SPEAKER_01Y'all, let me tell you something. Last month I was at my mom's house and she's 67, sharp as a tack her whole life, and she looked at me mid-sentence and just blanked, forgot the word she wanted, and I watched her face fall. That moment stuck with me for weeks.
SPEAKER_00Right? And that fear, that moment of watching someone you love search for a word, it's one of the most universally human anxieties there is. What's fascinating is that researchers are now looking at a supplement most people associate exclusively with gym culture as a serious candidate for slowing that exact kind of decline.
SPEAKER_01We are talking about creatine. Yes, that creatine. The thing your nephew probably has in a tub next to his protein powder. But here's the thing, though, the story of creatine in your brain is completely different from what most people know, and honestly, it surprised me.
SPEAKER_00And today we're going to dig into the evidence, not the hype. What does the science actually say about creatine cognitive aging and whether this is genuinely worth your attention?
SPEAKER_01So let's start from the beginning because I think most of our listeners hear creatine, and picture a 22-year-old at the squat rack.
SPEAKER_00Completely fair association. But creatine is not a synthetic compound. It's a naturally occurring molecule your body makes from three amino acids, arginine, glycine, and methionine. Your liver, kidneys, and pancreas produce it. You also get it from red meat and fish.
SPEAKER_01So it's not some lab-invented thing, it's already in us.
SPEAKER_00Exactly. And here's the part that surprised me when I first looked into this. About 95% of your body's creatine sits in skeletal muscle. But the remaining 5%? Concentrated heavily in the brain and heart. Two organs with extraordinarily high energy demands.
SPEAKER_01Which makes you think, why did we ever only talk about it for lifting weights?
SPEAKER_00Marketing mostly. The research on muscle performance came first and it was commercially driven. But the cognitive angle has been building quietly in academic literature for at least two decades now.
SPEAKER_01So let's talk about the brain piece. Because I think people need to understand why creatine would even matter up there. What's actually happening?
SPEAKER_00Right, so here's the core mechanism. Your brain cells' neurons run almost entirely on a molecule called ATP, adenosine triphosphate. Think of ATP as the currency your cells spend to do anything. Think, move, remember, breathe.
SPEAKER_01Okay, ATP equals brain fuel. Got it.
SPEAKER_00Creatine's role is to rapidly regenerate that fuel. It works with a compound called phosphocreatine to recharge ATP almost instantly when your neurons are working hard. Dr. Sergei Jerzinski and research teams, including work published in the journal Amino Acids, have documented how this system becomes less efficient as we age.
SPEAKER_01So your brain's ability to recharge its own batteries gets slower over time. That's what we're talking about preventing.
SPEAKER_00Or at minimum, slowing. And the critical question was always whether supplemental creatine can actually reach the brain. Because plenty of things you swallow never cross what's called the blood-brain barrier, which is essentially a very selective security checkpoint.
SPEAKER_01And the answer is?
SPEAKER_00Yes, it does. The transporter protein, SLC6A8, actively carries creatine into the brain. This has been confirmed through neuroimaging studies showing measurable increases in brain creatine levels after supplementation. Research from the University of Sydney, including work by Dr. Caroline Ray, has been particularly important here.
SPEAKER_01Okay, I want to talk about what the actual benefits look like in real people, because that's what our listeners need. One of the most cited studies is a randomized controlled trial, also from Dr. Say, the Docker's group at Sydney, published in the Proceedings of the Royal Society B. They gave participants five grams of creatine daily for six weeks and tested cognitive performance.
SPEAKER_01And what happened?
SPEAKER_00Significant improvements in working memory and processing speed, particularly on tasks requiring rapid mental calculation and holding information in mind while doing something else. The effect was more pronounced in vegetarians who have lower dietary creatine to begin with.
SPEAKER_01That vegetarian finding is really interesting to me because a lot of our listeners who are health conscious have already reduced their red meat. They might actually have the most to gain.
SPEAKER_00Brilliant point, Emma. And it extends to older adults generally. As we age, our endogenous creatine synthesis naturally declines. Research published in the journal Nutrients has examined this specifically in adults over 60 and found that supplementation can partially compensate for that natural drop.
SPEAKER_01Here's the thing though, I want to be honest about what we don't know yet, because I don't want anyone listening to think this is a magic pill situation.
SPEAKER_00No, and I appreciate you flagging that. The cognitive research is genuinely promising, but the field is still relatively young compared to the muscle performance literature. Most studies are short-term, 6 to 12 weeks. We don't yet have large-scale long-term trials spanning years for cognitive outcomes specifically.
SPEAKER_01So we're not at the point of saying, take creatine and you'll never have a senior moment.
SPEAKER_00Absolutely not. What we can say is that the mechanistic rationale is solid. Short-term cognitive benefits in controlled trials are real and replicated. And the safety profile for creatine monohydrate is exceptional. It is one of the most studied supplements in existence.
SPEAKER_01And that safety point matters enormously to me, especially for our listeners who are in their 50s and 60s and managing multiple health considerations.
SPEAKER_00Right. The International Society of Sports Nutrition has formally stated that creatine monohydrate is safe for long-term use in healthy adults. The main reported side effects are mild GI discomfort if taken in large doses at once and some water retention in muscle tissue.
SPEAKER_01I want to pause on the water retention thing because I know some of our listeners, especially women in perimenopause or menopause who are already feeling puffy or bloated, might hear that and immediately check out.
SPEAKER_00That's a legitimate consideration. The water retention is intramuscular, meaning inside the muscle cells, not subcutaneous. It's not the same as general bloating. And interestingly, for older adults experiencing muscle loss, which is called sarcopenia, that cellular hydration may actually be beneficial.
SPEAKER_01So potentially helpful on two fronts. The brain and maintaining muscle as we age.
SPEAKER_00Which matters enormously for overall health span. Dr. Darren Kando at the University of Regina has published extensively on creatine and aging across both muscle and bone metrics. His work suggests combining creatine with resistance exercise produces synergistic benefits in older adults.
SPEAKER_01Okay, so this brings us to how someone would actually use this practically. Because I know someone listening right now is thinking, great, but what do I do with this information?
SPEAKER_00The most well-studied form is creatine monohydrate, not the fancier, more expensive variants. Dose-wise, 3 to 5 grams per day is what the cognitive research has used. No loading phase necessary for brain benefits, which is different from traditional muscle protocols.
SPEAKER_01And the loading phase thing was always the part that felt extreme to me. Like I am not taking 20 grams of anything per day.
SPEAKER_00Which you don't need to for cognitive purposes. Just three to five grams consistently every day. Consistency matters more than timing. Take it with food if you have any stomach sensitivity.
SPEAKER_01One more question I keep getting from people in my life, and honestly it came from my husband too. Is this only for older people? Like, should someone in their 30s care?
SPEAKER_00The research shows benefits across ages for tasks requiring significant mental effort. Like the Sydney study included younger adults. But the urgency for brain-specific creatine supplementation, based on current evidence, is greater for adults over 45, especially those who eat little or no meat.
SPEAKER_01So if you're in your 30s and generally eating well, it's probably not the most pressing thing on your list. But if you're in the sandwich generation caring for aging parents while managing a career and still trying to think clearly at 10 p.m., the case is actually pretty compelling.
SPEAKER_00And that's a genuine argument from the evidence, not from enthusiasm.

The science on brain energy and cognitive aging is real, the safety record is solid. And three to five grams of plain creatine monohydrate per day is a genuinely low-barrier thing to explore with your doctor if you're over 45 and thinking about your long-term mental sharpness.
