleeping under six hours regularly.
SPEAKER_01Okay, so every parent of a newborn just felt personally attacked.
SPEAKER_00Temporarily elevated inflammation from acute sleep deprivation is different from the chronic pattern. But the direction is the same. Ultra-processed food is another major driver. Research published in the British Medical Journal by Dr. Byrne. Matilda Tuvier's team at Inserm in France found a dose-dependent relationship between ultra-processed food consumption and systemic inflammation markers.
SPEAKER_01Dose-dependent meaning the more you eat, the higher the inflammation.
SPEAKER_00Precisely. And then there is sedentary behaviour, which is distinct from exercise. You can go to the gym for 45 minutes and then sit for 10 hours. Research suggests that prolonged sitting independently elevates inflammatory markers, regardless of whether you also exercise.
SPEAKER_01That one genuinely surprised me when I first read about it. I thought I worked out this morning. I am covered.
SPEAKER_00Not entirely. The data suggests breaking up sitting, even with brief movement every hour, produces measurable anti-inflammatory effects.
SPEAKER_01Right, let's unpack the other side though, because this episode is not meant to scare people. What genuinely lowers CRP? What is actually evidence-based here?
SPEAKER_00Good, let's start with diet, because the research is robust. The Mediterranean dietary pattern consistently reduces high-sensitivity CRP across multiple trials. The pre-dimed trial, a large randomized controlled trial conducted in Spain, showed that a Mediterranean diet supplemented with olive oil or nuts reduced cardiovascular events and inflammatory biomarkers compared to a low-fat control diet.
SPEAKER_01I love that this is not about eliminating everything fun. Olive oil, fish, vegetables, legumes, these are delicious foods.
SPEAKER_00That is the somewhat cheering part of the evidence. You are not being asked to eat a punishing diet. You are being asked to eat a Mediterranean one. Omega-3 fatty acids, in particular, found in fatty fish like salmon and mackerel, have direct anti-inflammatory mechanisms at the cellular level.
SPEAKER_01And I want to flag something here for our pre-retiree and older listeners, especially. Gum disease, periodontal disease. I did not expect this to come up when I was researching, but it is real.
SPEAKER_00Entirely legitimate. Chronic periodontal infection is a significant and often overlooked driver of systemic inflammation. The bacteria involved can enter the bloodstream and directly elevate CRP. The American Heart Association has published statements on the relationship between periodontal disease and cardiovascular risk. Dental care is inflammation management.
SPEAKER_01Y'all floss your teeth. I cannot believe I am saying this on a wellness podcast, but apparently it matters for your heart.
SPEAKER_00And then there is sleep again. Because optimizing sleep is one of the most powerful levers available. Dr. Matthew Walker's work, detailed in his book Why We Sleep, summarizes research showing that improving sleep quality and duration produces measurable reductions in inflammatory markers. It is bidirectional.
SPEAKER_01Here's the thing though, and I want to name this because it matters. Not everyone can just sleep more. Caregivers, shift workers, people with chronic pain. The advice has to be honest about that.
SPEAKER_00That is a fair challenge. And I think the honest answer is that even incremental improvements in sleep hygiene, protecting the first and last hours of sleep, reducing light exposure, being consistent with timing, produce some benefit. It does not have to be perfect to move the needle.
SPEAKER_01I appreciate you saying that because the all or nothing framing is where people give up.
SPEAKER_00Agreed. And that is why the conversation about chronic inflammation is actually an optimistic one, despite how serious the underlying risk is. These are largely modifiable factors: diet, sleep, stress management, movement patterns. They are accessible levers.
SPEAKER_01So should everyone be asking for a high-sensitivity CRP test at their next physical?
SPEAKER_00It is worth a conversation with your doctor. The American Heart Association recommends it as a useful tool for cardiovascular risk assessment in individuals at intermediate risk. It is a simple blood test. It is not universally appropriate for everyone, but for adults over 40 or anyone with a family history of cardiovascular disease, it is a reasonable question to ask.
SPEAKER_01And knowing your number gives you something to actually work toward. That is actionable. Okay, so if you are walking away with one thing today, let it be this. Your inflammatory status is measurable, it is meaningful, and it is largely within your influence. Ask your doctor about high-sensitivity CRP. Look at your sleep, your food, your stress. These are not small things, they are the whole game.
SPEAKER_00Right. And to put a number on it, an optimal high sensitivity CRP is generally considered below 1 milligram per liter. Between 1 and 3 is elevated risk. Above 3 is high. Knowing where you sit is not cause for panic, it is cause for informed action, which is always preferable to comfortable ignorance.
