In this segment, Dr. Mistry and Donna Lee explain the common occurrence of penile shrinkage in overweight and/or aging men. What Dr. Mistry refers to as "shrinking penis syndrome" can have different causes, but is often related to weight gain and/or low testosterone. To improve this condition, Dr. Mistry has developed a "cock-tail" of treatment modalities that combine to increase the length of the penis up to an inch and a half while also increasing the girth of the flaccid penis. This cocktail includes: spark wave therapy to improve the erectile bodies of the penis, use of a vacuum erection or penile tension device to add length, a daily dose of Tadalafil (Cialis) to increase blood flow, and the normalization of testosterone levels. Together, these treatments can help restore length to your penis lost to "shrinking penis syndrome."

Donna LeeYou do get tired of me talking about moon phases and all the angry and sad employees and patients that show up every full moon.
Dr. MistryThat's correct.
Donna LeeThat's a thing. My dad was a PA in the ER.
Dr. MistryI'm very scientific. We don't believe in moon phases...
Donna LeeMy dad didn't either, but he was an ER PA, and every full moon he'd come home and tell me and my mom about how crazy all the patients were, and I thought he was crazy. Now I work with you.

Dr. MistryWhat a great segue into kind of that concept that we love here, which is the concept of the second opinion, which you can ask via question at armormenshealth@gmail.com. And then we can also take your consult through a telemedicine appointment. Even if it's complicated, we get medical records from all over the country. We have patients from all over the country that come and travel to us for advanced prostate cancer treatment. We have some patients that worry that they can't get in to see us, but we pride ourselves on availability because we try to make sure that we have space for those interesting and complex cases that people have not found a solace or care for. If you think that you have a condition that's being overlooked, we would encourage you to come see us. If you're interested in advanced therapies, we encourage you to see us. If you're sitting out there with BPH, just being, you know, managed on medications that are insufficiently working and want to hear about the next step-this week, I had a patient who gets up 10 times a night to pee, and he's seen two urologists and nobody was able to make a connection. In one appointment, we diagnosed him with something called nocturnal polyuria, that means he was making too much pee at night.
Donna LeeThe other guys couldn't figure that out?
Dr. MistryI guess they didn't give them a urinal. A urinal cost two bucks. In one week, we diagnosed him. He's making a liter and a half of urine at night. So we're going to get him, you know, we're gonna make sure his kidneys are working okay, we're going to put him on a medicine called Desmopressin, and we're going to get him fixed up. And I really think that we are going to do that. And so it's kind of that more holistic body approach, really approaching things in a, in a real smart, intelligent, thought out, algorithmic way. I think really sets us apart.
Donna LeeYou know , I have a question. We had a patient--we're kind of short staffed, so I'm answering calls here and there and taking new patients. Anonymously though, this was intriguing to me, though--the patient had since childhood, he had a lump on his penis and he's in his thirties now. And so he wanted to make an appointment because now it's starting to hurt. So I thought, "Well, I wonder why it never hurt before?" And I asked if there was any trauma, but maybe he had a hidden or microtrauma? But he said, "No nothing's happened. But I went to my primary care and he suggested I see a urologist."
Dr. MistryFor the primary care doctor, it's safest always to send these painful penile lumps. Usually when we hear about lumps, lumps, my penile lumps.
Donna LeeIsn't that a Fergie song? "My lovely lady lumps." Humps?
Dr. MistryThat's not...It was, it was inappropriate. So , if you have little lumps on your penis, a common lump that can happen maybe not from infancy, but from, from puberty onwards is a sebaceous cyst. It's like a little, an entrapped sweat gland.
Donna LeeSo you go to Dr. Pimple Popper for that?
Dr. MistryThat's gross. Come to Dr. Mistry, he'll take care of it better . Those sebaceous cysts can sometimes become infected. Are there can be other cysts of the skin that become infected. I've had people have veins that become infected. There are other types of ulcerations there that can also become infected.
Donna LeeSo cysts can last that long?
Dr. MistrySurely, there can be any number of things that can last even from not again, not from infancy, usually, usually from, you know , teenage years and only, you know, raise their head in adulthood. Well, that's a great question. I'm glad you asked. Is there a question from a listener that we can go over?
Donna LeeWe can, yes. All right, I cannot promise this is not from my husband--it might be.

Donna Lee"Good morning, Dr. Mistry. I'm in my fifties over the past several months, it seems my penis is noticeably smaller when erect. Not sure what might be causing this. Maybe just old age?"
Dr. MistryFirst of all, being 47 myself, I'm hoping that 51 doesn't qualify as old age. I'm still a young buck...The shrinking penis syndrome, which is so common that I really think we should make it like a syndrome. As men age, absolutely there is a shortening of the penis that some men will, will perceive. After prostate treatment, whether it be prostate cancer, BPH treatment, even HIFU, a lot of men will report a shrinking or a retraction of the penis. When we look at that, we try to think to ourselves what could be going on? The first thing that we think about is that maybe everything's the same size, but maybe you got fatter. 30 pounds gives you one inch. So if you have that FUPA--that mons pubis area that has more fat, definitely your penis is going to appear shorter. So 30 pounds, one inch. Now, to me, that seems like common sense. But so many men are just absolutely like surprised.
Donna LeeFascinated.
Dr. Mistry"Are you saying that it's the extra 30 pounds I'm carrying here that make my penis look shorter?" Yes. That's precisely, that's precisely what happened. And especially if it happens over a period of just several months. The second thing that we think about is a lower testosterone. So a lower testosterone will make your, just kind of like, what is the rigidity of your penis, you know, how are you carrying it? What is the health of your erectile bodies? Could make it seem like you're more retracted? I have seen patients with something called pelvic for spasticity-- that's where the pelvic floor gets real tight and that can actually pull up some of the musculature that allows the penis to suspend. So if you've had like a back injury or a hip injury, or you're not as active , you might notice that that could be going on. If you're on a medication for an enlarged prostate, like Finasteride, I think that there could be some risk of the way that the testosterone's being manipulated could cause that to get, you know, to perceive that you have a smaller penis. So when we have that in our office , we offer a multipronged therapy. Number one is we make sure that you are your testosterone levels normal. Number two, we have, you begin using a vacuum erection device or a penile tensioning device. Both of these can add up to one and a half inches of penile length if used regularly.
Donna LeeOne and a half inches? That's a lot. I got excited about that.
Dr. MistryYou got too excited. What do you care? You don't like...
Donna LeeI'm like, "Hey! I'm running away from that."
Dr. MistryThen there's the use of spark wave, which we have in our office, which is a type of treatment that uses low intensity acoustic or shockwave therapy to cause microtrauma to that, the erectile body, helps regenerate some healing, and then using that vacuum erection device or that penile tension device to help kind of give you more length. But then my, like my topper here is a daily Tadalafil. So the daily Tadalafil or Cialis commonly used for erectile dysfunction, not only will it help you get a better erection when you're trying to get one, but I think that it also helps carry a larger penis size and girth when it's flacid. And not because it's partially erect all the time. I think there's more blood flow to the penis, and if you can tolerate the medicine, which most people do, I think it leads to an overall better girth and length. And then that along with the vacuum erection device, the spark wave therapy if you're interested, and then normalizing testosterone would be kind of our cocktail for shrinking penis syndrome.
