SPEAKER_00Y'all, let me tell you about my January. I did a five-day modified fast, felt incredibly virtuous, lost a few pounds, and then promptly got every single cold my kids brought home for the next two months. I mean every single one. I thought I was being so healthy.
SPEAKER_01And that is genuinely a fascinating thing to unpack. Because there is now emerging research suggesting those two events. The fasting and the infections may not have been a coincidence at all.
SPEAKER_00Wait, seriously? Because I assumed I was just, you know, a tired mom in winter.
SPEAKER_01Right, here's what I've learned. A study published in Nature in 2024, out of the University of Pennsylvania, led by researcher Maite Suarez Farinus and the team of Anselm and Sappathy, found that extended fasting can actually deplete immune memory cells. The very cells that remember how to fight infections you have already beaten before.
SPEAKER_00So my body essentially forgot how to fight stuff it already knew how to fight. That is wild and a little bit terrifying.
SPEAKER_01That is essentially what the research suggests. And today we want to dig into what that actually means, what the nuance is, and critically, what practical fasting still looks like when you care about your immune health.
SPEAKER_00Because we are not here to scare you off fasting entirely. That is not the move. Let's get into it.
SPEAKER_01So let's define the territory first. When we say fasting, we are talking about a broad spectrum. There's your 16 to 18 hour intermittent fasting, your 24-hour fasts, multi-day extended fasts, things like five-day fasting mimicking protocols.
SPEAKER_00Right, and the thing that's everywhere right now. People doing extended fasts for metabolic health, weight loss, even longevity, it's not a fringe thing anymore.
SPEAKER_01Absolutely not. And the benefits for many of those shorter protocols are genuinely well supported. Improved insulin sensitivity, some cardiovascular markers, cognitive focus. The research from Volta Longo at USC on fasting mimicking diets is quite compelling.
SPEAKER_00So we are not throwing the baby out with the bathwater here. Good. Because I know some of you just panicked a little.
SPEAKER_01No panic warranted. But let's dig into the specific mechanism the 2024 research identified, because it's genuinely important.
SPEAKER_00Please, yes, explain it like I'm a tired parent on my second coffee.
SPEAKER_01Right? So your immune system has what are called memory T cells and memory B cells. Think of them as your body's filing cabinet of past battles. Every infection you've cleared, every vaccine you've had leaves behind these cellular memories.
SPEAKER_00So when the same bug comes back, your immune system goes, Oh, I know you, and shuts it down fast.
SPEAKER_01Exactly. Now what this pen research found, and it was done in mouse models with human immune cell validation, is that extended fasting, specifically beyond roughly 24 hours, triggered a significant reduction in these memory immune cells in the bone marrow.
SPEAKER_00Here's the thing though, that sounds really alarming, but walk me through why it happens. Is the body just being stupid?
SPEAKER_01Not stupid at all. It's actually the body being efficient in the wrong context. During extended food restriction, the body enters a conservation mode. It starts pulling resources from tissues it considers metabolically expensive. And maintaining a large immune memory cell population is metabolically expensive.
SPEAKER_00So it's essentially downsizing, like a company laying off experienced staff to cut costs.
SPEAKER_01That is a surprisingly accurate analogy. And just like that company, when the crisis is over and business picks back up, you've lost institutional knowledge that takes time to rebuild.
SPEAKER_00And the rebuilding, that's the part that scared me when I read about this. It's not instant.
SPEAKER_01No. The research suggested recovery of these cell populations can take several weeks after an extended fast ends, which maps onto your January experience rather neatly.
SPEAKER_00Oh, it maps perfectly. Okay, so if you are someone in the sandwich generation, maybe you're 50, you're managing your own health, maybe caring for aging parents, your immune resilience is already something you're thinking about. This feels especially relevant.
SPEAKER_01Brilliant point, Emma. And for our pre-retiree and active senior listeners, this is worth paying close attention to. Immune memory naturally declines with age. A process called immunosinescence, documented extensively by researchers like Janet Lord at the University of Birmingham. So you have less margin to absorb additional depletion.
SPEAKER_00And for my younger listeners who are thinking that's future Mies problem, just know that you are building your immune memory right now. What you do in your 30s affects the reserves you have later.
SPEAKER_01Now let's be fair and address where the science currently sits. This research is important but preliminary in humans. Mouse models are not always a perfect translation.

SPEAKER_00So what does the nuance look like practically? Because here's where I actually disagree with the doom and gloom interpretation I've seen online.
SPEAKER_01Go on, I'm curious.
SPEAKER_00I think the problem isn't fasting. I think the problem is long, unstructured fasting with no attention to what happens around it, nutrition-wise, sleep-wise, stress-wise. Context matters enormously.
SPEAKER_01I'd largely agree with that. And the research itself points to duration as the critical variable. The shorter intermittent fasting windows, your 16 to 18 hours, did not show the same degree of immune memory disruption. The alarm bells get louder beyond the 24 to 48 hour mark.
SPEAKER_00Which is honestly where most casual fasters never go anyway, right? Most people doing intermittent fasting are not doing five-day water fasts.
SPEAKER_01Correct. And Longo's fasting mimicking protocol specifically involves maintaining some caloric intake, roughly 700 to 1,000 calories on restricted days, which may preserve more of these cell populations than total caloric deprivation.
SPEAKER_00So the design of the fast matters, it's not just about the clock.
SPEAKER_01Precisely. And refeeding matters enormously too. What you eat when you break an extended fast has a direct bearing on how quickly immune cell populations can recover. Protein quality, specifically leucine-rich proteins, plays a role in stimulating the rebuilding of these populations.

If you are drawn to fasting for metabolic or longevity reasons, the evidence still supports shorter time-restricted eating windows. 16 to 18 hours is the sweet spot most research, including work from Satchin Panda at the SOLK Institute, endorses for metabolic benefit with lower immune disruption risk.
SPEAKER_00Guideline 2 from me. If you are going to do an extended fast, timing matters. Don't do it when your immune system is already under stress. Peak cold and flu season, major work deadlines, periods of poor sleep. Your system does not need another stressor right now.
SPEAKER_01Guideline 3. The refeed matters as much as the fast. Break extended fasts with protein-rich, nutrient-dense whole foods. Don't undo immune recovery with a refeeding pattern that's basically processed carbohydrates for three days.
SPEAKER_00Guideline 4, and this is personal for me, build in recovery time. If you fast for two or three days, assume your immune defenses are slightly lowered for the following week or two. Be a little more intentional. Prioritize sleep. Don't go lick doorknobs. Eloquently put, I mean it's solid advice regardless.
SPEAKER_01And guideline five, which I think is underappreciated. Work with someone who knows your history. A registered dietitian familiar with fasting protocols can help you design something that achieves your goals without unnecessary immune cost. This is not a situation where generic internet advice should be your only guide.
SPEAKER_00Especially if you are immunocompromised on medications managing a chronic condition. Please get a professional in your corner. We are your informed friends, not your doctors.
SPEAKER_01And one final nuance worth naming. Actually supports long-term you immune health. The research from Yoshinori Osumi, who won the Nobel Prize for his work on autophagy mechanisms, is relevant here. It is not a simple fasting bad for immunity story.
SPEAKER_00It is a how you fast and for how long story, which is honestly the story for most things in wellness.
SPEAKER_01Right, so if you take one thing from today, duration is the variable that matters most. Shorter fasting windows, under 18 hours or so, appear to carry far lower risk to your immune memory than extended multi-day fasts. The research is early, but the mechanism is real and worth respecting.
